OBJECTIVE:To review the status quo and problems in the study of TCM formula granule.METHODS:Recent literature concerning the preparation technology and the normalization of quality criteria,etc.of TCM formula granule reported were reviewed and analyzed.RESULTS&CONCLUSION:TCM formula granule was of many merits such as safe and sani-tary,accurate in dosage measurement,portable etc.However,there are also some problems yet to be solved.
2.Dose optimization of continuous renal replacement therapy in sepsis-induced acute kidney injury

Sepsis is a life-threatening organ dysfunction caused by dys-regulated host response to infection. Acute kidney injury (AKI) caused by sepsis is one of the most common and severe clinical disease, which incidence and mortality remains high level, and has poor clinical outcomes. Continuous renal replacement therapy (CRRT) is an indispensable tool for the treatment of critically ill patients with severe AKI, but there is no consensus on the appropriate treatment dose of CRRT. By reviewing the process of high volume hemofiltration (HVHF) and standard volume hemofiltration (SVHF), comparing their benefits and risks, and making suggestions for therapeutic schedule improvement, reference for clinical work was provided.
3.Complications of esophageal carcinoma treated with intra-cavitary radiotherapy

Purpose: At present ,the local control of esophageal carcinoma have been improved by using intra-cavitary irradiation, but the complications a much more than before. It is necessary to study the real condition of these complications.Materials and Methods: From August 1993 to May 1994,38 patients with esophageal carcinoma were treated with external beam combined with intra-cavitary radiotherapy in our hospital. Conventional radiotherapy was used 200cGy/F,5F/W.When the dose of external beam reached 3800cGy/19F,intra-cavitary radiotherapy was added with the dose of 500cGy/F/W(patient did not receive the external beam at the same day). The total dose of external and intra-cavitary radiotherapy were 5000cGy/25F/5W and 1500cGy/3F/3W at end of the course respectively.Results: ① 68.4%(26/38) patients had esophagitis.②25 patients suffered from the non-carcinoma esophagostenosis, which was related to the length of tumor before treatment, the shade of soft tissue around lesion, the esophagitis, the diameter of esophagus cavitary and stiff of esophagus wall before intracavitary radiotherapy.③10 of 38 cases had non-carcimoma ulcer. All patients had received 1500cGy/3F/3W intracavitary radiotherapy and occured serious esophagitis, 7 of 10 suffered from the non-carcinoma stricture and ulcer before or later.④ 6 of 38 cases had the fistula and 5 patients had been exposed 1500cGy/3F/3W intracavitary radiotherapy.Conclusion: Esophageal carcinoma treated by afterloading techniques may improve the serious obstruction immediately, but it is not sure the long-term survial yet. We also find the increasing serious complications and the decreasing life quality by using this treatment.We suggests that this method only can be used as an auxilliary way with the external beam radiotherapy.

5.Determination of Gastrodine Content in Tianma Formula Granule by HPLC

Objective To establish a HPLC method for the determination of gastrodine in Tianma Formula Granule.Methods A HPLC was performed on Shim-pack VP-ODS(4.6mm? 150mm,5? m).The mobile phase consisted of acetonitrile and 0.1 % H3PO4(3∶ 97).Detected wavelength was at 220 nm,the flow r ate was 0.5 mL? min-1,column temperature was at 25 ℃,and the injection volume was 5 ? L.Results The linearity was good in the range of 0.1020~0. 6120 ? g for gastrodine(r=0.9999).The average recovery rate was 100.5 %, and RSD was 1.01 %(n=5).Conclusion The established HPLC method is simple,accurate,and suitable for the determination of gastrodine in Tianma Formula Granule.
6.The application value of contrast-enhanced ultrasound in the BI-RADS-US 4 breast lesions

Objective To assess the value of contrast-enhanced ultrasound (CEUS) in differential diagnosis of BI-RADS-US 4 breast lesions.Methods A total of 123 lesions underwent CEUS.The CEUS features of lesions were categorized into 5 malignant or benign indexes respectively,lesions displaying any two of the five features were diagnosed as malignant or benign.The diagnostic effect of CEUS for BI-RADS-US 4 lesion was analyzed according to the pathological results as the gold standard.Results CEUS of the 123 BI-RADS-US 4 breast lesions indicated that 75 lesions were malignant while 48 were benign.Pathological results confirmed that there were 72 malignant lesions and 51 benign.The proportions of malignant lesions in 4A category,4B category and 4C category were 16.2 %,58.5 % and 93.3 % respectively.The accuracy,sensitivity,specificity,positive predictive value,and negative and positive predictive value of CEUS for the diagnosis of BI-RADS-US 4 lesions were calculated as 92.7%,95.8%,88.2%,92.0% and 93.7 % respectively.Lesions showing false positive in CEUS mainly needed surgical treatment,such as fibroma with active growth mesen-chyme,intraductal papilloma and granulomatous mastitts.Conclusions Surgical treatment rather than aspiration biopsy are suggested for those diagnosed being malignant in CEUS of the BI-RADS-US-4 lesions,as they can be treated as BI-RADS5 lesions.Short-term visit or aspiration biopsy are suggested for BI-RADS-US-4 lesions diagnosed being benign in CEUS.
7.Analgesic effect of oxysophoridine and its mechanism

