1.A correlation of pulse pressure and prognosis of refractory septic shock patients

Objective To explore the correlation of pulse pressure(PP) and outcome in refractory septic shock patients.Methods A total of 68 patients with refractory septic shock consecutively admitted in our ICU from January 2012 to December 2012 were retrospectively studied.Hemodynamic data and arterial lactate concentration were collected at the time of admission and 24 hours after admission.The outcome of Day 28 post-diagnosis was also recorded.Results (1) Compared with the survivors,heart rate(HR) at 24hours after admission was higher in non-survivors,while 24 h lactate clearance rate (rLac) was lower in them (P < 0.05).Other hemodynamic parameters showed no difference between the non-survivors and the survivors at 24 hours after admission,including central venous pressure (CVP),mean arterial pressure (MAP),systolic blood pressure(SBP),diastolic blood pressure(DBP),PP,pulse pressure/heart rate (PP/HR),pulse pressure/mean arterial pressure(PP/MAP),pulse pressure/systolic pressure(PP/SBP),pulse pressure/diastolic pressure (PP/DBP),the value of SBP above MAP (SMP) and the value of DBP below MAP(MDP).(2)The mortality rate was higher in the patients with HR≥100 b/min than those with HR < 100 b/min,but without statistical significance (56.25% vs 36.11%,P =0.096).Compared with the survivors,no matter with HR≥100 b/min or HR < 100 b/min,lactate(Lac) at the 24 hours after admission was higher in all the non-survivors (P < 0.05),while with lower rLac (P < 0.05).In those with HR ≥100 b/min,the following hemodynamic parameters were higher in the non-survivors than in the survivors,including PP,PP/HR,PP/MAP,PP/SBP,PP/DBP,SMP and MDP (all P values < 0.05),while no statistical difference was observed in those with HR < 100 b/min.(3)The mortality rate showed no statistical difference in those with MAP≥85 mmHg(1 mmHg =0.133 kPa) and with MAP < 85 mmHg(42.42% vs 48.57%,P =0.611).No matter MAP≥85 mmHg or MAP < 85 mmHg,compared with the survivors,all the non-survivors had higher Lac at the 24 hours after admission (P < 0.05),while with lower rLac (P < 0.05).In those with MAP≥85 mmHg,HR was higher in the non-survivors than the survivors (P < 0.05).In those with MAP < 85 mmHg,compared with the survivors,the non-survivors had higher PP,PP/MAP,PP/SBP,PP/DBP,SMP and MDP (P <0.05),while with lower DBP (P <0.05).Conclusion PP is correlated with the outcome in refractory septic shock patients.When the HR and MAP differ,PP has different effect on the outcome and contributes more to the tissue perfusion and outcome in those with higher HR and lower MAP.
2.Investigation of stroke-preventing knowledge and influential factors in hypertensive patients from the communities of Guangzhou

Objective To investigate the current situations in stroke-prevention knowledge and influential factors in hypertensive patients from the communities of Guangzhou.Methods 94 hypertensive patients visiting the community health service centers in Guangzhou participated in the survey using stroke-preventing knowledge questionnaire.Results The total score on the stroke-preventing knowledge was(62.70±18.39).By stepwise regression analysis,those female and well-educated scored higher. The score on monitoring the blood pressure was the lowest(43.09±25.77).There were mistakes in medication,physical exercises and living habits.Conclusions The hypertensive patients in the communities in Guangzhou lack of stroke-preventing knowledge.In the stroke-preventing education,those male patients as well as those with less education deserves more attention.
3.Metabolic changes in abnormal savda patients with different types of tumor: a clinical observation.

OBJECTIVETo explore in vivo metabolic changes in abnormal savda patients with different types of tumor.
METHODSA total of 142 abnormal savda patients with common cancer types were enrolled in this study, and 50 healthy volunteers were recruited as the control group. For each sample, the H Nuclear Magnetic Resonance (NMR) based metabonomic analysis was performed. The free attenuation signal was computed subsection integral. Data obtained were analyzed by the Orthogonal Partial Least-Squares Discriminant Analysis (OPLS-DA).
RESULTSCompared with the control group, leucine, isoleucine, valine, histidine, phenylalanine, tyrosine, alanine, creatine, lactic acid, inositol, alpha-and beta-glucose, unsaturated lipids, very low density lipoprotein (VLDL) significantly decreased (P <0.05), while glycoprotein and carnitine significantly increased (P <0. 05) in the abnormal Savda group.
CONCLUSIONAbnormal savda patients with different types of tumor had similar metabonomics changes.

4.Study on regulatory effect of Danshensu on lipid metabolism of hyperlipidemia rats.

