Objective To prevent data transfer or loss in application development when dealing with data exchange between Oracle database with different server's character set.Methods Environment variable is set to update Oracle client's character set so as to solve data transfer or loss caused by character set transformation.Results It was the environment variable not the register list that was reset,so Oracle databases with different server's character set were not connected at the same time.Conclusion An adaptive method for different server's character set is given to solve this problem.
2.Methods of assessing candidates to be fostered as leaders in various fields of learning

The authors first describe their preparation for and implementation of the assessment of candidates to be fostered as leaders in various fields of learning, viz. selecting assessment methods, identifying an assessment index system, and specifying the index weight and ways to keep the score and implement the assessment. Then they give an account of the assessment results. Finally they analyze the clinical abilities and research capacities examined in the assessment and sum up their practical experience as: ①emphasis by the leading group on the creation of the right environment and mechanism for the growth of leaders in various fields of learning; ②transformation of concepts regarding the use of personnel; ③specification of the function of the assessment; ④identification of strengthened assessment as the key to guaranteeing the quality in fostering leaders in various fields of learning.
3.A case of primary parahyperthyroidism with chief complaint of upper gastrointestinal bleeding

SUMMARY Hyperparathyroidism is the clinical syndrome that results from elevated circulating parathyroid hormones,leading to multiple systems manifestations,and is divided into three kinds: primary, secondary and ternary.This patient’s main chief complaint was intermittent melena for 3 days,with the diagnosis of upper gastrointestinal hemorrhage according to endoscopy. Hypercalcemia, hypophosphatemia and high level of circulating parathyroid hormone and chronic renal failure were also found during hospitalization. Furthermore, MIBI showed a parathyroid adenoma located near the isthmus in the lower part of the left lobe. The patient had undergone nephrectomy because of kidney carcinoma 2 years ago. Primary hyperparathyroidism usually has the common gastrointestinal syndrome , but upper gastrointestinal hemorrhage reported as the chief complaint is rare and we had never seen similar reports before.
4.The application of chromosomal microarray analysis in genomic diseases

Copy number variations in the human genome,one of the causes of complex diseases and genetic diseases,can lead to genomic disorders.As these diseases are difficult to diagnose,it is significantly meaningful to conduct genetic researches and molecular diagnosis.Chromosomal microarray can be used to detect copy number variations on a genome-wide scale.With the advantage of high throughput and resolution,chromosomal microarray is perceived as an important means of identifying copy number variations in genomic disorders.As technology advancements of chromosomal microarray and accumulations of clinical experiences,chromosomal microarray has played a significant role in etiological diagnosis of multiple malformations,mental retardation and autism.
5.Mammary dysplasia and abnormal estrogen/progesterone or their receptor expression induced by psychological stress in pregnant rats

Objective To study the effects of psychological stress on breast development and abnormal levels of related hormones or their receptors in pregnant rats .Methods Pregnant Wistar rats were randomly divided into control group and ex-perimental group.The experimental group rats were given unpredictable stress stimuli for 15 days, including noise, restraint, re-versal of day and night , swimming in cold water and tail pinch-induced pain.Gross changes of the breast were observed , the di-ameter and height of breast nipple was measured, and breast coefficient was calculated.DNA and RNA levels in the breast tis-sues were determined by optical density measurement.The estrogen (E2), progesterone (P), growth hormone (GH) and prolac-tin (PRL) levels in plasma and breast tissues were determined by radioimmunoassay.Bmax and Kd of estradiol and progesterone receptors were measured by radolig and bindng assay .Morphological changes of the breast tissues were examined by light micros-copy using HE staining.Results Compared with the control group, the nipple diameter and height, breast weight and breast co-efficient were significantly decreased in the experimental group (P<0.05, P<0.01), the levels of DNA, RNA and RNA/DNA ratio in the breast tissue were significantly decreased (P<0.05, P<0.01), the levels of plasma E2, P and GH were significant-ly decreased (P<0.05, P<0.01), the levels of E2 and GH in the breast tissue homogenate were decreased also (P<0.05, P<0.01), the Bmaxs of E2 receptor and P receptor were decreased, but their Kds increased significantly (P<0.05, P<0.01), and the number of lobules and diameter of acini were significantly decreased (P<0.05).Conclusions Psychological stress can lead to breast dysplasia and abnormal levels of estrogen and progesterone and their receptors in pregnant rats .
6.Some Thoughts on Education Innovation of Traditional Chinese Medicine

At present, the training pattern in the higher-education institutions of traditional Chinese medicine (TCM) is still characterized as a trilogy of basic medical theory courses, clinical medical courses and graduation clinic practice, which results in the slow-cultivation of the TCM talent, and brings into the severe problems of theory being divorced from practice and bad social adaptability in the graduates. Under this situation, it is imperative to reform the traditional training patterns and to advocate innovation in the education. It is suggested that creative teachers, students' creative study and a democratic, harmonious, relaxing and easy study atmosphere are helpful to the cultivation of TCM talents. Therefore, a student-orientating training pattern of classroom-teaching synchronized with the clinic practice, emphasizing clinic practice, and aiming to improve the scientific research ability is advocated in higher-education of TCM. This is expected to provide some thoughts for the development of TCM higher education in the new era.
7.PTEN-silenced bone marrow mesenchymal stem cell transplantation for acute myocardial infarction