Objective To investigate the analgesic effect of oxysophoridine(OSR)and the influence of verapamil(Ver)on the antinociception of OSR when two drugs were co-administrated in mice.Methods The number of writhing within 15 min after ip different doses of OSR was observed in painful mouse mo- dels caused by acetic acid.The hot plate method was used to assess nociceptive sensitivity of CaCl_2 and Ver before ip OSR.Nitric oxide(NO)in serum was measured by spectrophotometry.Results The number of writhing was decreased and the latency of licking the hind paws was prolonged in a dose-dependent manner after ip OSR.The antinociception of OSR could be antagonized by CaCl_2 and enhanced by Ver.No inter- ference was detected in serum volume of NO.Conclusion These results suggest that OSR can antagonize the acute pain caused by acetic acid and hot plate in a dose-dependent manner in mice.Calcium channel blocker could enhance the effect of OSR.
8.Comparative Study on Water Soluble Extract and Total Flavonoid Content between Jinzhen Suanzao Teabag and Decoction

OBJECTIVE:To compare the quality control index between Jinzhen Suanzao teabag and decoction,and explore the alternative supplement of teabag to decoction. METHODS:The test sample solutions of Jinzhen Suanzao teabag and decoction were prepared by hot-maceration method and water-decoction method. The contents of water soluble extract and total flavonoids were de-termined and compared between 2 kinds of preparation. The leaching rates of teabag were investigated at different soak time(0,5, 10,15,20,25,30 min) to optimize soaking time. RESULTS:The average content of water soluble extract were 50.56% and 44.45%(P<0.05) respectively for the teabag and decoction. The total flavonoids content were 0.64 mg/g and 0.69 mg/g (P<0.05). The dissolution amount of teabag were increasing and leaching rate increased within first 20 min,and reached balance gradu-ally 25 min later. CONCLUSIONS:According to the convenience of use and results of each index,the difference in quality control index is not great between 2 kinds of preparation. Teabag can be as the supplement of decoction.
9.Hypoxia effects on the proliferation and differention of hydrogen peroxide-pretreated MC3T3-E1

BACKGROUND:The intracel ular accumulation of reactive oxygen species leads to oxidative stress. Hypoxia is widespread in physiological and pathological condition. Variation of bone proliferation and differentiation when bone tissues cultured or bone cel s induced toxicity by reactive oxygen species under hypoxia have not yet been reported. OBJECTIVE:To observe the biological characteristics of MC3T3-E1 pretreated with different concentrations of hydrogen peroxide (H2O2) in hypoxia, thus understanding the cel mechanism underlying prolonged bone healing in the elderly with osteoporosis and diabetes. METHODS:The MC3T3-E1 cel s pretreated with different concentrations of H2O2 were cultured in different oxygen concentrations. The proliferation of MC3T3-E1 was detected by cel counting kit-8. The cel differentiation was detected through alkaline phosphatase staining and alizarin red staining. Total RNAs were extracted and used for analyzing the mRNA levels of col age type 1, alkaline phosphatase and Cbfa1. RESULTS AND CONCLUSION:When MC3T3-E1 pretreated with 200μmol/L H2O2 for 6 hours, the cel proliferation was increased with time, but lower than that in the control group. The alkaline phosphatase activity was weakened, and the number of mineralized nodes was decreased at the early stage of differentiation. When MC3T3-E1 pretreated with 400μmol/L H2O2 for 6 hours, the cel proliferation was decreased obviously. The alkaline phosphatase activity was stil weakened, and the number of mineralized nodes was decreased further, but not affected by hypoxia. When MC3T3-E1 pretreated with 400μmol/L H2O2 for 6 hours and then cultured in hypoxia, the mRNA expression of Cbfa1 was decreased, but the mRNA expressions of col age type 1 and alkaline phosphatase were significantly increased. These results suggest that MC3T3-E1 pretreated with low concentration of H2O2 show a significant decrease in proliferation, while MC3T3-E1 pretreated with a high concentration of H2O2 and cultured in hypoxia show a decrease in osteogenic differentiation, especial y at the early stage of alkaline phosphatase formation.
10.Sorafenib in combination with interferon-α-2b for the treatment of metastatic renal cell carcinoma: pre-liminary results

Objective To explore the efficacy and tolerability of sorafenib in combination with interferon-α-2b for Chinese patients with metastatic renal cell carcinoma. Methods Seventeen pa-tients with unreseetable metastatic renal cell carcinoma were enrolled and received sorafenib in combi-nation with interferon-α-2b. Sorafenib was continuously given at the dose of 400 mg twice per day, in-terferon-α-2b 300 MIU subscutaneously once per day for 5 d each week, until disease progression or intolerable toxieities occurred. Tumor responses were evaluated every two months by response evalua-tion criteria in solid tumor. Results The median treatment duration was 120(51-442)d. All 17 pa-tients were evaluable for efficacy and safety. Five patients achieved partial responses (PR), 1 uncon-firmed PR,9 stable diseases. Two patients had progressed diseases. The overall response rate was 29%(5/17)with the disease control rate of 88%(15/17). Due to short-time follow up, the median progression free and overall survival time were not yet available. The common side effects included: fever 82%(14/17), diarrhea 82%(14/17), hand-foot syndrome 71%(12/17), asthenia 65%(11/17), rash 53%(9/17), alopecia 41% (7/17), mucosities 41% (7/17), hypertension 29% (5/17), anorexia 24% (4/17), hoarseness 24% (4/17), myalgia 24 % (4/17), nausea/vomiting 12 % (2/17) and headach/ dizzle 12%(2/17). Eleven (65%) out of 17 patients developed neutropenia, 5(29%) thrombocytope-nia and 5 (29%)anemia. Four(24%) out of 17 patients had abnormally elevated ALT/AST. Conclu-sion Sorafenib in combination with interferon-α-2b for metastatic renal cell carcinoma improves re-sponse rate with manageable toxicity.
