3.The diagnostic value of motor evoked potentials of musculus rectus abdominis in thoracic spinal cord injury

Objective To investigate the clinical application of transcranial magnetic stimulation motor evoked potentials (TMS-MEP) of rectus abdominis in the diagnosis of thoracic spinal cord injury. Methods The innervation of rectus abdominis and dermatomal distribution were studied in 5 adult cadavers. MEP of bilateral rectus abdominis in different segments was measured and recorded in 25 normal subjects for reference and in 23 patients with T4-T12 spinal cord injury. Results The anatomic study showed that rectus abdominis was innervated by 7 pairs of intercostal nerve from T6 to T12, useful to determine the MEP recording points in the body surface. In all normal subjects, T6-T12 MEP of bilateral rectus abdominis were recorded successfully and the referential range of normal MEP was established. The MEP peak latencies of each level were significantly different each with other. Among 23 patients suffering from thoracic spinal cord injury, 12 cases with complete paralysis had no MEP record below the injured level and no neurological recovery during the follow-up; while in 11 cases with incomplete paralysis, abnormal MEP were recorded in all but one with T4 fracture, the MEP of this patient disappeared below T6. The change of MEP of rectus abdominis was consistent with the level of fracture except in 1 case. During follow-up, 9 cases with incomplete paralysis patients showed recovery to different extent. Conclusion According to neuroanatomy, segmental MEP of rectus abdominis can be evoked and recorded in all normal subjects. The MEP peak latencies are significantly different in each level. MEP of rectus abdominis is proved to have the localizing and characteristic diagnostic value in the evaluation of the thoracic spinal cord injury.

Objective To investigate the effect of Xingnaojing injection solution in treating severe burn patients with sepsis. Methods 41 cases of burn patients with sepsis were randomly divided into experimental group and control group. The experimental group were intravenously injected 40ml/day Xingnaojing injection solution for 7 days. The control group received routine therapy. Bacteria counts in burn wound, the survival rate of the patients and the positive rate of blood culture in the both groups were determined and compared. 5.Hand hygiene among healthcare workers in primary medical institutions in Shijiazhuang

Objective To understand hand hygiene(HH) among health care workers(HCWs) in primary medical institutions in Shijiazhuang,and evaluate HH compliance,so as to formulate the strategy for HH management.Methods In 2015,one county-level comprehensive medical institution and two township medical institutions from each of seven counties of Shijiazhuang city were randomly selected,HH qualified rates of HCWs in all divisions of internal medicine before clinical procedures and after HH were analyzed statistically.Results 118 HCWs before clinical procedures and 130 HCWs after HH were monitored.The qualified rate of HH of physicians and nurses before clinical procedures were 44.26% (27/61) and 40.35% (23/57) respectively;HH qualified rate of nurses in township hospitals before clinical procedures were higher than county-level hospitals(x2 =6.447,P =0.011);the qualified rate of HH of physicians and nurses after HH were 98.41 % (62/63) and 98.51 % (66/67) respectively;there were significant differences in the qualified rates before clinical procedures as well as after HH in HCWs at different levels and types of medical institutions(all P<0.05).Conclusion The qualified rate of HH among HCWs in primary medical institutions in Shijiazhuang before clinical procedures is low,medical institutions should improve the HH compliance among HCWs from various aspects.
6.Temporary vena cava filter for the thrombolytic treatment of venous thrombosis

Objective To study the value of the temporary vena cava filter in the prevention of ectopic embolism formation in cases with thrombolytic therapy of venous thrombosis.Methods Of 11 patients, 10 suffured from venous thrombosis of lower limb and 1 from thrombosis of right internal jugular vein with persistent truncus arteriosus were treated by inserting permanent or temporary vena cava filter via median vein. Results In all 11 case control of symptoms followed this treatment occurred 7 to 15 days, thrombi were completely or partly dissoluted, the blood vessels recanalized, and finally removed the temporary vena cava filter. There were no complication and ectopic emboli related to thrombolytic therapy. Conclusions Our observation indicated that temporary vena cava filter insertion is safe and effective to prevent ectopic embolism in thrombolytic therapy of venous thrombosis.
7.Isolinetic test on evaluation of manipulation therapy for frozen shoulder

Objetive: To study the result of manipulation therapy(MT) for frozen shoulders. Method: Before and after MT, the parameters of involoved shoulder extensors and flexors , abductors and adductors were isokinetically recorded on 20 patients with frozen shoulder twice. Result: Compared with the parameters measured pre-therapy most of those factors measured increased significantly afterwards (P<0.01 ). Conclusion:Effect of MT on frozen shoulder are satisfactory and ROM are accurate and reliable.
8.Construction of Nanog lentiviral expression vector and its application in differentiation regulation of embryonic stem cells

