1.Epidemic characteristics of student tuberculosis in Xuzhou, Jiangsu, 2016-2020

Abstract: Objective The tuberculosis data of students registered and reported in Xuzhou from 2016 to 2020 were analyzed to provide a basis for Xuzhou to formulate tuberculosis prevention and control measures in schools. Method The epidemiological characteristics of tuberculosis among students in Xuzhou from 2016 to 2020 were analyzed based on the data from Tuberculosis Information Management System and Xuzhou Statistical Yearbook 2021. Results From 2016 to 2020, a total of 574 student tuberculosis patients were registered in Xuzhou, with an annual average registration rate of 5.64/100 000. The reported incidence of tuberculosis among students in Xuzhou showed an increasing trend year by year (χ2trend=22.521, P<0.001). There were 365 males and 209 females, respectively, with a sex ratio of 1.75:1. The tuberculosis patients were mainly in the 15-19 years old group and 20-24 years old group. According to the statistical analysis by different levels of education, students from ordinary higher institutions accounted for 48.61% of the total number of confirmed cases, followed by senior high schools and secondary vocational schools, accounting for 20.91% and 14.46%, respectively. Preschool education had the lowest number of confirmed cases, accounting for 1.05%. There was significant difference in the reported incidence of tuberculosis among students at different educational levels (χ2=1 522.39, P<0.001). The highest incidence was reported in ordinary higher institutions, followed by senior high schools and secondary vocational schools, and the lowest incidence was reported in preschool education. Conclusion Xuzhou city school tuberculosis epidemic integral is on the rise, we should further strengthen tuberculosis control work of school tuberculosis prevention and control measures of implementation in the future, at the same time to strengthen school tuberculosis prevention knowledge propaganda and standardize the schools for disposal of tuberculosis epidemic situation. We should also further strengthen the newborn matriculation examination and students absent from class because of illness monitoring work.
2.Therapeutic potential of brain-derived neurotrophic factor and Schwann cells transplantation in experimental autoimmune neuritis

Objective To investigate the therapeutic potential of brain-derived neurotrophic factor (BDNF) and Schwann cells(SCs) in experimental autoimmune neuritis (EAN) and assess the effect and mechanism.Methods EAN model was established by immunization of Lewis rats with 400 μg of specific peptide P2(57-81)and complete Freund adjuvant.In the therapy group,the SCs (n =28) and the combination of BDNF administration and SCs (n =48) were labeled by the nuclear fluorescent dye injected into the intracerebroventricularly in 14 d after immunization.Transplanted cell migration tracking respectively were at 25,35 and 45 days after immunization.The rats were observed for signs of disease daily and subjected to clinical score,of which the sciatic nerves were subjected to histopathological examination (hematoxylin eosinstaining,luxol-fast-green and immunohistochemical staining).The inflammatory cell infiltration and demyelination were assessed,and the CD4,CD8,CD68,S-100 and nerve growth factor (NGF) positive cells numbers were compared among the 3 different groups.Results AIl the rats had the neurological deficits.Compared with control group,there were no significant differences in SCs therapy group.In SCs + BDNF therapy group,the recovery of paralytic symptom was faster and the score was lower after immunization 45 d.After immunization 25 and 35 days,both the inflammatory cells infiltration (EAN model group:325.8 ±10.8,221.4 ± 35.2;SCs + BDNF transplantation group:307.3 ±4.6,197.2 ± 16.8; t =2.172,P =0.031 ;t=3.756,P=0.000) and the expression of CD4+,CD8+ T cells and CD68+ macrophages were reduced.After immunization 35,45 days,the demyelination degree (EAN model group:3.4 ± 0.5,2.9 ± 0.8 ; SCs +BDNF transplantation group:2.9 ±0.8,2.3 ±0.5) was reduced (t =-7.408,P =0.000;t =-6.092,P =0.000),the expression of S-100 is higher,and NGF was lower than the control group in each time point after immunization.Conclusions SCs transplanted into the cerebellar ventricle of animals can migrate into the sciatic nerve.The combination of BDNF administration and SCs transplantation may represent an effective strategy by reducing inflammation reaction,improving the expression of S-100 in the donor cell,and reducing NGF irritability heighten in sciatic nerve.However,delivery of SCs alone is inefficiency to the treatment of EAN.