OBJECTIVETo explore the effect of Danshensu on the lipid metabolism of hyperlipidemic rats.
METHODSixty clean male SD rats were selected. Twelve of them were selected in the basic control group and fed with common foods, and the remaining rats were fed with the high-fat feeds. After the successful modeling, they were randomly divided into the high-fat control group and low dose (10 mg x kg(-1) x d(-1)), medium dose (20 mg x kg(-1) x d(-1)) and high dose (40 mg x kg(-1) x d(-1)) Danshensu (dissolved in saline) groups. Both of the two groups were abdominally injected with the same volume of normal saline once a day for consecutively 30 days. The serum TG, TC, HDL-C and liver ACC1, FAS, HMGR, CPT-I mRNA expressions were detected.
RESULT AND CONCLUSIONDanshensu could inhibit the LDL-C level, timely clear redundant cholesterol and effectively regulate the lipid metablism of hyperlipidemic rats by reducing the TC content, decrease the fatty acid by reducing the FAS mRNA expression, and reduce the synthesis levels of endogenous cholesterol by inhibit the HMGR mRNA expression.

5.The imaging performance and mechanism of acute slipped patellofemoral joint impingent syndrome

Objective To study the imaging performance and mechanism of acute patellofemoral joint impingent syndrome.Meth-ods The MR images and clinical data of acute patellofemoral joint impingent syndrome were analysed retrospectively in 10 patients, which were confirmed by clinic .MRI and DR examination were performed in all patients,in which 6 patients had complete knee ar-throscopy material.The imaging performance and mechanism of acute patellofemoral joint impingent syndrome were summarized re-spectively.Results All cases had clear history of buckling ectropion trauma;Patellar dislocation or subluxation were detected in 9 cases on DR examination,10 cases on MRI;Patellar medial fracture or osteochondral fracture caused by acute slipped patella was de-tected in 1 case on DR examination,2 cases (3 places)on MRI;Medial patellar retinaculum injury were detected in 10 cases on MRI,which were corresponding to that on surgery.Articular injury and subchondral marrow edema in particular parts of joint (an-terolateral of lateral femoral and medial patellar)were detected in 9 cases on MRI.MRI features of articular cartilage injury were confirmed by arthroscopy.MRI showed ACL or PCL injury in 3 cases,meniscus tear in 4 cases,which were quite consistent with the arthroscopy findings.Conclusion MRI can effective display joint cartilage damage,subchondral marrow edema in particular parts of the joint,which is useful to clinical prediction and treatment at early stage.
6.The clinical effects of traditional Chinese medicine on Kawasaki disease:a systematic review of randomized controlled trials and Meta analysis

Objective To systemically evaluate the clinical therapeutic effect and safety of traditional Chinese medicine on Kawasaki disease. Methods Through retrieval of the reports in PubMed, the Cochrane Library, China National Knowledge Internet (CNKI), VIP database, Wanfang database by computer, the correlative articles of clinically randomized controlled trials (RCTs) published in China during the period from 1990 to 2013 were collected, and their quality evaluation was made. All the articles were screened according to inclusion and exclusion criteria, in which interventions were traditional Chinese medicine therapy and conventional western treatment. The articles selected were assessed with the software of RevMan 5.2. Results There were 212 articles searched out, but only 13 articles, 656 children with Kawasaki disease were conformed to the inclusion criteria. The Meta analysis indicated that the total effective rate in the traditional Chinese medicine group was superior to the group treated with western medicine. The difference in the curative effects between the two groups had notable significance [odds ratio (OR) = 3.58, 95%confidence interval (95%CI) was 2.16-5.93, test for overall effect Z=4.94, P<0.000 01]. No difference was seen in the curative effect of improving coronary artery changes between the traditional Chinese medicine group and western medicine control group (OR=2.03, 95%CI was 0.74-5.62, test for overall effect Z=1.37, P=0.17). Conclusion The clinical therapeutic effect of traditional Chinese medicine on Kawasaki disease is superior to that treated by western medicine.
7.Value of ultrasound-guided fine needle aspiration biopsy in diagnosing thyroid nodules of TI-RADS 4-6 categories

Objective To explore the application value of ultrasound-guided fine needle aspiration biopsy(FNAB) in diagnosing thyroid nodules of TI-RADS 4-6 categories.Methods A retrospective analysis was made on the pathologic data of 494 patients (totally 501 nodules) who underwent TI-RADS ultrasonographic stratification and ultrasound-guided FNAB.168 thyroid nodules of TI-RADS 4-6 categories were selected from 163 patients and classified into three groups:group T,TI-RADS 4A categories;group Ⅱ,TI-RADS 4B category; group Ⅲ,TI-RADS 5-6 category(the pathologically confirmed cases in category 6 had been removed).The ultimate pathologic findings were taken as golden standards to calculate the accuracy,sensitivity,specificity,positive predictive value of ultrasound-guided FNAB in the three groups.ROC curve was drawn to evaluate the diagnostic value of FNAB.Results The accuracy,sensitivity,specificity,positive predictive value of the ultrasound-guided FNAB in the three groups were listed as follows:group Ⅰ 74.4%,73.3%,75.0%,64.7%;group Ⅱ 83.9%,87.5%,71.4%,91.3%;group Ⅲ 89.3%,91.7 %,75.0%,95.7 % respectively.The positive predictive values of the ultrasound-guided FNAB for group Ⅰ and group Ⅱ was higher than that of conventional TI-RADS classification criteria,so the difference was statistically significant; while the positive predictive values for group Ⅲ was close to the conventional ultrasound TI-RADS classification criteria,the difference was not statistically significant.Conclusions The combination application of TI-RADS classification criteria and FNAB can improve the accuracy of ultrasound diagnosis in group Ⅰ and group Ⅱ,and reduce the puncture rate of group Ⅲ.

