BACKGROUND: Dyslexia is the most common in the study of learning disabilities, it can affect various aspects of children, including behaviors,cognition, emotion, social adaptation, etc., and seriously block their obtaining of knowledge and improvement of ability in children.OBJECTIVE: To study the changing law of regional cerebral blood oxygenation in children with Chinese dyslexia in the process of recognizing Chinese characters, and investigate the neurophysiological basis of dyslexia in children.DESIGN: A case-control study.SETTING: Department of Child and Adolescent Health and Maternal Care, School of Public Health, Tongji Medical College, Huazhong University of Science and Technology.PARTICIPANTS: The study was carried out in Huazhong University of Science and Technology from June to September in 2003. Forty-five primary students of 8 to 12 years old, who were grade 3 to 5 in Wuhan city,were enrolled in this study, including 26 dyslexic children (dyslexia group)and 19 normal readers (control group). All the enrolled children were righthanded. Informed consents were obtained from all the participants and their parents (guardians) after explanation of aims and steps of this study.METHODS: The functional near-infrared imager (fNIRI) was applied to detect the changes of cerebral blood oxygenation in left prefrontal lobe of dyslexic children and normal children in the primary processing (viewing passively, reading aloud, producing an action word) and secondary processing of Chinese characters (outputting task, action words association) of Chinese characters.MAIN OUTCOME MEASURES: The changes of cerebral blood oxygenation in the primary processing and secondary processing of Chinese characters were observed in both groups.RESULTS: All the 26 children with Chinese dyslexia and 19 normal children were involved in the analysis of results. ① Changes of the contents of blood oxygenation in the primary processing of Chinese characters:When viewing the Chinese characters passively, the contents of blood oxygenation on channel A, B and C were all higher in the dyslexia group than in the control group (0.378 0, 0.134 6; 0.085 8, 0.051 9; 0.109 3, 0.069 2;P < 0.05); there was no obvious difference on channel D between the two groups (P > 0.05). When reading aloud, the contents of blood oxygenation on channel A, B, C and D were all unobviously higher in the dyslexia group than in the control group (P > 0.05). In the process of generating action words, the contents of blood oxygenation on channels A and C were all higher in the dyslexia group than in the control group (0.411 2, 0.139 7;0.141 3, 0.078 4; P < 0.05), and there were no obvious differences on channels B and D between the two groups (P > 0.05). ② Changes of the contents of blood oxygenation in the secondary processing of Chinese characters: In the process of outputting task, the changes of blood oxygenation was lower in the dyslexia group than in the control group (-0.025 7, 0.089 0,F=16.25, P < 0.01); In the process of action words association, the changes of blood oxygenation was obviously higher in the dyslexia group than in the control group (0.073 0, -0.072 1, F=15.59, P < 0.01).CONCLUSION: The cortexes of left prefrontal lobe in dyslexic children are activated in the process of recognizing Chinese characters, but the degree and pattern of activation are different from those of normal children.The functional abnormalities of specific brain area may be the biological basis of Chinese dyslexia.Song RR, Wu HR.Changes of regional cerebral blood oxygenation in recognizing Chinese characters in children with Chinese dyslexia.
3.Exploration of clinical value of colposcopy directed biopsy in diagnosis of vaginal intraepithelial neoplasia

Objective To explore the clinical value of colposcopy directed biopsy in diagnosis of vaginal intraepitheal neoplasia (VaIN).Methods The clinical data of 106 VaIN patients diagnosed by colposcopy directed biopsy were reviewed and analyzed retrospectively.In the past medical history of 106 patients,46 patients had cervical cancer history,43 patients had cervical intraepithehal neoplasia history,13 patients had previous hysterectomy because of benign disease and 4 patients had uncertain past medical history.Results Among 106 patients,97 patients(91.51%,97/106) had abnormal pap smears of vagina,9 patients (8.49%,9/106) had abnormal pap smears of cervix.Histological analysis revealed that 69 patients were VaIN Ⅰ,24 patients were VaIN Ⅱ,13 patients were VaIN Ⅲ.The high-risk human papilloma virus (HPV) test at diagnosis were positive in 98 patients (92.45%,98/106),negative in 8 patients (7.55%,8/106).Eighty-seven patients VaIN were localized in the upper one third of the vagina,19 patients VaIN were localized in other parts of vaginal wall.Ninety-eight patients VaIN affected to the multifocal vaginal wall,8 patients VaIN affected to the focal vaginal wall.Conclusions It has highly therapeutic value of colposcopy directed biopsy in diagnosis of VaIN.But should closely correlated to past medical history,colposcopy should performed on the whole vagina wall and multiple punch biopsy to elevated positive rate.
4.Nodular Lesions of Cirrhotic Liver: Imaging Manifestations with Pathologic Correlation

