1.Effects of mindfulness behavior training on emotional disorder and immunological function in elderly patients after hip fracture fixation surgery

Objective To investigate the effects of mindfulness behavior training on emotional disorder and immunological function in elderly patients after hip fracture fixation surgery.Methods Sixty elderly patients with fixed hip fracture by operation were randomly divided into a training group and a control group with 30 cases in each group.The training group received mindfulness behavior training in addition to routine rehabilitation training,while the control group only received routine rehabilitation training.The mindfulness behavior training was administered 90 min for once a week,lasting for 5 weeks.The emotional disorder was assessed with the profile of mood states (POMS),and the immune system function were evaluated by the number of CD3 + 、CD4 + 、CD8 + in T cell subgroup before and after 5 weeks of training.Results Before training,there was no significant difference in scores of POMS and the cells number of CD3 +,CD4 +,CD8 + (P > 0.05) between the two groups.After treatment,scores of POMS including tension-anxiety (13.14 ± 3.36 vs 15.90 ± 4.28),depression-dejection (18.10 ± 6.24 vs 20.06 ± 5.42),fatigue-inertia (16.53 ± 3.50 vs 18.98 ± 4.88),vgor-activity (26.68 ± 5.22 vs 22.43 ± 5.20) (P < 0.05),the level of mindfulness practice(58.00 ± 4.78 vs 54.00 ± 4.89) and the cells number of CD3 +,CD4 +,CD8 + (75.75 ± 5.40vs 69.91 ± 4.42 ;39.54 ± 3.29 vs 34.44 ± 4.21 ; 39.82 ± 3.55 vs 36.82 ± 3.55) in experimental group improved more than those in control group (P < 0.05).Conclusion Mindfulness behavior training can improve the patients' mood state and have positive promotion on immunological function.
2.Effects of Mindfulness Behavior Training on Mood States and Quality of Life in Old Patients post Fixation for Hip Fracture

Objective To investigate the effects of mindfulness behavior training on mood states and quality of life in old patients post fixation for hip fracture. Methods Patients were randomly divided into experiment group (n=30) and control group (n=30). The control group received routine rehabilitation, while the experiment group received mindfulness practice therapy in addition. They were evaluated with Profile of Mood States (POMS), Short Form of Health Survey (SF-36), and Mindful Attention Awareness Scale (MAAS) before and 5 weeks after treatment. Results There was no difference in the scores of POMS, MAAS and SF-36 before treatment (P>0.05). The scores of MAAS and SF-36, the subscores of tension-anxiety, depression-dejection, fatigue-inertia and vigor-activity of POMS improved more in the experiment group than in the control group (P<0.05). Conclusion Mindfulness behavior training can improve the mood state and quality of life of old patients post fixation for hip fracture.
3.Effect of interferon gamma on immune factor and CD69 , CD107 a in patients with delayed type hypersensitivity

Objective To investigate effect of interferon gemma on immune factor and CD69, CD107a in patients with delayed type hypersensitivity by interferon gamma.Methods 76 cases with delayed type hypersensitivity were selected and divided into 2 groups.38 cases in control group were treated conventional therapy, 38 cases in experiment group were treated by interferon gamma.Peripheral blood CD69, CD107a, immune factor, T cell subsets and the treatment efficiency were compared after the treatment.Results Compared with the control group, the serum CD69 and CD107a levels were lower in the experimental group (P<0.05), serum IgG and IgA levels were higher, the serum IgE level was lower (P<0.05), and the CD3 +,CD4 +,CD4 +/CD8 +level was higher, serum CD8 + was lower (P<0.05) .The effective rate of the treatment group was significantly higher than that of the control group (P<0.05) .Conclusion Interferon gamma has good clinical effect in the patients with delayed type hypersensitivity, and can effectively reduce the levels of CD69 and CD107a in the peripheral blood, and regulate the immune function of the body.
4.Anti-Tumor Effects of Local Injection With Oncolysate Transfected by Recombinant Vaccinia Viruses Encoding Human IL-2

