2.Analysis on pathogenesis of keratoconus with chronic corneal trauma

Keratoconus is a progressive, non - inflammatory corneal thinning disorder that produces a unique spectrum of change in the surface curvature of the cornea. The suspected factors associated with the cause of keratoconus include genetic mechanisms, familial associations, allergy, atopy and so on, but environmental factors, such as abnormal rubbing, contact lens wearing and ultraviolet radiation may be easily ignored. Although it is commonly thought that keratoconus is a result of various pathological progress mediated by multiple factors, those chronic trauma could play important roles in the pathological progress. The classic definition of keratoconus is still referred as a non - inflammation disorder with malnutrition of keratectasia, more and more clinical evidences and experimental results that may refute this concept. It may be due to the long-term persistent chronic trauma resulted from eye rubbing, contact lens or ultraviolet radiation. Under this conditions, the increased release of inflammatory mediators and matrix metalloproteinases may accelerate keratocyte apoposis and enzyme metabolism disorders, with the result of matrix cell lost, tensile strength decreased and keratectasia. Here, for some papers known about the relationship between chronic cornea trauma and keratoconus were reviewed and discussed its possible causal relationship.
3.The clinical value of olfactory functional magnetic resonance imaging in early diagnosis of Alzheimer's disease

Objective To investigate the clinical value of olfactory functional magnetic resonance imaging in early diagnosis of Alzheimer's disease (AD).Methods 43 patients with mild cognitive impairment were selected,in the same period,49 cases of patients with AD were selected as the AD patients group,and 53 normal populations were selected as the control group.The neuropsychological of all cases were assessed by using MMSE scale,MoCA scale and CDR Scale.Siemens 3.0T MRI machines were used according to event -related design approach for olfactory cor-tex conducted fMRI scans.The matlab7.0 and SPM8 data preprocessing tools were used to image analysis.The prima-ry olfactory cortex volumes and activation voxels numbers of the three groups were compared.The correlations of acti-vation voxels numbers and primary olfactory cortex volumes and neuropsychological scores were analyzed.Results The primary olfactory cortex volumes of the control group,patients with mild cognitive impairment and AD patients were (3 846 ±517)mm3 ,(2 863 ±367)mm3 and (2 214 ±283)mm3 ,respectively,pairwise comparison,the control group >mild cognitive impairment >AD patients,the differences were statistically significant (compared with the con-trol group,t =16.835 and 34.716,compared with mild cognitive impairment group,t =19.753,P <0.05).The acti-vation voxels numbers of the control group,patients with mild cognitive impairment and AD patients were (193 ±84), (104 ±52)and (51 ±27),pairwise comparison,the control group >mild cognitive impairment >AD patients,the differences were statistically significant (compared with the control group,t =47.916 and 72.954,compared with mild cognitive impairment group,t =37.382,P <0.05).Partial correlation analysis showed that the activation voxels num-bers of the three groups were positively correlated with primary olfactory cortex volume,MMSE score and MOCA score (r =0.397,0.462 and 0.494,all P <0.05).Conclusion Olfactory fMRI in patients with AD might reflect the changes in the entorhinal cortex caused by pathological changes.It could provide clues and information for the early diagnosis of AD.
4.Effect of dexmedetomidine on apoptosis and respiratory burst of polymorphonuclear neutrophils in vitro

Objective To investigate the effect of dexmedetomidine (Dex) on the apoptosis and respiratory burst of human polymorphonuclear neutrophils (PMN) in vitro. Methods The peripheral venous blood was collected from healthy volunteers. Isolation of PMN was performed by using the discontinuous plasma-Percoll gradient technique. PMN was randomLy divided into four groups: control group and three different concentrations of Dex (1 ng/mL,10 ng/mL,100 ng/mL) groups. PMN was cultured in enriched RPMI-1640 media at 2 × 106/mL for 24 h. The caspase-3 activity, apoptosis rate and respiratory burst of PMN were detected at 2 and 24 h. Results Compared with the control group, the PMN which was treated with three different concentrations of Dex showed no significant difference in caspase-3 activity, apoptosis rate and respiratory burst of PMN after 2 h of incubation. Compared with the control group, 1 ng/mL concentration of Dex did not affect the caspase-3 activity, apoptosis and respiratory burst of PMN cultured for 24 h. However, 10 ng/mL and 100 ng/mL concentrations of Dex promoted the caspase-3 activity, increased apoptosis rate and reduced the respiratory burst of PMN after 24 h of incubation. Compared with 10 ng/mL Dex group, 100 ng/mL concentrations of Dex promoted the caspase-3 activity, increased apoptosis rate and reduced the respiratory burst of PMN after 24 h of incutation. Conclusion Clinically relevant concentration of Dex does not affect apoptosis and respiratory burst of PMN, while high concentration of dexmedetomidine can induce apoptosis of PMN.
5.Otoplasty of slited cartilage to correct flaring ear

