1.Effects of long-term administration of amlodipine on the intima-media thickness and the endothelium-dependent vasodilative function of carotid artery in elderly patients with hypertension

Objective To evaluate the clinical effects of amlodipine on structure and function of atherosclerotic artery in elderly hypertensive patients. Methods The intima-media thickness(IMT) and the inner diameter of carotid artery at rest and overpressure of brachial artery as well as after sublingual nitroglycerin administration were measured with amlodipine by color Doppler ultrasonograms before and after long-term (36 months) treatment. The hypertensive patients (76 cases) were divided into 3 groups according to the value of IMT. Group A, n = 21, IMT +1s. Results The blood pressure was controlled to therapeutic aim and maintained stable after 8 weeks treatment with amlodipine. The carotid artery IMT of hypertensive patients was lowered significantly and the endothelium-dependent vasodilative function of common carotid artery was improved dramatically after 36 months therapy with amlodipine. The IMT and endothelium-dependent vasodilative function came up to normal standards in group A and approached the normal level in group B. Conclusions Amlodipine can improve the endothelium-dependent vasodilative function, retard or even reverse the progression of carotid artery IMT, when it lowers blood pressure stably.
2.Essential Drug and Pharmacoepidemiology

OBJECTIVE:To provide suggestion and advice for the selection and utilization of essential drug. METHODS:WHO standards for the selection of essential drug and the formulation process of National Essential Drug?Part for Primary Medical and Health Institutions (2009 edition) were analyzed. The significance of theory and methods of pharmacoepidemiology in the selection, utilization and performance evaluation of essential drug were put forward. RESULTS & CONCLUSION:National essential drug should be selected according to evidence-based medicine and pharmacoeconomics based on pharmacoepidemiology, in order to take advantage of a drug epidemiology-based evidence-based medicine and drug selection based on economics, the study concludes. Dialectical thinking about safety and effectiveness of essential drug should be conducted.
3.Proceedings of the international meeting of the 10th anniversary of the AADO

The international meeting of the 10th anniversary of the AADO, convoked from Oct.8 to Oct. 10 in 2002 in Shanghai, was sponsored by AADO and organized by the two orthopedics departments of Changhai Hospital and Ruijin Hospital, Shanghai. The supporters of this meeting were Stryker Pacific, Stryker Trauma and Stryker China. There were 180 conventioneers who came from all over the mainland China, America, Germany, India, Japan, Hong Kong and Taiwan. They were clinical specialists in traumatology, managers from traffic departments and experts in vehicle design. 17 theses were selected for the meeting. The theme of the meeting was musculoskeletal trauma in developing countries with focus on road traffic injuries &geriatric fall injuries. Many experts in orthopedic trauma were invited to give presentations at the meeting on the theme.
4.Analysis of Physical Fitness Participation of 1243 Community Residents

Objective To Analysis the physical fitness participation of 1243 community residents and to provide the scientific basis for developing intervention strategies. Methods The study was based on data of Physical Activity Questionnaire collected in Nutrition and Health Survey 2006 in Putuo, Shanghai. 1 243 subjects aged 6 years and above were involved. Results 43.8% of community residents had participated in physical exercise. Participation ratio was 41.4% in male, lower than that in female (45.8%). Participation ratios among aged 6yr~, 18yr~, 45yr~, 60yr~ were 48.1%, 25.2%, 22.0%, 64.9% in male and 40.8%, 22.5%, 40.1%, 67.1% in female. Participation ratios among age groups were significantly different (P=0.000). Proportion for frequent exercise of all age groups were 25.0%, 11.9%, 19.7%, 64.9% for male and 21.1%, 15.0%, 37.4%, 66.3% for female. There were difference among age groups (P= 0.000). Conclusion Specific intervention measures should be developed for different kind of people.
5.Analysis on the Utilization of TCM Injections in Our Hospital During the Period of 2005~2008