In this experiment, the murine melanoma cell B16-F10 oncolysates transfected by recombinant vaccinia viruses encoding human IL-2(IL-2VBO) were used as vaccine. When the tumor-bearing mice were treated by following injection of IL-2VBO into tumor site, the tumor growth was inhibited and the survival time prolonged. The PBL from IL-2VBO treated mice showed higher cytotoxicity to the wild type B16-F10 but not to YAC-1 cells than that from control groups. The results showed the active specific immunity was induced by the IL-2VBO. On accout of its stronger immuogenicity and some advantages in preparing and storing, the IL-2VBO might be used as a kind of effective vaccine in the cancer active specific immunotherapy.
5.In vitro Observation on the Immunological Characters of IFN-? Induced, IL-2 Gene-Modified Human Primary Fibroblasts

It was an important strategy in cancer gene therapy that transferring foreign gene to fibroblasts to express synthetic protein to exert antitumor effect. But most of studies regarded the foreign gene-modified fibroblasts as a by-stander, only a delivery source of synthetic protein, and neglected their irnmunological characters. In this paper, human primary dermal fibroblasts were modified by human IL-2 gene by retrovirus vector and induced by human IFN-?. The results showed that the MHC- I , MHC- II and CD40 expression on the surface of IFN-? induced fibroblasts were up-regulated significantly in comparison with those of non-induced cells. The IL-2, IL-1, IL-6 secretion were detected as a increased level in supematants of the IFN-?-induced, IL-2 gene modified fibroblasts. Because these molecules and cytokines play important roles in antigen presentation and effector activation, it can be inferred that the IFN-y induced, IL-2 gene modified fibroblasts could be used as antigen presenting cells in addition to delivery cells to activated the effector cells.
6.Risk factors and prognosis of Clostridium difficile infection among 216 hospitalized patients with inflammatory bowel disease

Objective To analyze the risk factors and prognosis of Clostridium difficile infection (CDI) among hospitalized patients with inflammatory bowel disease (IBD).Methods From March 2014 to February 2015,216 first-time hospitalized patients with IBD were enrolled.Once hospitalized,stool samples were obtained for Clostridium difficile culture immediately,and at the same time toxins of Clostridium difficile (tcd) A and tcdB were detected by polymerase chain reaction(PCR).The incidence and risk factors of CDI in enrolled patients were analyzed.Chi square test and nonparametric test were performed for statistical analysis.Results Among the 216 IBD patients,there were 73 patients with ulcerative colitis(UC) and 143 patients with Crohn's disease(CD).The positive rate of CDI was 13.9 % (30/216) in IBD patients.The positive rate of CDI was 24.7 % (18/73) in UC patients,which was higher than that of CD patients (8.4%,12/143),and the difference was statistically significant (x2 =10.690,P<0.01).As the severity of the disease increased,the rate of CDI positive patients in UC patients increased.Among three patients at remission period,17 patients at mild active stage,23 patients at moderate active stage and 30 patients at severe active stage,the CDI positive patients were zero,two,five,11,respectively,and the difference was statistically significant (x2 =4.820,P=0.028).In UC patients,the incidences of patients with hospitalization history in other hospitals within three months before admission in CDI positive group and negative group were 6/18 and 7.3% (4/55),respectively,and the difference was statistically significant (x2 =5.740,P=0.020).In 30 days before admission,the rates of patients with proton pump inhibitor (PPI) taking history of two groups were 7/18 and 7.3% (4/55),and the difference was statistically significant (x2=8.270,P=0.004).The length of hospital stay in CDI negative UC patients was 7.0 d (5.0 d,12.0 d),which was shorter than that of CDI positive patients (10.5 d (8.8 d,18.2 d)),and the difference was statistically significant (U=277.000,P=0.005).The rate of patients underwent intestinal operation in CDI negative CD patients within three months after CDI examination was 10.7 % (14/131),which was significantly lower than that of CDI positive patients (5/12) (x2 =4.520,P=0.010).Conclusions The incidence of CDI in hospitalized IBD patients was high,especially in UC patients;the risk factors included disease severity,prior hospitalization history within three months before admission and PPI taking history within 30 days before admission.CDI was correlated with poor prognosis.

8.Post-stroke dysphagia treated with acupoint injection combined with neural electrical stimulation.

