ObjectiveTo investigate the effect of nickel-titanium(Ni-Ti) shape memory alloy embracing internal fixer on oblique or spiral fracture.Methods23 cases with spiral or oblique fracture accepted internal fixation with Ni-Ti shape memory alloy embracing.The time of bone healing,articular function,the appearance and function of the wound extremity were evaluated after 9～34 months follow-up.ResultsAll fractures healed in 4～16 weeks after operation.None infection,nonunion,embracing emersion or break had been found. The fine rate was 91.3%.ConclusionThe effect of Ni-Ti shape memory alloy embracing internal fixer on long spiral or long oblique fracture is satisfactory.

[Objective]To analyze the factors of post-operative deformity after posterior approach vertebral pedicle fixation in treatment of dorso-lumbar spinal fractures.[Method]Between 2000.10～2005.11,32 patients of single dorso-lumbar spinal fracture were hospitalized.compressive type 5 cases,explosive type 27 cases.T11 12 cases,T12 11 cases,L1 5 cases,L2 3 cases,L3 1 case.The patients were devided into 3 groups according to the severity of related chondral plates injuly,group 1: without chondral fracture 11cases,group 2: with upper chondral plate fracture 15 cases,group 3: with both upper and lower chondral plate fractures 6 cases.By the formula to measure the average height of fractured vertebral body and its kyphotic Cobb's angle before operation,after operation and at time of follow-up,and calculate the result stastically.[Result]All 32 patients were followed-up for 10～56 months(average 23 months).The results indicated that the patient's preoperative condition was negatively correlated with the average vertebral height which is followed-up.The worse condition was,and the greater loss of postoperative vertebral height was.Furthermore,the patient's situation before operation had a positive correlation with the Cobb's angle of vertebral followed-up.When the patient's condition was worse,the postoperative angle of deformity was larger.[Conclusion]Posterior approach vertebral pedicle fixation in treatment of dorso-lumbar spinal fractures may result in reccurence of vertebral deformity,which is related to the severity of injury of vertebra and its chondral plates,so that to those patients related with chondral plates injuries,the vertebral pedicle fixation should be combined with anterior bone grafting,or with bone grafting into vertebral body after reduction.

Objective To explore the relationship between Sirt1 single nucleotide polymorphisms ( rs2236319 ) and chronic obstructive pulmonary disease ( COPD ) susceptibility. Methods The subjects included 149 patients with stable COPD in the First People′s Hospital of Jining outpatient or ward from March 2014 to March 2015, and 160 cases of the same period health check. Sirt1?rs 2236319 genotype distribution frequency was detected by real?time PCR,and the levels of inflammatory factors were compared,including IL?6 and CRP. Results The AA,AG,GG genotype frequency of COPD group were 59. 1%( 88/149) ,35. 6%( 53/149),5. 4%(8/149) respectively,of control group were 45. 0%(72/160),48. 8%(78/160),6. 3%(10/160) respectively,the allele A,G frequency of COPD group were 76. 8%(229/298),23. 2%(69/298) respectively,of control group were 69. 4%( 222/320 ) , 30. 6%( 98/320 ) respectively, there were statistically significant differences between the two groups(χ2=6. 23,6. 06,P<0. 05) . IL?6 and CRP levels were significantly increased in COPD group carrying AA genotype as compared to COPD group carrying AG+GG genotype((10. 30±0. 40) mmol/L,(10. 16±0. 36) mmol/L;(2. 56±0. 20) mg/L,(2. 46±0. 22) mg/L;F=4. 52,8. 04,P<0. 05);There were no significant differences in terms of IL?6 and CRP levels in control group carrying AA genotype as compared to control group carrying AG+GG genotype ( P>0. 05 ) . Both the level of IL?6 and CRP were significantly increased in COPD group carrying AA genotype and AG+GG carriers compared to controls((10. 30 ± 0. 40 ) , ( 9. 88 ± 0. 56 ) mmol/L, ( 10. 16 ± 0. 36 ) , ( 9. 86 ± 0. 57 ) mmol/L;( 2. 56 ± 0. 20 ) , ( 2. 28±0. 25) mg/L,(2. 46±0. 22),(2. 26±0. 26) mg/L;F=34. 52,11. 09,73. 06,19. 38;P<0. 01). Conclusion The polymorphisms of Sirt1?rs2236319 may be an additional risk factor for COPD and associate with inflammatory factors. Furthermore the involvement of rs2236319 in the initiation and progression of COPD may relate to inflammatory processes.