OBJECTIVE: To analyze the utilization and tendency of TCM injections in our hospital, and to provide reference for clinical use of drugs. METHODS: Based on hospital database, the category, consumption sum and DDDs of TCM injections in our hospital during the period of 2005~2008 were analyzed statistically. RESULTS: The proportion of consumption sum of TCM injections in the total consumption sum from 2006 to 2008 were 13.06%, 9.33%, 9.97%, 9.08% respectively. The consumption sum of TCM injections was increased year by year. Stasis-removing agent for cardio-cerebral-vascular system took up the first place in the list of category of drugs, comsunption sum and DDDs. CONCLUSION: TCM injections are widely applied in our hospital. The utilization structure of TCM injections is the reasonable basically. But great importance should be attached to the safety and effectiveness of TCM injections and reasonable choice.
6.THE PROJECTIONS FROM LAMINA Ⅱ OF MEDULLARY DORSAL HORN TO PARABRACHIAL REGION, CAUDAL VENTROLATERAL MEDULLA AND SPINAL CORD IN THE RAT

After injecting retrograde tracer fiuoro-gold (FG) into the parabrachial region(PB), caudal ventrolateral medulla(CVLM) and the fourth segment of cervical spinal cord (C4), respectively, neurons in laminae I ~ Ⅱ of the medullary dorsalhorn projecting to the above mentioned brain areas were observed. PB received projections from bilateral laminae I and Ⅱ withan ipsilateral dominance; CVLM and C4 received projections from ipsilateral laminae I and Ⅱ. Neurons projecting to C4 werevery sparsely distributed in laminae I and Ⅱ of the medullary dorsal horn. The projecting neurons in outer part of lamina Ⅱwere more than those in inner part of lamina Ⅱ . Combined with immunofluorescence histochemistry for calbindin-D28k(CB) andparvalbumin(PV), it was demonstrated that a part of neurons projecting to PB or CVLM showed CB-like immunoreactivity, butnone of them exhibited PV-like immunoreactivity. There were only a few neurons in lamina Ⅱ projecting to C4 and they exhibitedneither CB- nor PV-like immunoreactivity. The present study provides further evidence for the existence of projecting neurons inlamina Ⅱ and suggests that immunostaining against CB and PV may distinguish two neuronal subpopulations in lamina Ⅱ .
7.Clinical Observation on Yin-Nourishing and Blood-Activating Herbal Medicine for Seminal Delayed Liquefaction

Objective To investigate the therapeutic effect of Chinese herbal medicine for seminal delayed liquefaction(SDL).Methods Two hundred and ninety SDL patients were randomized into two groups.Group A(N=230)received oral use of yin-nourishing and blood-activating herbal medicine(mainly composed of Radix Ophiopogonis,Radix Scrophulariae,Fructus Ligustri Lucidi,Herba Ecliptae,Radix Salviae Miltiorrhizae,Flos Carthami,Rhizoma Sparganii,Eupolyphaga seu Steleophaga),and group B(N=60)received intramuscular injection of chymotrypsin and oral use of vitamin C and zinc gluconate.The therapeutic effect in the two groups were evaluated after treatment,and the changes of main parameters of seminal liquid as well as the seminal plasma contents of prostate specific antigen(PSA),acid phosphatase(AP),citric acid(CA)and zinc were observed before and after treatment.Results In group A,156 patients were cured,57 effective,17 ineffective and the total effective rate was 92.6%;in group B,21 patients were cured,17 effective,22 ineffective and the total effective rate was 63.3%.The Therapeutic effect was better in group A than that in group B(P
8.Inferior displacement sign in fracture of humeral greater tuberosity