Objective To study a new method of correcting flaring ear. Methods Auricular cartilage was slited from posterior auricula. On Gibson's principle of cartilage distortion antero-lateral cartilage membrane was scarified in order of the involute interpolation of anthelix and cartilage membrance was ground. Then the cartilage outside the slited line was put behind the cartilage inside the slited line. Finally,cartilage was operated with Mustarde's mattres suture. Results 12 patients with flaring ear ( 14 ears) were corrected with this operating method and appearance of corrected ears were very good after operation. Following up for 6-18 months, no one recurred. Conclusion Otoplasty of slited cartilage is an effective method of correcting flaring ear.
6.Improvement of palliative management of the complications in 26 patients with advanced prostate cancer.

Objective To improve the management of the complications in patients with advanced prostate cancer. Methods Bone pain, anemia, urinary tract obstruction, hematuria and edema in the lower extremities and scrotum of patients with prostate cancer were managed with analgesics(24 cases), bisphosphonates(14 cases), isotope strontium-89(6 cases), external beam irradiation(9 cases), blood transfusion(9 cases), TURP(7 cases), superpubic cystostomy(8 case), percutaneous nephrostomy(4 case) and cysto-irrigation(6 case). Results 16 patients were alived and 10 were died of respiratory or circulation failure . The patients were relieved from pain and the other complications for several months after the palliative measures had been taken. Conclusions Bone pain, anemia, urinary tract obstruction and hematuria are common complications of advanced prostate cancer. They may have great impact on the life quality of patients. Diverse therapeutic modalities should be employed to palliate these disabling symptoms.
7.Determination of irigenin in Shegan Kangbingdu Injection by RP-HPLC

AIM: To develope the quantitative analysis approach for irigenin (IRG) in Shegan Kangbingdu Injection(Rhizoma Belamcandae, Flos Lonicerae, Radix Bupleuri, etc.). METHODS: The operation was carried out on the Kromasil ODS column (5 ?m, 4.6 mm?200 mm) with the mobile phase comprised of a mixture of water-methanol-acetonitril(50∶46∶5) adjusted pH3.0 by phostrate acid, the flow rate of 0.8 mL?min -1 , the UV detection wavelength at 265 nm and the temperature at (30.5?1) ?C . RESULTS: The linear range was in the range of 0.028 2-9.4 ?g(r=0.999 9). The relative standard deviations of peak areas for IRG was 1.2% and the standard was stable within 18 h(RSD=0.72%). The LOD (S/N=3) was 3.2 ng, and the limit of quantification (S/N=10) was 10.6 ng and the average recovery was 99.7%. CONCLUSION: The method is simple, rapid and accurate. It can be used for the quality control of Shegan Kangbingbu Injection.

9.The Effect of Xuefuzhuyu Granule Combined with Irbesartan in Reversing the Carotid Vascular Remodeling of Hypertension Patients

Objective To investigate the effect of Xuefuzhuyu granule combined Irbesartan in reversing the carotid vascular remodeling of essential hypertension(EH) patients. Methods 61 EH patients were enrolled in this study. 29 cases of EH were treated with Irbesartan, 32 cases were treated with Xuefuzhuyu granule and Irbesartan, and 28 normotensive subjects served as control. Color dopplor ultrasound were adopted to measure IMT and LD of carotid. Results After treated for 9~12 months, the IMT and IMT/LD in EH patietns treated by Xuefuzhuyu granule and Irbesartan significantly decreased than those in EH patients treated only by Irbesartan. Conclusion Xuefuzhuyu granule was beneficial to reversing the carotid vascular remodeling in EH patients.
10.Destructive damage of bile duct induced by transcatheter arterial embolization for hepatic hemangiomas: Report of 2 cases and literature review

Two cases of hepatic cavernous hemangioma underwent transcatheter arterial embolization and the bile duct damage occurred after operation.Chemical cholecystitis developed in one case,gallbladder necrosis in another and hilar bile duct stricture occurred in both cases.Thirty reported cases of destructive damage of bile duct induced by transcatheter arterial embolization were found from searching PubMed databases and CNKI net.The main complications included acute cholecystitis,hepatonecrosis,biliary cirrhosis and bile duct damage.The literature review indicates that thanscatheter arterial embolization may cause severe bile duct damage and should be chosen and operated with caution.
