1.Two Cases of Right Homonymous Hemianopsia and Total Blindness due to Cerebral Paragonimiasis.

We present two cases of cerebral paragonimiasis in a 23-year-old Korean army soldier and in a 18-year-old Korean girl, respectively. The diagnosis was made on the basis of clinical, laboratory and x-ray findings, and was confirmed by the histopathological examination of the excised cyst. P.W. skin test showed positive reaction, whereas C.S. test negative. X-ray study on skull revealed multiple globulated round cystic formations in occipital region. Two months after surgical removal of cysts, visual field were not improved. Pertinent literatures are reviewed.

2.A Case of Ocular Cysticercus in Severe Systemic Cysticerosis.

A case of iridocyclitis due to cysticercus cyst in the anterior chamber and exophthalmos due to in the orbit. A case of iridocycleitis due to cysticercus cyst in the anterior chamber of the left eye, bilateral exophthalmos due to in the orbit and generalized multiple cysticercus cyst in the subcutaneous tissue, brain and the internal organs is reported in a twenty one year old Korean male. To watch characteristic heaving and undulatory movements of the live cysticercus cyst in the anterior chamber of the eye is fascinating. Iridocyclitis was improved fairly after subconjunctival injection of hydrocortisone acetate 5mg and with general treatment for iridocyclitis. Regarding to literature, ocular cysticercosis is known as a very rare case in Korea. A brief review of the related literatures is presented.

3.Isolation and Characterization of Tumor Cell - Derived Immunoregulatory Factors.

It has been known that the immunological functions against cancer cells were diminished, and these phenomena result from the inhibition of cell-mediated immunity by substance(s) secreted from cancer cells. It was also reported that the immunological functions decreased in patients with stomach cancer, which is the most frequent cnacer in Korean. However, the nature and function of the inhibitory factor(s) orignated from stomach cancer have not been identified. To elucidate effects of immuological inhibitory factor(s) secreted from cancer cells, SNU-1 (stomach cancer) and SW480 P109/R3P2 (colon cancer) were used in this study. Jurkat T cell line, an acute T cell leukemia, was pre-incubated with fractionated cancer cell culture supernatant for 3 days, then was stimulated with PMA, PWVanti-CD28 mAb or PMA/ionomycin for 8 hrs respectively. Fraction of SNU-1 (3 - 10 kDa) and above 10 kDa of SW480 P109/R3P2 inhibited the expression of IFN-r mRNA when Jurkat T cell was stimulated with PMA. However, there were no difference in IL-2 and IL-4 gene expression response to either PMA/anti-CD28 mAb or PMA/ionomycin. These results show that cancer cells secret some inhibitory factor(s) acting on the immune response, especially IFN-r gene expression of the Jurkat T cells response to PMA. Therefore, it suggests that the inhibitory factor(s) secreted from cancer cells influences on. the PKC-dependent pathway related to the signal transduction by PMA.

4.Therapeutic Efficacy of Phacoemulsification and Intraocular Lens Implantation in Phacomorphic Glaucoma.

PURPOSE: To evaluate the therapeutic efficacy of phacoemulsification in treating phacomorphic glaucoma. METHODS: We observed 25 eyes of 25 patients who had undergone phacoemulsification and intraocular lens implantation for the treatment of phacomorphic glaucoma. Intraocular pressure (IOP), best corrected visual acuity (BCVA), depth of anterior chamber, management and complications between preoperative and postoperative periods were analyzed retrospectively. RESULTS: The mean IOP was 49.0 +/- 10.6 mmHg (range: 31~70 mmHg) before the operation and 13.2 +/- 2.8 mmHg (range: 8~20 mmHg) at 36 months after surgery (Paired T-test, p<0.0001). We could not find any case with increased IOP in our series for more than 3 years. The postoperative IOP decreased significantly the day after surgery (Paired T-test, p<0.0001). Improvement of postoperative visual acuity was remarkable from the first week after operation (Paired T-test, p<0.0001). Postoperatively, the visual acuity was improved except for one eye with preoperative visual acuity of no light perception. Especially, BCVA in 15 cases (60%) was above 20/40. Preoperatively, the anterior chambers were extremely shallow (average: 1.5 +/- 0.3 mm) by the swollen lens, and the average postoperative anterior chamber depth increased significantly by 2.6 +/- 0.1 mm on A-scan. The ratio of lens thickness to axial length factor was 2.5 +/- 0.1, indicating the shallow anterior chamber. Continuous postoperative medical treatment for IOP control was needed in one case, where peripheral anterior synechiae were observed by gonioscopy immediately after surgery. CONCLUSIONS: Phacoemulsification is a very safe and effective procedure for the treatment of phacomorphic glaucoma.

