3.Effect of intra-dialytic exercises on micro-inflammatory state in maintenance hemodialysis patients
Modern Clinical Nursing 2016;15(8):54-58
Objective To investigate the effect of intra-dialytic exercises on micro-inflammatory state in maintenance hemodialysis (MHD) patients. Methods Sixty clinically stable MHD patients from the Hemodialysis Center were enrolled in the study . The patients were divided into exercise group and control group with 27 cases in each group by using the random digit number table. The patients in the exercise group underwent a 6-month intra-dialytic exercise, in which the patients conducted physical exercises supinely on a cycle ergometer each for 50 minutes, 3 times a week. The patients in the control group were treated without any exercise intervention. Serum interleukin-6(IL-6), C-reactive protein(CRP) and tumor necrosis factor-α(TNF-α) were determined after the study for 0 and 6 months. Results After the 6-month intradialytic exercise program, serum IL-6, CRP and TNF-α were all decreased in the exercise group (P < 0.05). These indices were unchanged in the control group (P > 0.05). Conclusion The intra-dialytic exercises can alleviate micro-inflamatory state in maintenance hemodialysis patients.

Objective To explore the effect of nuclear factor-kappa B (NF-κB) decoy oligode-oxynucleotides (ODN) on respiratory function and expressions of IL-1β and IL-13 in serum following se-vere lung contusion in rabbits. Methods A total of 40 New-Zealand rabbits were randomly divided into four groups, ie, severe lung contusion group (Group A, n=12), severe lung contusion with NF-κB scrambled decoy ODN intervention group (Group B, n=12), severe lung contusion with sense NF- B de-coy ODN intervention group (Group C, n=12) and normal control group (Group D, n =4). After the contusion model was set up, the sense and scrambled NF-κB decoy ODN were infused into the rabbits via the jugular veins in different groups, with 20 g per experimental rabbit. After contusion, respiratory fre-quency, tidal volume, airway pressure, respiration flow rate curve and end expiration nitric oxide concen-tration were detected at 1, 2, 3 and 4 hours. The expressions of IL-1β and IL-13 in serum were observed by means of ELISA. Results After sense NF-κB decoy ODN intervention, alveolar ventilation, arteri-al PO_2 and pulmonary compliance were improved, compared with Group A and Group B, with statistical difference (P<0.01). The expression of IL-1β was decreased and that of IL-13 increased after sense NF-κB decoy ODN intervention to the severe lung contusion, compared with Groups A and B, with statis-tical difference (P <0.01). The expression of IL-1β was increased to peak level at 1 hour after contu-sion, which continued to the end of the experiment. While expression of IL-13 was decreased at 1 hour af-ter contusion and reached the minimum level at 4 hours. With intervention with sense decoy ODN, the in-creased expression of IL-1β was down-regulated, but expression of IL-13 remained at high level, with sta-tistical difference compared with Group A and Group B (P < 0.01). Conclusions Intervention with sense NF-κB decoy ODN can significantly protect the respiratory function, reduce the expression of IL-1β and increase expression of IL-13 after severe lung contusion.
7.Clinical analysis of extrinsic allergic alveolitis in 14 patients
Chinese Journal of Practical Internal Medicine 2000;0(12):-
20% lymphocytes)was documented in all patients who underwent bronchoalveolar lavage(BAL).In addition, inversion of the CD4/CD8 ratio(mean value:0.64?0.21)was observed in the patients.Non-specific IgM and IgG antibodies to antigens were documented in 13/14(92.9%)of BAL patients.Ten patients were cured by glucocorticoid, others by anti-infective therapy. We rechecked the HRCT which displayed the shade of lung in 12 patients(85.7%)vanishing completely,2 patients'(14.3%)partly.Conclusion There is no specificity in the clinical manifestations, change of mageology, pulmonary function and the immunologic test. However, compared with other methods,we find the test of BAL is more valuable to the diagnosis. It is critical to avoid the antigen, and we find the curative effect of the glucocorticoid is significant.

Journal of Endocrine Surgery 2010;04(6):390-393
Objective To improve the ultrasonic differential diagnosis of thyroid lump through analyzing pathological and ultrasonic features of papillary thyroid carcinoma. Methods Records of 173 cases papillary thyroid carcinoma were analyzed retrospectively in terms of preoperative ultrasonic manifestation, lesion size, echo, boundary, calcification, blood supply, lymph node metastasis and accompanying diseases etc. Results The avthe 173 papillary thyroid carcinoma cases, the number of cancer nodule was 203. There was no statistical difference between papillary thyroid carcinoma lesion and papillary thyroid microcarcinoma lesion in terms of low echo and ill-defined borders (P > 0. 05). The former is higher than the latter in blood flow speed, microcalcification rate, and preoperative ultrasonic diagnosis rate (P <0. 01, P <0. 05, P <0. 01 respectively). The mulifocality rate for the 173 cases with papillary thyroid carcinoma is 33.53% (58/173). There was no statistical difference between papillary thyroid carcinoma and papillary thyroid microcarcinoma in cancer lesion(P >0. 05), while the former has a higher rate of lymph node metastasis than the latter(P <0. 01). Preoperative diagnosis rate is higher in papillary thyroid microcarcinoma cases without other thyroid diseases than in cases with other thyroid diseases (P < 0. 05). No statistical difference of preoperative diagnosis rate was seen between papillary thyroid carcinoma patients with other thyroid diseases and those without other thyroid diseases (P > 0. 05). Conclusions The overall evaluation of thyroid nodule diameter, boundary, echo, small calcification, lymph node metastasis, blood supply, accompanying diseases and so on can improve diagnosis of primary papillary thyroid carcinoma.