Aim To study the effect of oxymatrine (OMT)on protecting human umbilical vein endothelial cells (HUVECs)from toxicity induced by high glucose in vitro.Methods The HUVECs were cultured with medium containing different concentrations of glucose or OMT. The cells were randomly divided into 6 groups:5.5 mmol·L -1 control group(Control),22.2 mmol·L -1 high glucose (22.2 mmol·L -1 ),44.4 mmol·L -1 high glucose (44.4 mmol·L -1 ),control+OMT(control +OMT),22.2 mmol·L -1 high glu-cose +OMT(22.2 mmol·L -1 +OMT),44.4 mmol ·L -1 high glucose +OMT (44.4 mmol · L -1 +OMT).The protective effect of oxymatrine was as-sessed by MTS assays.The expression of A2B in HU-VECs was detected by Real-time quantitative PCR (qPCR)and Western Blot methods.Results The glucose was shown to have caused cytotoxicity in HU-VECs.Oxymatrine (3 μmol·L -1 )was found to have protected HUVECs from glucose toxicity effectively, and reduced the expression of A2B significantly.Con-clusion Oxymatrine can obviously protect HUVECs from cytotoxicity induced by high glucose and the effect is performed partly by decreasing A2B expression.
5.Relation of severity of rhegmatogenous retinal detachment with the levels of amino acids and VEGF in the serum and in the subretinal fluid

Abstract? AIM: To analysis the relation of severity of rhegmatogenous retinal detachment with the levels of amino acids and vascular endothelial growth factor ( VEGF) in the serum and in subretinal fluid.? METHODS: Forty -eight patients ( 52 eyes ) with rhegmatogenous retinal detachment treated in our hospital were selected.According to the degree of retinal detachment, patients were divided into <1/2 quadrant group, 1/2-3/4 quadrant group and>3/4 quadrant group. Fifty-five healthy objects for physical examination in our hospital were selected as the control group, to compare the differences of amino acids and VEGF levels in the serum.Correlation analysis on VEGF levels and amino acids in the serum and in subretinal fluid among patients with different grades of rhegmatogenous retinal detachment was conducted.? RESULTS: In patients with rhegmatogenous retinal detachment, the tryptophan in serum was 28.59±4.46mg/L, phenylalanine 8.95 ±2.55mg/L, methionine 8.15 ±2.17mg/L, valine 28.62 ±5.29mg/L, histidine 18.96 ±1.85mg/L and VEGF 589.92 ±185.34μg/L, which were higher than those in the control group, and the difference was statistically significant(P<0.05).The levels of phenylalanine was 9.85 ±1.21mg/L, histidine 20.63 ±2.07mg/L and VEGF 718.69 ± 283.34μg/L in the subretinal fluid of>3/4 quadrant group, which were significantly higher than those in the <1/2 quadrant group and 1/2-3/4 quadrant group ( P<0.05). VEGF in the subretinal fluid of VEGF in the rhegmatogenous retinal detachment group were positively correlated with phenylalanine (r=0.542, P<0.001), and histidine (r=0.782, P<0.001).?CONCLUSION: The levels of amino acids and VEGF in the subretinal fluid of patients with rhegmatogenous retinal detachment was higher than those in normals and increased with the severity of retinal detachment.
6.Effect of tumor necrosis factor alpha treatment on cerebral ischemia-reperfusion injury in rats