Objective To introduce the new nomenclature scheme of the International Working Group (1995) on hepatic nodules, and summarize the imaging features of various hepatic nodules in light of their pathological characteristics, and evaluate the diagnostic values of various imaging facilities.Methods Ultrasound, computed tomography(CT), magnetic resonance imaging(MRI), and angiographic CT were reviewed and introduced.Results Many of these types of hepatic nodules play a role in the de novo and stepwise carcinogenesis of hepatocellular carcinoma(HCC) in the following steps: regenerative nodule, low grade dysplastic nodule, high grade dysplastic nodule, small HCC, and large HCC. Accompanying such transformations, there are significant alterations in the blood supply and perfusion of these hepatic nodules.Conclusion Modern state of the art medical imaging facilities can not only delineate and depict these hepatic nodules, but also provide important clues for the characterization of focal hepatic lesions in most cases, thus facilitating the early detection, diagnosis and management of HCC in its early stage.

6.Clinical Significance of Routine Application of Choledochoscope During Biliary Duct Operation

Objective To investigate the clinical significance of routine application of choledochoscope during operation of biliary duct.Methods The clinical data of 136 patients with the routine application of intraoperative choledochoscope dealing with bile duct diseases in this hospital from October 2003 to July 2009 were analyzed and summarized.Results Intraoperative choledochoscope inspection,taking stones and targeted surgery were performed in 116 cases with extrahepatic and (or) intrahepatic bile duct stones.The taking out rate of extrahepatic bile duct stones was 100% (85/85),the residual stone rates of extrahepatic and intrahepatic bile duct were 0 (0/85) and 22.6% (7/31),respectively,with the total residual stone rate was 6.0% (7/116).By using intraoperative choledochoscope,benign intrahepatic bile duct stricture was found in 13 cases,malignant extrahepatic and intrahepatic bile duct stricture in 8 cases.Four cases of hilar cholangiocarcinoma and 4 cases of common bile duct cancer,2 cases of lower segment of common bile duct polyp,3 cases of hepatolithiasis with the left hepatic bile duct carcinoma were diagnosed by biopsy via choledochoscope.Causes were confirmed by applying choledochoscope in 16 patients with obstructive jaundice.The use of choledochoscope with surgical treatment enabled benign and malignant bile duct stricture to achieve good results,without serious complications such as bile duct dilaceration,subphrenic abscess or acute cholangitis.Conclusions Using choledochoscope can tremendously reduce the residual rate of stone in biliary duct surgery,increase the definite diagnosis rate of biliary duct diseases and play a role of reasonable instruction in its treatment options.It has unique advantages in identifying causes of jaundice.It is remarkable that the role of applying choledochoscope to diagnose and cure biliary duct diseases.It should be widely used.

8.Evaluating the role of Her-2 in endocrine therapy for breast cancer

How to select endocrine agents for patients with ER-positive/ Her-2 -positive breast cancer has been a difficult question. Increasing evidences shows a consistent results that an association between Her-2 overexpression and poor response of endocrine treatment, tamoxifen and the third generation aromatase inhibitors has the similar poor efficacy. The treatment strategy for such patients should prefer chemotherapy alone or in combination with trastuzumab, and endocrine therapy combining with trastuzumab may be the development strategy.
9.The development of predicting the lung cancer resistance to gemcitabine by the expression levels of RRM1

Gemcitabine is important pyrimidine antimetabolites drug. Ribonucleotide reductase is oneof its targets whose overexpression has been observed in several gemcitabine- resistant cell lines. Meanwhile,the NSCLC patients with high RRM1 expression levels have a relatively poor survival when treated with gem-citabine. To this day, the relationship between RRM1 expression levels in peripheral blood mononuclear cellsand gemcitabine is still uncertain and it deserves further research.
