Journal of Xi'an Jiaotong University(Medical Sciences) 2017;38(3):337-342
Objective To understand the status of perinatal folic acid intake by childbearing-aged women in Shaanxi Province and analyze the influencing factors so as to provide data support for current policy adjustment. Methods We sampled in 10 cities of Shaanxi and Yangling Demonstration area, and chose women who had been pregnant in the past 3 years or were pregnant at the time of the survey to answer questionnaires. The unconditional logistic regression model was used to analyze the influencing factors. Results At the whole provincial level, the intake rate of folic acid was 84.1%.The intake rate of folic acid varied significantly among different regions, being 88.9% in southern Shaanxi,85.7% in Guanzhong area, and 88.9% in northern Shaanxi (x2=24.011,P<0.001).It also significantly differed between urban and rural areas (89.5% vs.80.6%,x2=19.974,P < 0.001).The intake rate of folic acid was different significantly among different age groups, too, being lower in the old-age group than in the young-age group (x2 =13.637,P=0.003).The rate of free access to folic acid and duration of folic acid intake also significantly differed among urban and rural areas, different regions and different age groups (P < 0.05).By using logistic regression model we found that the childbearing-aged women's knowlage of folic acid was the most important influencing factors besides urban/rural area, region, age and education level.Among these factors the odds ratio of awareness intake of folic acid during perinatal period was 14.13 (95% CI:6.16-32.44),the odds ratio of knowing that folic acid can prevent neural-tube defects was 2.71 (95% CI:1.43-5.14) and odds ratio of knowing the policy of free distribution of folic acid supplements was 2.88 (95% CI:1.52-5.47).Conclusion The rate of folic acid intake among childbearing-aged women in Shaanxi Province is relatively high, but the duration of folic acid intake is still short. The understanding about folic acid is an important factor which affects the use of folic acid.Improving the cognitive level of folic acid among childbearing-aged women is the most useful way to enhance the effective prevention of neural tube defects.
2.Research advances in immune tolerance in chronic hepatitis B virus infection
Journal of Clinical Hepatology 2017;33(5):949-953
Immune tolerance refers to the specific non-response or negative response of T and B lymphocytes to antigens.According to the period of formation, immune tolerance can be classified into central tolerance and peripheral tolerance.Immune tolerance to hepatitis B virus (HBV) after HBV infection is considered a major cause of chronic HBV infection. This article briefly reviews the roles of HBeAg,functional defects of dendritic cells, low response of cytotoxic T lymphocytes,T helper cells and cytokines, variations of HBV genotype and genome, and host gene polymorphisms in the development of immune tolerance in chronic HBV infection,as well as related research advances.

Objective:To discuss the clinical efficacy of minimally invasive locking plate and the traditional open reduction with internal fixation in the treatment of proximal humerus fractures.Methods:89 cases of patients with proximal humeral fractures were selected and divided into two groups according to different surgical methods. The observation group (45 cases) was given minimally invasive locking plate, while the control group (44 cases) was treated with the traditional open reduction and internal fixation. The operation time, hospitalization time, fracture healing time, intraoperative blood loss,Neer score and Constant-Murley score at 1 month after postoperatiion were compared between two groups. Results:The operation time, hospitalization, fracture healing time of observation group were significantly shorter than those of the control group, and the bleeding amount of observation group was less than that of the control group (P<0.05).At 1 months after operation, the Constant-Murley scores of observation group were significantly better than those of the control group (P<0.05).The For Neer scores, excellent rate of observation group (91.1%) were significantly higher than those of the control group (68.1%,P<0.05).Conclusion:Compared with the traditional open reduction with internal fixation, minimally invasive locking plate was better, safer, faster and more effectively for promoting the recovery of shoulder function of patient with proximal humerus fractures.

In accordance with students' learning needs and future professional development, individualized practices of formative assessment are analyzed in terms of diverse evaluators, accumula-tive learning contents, after-class tutoring, thus to maximize student's learning potentials, to be capa-bility-oriented, to make students highly qualified and competent for their future clinical position and academically developed. Additionally, the assessing competence of teachers is vital to the successful implementation of formative assessment. Importantly, each medical university can explore further into the practice of formative evaluation, based on the orientation of itself.

Based on the educational structure theory, the difference between mass education and elite education is probed at the macroscopic level to make it clear that the essence of Physician education and training program of excellence is elite education for occupational purposes; at the mi-croscopic level the orientation of English teaching for the class ofexcellent physicians is determined as English for occupational purposes. Moreover, the curriculum system of medical English is designed, and teaching materials associated with clinical medicine are selected, the language laboratory simulat-ing hospital scenes is constructed, and the transboundary team of teachers is built comprising university teachers and doctors in hospitals. At the level of individual, the competency model of English forexcellent physicianshas been developed, English competence requirements in medical industry being clearly established . In a word , the study of macrostructure points out the direction and target of English teaching for excellent physicians; the study of microstructure perfects the mode of English teaching forexcellent physicians;and the study of individual structure indicates the ultimate foothold of English education forexcellent physicians. Three aspects are integrated into an organic whole.

Objective To explore acute cerebral vascular accident factors in patients with nosocomial infection. Methods Clinical data with 680 cases with acute cerebral vascular accident were retrospectively analyzed, and the patients with hospital flu infected as the observation group, selected in accordance with 1 ∶ 1 over the same period without cerebrovascular accident combined hospital patients feel as control group. The patient age, gender, state of consciousness , invasive operation, dehydrating agent application time, whether use of antibiotics and other differences were compared . Results In 680 cases of patients with acute cerebral vascular accident, there were 90 cases of hospital infection; two groups gender, dehydrating agent application time had no difference(P > 0. 05 ); the observation group compared with the control group older, unconscious, to implement invasive operation to prevent high proportion of antibiotics(P <0. 05), hospital infection-related factors. Conclusion Acute cerebral vascular accident patients should have a higher incidence of nosocomial infection, and be related with many factors. Taking corresponding measures against the relevant factors could prevent hospital infection.

Objective To establish a cell line with overexpression of rat serotonin1A receptor (5-HT1AR).Methods Human neuroblastoma cells-SH-SY5Y were donated by cancer institute attached to the 4 th Affiliated Hospital, Hebei Medical University. Total RNA was extracted from brain tissues of male SD rats and rat 5-HT1A R was obtained by RT-PCR. Plasmid pc-DNA3. 1/hisC containing the rat 5-HT1AR (pc-DNA3.1/hisC-Rat-5-HT1AR)was constructed and transfected into SH-SY5Y cells. The transfected cells were isolated by G418 selection and SH-SY5Y-Rat-5-HT1A R cells were obtained. Expression of 5-HT1A R was detected by Western blot analysis. Cell viability was evaluated by MTT assay. SH-SY5Y-Rat-5-HT1AR cells were further observed for 5-HT1AR by immuno-fluorescence staining. Results Plasmid pc-DNA3. 1/hisC-Rat-5-HT1AR was successfully constructed by linking Rat-5-HT1A R with pc-DNA3.1/hisC and transfected into SH-SY5Y. The SH-SY5Y-Rat-5-HT1A R cells were more slender than SH-SY5Y cells with less and longer processes. MTT showed that the viability of SH-SY5Y-Rat-5-HT1A R cells was much lower than SH-SY5Y. Rat 5-HT1A R was expressed efficiently on the membrane of SH-SY5Y-Rat-5-HT1A R cells. Conclusion A cell line with overexpress of rat 5-HT1A R is successfully established.
