2.Multicentric Extrahepatic Tumor Involving the Gallbladder and Common Bile Duct A Case Report of Longterm Survival.

A 60-year-old woman has experienced of right upper quadrant abdominal discomfort. This partient took cholecytectomy due to gall stone 15 years ago. At that time microscopic finding the neck of gallbladder showed well differentiated papillary adenocarcinoma confined to superficial part of musele layer. 3 years before admission she took T-tube drainage due to obstructive jaundice. The microscopic finding af distal common bile duct showed well differentiated adenocarcinoma. Recently she complained of pain and discomfort on T-tube site and so we perforrned endoscopic retrograde biliary drainage after removal of T-tube. The computed tomogram showed no mass in liver or lymph node enlargement. 4 months after procedure of ERBD; we changed to the new catheter for the prevention of cloggiag of prosthesis. We report here a case of longterm survival over 15 years after the diagnosis of cancer of extrahepatic bile duct.

3.Psychiatric Consultation for 5 Years in a University Hospital.

OBJECTIVE: This study aimed to investigate the pattern of psychiatric consultation-liaison in a new general hospital over a 5-year period. METHOD: Retrospective chart review was conducted for psychiatric consultations performed from May 2004 to December 2008 in a new general hospital in Hwasun, Korea. RESULTS: A total of 1,852 patients who were referred to the Department of Psychiatry for consultation were included in the analysis. The main reasons for psychiatric consultation were changes in mental status(20.5%), depression(16.8%), insomnia(12.8%), and anxiety(7.9%). Psychiatric consultations were conducted with diagnoses of delirium(39.7%), depressive disorder(28.2%), adjustment disorder(7.9%), and anxiety disorder(4.1%). Patients with delirium were significantly older than were those with other psychiatric diagnoses(p<0.001). Delirium was more common in male patients than in female patients(47.1% vs. 29.9%, respectively), and depression was more common in female patients than in male patients(48.3% vs. 33.9%, respectively)(p<0.001). Delirium was more common in patients who underwent surgical operation than those who did not(p=0.010). CONCLUSION: Delirium was the most common diagnosis for psychiatric consultation followed by depression in a university hospital. Delirium was more prevalent in men than in women, while depression was more common in women.

7.Role of Apoptosis in the Differentiation of the Parietal Epithelium of the Renal Corpuscle in the Developing Rat Kidney.

The purpose of this study was to establish the role of apoptosis in the developing renal corpuscle in the prenatal rat kidney. Kidneys from 14-, 16-, 18-, and 20-day-old fetuses[E-14, E-16, E-18, and E-20] were preserved by immersion or perfusion via the heart using Bouin`s fixative. Apoptosis was detected`by in situ nick end labeling method using ApopTag kit. In kidneys from E-14, apoptotic cells and bodies were found only in the mesenchymal tissue surrounding the developing nephrons. In kidneys from E-16, E-18, and E-20, apoptotic cells and bodies were located mainly in the columnar distal epithelium of the renal vesicle[future parietal epithelium] as well as in the parietal epithelium of the renal corpuscles of S-shaped bodies, stage III and IV nephrons. Apoptosis was not observed in the proximal part of renal vesicles or in the podocytes in renal corpuscles. In contrast, strong bel-2 immunoreactivity was present in the proximal part of the renal vesicle and in podocytes in S-shaped bodies, but gradually decreased in stage III and IV nephrons. The distal part of the renal vesicle had weak staining for bcl-2, and there was no bel-2 immunoreactivity in the parietal epithelium of S-shaped bodies, and stage III and IV nephrons. We conclude that bcl-2 is involved in the regulation of apoptosis during the differen-tiation of the parietal epithelium of Bowmann`s capsule.

8.Pseudomembranous Colitis in a Child of Chronic Diarrhea.

Chronic diarrhea in children is a common problem with numerous causes. Although most of these causes are benign, critical illness may present as chronic diarrhea. In a patient of chronic diarrhea, gastrointestinal infections are the most common causes in children of all ages and antibiotics may cause chronic diarrhea by altering intestinal microflora, which can result in the emergence of bacterial overgrowth. Overgrowth of Clostridium difficile may cause pseudomembranous colitis. We experienced 25-month-old boy who suffered from chronic diarrhea and partially treated with antibiotics irregularly. Colonoscopic findings of this child showed multiple plaques with white to yellowish exudate which adhere to the mucosal surface of a variable length of rectum. Histollogically, each plaque comprised a pseudomembrane of mucous debris, inflammatory cells, and exudate overlying groups of partially disrupted glands. A latex agglutination test on patient's stool was positive to toxin A of Clostridium difficile. He was recovered after stopping the antibiotics he has been prescribed, and being given vancomycin for 2 weeks. We report this case with brief review of literature.

9.Atypically Large Calcific Tendinitis of the Shoulder: A Case Report.

Calcific tendinitis of the shoulder joint is common disease causing acute pain, mainly involving the supraspinatus or infraspinatus muscle, and less frequently the teres minor or subscapularis muscle. This study reports on the satisfactory arthroscopic removal of calcium deposits as well as infraspinatus and supraspinatus muscle repair without relapse via minimal incision using suture anchors. This was a case of atypically extensive calcific tendinitis involving the infraspinatus muscle, with a bursal side partial rupture of the supraspinatus muscle in a 61-year-old female whose chief complaint was chronic pain of the right shoulder exacerbated by limited movement.

10.Ossification of the Coracoacromial Ligament in Subacromial Impingement Syndrome: A Case Report.

Here, a case of a 59-year-old man with rotator cuff tear and impingement syndrome caused by an ossified coracoacromial ligament is presented. Ossification of the coracoacromial ligaments can occur because of degenerative changes due to trauma or repeated stress, which can lead to impingement syndrome. Therefore, when coracoacromial ligament ossification is present, rotator cuff damage due to impingement syndrome should be considered. Here, we conducted arthroscopic subacromial decompression, removal of the ossified coracoacromial ligament, and supraspinatus and subscapularis tendon repairs. We achieved satisfactory surgical outcomes without relapse; therefore, we report this case with a literature review.