BACKGROUND: Some studies suggest that pre-injection of tumor necrosis factor-α (TNF-α)can protect focal cerebral ischemia in mice. Cerebral ischemia tolerance is related to the increase of TNF-α level; on the other hand, TNF-α is an injurious cytokine associated with stroke. Circulating antibody against anti-TNF-α can protect reperfused injury.OBJECTIVE: To study the effects of TNF-α pretreatment and post-treatment on cerebral ischemia-reperfusion injury and explore possible mechanism.DESIGN: Randomized controlled study.SETTING: Neurological Department, the Second Hospital Affiliated to Harbin Medical University.MATERIALS: The experiment was conducted at the Animal Experiment Center of Harbin Medical University from January to April 2002. Totally 120 healthy adult male Wistar rats were randomly divided into the following 8 groups: TNF-α 0.05 μg, 0.5 μg and 1.0 μg pretreatment groups and PBS group, TNF-α 0.05 μg, 0.5 μg and 1.0 μg post-treatment groups and PBS group with 15 in each group.METHODS: The focal brain ischemia model of middle cerebral artery occlusion (MCAO) was made using inserting thread method. TNF-α of different doses (0.05 μg, 0.5 μg or 1.0 μg) or PBS was injected intracisternally and 22-hour reperfusion, 8 rats from each group were killed. Then the perhour reperfusion, 7 rats from each group were killed. Then pathological changes were observed, glial fibrillary acidic protein (GFAP) and intercellular adhesion molecule-1 (ICAM-1) expression were inspected by immunohistochemical method. Histopathological and immunohistochemical evaluation was made with the computer-assisted image analyzing system,and the number of GFAP positive cells and ICAM-1 positive vessels in each hemisphere was counted.riliary acidic protein and ICAM-1.infarct volume: TNF-α 0.5 μg and TNF-α 1.0 μg pretreatment groups showed reduced volume of lesion; infarct volume reduced by 70.9% in TNF-α 0.5 μg pretreatment rats and 66.5% in TNF-α 1.0 μg pretreatment rats. TNF-α 0.5 μg and TNF-α 1.0 μg post-treatment groups showed increased volume of lesion; infarct volume increased by 22.3% in TNF-α 0.5 μg post-treatment rats and 46.7% in TNF-α 1.0 μg post-treatment rats.TNF-α 0.05 μg and 1.0 μg pretreatment groups did not differ significantly (P > 0.05), but there was an obvious difference between TNF-α 0.5 μg and pared with PBS pretreatment group, TNF-α 0.5 μg and 1.0 μg pretreatment groups showed lessened tissue damage and edema. Compared with PBS post-treatment group, TNF-α 0.5 μg and TNF-α 1.0 μg post-treatment fibriliary acidic protein and ICAM-1: TNF-α 0.5 μg and TNF-α 1.0 μg pretreatment groups showed reduced volume of glial fibriliary acidic protein and ICAM-1 (P < 0.05); but TNF-α 0.5 μg and TNF-α 1.0 μg posttreatment groups showed increased volume of glial fibriliary acidic protein and ICAM-1 (P < 0.05). TNF-α 0.05 μg and 1.0 μg pretreatment groups did not differ significantly (P > 0.05); but there was an obvious difference between TNF-α 0.5 μg and 1.0 μg post-treatment groups (P < 0.05).cerebral ischemia reperfusion injury. This effect is not related to the repair given after cerebral ischemia reperfusion, ischemia exacerbates, which is α are determined by whether TNF-αis given before or after cerebral ischemia in a dose-dependent manner.
7.Clinical study of ginkgo biloba extract dripping pills on ACS patients after PCI and its effects on PAR, ACT and ATⅢ