BACKGROUND:Bone marrow mesenchymal stem cel transplantation for myocardial infarction becomes popularized in recent years, but transplanted cels cannot survive and proliferate under early inflammatory reaction or local ischemia/hypoxia microenvironment, eventualy hampering the therapeutic outcomes.
OBJECTIVE:To investigate the therapeutic effect of PTEN-silenced bone marrow mesenchymal stem cels on acute myocardial infarction.
METHODS:(1) Bone marrow mesenchymal stem cels from Sprague-Dawley rats were randomly assigned to receive no treatment, NCsiRNA transfection using Lipofectamin2000orPTEN siRNA transfection using Lipofectamin2000. Cel growth curves were described using MTT method to detect cel cycle using flow cytometry. (2) Thirty Sprague-Dawley rats were selected to prepare myocardial infarction models that were randomized into three groups (n=10 per group): blank control, negative control and RNAi group. Six hours after modeling, bone marrow mesenchymal stem cels transfected with nothing, NCsiRNA and PTEN siRNA were respectively injected into the infarcted center of the left ventricular anterior wal in these three rat groups. After 4 weeks, al rats were subjected to cardiac function detection using echocardiography, and the survival and proliferation of bone marrow mesenchymal stem cels in the rats were observed by fluorescence microscopy.
RESULTS AND CONCLUSION:Compared with the other two groups, a significant increase in the absorbance values at different culture time, the proportion of cels in S+G2phase, and the number ofbone marrow mesenchymal stem cels in the myocardial tissue was found in the RNAi group (alP< 0.05). Additionaly, the left ventricular ejection fraction and left ventricular shortening fraction were significantly reduced in the RNAi group than the blank control and negative control groups at 4 weeks after cel transplantation (P< 0.05). Bothin vivoandin vitroexperimental findings showed that PTEN silencing could effectively improve cel survival and proliferation in the infarcted myocardium. Moreover, in thein vivoexperiment, an overt improvement in rat’s cardiac function was achieved.
8.Clinical features of diabetes insipidus with urinary tract dilatation

Diabetes insipidus ( DI) could lead to urinary tract dilatation, even renal dysfunction. This study compared clinical features of DI with or without urinary tract dilatation. The results showed that the former had earlier onset age, more male patients, longer duration, and higher serum creatinine, as well as lower urine osmotic pressure and urine specific gravity after injection of vasopressin. But only disease duration was the independent risk factor (OR=1. 248). More nephrogenic DI and more hereditary DI were with urinary tract dilatation compared with central DI or acquired DI.
9.Clinicopathological analysis of orbital neurilemoma:a report of 8 cases

Objective The purpose of this study is to study the clinical and pathological features of neurilemoma .Methods We observed the clinicopathologic features and immunohistochemical staining from eight patients with orbital neurilemoma between 2010.1~2012.12.Results Eight patients with classic neurilemoma were included in the study ,in which there were five males and three females ,aged between 21 and 63,mean age 35.The main symptom of the patients was exophthalmos ,including five cases of right eyes and three left eyes;2 cases of orbital floor and six above orbit ,lasting for one to ten years .The tumor diameter ranged between 1cm and 5 cm,an average of 3 cm,being pale and light yellow color .There were five cases of type Antoni A and one case of type was Antoni B among the six classic type neurilemoma .Two cases of ancient were neurilemoma ,in which one case was the histological structure of the classic type neurilemoma ,but there were more hypertrophy tumor cells , chromatin was coarse block atypia cells .The other one case with cells arranged disorderly ,which was mainly fine striated cells with scattered deeply stained atypia cells ,stromal transparent degeneration ,cystic degeneration .Im-munohistochemistry results showed that S -100(+),vimentin(+),ki67(-).Conclusion Antoni type B and ancient schwannoma are rare ,with complicated histologic characteristics .Combined with clinical features and im-munohistochemistry staining ,it can be diagnosed .
10.Approach to the patients with adrenocortical insufficiency combined with the syndrome of inappropriate secretion of antidiuretic hormone

To summarize the clinical data of two cases with severe hyponatremia diagnosed as adrenal insuffiency combined with syndrome of inappropriate secret on of antidiuretic hormone(SIADH),and to review related literatures.Case 1 diagnosed as Addison's disease for 27 years and developed severe hyponatremia again but did not response well to sufficient glucocorticoid.Further examination showed SIADH caused by lung cancer and tolvaptan worked well.Case 2 was diagnosed as SIADH caused by lung cancer and responsed well to tolvaptan.However,hyponatremia reoccurred with the decreasing level of ACTH and cortisol during the chemotherapy.It was thought that hyponatremia was caused by drug-related adrenal insuffiency and glucocorticoid replacement therapy achieved good response.Both primary/secondary adrenal insuffiency and SIADH can lead to severe hyponatremia,but it is rare that the two situations exist in one patient and occur in different time.We should consider the possibility of the situations when we make differential diagnosis of refractory hyponatremia,monitoring the curative effects carefully,then correct the diagnosis timely,and reduce missed diagnosis and misdiagnosis.