Objective To observe the protective effect of spironolactone on heart of hypertensive SD rats.Methods 30 male SD rats were randomly divided into the control group,hypertensive group(HS group),low spironolactone dose group(HSL group)and high spironolactone dose group(HSH group).The animals except the control group were treated with L-NAME 50 mg/kg/d.Then the animals of the HS group were fed with 1% sodium chloride water solution.Those of the HSL and HSH groups were treated same but the former added with spironolactone 20 mg/kg/d,the later added with spironolactone 100 mg/kg/d.8 weeks late,the animals were killed and hearts were taken out to observe following items:myocardial cross-sectional area,ratio of per-arteriolar fibrous area/arteriolar luminal area of arterioles,ratio of collagen area/total myocardial area,myocardial necrosis and inflammation.Results The myocardial cross-sectional areas in the HS group were significantly more than those in other 3 groups(P<0.01).Collagen ratios of the HS group were significantly higher than others(P<0.01).The ratios of arteriolar wall area versus luminal area in the HS group were significantly higher than those of control and HSH groups(P<0.01).Myocardial injuries in the HS group were more severe than those of other 3 groups.There were no differences between HSL and HSH groups.Conclusion Spironolactone has protective effect on heart of hypertensive SD rats.

To compare the differences of the active ingredient contents and the sulfur dioxide residue in Astragali Radix before and after sulfur fumigation and provide a basis for establishing an alternative processing method. Astragali Radix, harvested at the same time in Longxi Gansu, were processed with different methods. high performance liquid chromatography (HPLC) was used to determine the contents of the active ingredients in Astragali Radix and the revised method of the pharmacopoeia of China in 2011 was applied to determine the sulfur dioxide residue. The results show that the three-fold sulfur-fumigation group has the highest level of astragaloside IV and the dried sulfur-fumigation group with 10% water has the lowest level; the content of calycosin-7-O-β-D-glucoside is the highest in naturally dried group and the lowest in the group of sulfur fumigating for 3 times; the sulfur dioxide residue of all sulfur-fumigation groups exceeds certain limit significantly and the group of sulfur fumigating for 3 times reaches the highest level.

Objective To clone and express the outer surface protein C ( OspC) from a Chinese Borrelia afzelli FP1 strain and to evaluate the immune protectivity of the recombinant OspC protein ( rOspC) . Methods The gene encoding OspC protein of Borrelia afzelli FP1 strain was amplified by polymerase chain reaction (PCR) and then inserted into pET-30a plasmid to construct the recombinant expression plasmid pET-30a-OspC. The transformed E. coli BL21 strains carrying pET-30a-OspC plasmid were induced by IPTG to express OspC protein. The expressed proteins were purified by Ni-IDA resin chromatography and analyzed by SDS-PAGE and Western blot assay. Indirect immunofluorescence assay ( IFA) was performed to detect anti-rOspC protein antibodies in serum samples from rabbits immunized with rOspC protein. In vitro neutral-ization test was performed for evaluation the immune protectivity of rOspC protein. Results The recombi-nant expression plasmid pET-30a-OspC was successfully constructed and highly expressed in E. coli BL21. A strong antigen-antibody reaction between the rOspC protein and polyclonal antibody against Borrelia afzelli FP1 strain was detected by Western blot assay. The titers of IgG in serum samples from rabbits immunized with rOspC protein were significantly elevated. The in vitro neutralization test indicated that 106/ml of Borre-lia afzelli FP1 strains were neutralized by every anti-OspC protein serum sample from the experiment group. Conclusion The rOspC protein showed a strong immune protectivity against Borrelia afzelli, which could be used in the development of polyvalent subunit vaccine against lyme disease.