BACKGROUND:There is a lack of safe and effective modular lentivectors in differentiation regulation of embryonic stem cel s. OBJECTIVE:To construct a modular lentivector, Puro-Nanog-hrGFP, with Nanog promoter-control ed humanized renil a reniformis green fluorescent protein (hrGFP) and puromycin and to observe the expression of Nanog during the differentiation of Nanog-hrGFP-transfected mouse embryonic stem cel lines. METHODS:After PCR amplification, Nanog, hrGFP and entry vector were recombined into the pDest-puro vectors to generate the lentiviral expression vector, Puro-Nanog-hrGFP, by the LR reaction. Through lentivirus production, transduction and puromycin screening, the transduced cel lines with Nanog-hrGFP gene were generated and identified. Expression of Nanog during the differentiation of transgenic mouse cel lines was detected. RESULTS AND CONCLUSION:The lentiviral expression vectors Puro-Nanog-hrGFP were constructed successful y by Gateway technology, and then the transducted cel lines were obtained by lentiviral infection. The expression of Nanog was gradual y decreased during the process of transgenic cel lines differentiation, which provides a new tool for further investigation on regulation of stem cel differentiation.
9.Distribution of Common Pathogens in Clinical Blood Culture and Their Antibiotic Resistance Analysis

OBJECTIVE To understand the distribution of common pathogens and their drug resistance trend and to provide the basis of the correct selection of antibiotics for clinic.METHODS Retrospective analysis was taken for blood culture specimens during 2 years in our hospital.And statistical analysis was done.All of 4028 cases of blood culture specimens were detected on automatic BacT/Alert3D rapid blood culture system,strains isolated were taken to VITEK-2 automatic microbiological analysis/sensitivity system for identification and drug susceptibility testing.Drug susceptibility results were analyzed using WHONET 5.3 software.RESULTS Totally 435 pathogen were isolated from 4028 cases of blood culture sample and the positive rate was 10.8%.195 Gram-negative bacteria strains accounted for 44.8%,mainly Escherichia coli,Klebsiella pneumoniae,Burkholderia cepacia,Acinetobacter baumannii,and Pseudomonas aeruginosa.Gram-positive bacteria accounted for 49.9%;mainly Staphylococcus epidermidis,S.haemolyticus and S.aureus dominated.Fungi were 23 strains(5.3%),mainly Candida albicans.Among them,extended-spectrum ?-lactamases(ESBLs) produced by E.coli and K.pneumoniae strains were 43.9% and 48.8%,respectively.Meticillin-resistant S.aureus and coagulase-negative staphylococci were 35.3% and 72.9%,respectively.But vancomycin-resistant S.aureus was not found.CONCLUSIONS The bacteria identification detected in blood culture class is quite complicated,and the drug resistance is high.Laboratories should increase the detection rate of bacterial culture and provide drug monitoring results for the clinics on time based on CLSI norms.Clinicians should use the antibiotics reasonably based on the drug susceptibility results in order to reduce nosocomial infections and the emergency of multiple drug-resistant strains.
10.Anti-BP230-type bullous pemphigoid associated with linagliptin treatment: first case report in China

A 66-year-old female patient presented with blisters on the scalp and neck for 1 month. She had a history of type 2 diabetes for 6 years, and started taking the dipeptidyl peptidase-4 inhibitor linagliptin at a dose of 5 mg once a day 6 months before the onset of eruption. Skin examination showed scattered mung bean- to peanut-sized blisters on the scalp, and some blisters broke with exudation and crusts. There was a pigeon egg-sized bulla and two mung bean-sized blisters on the left neck, with tense blister walls, clear blister fluids, non-erythematous base, and Nikolsky′s sign was negative. Enzyme-linked immunosorbent assay revealed that the serum levels of anti-BP180 NC16A and anti-BP230 antibodies were 5.81 and 139.76 kU/L respectively. Histopathological examination of the blister on the neck showed subepidermal blister formation, and infiltration with neutrophils and a few eosinophils in the blister. The patient was finally diagnosed with localized anti-BP230-type bullous pemphigoid. This case of anti-BP230-type bullous pemphigoid associated with the dipeptidyl peptidase-4 inhibitor linagliptin was firstly reported in China.