OBJECTIVETo observe the effect differences on post-stroke dysphagia among acupoint injection combined with neural electrical stimulation, acupoint injection, neural electrical stimulation, and swallowing training respectively, so eligible intervention can be applied to this kind of disease.
METHODSOne hundred and eight-three patients of post-stroke dysphagia were randomized into a comprehensive treatment group (42 cases), an acupoint injection group (44 cases), a neural electrical stimulation group (49 cases) and a swallow training group (48 cases) and were treated with the comprehensive therapy of acupoint injection and neural electrical stimulation, acupoint injection, neural electrical stimulation and swallowing training separately. The treatments for 10 days made one session. There were 3 days at the interval among treatment sessions and 3 sessions were required totally. The cases in those treatment groups were blankly controlled with the other 47 patients of post-stroke dysphagia. All the patients received basic rehabilitation treatment. The modified water swallowing test was conducted to assess the efficacy before treatment, 10 days after treatment and 30 days after treatment in each group separately. The clinical efficacy, score of water swallowing test and improvement in water swallow test were compared among the groups.
RESULTSAfter 10-day treatment, the differences in efficacy and score of water swallow test were not significant in each group (all P > 0.05). After 30-day treatment, the effective rate (94.29%, 33/35) in the comprehensive treatment group was apparently better than 68.75% (22/32) in the acupoint injection group, 80.00% (32/40) in the neural electrical stimulation group, 67.50% (27/40) in the swallowing training group and 42.86% (12/28) in the blank group separately. The score in water swallow test in the comprehensive treatment group was lower than that in each of the other groups (1.37 ± 0.60 vs 2.03 ± 1.00, 1.90 ± 0.90, 2.20 ± 0.72, 2.71 ± 0.90, all P < 0.05). The differences in the effective rate and score in water swallow test were not significant among the acupoint injection group, neural electrical stimulation group and swallowing training group (all P > 0.05), which indicated that the improvement in swallowing function in the comprehensive treatment group was significantly superior to the other groups (all P < 0.05).
CONCLUSIONThe comprehensive therapy of acupoint injection and neural electrical stimulation achieves the much better efficacy on post-stroke dysphagia.

9.Clinical utility of high avidity anti-dsDNA antibody in the diagnosis of systemic lupus erythematosus

Objective To investigate the clinical utility of anti-dsDNA antibody in the diagnosis of systemic lupus erythematosus (SLE).Methods Total and high affinity anti-dsDNA antibodies of 431 serum samples were measured by total and high affinity anti-dsDNA antibody EIA kits.Modified SLE disease activity index (M-SLEDAI) was scored for each SLE patients at the time of serum collection.The agreements between total and high affinity anti-dsDNA EIA tests and the correlation of anti-dsDNA antibody levels with disease activity and clinical manifestations were analyzed.Statistical analysis was performed using x2 test,Spearman correlation,Mann Whitney U test & Fisher exact,Student t test.Results ① The overall agreement between total and high affinity anti-dsDNA antibodies was 93.3%,and the total agreement of SLE patients was 89.6%.Twenty-two out of 23 (95.7%) SLE patients with positive total anti-dsDNA antibody but high affinity antidsDNA antibody were inactive with the M-SLEDAI score less than 4.Four patients with other autoimmune diseases had positive total anti-dsDNA antibody but negative on high affinity anti-dsDNA antibodies.② The total and high affinity anti-dsDNA antibody levels were significantly positively correlated with disease activity (M-SLEDAI) and negatively correlated with serum C3 and C4 levels (P<0.01).③ The ratio of high affinity to total anti-dsDNA antibody was significantly higher in patients with M-SLEDAI ≥4 or with active kidney damage (P<0.01).Conclusion Both total and high affinity anti-dsDNA antibodies are significantly correlated with SLE disease activity and kidney damage.However,the high affinity anti-dsDNA antibody may be more specific for active SLE and helpful in the differential diagnosis with other autoimmune diseases.High affinity anti-dsDNA antibody is more valuable in monitoring SLE disease activity status,especially on kidney damage,compared to total anti-dsDNA antibodies.
