OBJECTIVETo investigate the technique and its therapeutic effect of synchronous correction of lip and nasal deformity in complete bilateral cleft lip.
METHODS29 patients with complete bilateral cleft lip underwent synchronous correction of lip and nasal deformity with the modified Mulliken method for Asians. The philtral flap was 6-7 mm in length, and 3-4 mm in width at the collumellar-labial junction. The distance between the peaks of cupid's bow was 4-5 mm. The bilateral edge of philtral flap was de-epithelialized and advanced to form philtrum column. The lateral lip was advanced to the medial site, and the central vermilion tubercle was constructed with the bilateral vermilion-mucosal flap. Through the alar rim incision, the displaced cartilage was dissected and repositioned to raise the nasal tip. The follow-up period was 6 months to 6 years.
RESULTSSatisfactory results were achieved in all patients. The reconstructed upper lip had invisible scar with natural philtrum and column. The vermilion had good appearance with tubercle. The length of nasal column was not decreased and depression of nasal tip and alar was greatly improved.
CONCLUSIONSOur modified Mulliken method is effective in synchronous correction of lip and nasal deformity in bilateral cleft lip.

Objective To detect the relationship between the genotypes of the 2642 deletion polymorphism (delta 2642) in IT15 gene and the age of onset of Huntington disease (HD). Methods Peripheral blood samples were collected from 29 patients with HD and 38 gender- and age-matched controls. All patients with HD were diagnosed by gene diagnosis. The CAG trinucleotide repeats of the 29 patients with HD outnumbered 40. Polymerase chain reaction-restriction fragment length polymorphism and polyacrylamide gel electrophoresis technique were used to detect the genotypes of delta 2642. Results No B/B genotype was detected in both 2 groups. The genotype frequencies of A/A and A/B in the IT15 delta 2642 polymorphism was 65.5% and 34. 5% in HD patients,92. 1% and 7. 9% in the controls respectively (x2 = 7. 435, P =0. 006). The frequency of the B allele was 17.2% in the HD group and 3. 9% in the control group (P = 0. 010, OR = 5.07, 95% CI 1.47-15.12). Analysis showed no significant difference between A/A genotype patients and A/B genotype patients for CAG trinucleotide repeats(P =0. 188). HD patients with A/B genotype (37.33±6. 46) had an earlier onset than the patients with A/A genotype (47.10± 10. 86, t = 2. 491, P = 0. 019). Conclusions These data demonstrated that variations in IT15 delta 2642 polymorphisms may be a genetic factor that influences the variability in HD age of onset. HD patients with delta 2642 A/B genotype have an earlier onset than the patients with A/A genotype.

Objective To investigate the changes in the expression of calcitonin gene-related peptide (CGRP), substance P (SP) and brain-derived neurotrophic factor (BDNF) in dorsal root ganglion (DRG) and the effect of electroacupuncture (EA) on morphine tolerance in rats with chronic inflammatory pain (CIP) and morphine tolerance. Methods Twenty-five 8-month-old male SD rats weighing 230-250 g in which intrathecal (IT)catheters were successfully implanted without complications were randomly divided into 5 groups ( n = 5 each):groupA CIP + normal saline (NS) 10 μl IT twice a day for 7 consecutive days; group B CIP + morphine 10 μg/kg ( 10 μl) IT twice for the first day only; group C CIP + morphine 10 μg/kg ( 10 μl) IT twice a day for 7 consecutive days; group D CIP + EA (intensity 2 mA, frequency 2 Hz, wave length 0.6 ms) + morphine 10 μ g/kg (10 μl) IT twice a day for7 consecutive days; group E CIP + EA (intensity 2 mA, frequency 15 Hz,wave length 0.4 ms) + morphine 10μg/kg (10 μl) IT twice a day for7 consecutive days. CIP was induced by injecting complete Freund's adjuvant (CFA) into the ankle joint of the left hindlimb. IT morphine or NS was started on the 4th day after induction of CIP. EA of Yanglingquan and Zusanli lasting 30 min was performed once a day after first IT administration of morphine for 7 days. Paw withdrawal latency (PWL) to a thermal nociceptive stimulus was measured before induction of CIP, 1 day before (baseline) and at day 1-7 after administration (T0-8) . The animals were sacrificed after the last PWL measurement. The DRGs of the lumbar segment (L4-6) were removed for determination of CGRP, SP and BDNF mRNA expression using RT-PCR. Results PWL was significantly shorter at T1 than at T0 in all groups, and at T3-8 than at T2 in group B-E, while longer at T2 than at T1 in group B-E ( P <0.05). PWL was significantly longer in group B-E and CGRP, SP and BDNF mRNA expression higher in group C than in group A ( P < 0.05). PWL was significantly longer in group C-E than in group B ( P < 0.05). PWL was significantly longer and CGRP, SP and BDNF mRNA expression lower in group D and E than in group C ( P <0.05). PWL was significantly lower and CGRP, SP and BDNF mRNA expression higher in group E than in group D ( P < 0.05). Conclusion Up-regulation of the expression of CGRP, SP and BDNF mRNA in DRG is involved in the devepopment of morphine tolerance. EA can inhibit the devepopment of morphine tolerance by inhibiting the expression of CGRP2 SP and BDNF mRNA.

Chinese Journal of Tissue Engineering Research 2010;14(1):11-14
BACKGROUND: Non-adherent mesenchymal stem cells (NA-MSCs) can form colony forming unit of fibroblasts and induce the differentiation into adipocytes, osteoblasts, and chondrocytes. OBJECTIVE: To determine the effect of epithelial growth factor (EGF) on colony formation of fibroblasts and differentiation into neuron-like cells from NA-MSCs.METHODS: Bilateral femur and tibia as well as total MSCs were separated, and repeated-transfer was employed to purify NA-MSCs. The fifth-passage total MSCs and NA-BMCs were induced in a medium containing EGF and basic fibroblast growth factor (b-FGF) for 2 weeks. Colony unit formation of fibroblasts, effect of EGF on colony-forming unit of fibroblasts, and relative protein expression detected by toluidine blue and immunocytochemical staining were observed. RESULTS AND CONCLUSION: Both total MSCs and NA-BMC could generate colony-forming unit of fibroblasts. After treatment of EGF, colony-forming unit of fibroblasts from NA-BMC was increased significantly. Immunocytochemical staining demonstrated that two weeks later both neuro-specific NeuN and NF-200 were observed in total MSCs and NA-BMC; while, toluidine blue staining indicated that neuron-specific Nissl body was observed in some cells. EGF can effectively promote colony-forming unit of fibroblast from NA-BMC, and repeated-transfer NA-BMC can induce differentiation into neuron-like cells.