6.Nasal Bone 3 Dimentional Computer Tomography Guided Tailored Osteotomy of the Bony Deviated Nose.

BACKGROUND AND OBJECTIVES: Correction of deviated nose is considered one of the most difficult and serious surgical operations in rhinoplasty. The correction of bony deviation, in particular, requires osteotomy. Although there are individual differences in the shapes or features of bony deviations of each patient, the techniques of osteotomy are limited. Therefore, for proper correction of bony deviation, it is necessary to set up a precise pre-surgical plan. SUBJECTS AND METHOD: We categorized 187 patients with bony deviated nose into 5 types based on the analysis of preoperative nasal bone 3D computer tomography, and studied surgical method for each type. RESULTS: Of 187 patients, 92 (49%) patients are categorized into type I, 62 (36%) patients into type II, 12 (6%) patients into type III, 2 (1%) patients into type IVa, 7 (4%) patients into type IVb and 7 (4%) patients into type V. Only did eight patients (5%) undergo revision surgery due to unsuccessful outcomes. Major complications like infection, abscess, arteriovenous fistula, epiphora, and saddle deformation did not occur at all. CONCLUSION: Preoperative planning under the analysis and categorization using 3D CT is considered helpful for the precise and individualized surgery in corrective rhinoplasty.

7.Neurophysiological Changes Associated With Listening Effort in Normal Adults

Background and Objectives:
Several studies have proven that listening efforts cause neuroanatomical and physiological changes in the frontal lobe in adults with hearing loss as well as in the elderly with normal hearing. However, there is still a lack in research into how brain function changes when the cognitive load is increased based on noise level. We conducted a preliminary study using functional near-infrared spectroscopy to determine differences in listening effort according to varying listening conditions.Subjects and Method Five adults with normal hearing participated in the study. Listening effort was measured in three listening environments (resting, quiet, and noise) using NIRSIT Lite (OBELAB, Seoul, Korea). The measures variables were concentration changes in oxygenated hemoglobin (ΔHbO2) and the percent correct response rate.
Results:
As a result of ΔHbO2, channels 1 (p=0.045), 5 (p=0.042) and 7 (p=0.033) (right channels) showed significantly higher ΔHbO2 in the noise condition than in the resting condition. In addition, in channel 1, ΔHbO2 was significantly higher even in the quiet condition than in the resting condition (p=0.041). The percent correct response rate was significantly decreased in the noise condition (35.6%) as compared to the quiet condition (64.4%) (p=0.024).
Conclusion
We observed that a decrease in cerebral localization to compensate through the right hemisphere was presented with increased listening effort. The results of the study will be useful in situations where listening effort need to be accurarely measured.

Thoracopagus is very rare congenital anomaly with an incidence estimated at 1 in 50,000 deliveries or 1 in 600 twin deliveries. If twining is initiated after the embryonic disc and the rudimentary amnionic sac have been formed, and if division of the embryonic disc is incomplete, conjoined twins result. We experienced a case of thoracopagus in 28 years old woman. We present this case with a brief review of literatures.

10.Dose Related Effect of Extracorporeal Shock Wave Therapy in Lateral Epicondylitis.

The aim of this study was to investigate the dose-related effects of extracorporeal shock wave therapy (ESWT) on the lateral epicondylitis of the elbow. Between March 2005 and March 2008, 66 patients who had been treated with extracorporeal shock wave therapy due to lateral epicondylitis of elbow formed the subjects. The subjects were divided into the 1st, 2nd and the 3rd treatment group and evaluated the clinical outcomes by visual analog scale (VAS) and a simple elbow test (SET) at immediate treatment, posttreatment 6 and 12 months, retrospectively. Changes in VAS score between the 2nd and 3rd treatment group and between the 1st and 3rd treatment group showed significant difference only at posttreatment 1 month group (p=0.001, 0.2, 0.1), (p=0.03, 0.08, 0.3), but Visual Analog Scale score at posttreatment 1 month showed no difference within the groups (p=1.0, 0.2, 0.07). SET within and between the groups showed significant difference at posttreatment 6 and 12 months (p<0.05).
