4.Clinical Observation on Benign Prostatic Hypertrophy.

A clinical study was made on 112 cases of benign prostatic hypertrophy, admitted to the Department of Urology, East City Hospital, Seoul, Korea during the period from January 1979 to December 1982.The results were summarized as follows.1. Benign prostatic hypertrophy were 112 cases of all 761 cases hospitalized, admission rate was 14.7%. 2. Majority of cases was found in 6th and 7th decades with mean age of 72,4 years old. 3. In composition of sexual life during 2th to 5th decades, long-term obstinence or irregular sexual life were 57 cases, long-term bachelohood were 38 cases, normal sexual life were 17 cases. 4. In most cases, the duration of the obstinence or the irregular sexual life was 11 years to 25 years. 5. The presenting symptoms were frequency in 94 cases, acute urinary retention in 59 cases and dysuria in 37 cases etc. 6. The blood chemistry showed elevated BUN in 83 cases and elevated creatinine in 61 cases. 7. The endoscopic examination was performed in 98 cases which revealed trabeculation in 87 cases, inflammation in 61 cases, kissing of lateral lobes in 46 cases and diverticular formation in 15 cases. 8. Hypertension in 12 cases and pulmonary tuberculosis in 12 cases were frequently associated with benign prostatic hypertrophy. 9. Methods of operative treatment were suprapubic prostatectomy in 24 cases, transurethral resection 21 cases and suprapubic cystostomy in 33 cases. 10.Postoperative complications were temporary incontinence in 33 cases, delayed bleeding in 8 cases, acute epididymitis in 3 cases and wound infection in 2 cases.

6.Experimental Studies on the Release of 17-Hydroxycorticosteroids from Soaked Soft Contact Lenses.

The therapeutic application of the soft contact lenses are popular in ophthalmic use recently. Drug effects of greater magnitude and duration may be achieved by using a soft contact lens soaked with drugs. Using mycon-soft lenses, with different diameter (14.5mm, 12.0mm) and thickness (0.2mm, 0.1mm), soaked in 2.5% hydrocortisone acetate or 2.5% hydrocortisone succinate for 15 minutes and 30 minutes according to the protocol, and elution studies were performed. Elution rate were not influenced by the thickness of contact lens and soaked time in drugs, but it was influenced by the size of contact lens. It showed that increased elution rate in larger contact lens than smaller one. It was suggested that neither hydrocortisone acetate nor hydrocortisone succinate were absorbed by the soft contact lens but were adhered to the lens surface and released from it gradually.

7.The Use of Eradacil(Rosoxacin) in Acute Gonococcal Urethritis.

From July 1982,30 male outpatients with acute gonococcal urethritis, bacteriologically confirmed to be positive for gonococci, were entered in this study. New bactericidal agent Eradacil was administered orally in a signal dose of 300mg in 30 case of acute gonococcal urethritis and following results were obtained. 1. All cases of acute gonococcal urethritis were cured. 2. In 4 cases, there was recurrence of urethral signs and symptoms 3 to 10 days after initial clearing following Eradacil therapy. These 4 case were retreated successfully with Doxycycline. 3. No serious side effects were observed except slight dizziness in one case.

BACKGROUND: Colloid solutions are used to treat hypovolemia and expanding plasma, but they may inhibit platelet function and reduce the level of coagulation factors during surgery. This study was conducted to compare the effects of hydroxyethyl starch (HES) on adenosine diphosphate (ADP)- and collagen-induced platelet aggregation in patients undergoing total intravenous anesthesia. METHODS: Patients undergoing endoscopic sinus surgery under total intravenous anesthesia with propofol and remifentanil were divided into a group that underwent fluid management with only crystalloid solution (n = 15) and a group that was managed with crystalloid solution that included 6% HES (130/0.4) (n = 15). ADP- and collagen-induced platelet aggregation were measured 5 minutes before induction, after the first intraoperative hour, and one hour postoperatively. RESULTS: Significantly diminished ADP- and collagen-induced aggregation values were observed intraoperatively when compared with the preoperative value in the patients that were managed with colloid solution that included HES. In addition, significantly diminished collagen-induced aggregation values were observed intraoperatively when compared with the preoperative value in the group that was managed with the solution that only contained the crystalloid. However, ADP- and collagen-induced platelet aggregation were recovered postoperatively in both groups. CONCLUSIONS: The results of this study indicated that fluid therapy with colloid solution that contained 6% HES (130/0.4) may diminish ADP-induced platelet aggregation intraoperatively in patients subjected to total intravenous anesthesia.

9.Relationship between neuronal nitric oxide synthase and NADPH-diaphorase in the dorsal root ganglia during neuropathic pain.

BACKGROUND: Changes in nitric oxide (NO) production in the dorsal root ganglia (DRG) may contribute to allodynia after nerve injury. It is known that the histochemistry of NADPH-diaphorase (NADPH-d) is known to be not always coincident with NOS. This study was conducted to investigate the relationship between nNOS and NADPH-d expression in the DRG in a spinal nerve injury model of neuropathic pain, and to elucidate role that NO plays in neuropathic pain. METHODS: nNOS immunohistochemistry and/or NADHP-d histochemistry were conducted in the DRG of a spinal nerve transection model of neuropathic pain, and the pain behavior was then measured by a von Frey filament test of the hindpaws of wild type and nNOS knock-out mice. RESULTS: nNOS immunoreactive neurons and NADPH-d stained neurons were not always identical. Additionally NADPH-d increased, but nNOS did not increase significantly in the DRG after spinal nerve transection. Neuropathic pain behavior increased in the hindpaw of nNOS(-/-) mice after spinal nerve transection, but was lower than that of wild type mice after spinal nerve transection. CONCLUSIONS: nNOS immunoreactive neurons and NADPH-d stained neurons were not always identical in the DRG, and a novel NADPH-d positive source may be involved in neuropathic pain after spinal nerve transection. Changes in nNOS expression in the DRG were not the primary cause of neuropathic pain behavior in a spinal nerve transection model of neuropathic pain.

10.Serum Epeidermal Growth Factor Receptor (EGFR) in Cervical Cancer.

Epidermal growth factor receptor (EGFR) is overexpressed in various malignancies including carcinoma of the breast, lung, esophagus, cervix, and stomach. In patients with cervical carcinoma, its overexpression may be associated with advanced stage and poor prognosis. So, we evaluated the levels of serum EGFR in patients with cervical carcinoma. The level of EGFR extracellular domain was determined in serum from 57 cervical carcinoma patients(adenocarcinoma: 2, squamous cell carcinoma: 39, carcinoma in situ(CIS): 16) and 28 cases of healthy control using enzyme-linked immunosorbent assay(Calbiochem). In invasive carcinoma, serum EGFR level was measured in 11 cases of Stage Ia, 9 cases of Stage Ib, 4 cases of Stage IIa, 15 cases of Stage IIb, 2 cases of stage III patients. The mean ages of the healthy controls, of the wome with carcinoma in situ(CIS), and with invasive cervical carcinoma were not different(49.3, 44,4, 49.5, respectively, p 0.241). The mean serum level of EGFR in healthy control(n 28), carcinoma in situ(CIS)(n 16), and invasive carcinoma patients(n=41) were not significantly different(71.4+/-12.8fmol/ml, 79.2+/-26.8fmol/ml, 61.8+/- 18.4 fmol/ml, respectively, p=0.071). In conclusion, the expression of EGFR was not increased in patients with cervical cancer compared with normal women. And no significant differences were found depending on the clinical stage.