Objective To evaluate the value of material decomposition imaging of spectrum CT in overcoming renal cyst pseudoenhaneement.Methods Totally 80 patients with renal cysts (total 75 cysts) who underwent CT imaging with GSI mode were collected.The renal cysts were divided into 3 groups according to diameters,group A (diameters 0.5-＜1.5 cm,n=25),B (1.5-＜2.5 cm,n=25) and C (2.5-＜3.5 cm,n=25) respectively.The iodine-water density imaging was reconstructed by using the GSI Viewer analysis software.The CT value and iodine-water concentration of the cysts were recorded.The difference of CT value,iodine-water concentration in unenhanced and enhanced dual phases in each group was compared.Results The difference of CT value between plain scan and parenchyma phase among the 3 groups had statistically significant difference (F=204.128,P＜0.001),and the differences comparing any two were statistically significant (all P＜0.05).The postcontrast attenuation increased more than 10 HU in group A and B,indicating renal cyst pseudoenhancement,and less than 10 HU in group C,which had no pseudoenhancement.There were statistical difference in iodine concentration of the cysts of the 3 groups in unenhance,cortical and parenchyma phases (all P＜0.001),but the difference value in unenhance,cortical and parenchyma phases were less than 10 (100 μg/cm3),and the difference value of the 3 group was group A＞group B＞group C (all P＜0.05).The water concentration of the cysts in group A descend in renal cortical and parenchyma phase with statistical difference (P＜0.001),but the difference value was less than 10 mg/cms.Conclusion The measurements of iodine-water concentration appear to drift as well,the smaller the greater,The degree of the iodine concentration shifting is more obvious than water concentration.

Objective To establish a method for detection of serum urocanic acid (UCA) by high performance liquid chromatography (HPLC),and explore the clinical significance of serum UCA concentration for children acute leukemia.Methods The chromatographic conditions of HPLC were set up and optimized,and the linearity of standard curve,precision,accuracy and stability were validated.Then the serum from ninety acute leukemia children and ninety non-tumor blood disease children was collected,the concentration of serum UCA was detected with HPLC,and the differences of two groups were compared to study the clinical significance of UCA in children acute leukemia.Results The HPLC method for detecting serum UCA was successfully established and optimized.The standard curves of trans-UCA and cis-UCA both showed good linearities(R2=0.999 6 and 0.999 9) at the condition of the mobile phase of acetonitrile-20 mmol/L KH2PO4,pH 3.7(5:95,V/V),flow rate of 1.2 mL/min,detection wavelength of 264 nm in HPLC.The relative standard deviation RSD％ of intra-assay and inter-assay were lower than 5％.Compared with non-tumor blood disease,the serum concentration of cis urocanic acid (cis-UCA) and trans urocanic acid (trans-UCA) of children with acute leukemia were significantly increased (P＜0.001).Compared with cis-UCA,trans-UCA was more valuable for risk classification of acute lymphoblastic leukemia (ALL).Conclusions HPLC is a good technology to titrate of UCA in serum.The concentration of serum UCA in children with acute leukemia may provide the clues for diagnosis and prognosis,with important clinical significance.

Objective To evaluate the application of inguinal incision in retroperitoneal laparoscopic nephroureterectomy(LNU)in the treatment of upper urinary urothelial carcinoma(UUUC).Methods From Mar.2007 to Jan.2012,186 retroperitoneal LNU procedures on 115 males and 71 females for the treatment of UUUC were performed in our institute.All cases were grouped as inguinal incision group(n =112)and lumbar incision group(n =74)according to specimen retrieval incision.Operative time,estimated blood loss,postoperative analgesia,hospital stay,incision complications,cosmetic satisfaction and tumor recurrence were compared between the 2 groups.Results All the 186 cases of operation were successfully accomplished.There were no differences in tumor stage,tumor grade,mean operative time,blood loss between the 2 groups.In inguinal incision group,the incidence of incision fat liquefaction,incision hernia,incision bulging,lumboabdominal unsymmetry and postoperative analgesia was less than that of lumbar incision group.In inguinal incision group,the mean hospital stay was shorter,cosmetic satisfaction(Ⅰ/Ⅱ/Ⅲ)was better(7/24/81 versus 22/18/34,P ＜ 0.01).Recurrence rate of UUUC in middle and inferior segment of ureter was fewer than that of lumbar incision group(5.3％ versus 35.0％,P ＜0.01).Conclusion Retroperitoneal LNU for UUUC combined with inguinal incision offers advantage of less trauma,less complications,higher cosmetic satisfaction and lower tumor recurrence.
