1.A Case of Basosquamous Cell Carcinoma of the Eyelid.

The authors experienced a case of basosquamous cell carcinoma occuring in the eyelid, which was confirmed by the biopsy. Histological findings showed the infiltrating nests of malignant squamous cell with nests of basal cell infiltrating into the dermis. A huge ulcerating, infected lesion with irregular indurating border involved the entire lid and extended onto the orbital margin and nose with the nasolacrimal duct exposed. Because of the unusual case, it was felt that literatures review would be instructive.

4.Evaluation in Systemic Adverse Reactions after Instillation of Phenylephrine HCI.

Phenylepherine HCI widely used as a mydriatic and vasoconstrictor, have been generally considered safe and innocuous. But hypertension caused by instillation of phenylepherine HCI is frequently reported, and even severe systemic adverse reactions, such as myocardial in farction, sudden death etc. are reported, recently. For the purpose of studying alteration of B.P. and systemic adverse reactions that are caused by instillation of phenylephrine HCI. we divided 42 patients into 2 groups; group 1 includes 20 patients who are instilled once a drop of 10% phenylephrine HCI viscous solution into the conjunctival cul-de-sac of bOth eyes, group 2 includes 22 patients who are instilled twice a drop at 2 minute intervals. We observed alteration of B.P. and systemic adverse reactions in both groups, next made a comparative study, and mydriatic effect too. The results was generally there was no severe reaction, but marked increase of B.P. that statistically significant was observed in 3 patients. There was no significant difference between the 2 groups. In mydriatic effect, the significant difference was not observed between the 2 groups or according to the age. When a clinician does its instillation, he should keep possible adverse reactions in mind. and especially in cardiac disease, hypertensjon, aneurysms, advanced arteriosclerosis, infants and the elderly etc., who have much chance of accompanying severe adverse reactions, he should use phenylepherine HCI cautiously. (this time it is desirable that he use low percentage of phenylepherine HCI).

5.The Effects of Follicular Fluid on in Vitro Maturation of Bovine Follicular Oocytes.

The present experiments were conducted to evaluate the effects of follicular fluid on maturation of bovine follicular oocytes. TC medium 199 seemed to be in adequate for this purpose since a high proportion (ranging 84.1. to 97.0%) of the oocytes were able to resume meiotic division in both media-with or without the addition of follicular fluid. This implies a possible similarity between TC medium 199 and follicular fluid with regard to the components initiating meiosis. Actually, TC medium 199 contains amino acids, vitamins and carbohydrates most of which are also found in follicular fluid. For this reason, the effect of follicular fluid on the ovum maturation was investigated by applying Krebs-Ringer bicarbonate solution (SECM), which was main1y composed of the salts, pyruvate and lactate. When the oocytes were cultured in SECM without the addition of follicular fluid, only 7-14% of them resumed meiotic division within 30 hours. However, when follicular fluid was added, the proportion of oocytes undergoing maturation was sharply increased to about 70%. Among the groups cultured in media containing different concentrations of follicular fluid, the proportion of the oocytes in meiosis remained constant, In pure follicular fluid, 87-89% of the oocytes showed enhancement of meiotic division. The presence of the follicular fluid contributed to a decrease in the production of degenerative oocytes. As a consequence it has been noted that addition of follicular fluid to the culture medium provides a more beneficial environment for cow oocytes. Meiotic division is initiated and production of degenerative oocytes is inhibited.

6.The Maturation in Vitro of The Rabbit Oocytes I. Amino Acids Support the Maturation In Vitro of the Rabbit Oocytes.

Rabbit follicular oocytes were cultured in a medium supplemented with various elements such as bovine serum(RS), bovine serum albumin(BSA), amino acids and chorionic gonadotrophic hormone(HCG) in order to find which factors among them were most effective for oocyte maturation. The presence of BSA in the basic medium (modified Krebs-Ringer bicarbonate) did not elevate the proportion of oocyte maturation. When BS alone was added to the medium, only a few oocytes could reach to metaphase I and most of them were in degeneration. This implies that BS may act as an inhibitory or a toxic agent to the rabbit oocytes. It was found that the medium supplemented with 0.4% BSA and amino acids together raised the proportion of the oocyte maturation (54-62%). Especially the presence of proline, or of both proline and glutamine, gave a more favourable condition for the initiation of meiotic division than other amino acids. Addition of HCG to the medium did not promote the proportion of the oocyte maturation. As a consequence, it is apparent that amino acids in the medium are the most essential factors in inducing oocyte meiotic division.

8.Differentiation of amebic versus pyogenic liver abscess with US and CT.

To differentiate amebic and pyogenic liver abscesses, sonography(US) and CT findings of 21 amebic and 22 pyogenic liver abscesses were reviewed retrospectively. US was performed in all cases and CT in 15 cases of each group. Eleven cases of amebic abscesses had well defined margin which was observed in 10 cases of pyogenic abscessed. Amebic abscesses showed hyperechoic wall in 7 cases and calcification in 4 cases. Ten cases of pyogenic abscesses had mearby intrahepatic duct dilatation which was observed in 2 cases of amebic abscesses. Pneumobilia was seen in 6 cases of pyogenic abscesses. US findings of clear margin and hyperechoic wall, and CT findings of intrahepatic duct dilatation, pneumobilia, and calcification were helpful in differentation of amebic pyogenic liver abscesses.

9.Electron Microscopic Studies of Mouse Oocytes and Two-cell Embryos exposed to Progesterone in Vitro.

This experiment was undertaken in order to find out if there is any morphological change in oocytes and two-cell embryos whose development have been suppressed by progesterone for six hours in vitro. It can be observed that some part of the outer side of nuclear membrane of the suppressed oocytes was damaged. The number of nuclear pores has decreased in suppressed oocytes and this suggests that progesterone might suppress the transport of intermediary metabolites between cytoplasm and nucleus. Sometimes, closely packed aggregates of parallel or irregular endoplasmic reticula were observed in suppressed oocytes. Microvilli of suppresed oocytes showed signs of degradation and the perivitelline space became apparent. Thus it is presumed that the egg membrane has constricted during cultivation under progesterone in vitro. The other cell organelles such as mitochondria, multivesicular bodies, cortical granules and fibrillar lattices showed no difference in morphology between treated and control (intact) oocytes. In two-cell embryos, there was also no evident morphological change except for the fact that many vacuoles appeared clearly in suppressed embryonal cells. In brief, there was no fundamental morphological change in the oocytes and the embryonal cells exposed to progesterone for six hours even though it inhibits their development. The action of progesterone should be investigated thoroughly.

10.Development of the implant surgical technique and assessment rating system.

PURPOSE: There has been no attempt to establish an objective implant surgical evaluation protocol to assess residents' surgical competence and improve their surgical outcomes. The present study presents a newly developed assessment and rating system and simulation model that can assist the teaching staffs to evaluate the surgical events and surgical skills of residents objectively. METHODS: Articles published in peer-reviewed English journals were selected using several scientific databases and subsequently reviewed regarding surgical competence and assessment tools. Particularly, medical journals reporting rating and evaluation protocols for various types of medical surgeries were thoroughly analyzed. Based on these studies, an implant surgical technique assessment and rating system (iSTAR) has been developed. Also, a specialized dental typodont was developed for the valid and reliable assessment of surgery. RESULTS: The iSTAR consists of two parts including surgical information and task-specific checklists. Specialized simulation model was subsequently produced and can be used in combination with iSTAR. CONCLUSIONS: The assessment and rating system provided may serve as a reference guide for teaching staffs to evaluate the residents' implant surgical techniques.