Objective To explore the clinical significance of the inferior displacement sign on the initial anterioposterior (AP) view X-ray for fractures of humeral greater tuberosity.Methods This study retrospectively analyzed the imaging and clinical data of 24 patients with isolated fracture of humeral greater tuberosity who had sought medical treatment during the period from January 2008 to June 2015.They were 10 males and 14 females,with an average age of 54 years (from 19 to 68 years).Laterality:8 left sides and 16 right sides.The AP view X-ray films of the 24 patients present the inferior displacement sign defined as the inferior cortical margin of the greater tuberosity overlapping the proximal humeral shaft on the initial AP view.The patients received further CT examination to determine the displacement direction and whether the humeral neck fracture was complicated.Results A slight displacement of humeral anatomic neck fracture was shown on the initial AP view X-ray in 10 patients.Further CT examination revealed that humeral anatomic neck fracture was confirmed in 23 patients,the greater tuberosity was displaced posteriorly in 22 patients and posteroinferiorly in 2 patients.Radiographic measurements showed:the mean posterior displacement was 10.5 ±4.5 mm;the displacement between the humeral head and the humeral shaft was 2.5 ± 1.5 mm;the neck shaft angle was 147.2° ± 9.2°;the distance between the humeral head and greater tuberosity was 10.8 ± 3.4 mm;the overlap between the inferior edge of greater tuberosity and the upper edge of lateral humeral shaft was 12.5±5.9 mm.Pearson correlation analysis showed no correlation between the overlap and the head-tuberosity distance (P > 0.05),but a significant correlation between the overlap and the neck-shaft angle (P < 0.05).Conclusions The inferior displacement sign on the initial AP view X-ray is actually the overlap of posterior displaced greater tuberosity fracture shown on the X-ray.The posterior displaced greater tuberosity fracture complicated with slight displaced anatomic neck fracture is a presentation of the valgus impacted fracture of the proximal humerus.The inferior displacement sign in the fracture of humeral greater tuberosity can highly suggest the presence of occult or slight displacement of the humeral anatomic neck fracture.
9.Effect of Pirfenidone on Activity of Hepatic Cytochrome P450 in Rats

Objective To study the effects of pirfenidone on total enzyme and isoenzyme of liver microsomal cytochrome P450 in rats. Methods The activites of liver microsomal dimethyl nitrosamine N-demethylase( NDMA )and erythromycin demethylase( ERD ) were determined. Fifty-six SD rats were randomly divided into six groups,in which they received CMC,dexamethasone 100 mg·kg-1 ,ketoconazole 40 mg·kg-1 ,pirfenidone 25,50,100 mg·kg-1 ,respectively. After administration for 6 and 12 days,livers were prepared liver microsome,the concentration of proteinum in microsome and shade selection to plasma samples were determined by spectrophotometer. Results After administration of pirfenidone for 6 days, cytochrome P450 was significantly increased in 50 and 100 mg·kg-1 pirfenidone groups,as compared with solvent control group (1%sodium carboxymethylcellulose)(P﹤0. 01). After administration of pirfenidone for 6 and 12 days,NDMA of liver microsome was not changed significantly(P﹥0. 05). ERD of liver microsome was significantly increased in 100 mg·kg-1 pirfenidone group after administration for 12 days(P﹤0. 01). Conclusion Pirfenidone can induce P450 and ERD activities in a dose-and time-dependent manner.
10.Biopolymer poly hydroxybutyrate-hydroxyvalerate membrane

BACKGROUND: Poly hydroxybutyrate-hydroxyvalerate (PHBV) has been used to construct bioprosthetic heart valve. It remains unclear whether it can be used as membrane for guided bone regeneration. OBJECTIVE: To investigate the biocompatibility of PHBV membrane and evaluate its efficiency of promoting bone regeneration in vivo. METHODS: Effects of 100%, 75%, 50%, 25% PHBV extract solution on relative growth rate of dog bone marrow mesenchymal stem cells were measured by MTT method and cytotoxicity of the biomaterials was evaluated. Bone defects were made on distal bilateral tibias and treated with PHBV membrane; the proximal bilateral tibias undergoing reduction of periosteal flap and were used as control. RESULTS AND CONCLUSION: The toxicity gradation of PHBV membranes was grade 0-1. That is, they were not toxic to growth and proliferation of bone marrow mesenchymal stem cells. New bone regeneration was observed in the defects covered with PHBV membranes at week 2 post-surgery. The defects covered with PHBV membranes were filled with mature bone at week 12 post-surgery. The bone repair in experimental groups was earlier and better than that in control groups. Results demonstrated that PHBV membrane, which has no cytotoxicity to mesenchymal stem cells in a wide range of extract concentration, could be a promising biopolymer membrane for guided bone regeneration.