The anti-aging effect of Yang Shou Dan (YSD) was studied. It was shown that YSD could obviously increase the activity of superoxicde dismutase (SOD) in erythrocyte, decrease lipid peroxides (LPO) in serum and in liver tissue and reduce the content of lipofucin in myocardium of mice. It could also prolong the mean life span, the half lethal period and the longest life time of drosophila melanogater. It is suggested that YSD exerts an anti-aging effect to a certain extent.
9.CT and MRI features of dermatofibrosarcoma protuberans

Objective To investigate CT and MRI features of dermatofibrosarcoma protuberans (DFSP).Methods Totally 16 patients with DFSP confirmed by pathology were enrolled.Tumor morphology,CT and MRI imaging appearance (11 cases underwent plain and enhanced CT,5 cases underwent plain and enhanced MRI) were analyzed retrospectively.Results DFSP usually occurred in the skin of truck,head and neck,protruding from the skin surface in different extent.Some lesions even suspended out of the skin.The lesions were divided into nodular type (n=12) and diffuse type (n=4) according to their morphological appearance.The tumors usually demonstrated as iso-density or slightly low density solid mass compared to muscle on CT.On MRI,it usually demonstrated as low signal on T1WI and high signal on T2WI.Tumor blood supply was rich,and it usually showed progressively moderate to strong enhancement.The signs within DFSP include hanging sign (n=2),skin tail sign (n=6),fascia tail sign (n=l),fat tail sign (n=4).Conclusion DFSP can be characterized by nodular or diffuse lesions,the manifestations of different form are slightly different,but still have a certain characteristic.
10.Effects of multimodal combination dialysis on Klotho protein, FGF-23 and BNP in patients with maintenance hemodialysis

Objective To discuss the effects of multimodal combination dialysis on Klotho protein, fibroblast growth factor-23 (FGF-23) and brain natriuretic peptide (BNP) in patients with maintenance hemodialysis (MHD). Methods A randomized controlled trial (RCT) was conducted. 120 patients who was diagnosed with chronic renal failure (CRF) uremia receiving MHD over 3 months admitted to Blood Purification Centre of Department of Nephrology of the Second People's Hospital of Guiyang from December 2015 to December 2016 were enrolled, who were randomly divided into hemodialysis (HD) group (HD for 8 times a month), HD + hemofiltration (HF) group (HD for 8 times a month + HF once a month), and HD + HF + hemoperfusion (HP) group (HD for 8 times a month + HF for 4 times a month + HP once a month), with 40 patients in each group. Before and after treatment for 6 months and 12 months, blood was taken from venous circuit tube, the serum Klotho protein and FGF-23 levels were determined by enzyme linked immunosorbent assay (ELISA), and the serum BNP level was determined by electrochemiluminescence. Results 120 patients with MHD were enrolled in the final analysis without withdrawal. There were no significant differences in the levels of Klotho protein, FGF-23, or BNP before enrollment among the three groups (all P > 0.05). Compared with those before enrollment, the levels of serum Klotho protein after enrollment in three groups showed a sustained upward tendency, which were higher in HD + HF + HP group than in HD + HF group and HD group (μg/L: 2.59±0.61, 1.63±0.35, 1.13±0.26 at 6 months, F = 119.374, P = 0.000; 6.98±1.21, 3.57±1.03, 2.12±0.43 at 12 months, F = 275.675, P = 0.000); the levels of FGF-23 showed a sustained downward tendency, which were lower in HD + HF + HP group than in HD + HF group and HD group (ng/L: 69.22±38.26, 132.28±61.18, 178.50±74.64 at 6 months, F = 33.509, P = 0.000; 32.81±17.32, 87.93±43.27, 146.33±69.28 at 12 months, F = 55.466, P = 0.000);the BNP showed a similar tendency as FGF-23 (ng/L: 4083.39±2864.53, 7245.69±4643.81, 7969.12±5360.85 at 6 months, F = 8.758, P = 0.000; 1521.86±894.63, 4554.32±1969.84, 5013.89±2033.64 at 12 months, F = 49.003, P = 0.000). Conclusion Multimodal combination dialysis can increase the Klotho protein level, and decrease the levels of FGF-23 and BNP in MHD patients with CRF uremia.
