3.Malignant Fibrous Histiocytoma: A Case report

A case of malignant fibrous histiocytoma in the right thigh is reported together with a brief review of literature in this paper. This tumor characteristically consists of a solitary circumscribed and firm mass 5 × 5 × 4cm in dimension, which has originated in the deep soft tissues. Wide local excision has been the most common form of treatment for this lesion, after open biopsy has been performed for the diagnosis. Microscopically, there is a pattern of histiocytes and bizarre histiocytic giant cells intermingled in a fibrous stroma with fibroblasts arranged in storiform pattern. Foam cells and abnormal mitoses are also frequently found in this tumor. Local recurrence is common. Metastasis is less common but with metastasis the prognosis is poor. In this case local recurrence was noted two months after excisional biopsy but metastasis was not occurred.

4.A Clinical Result of the Fracture of the Forearm Bone Shaft in Adult

Thirty-seven patient who had forearm bone fracture were treated by four different methods at the Department of Orthopedic Surgery, Soonchunhyang University Hospital from Jan. 1979 to March 1982. The obtained results are as follow; 1. Nineteen cases out of 37 cases of the forearm bone fracture were both forearm bones, 10 cases were ulna and 8 were radius fracture. 2. Twenty-three cases out of 37 were closed and 13 cases were open fracture, respectably. 3. The applied treatment were manual reduction with cast immobilization, intramedullary naliing, plate and screw fixation and combined. 4. Mean duration of primary bone union of the forerm bone fracture which were treated by plate and screw was 12. 4 weeks, combined fixation was 12.8 weeks, intramedullary nailing was 15.3 weeks and that of manual reduction and cast was 16 weeks in order. 5. Functional result was classified according to the rating system of Smith and Sage. The best method was plate and screw and the worst was manual reduction and cast immobilization. 6. D.C.P. fixation seems to be one of the best method in this series. Combined method, in a way of D.C.P. for radius and I–M nailing for ulna, is a method when shortening of operation time is inevitable.

6.The Association of the Serum Magnesium with Hearing Loss Among Noise Exposed Male Workers.

OBJECTIVES: This study was conducted to investigate that the chronic noise exposure is associated with decreased serum magnesium concentrations and evaluate whether decreased serum magnesium is associated with noise induced hearing loss. METHODS: One hundred seventy-eight male workers exposed to noise were selected and classified three groups by the degree of hearing loss. Hearing threshold levels were less than 30 dB at 1,000 Hz or less than 40 dB at 4,000 Hz in group I, more than 30 dB at 1,000 Hz or more than 40 dB at 4,000 Hz and 15 dB and less of pure tone average(PTA: (500 Hz+1,000 Hz+2,000 Hz)/3) in group II, more than 30 dB at 1,000 Hz or more than 40 dB at 4, 000 Hz and over 15 dB of PTA in group III. RESULTS: Serum magnesium concentrations were 2. 42+/-0. 26 nc/dt in group I, 2. 35+/-0.23 mg(dl in group II, 2.26+/-0.24 ne/dl in group III, respectively and significantly different between group I and group III (p<0. 01). It was negatively correlated with duration of the noise exposure as correlation coefficient(r) of -0.194 (p<0.05). Analysis of the multiple regression on hearing threshold levels showed that serum magnesium, diastolic blood pressure, duration of the noise exposure were statistically significant at 4,000 Hz(p<0.05). While only age was statistically significant at 1,000 Hz(p<0.05). CONCLUSIONS: These results suggest that chronic noise exposure may induces decrease in serum magnesium concentrations and that its decreased concentration is related with noise induced hearing loss.

7.A Case of Fetal Nuchal Cystic Hygroma.

Fetal cystic hygroma is a rare congenital malformation of the lymphatic system appearing as a single or multiloculated fluid-filled cavity, most often in the neck. A case of fetal nuchal cystic hygroma was diagnosed by Ultrasonography at 22 weeks of gestation and the diagnosis was confirmed at autopsy. We present the case with a brief review of literature.

8.Endoscopic Findings of Tuberculous Colitis.

The purpose of this paper is to review the colonofiberscopic findings in the 21 patients with tuberculous colitis, diagnosed by colonofiberscopic findings including histopathology and by follow-up observation after anti-tuberculosis chemotherapy. The results are as follows: 1) The male to female ratio was l. 33(R:4) and the average age of the patiets were 34. 9 years in men and women respectively. 2) The chief complaints were abdominal pain in 15 cases(71.4%), diarrhea in 14(66.7%), weight loss in 12(57. 1%), anorexia in 8(38, 1%), and so on. 3) Anemia and an increase in ESR were major abnormalities in hemogram. 4) On the chest X-ray examination, 65% of the cases showed active tuberculous lesions and the barium enema disclosed the changes suggesting tuberculous colitis in 52. 6% of the cases. 5) On colonofiberscopy 8 cases were ulcerative, 6 were hypertropbic and 7 were ulcerohypertrophic. 6) Histopathologic examination of the biopsy material from the colonofiberscopy revealed granulomata with caseation necrosis in 2 cases, granulomata without caseation necrosis in 8 cases, and the findings of chronic inflammation in 11 cases. 7) This study suggests importance of colonoscopic examination including histopathologic study of the biopsy material in the diagnosis of tuberculous colitis. The diagnostic efficieocy would be much improved if bacteriological examinations of the specimen could be developed.

9.Mesenteric Cystic Lymphangioma with Sustained Abdominal Pain : Report of a case.

A case of cystic lymphangioma of the mesentery with severe and persistent abdominal pain in a 22-year-old man is presented. The cyst was filled with chylous fluid. Microscopically, numerous nerve bundles were incorporated within the lymphangiomatous walls, and some protruded into the lumen. The above findings lead to a suggestion that mesenteric lymphangioma may differ from those in the other sites by its abundance of incorporation of nerve bundles into the lymphangiomatous walls, and that increase of tumor size by intracystic accumulation of chylous fluid may subsequently result in increase of intraluminal pressure to compress the nerve bundles with which abdominal pain is much enhanced.