Objective To investigate curative efficacy of ginkgo biloba extract dripping pills in treatment of acute coronary syndrome(ACS) after percutaneous coronary intervention(PCI) and its effects on platelet aggregation rate(PAR), activated clotting time(ACT) and antithrombin(AT)Ⅲ. Methods 90 patients of ACS treated with PCI who received therapy from January 2014 to October 2016 in Zhejiang green town cardiovascular hospital were selected and randomly divided into the observation group and the control group , 45 cases in each group.The control group was treated with routine treatment after PCI, while the observation group was combined with ginkgo biloba extract dripping pills.After treatment of seven days, the changed of PAR, ACT, ATⅢ and adverse cardiovascular events were compared, after treatment three months, the seattle angina scale were compared. Results After treatment, the levels of PAR in the observation group were significantly lower than that of the control group, and the levels of ACT and ATⅢ were significantly higher than that of the control group, the difference was statistically significant (P<0.05), the total incidence of adverse cardiovascular events in the observation group was significantly lower than that of the control group , the difference was statistically significant ( P<0.05), in the seattle angina scale, the scores of stable state of angina pectoris, the attack of angina pectoris, physical activity limitation, treatment satisfaction in the observation group were significantly better than that of the control group, the difference was statistically significant (P<0.05). Conclusion Ginkgo biloba extract dripping pill is well for ACS after PCI, which can effectively relieve clinical symptoms, to improve the expression of PAR, ACT and ATⅢ, helps to reduce the incidence of adverse cardiovascular events.
8.Significance of neuropsychological single-item scales in the diagnosis of mild cognitive impairment

Objective To identify the significance of neuropsychological single-item scales in the diagnosis of mild cognitive impairment (MCI) and Alzheimer's disease (AD).Methods A total of 676 elderly people living around Dongzhimen district in Beijing were recruited using multistage sampling method.Cognitive function was assessed by minimum mental state examination (MMSE)and other scales.MCI was diagnosed based on the criteria proposed by Petersen (1999),and AD was diagnosed based on the NINCDS-ADRDA.Subjects were divided into normal cognitive group (n=213 cases,31.5%),Alzheimer's disease (AD) group (n=167 cases,24.7%),amnestic mild cognitive impairment (aMCI) group (n=186 cases,27.5 %) and non-AD dementia group (110 cases,16.3 %).Results The MMSE scores in AD,aMCI and normal groups were 23.0 ± 5.9,25.9±2.6,8.1 ± 1.7,respectively.There were significant differences in MMSE scores between AD,aMCI and NC groups separately (all P<0.01).Compared with MCI and NC group,th scores of time orientation,attention,calculation and immediate recall and delayed recall were significantly decreased in AD group (all P<0.01).Comparing with normal group,the scores of attention and calculation were reduced in aMCI group (both P<0.01).The delayed story recall (DSR) scores in AD,aMCI and NC groups were 15.7 ± 11.7,7.6 ± 4.9,26.5 ± 9.3,respectively.There were significant differences in mean DSR scores between AD,aMCI and NC groups separately (all P<0.01).Compared with normal group,the clinical dementia rating (CDR) scores were higher and the mean clock drawing task (CDT)scores were lower in AD and aMCI groups (both P<0.01).Conclusions Both neuropsychological assessment tools such as MMSE and single-item scales such as delayed story recall have the certain significance in the diagnosis of MCI and AD.
9.Effect of levosimendan on SDNN and SDANN changes in patients with acute decompensated heart failure and its clinical efficacy and safety

Objective To investigate the therapeutic effect of levosimendan on acute decompensated heart failure and its effect on SDNN and SDANN values.MethodsThe clinical data of patients with acute decompensated heart failure treated in Zhejiang Greentown Cardiovascular Hospital from January 2014 to August 2016 were retrospectively analyzed.According to their treatment methods, they were divided into control group and observation group.There were 49 cases in the control group and 71 cases in the observation group.The control group was given conventional drug treatment, the observation group was given levosimendan treatment on the basis of the control group.The therapeutic effects of the two groups were observed.The difference of cardiac function, SDNN, SDANN value and adverse reaction between the two groups were compared before and after treatment.ResultsAfter treatment, the observation group efficiency was 98.59%, significantly higher than the control group;the observation group LAD, RAD, LVESD, LVEDD and BNP lower than the control group, the level of LVEF was higher than the control group, the difference was statistically significant (P<0.05);the observation group SDNN, SDANN, RMSSD and PNN50 (%) was higher than that of the control group, the difference was statistically significant (P<0.05).ConclusionLevosimendan has good therapeutic effect on acute decompensated heart failure, can significantly improve the level of SDNN, SDANN, and the incidence of adverse reactions is low.
