1.The application of high-quality nursing care principle in nursing students′clinical tutoring at oral and maxillofacial-head and neck oncology ward

Objective To apply high-quality nursing principles in the nursing education, allow nursing students to adapt to the modern nursing pattern more effectively in the oral and maxillofacial-head and neck oncology ward, and then to evaluate the effect of this application. Methods From June 2013 to March 2014, 60 nursing students had practiced in our department were randomly divided into the control group (30 cases, traditional teaching method) and the experimental group (30 cases, high-quality nursing teaching method). Nursing expertise, nursing procedures, teachers′ evaluation and patients′ satisfaction degree were evaluated. Results The nursing expertise, nursing procedures, teachers′ evaluation and patients′ satisfaction scores was (92.97 ± 5.65), (92.93 ± 4.38), (94.20 ± 2.95), (93.60 ± 3.97) in the experimental group respectively, while in the control group was(87.93 ± 4.62),(87.55 ± 7.54),(89.67 ± 6.21),(89.50 ± 5.80)respectively. All the four evaluation indexes of the experimental group were better than that of the control group(t=-3.78,-3.38,-3.61,-3.20), P﹤0.05. Conclusions Applying high quality nursing care principle in the nursing education allows students to adapt to the modern nursing pattern more quickly, and to establish foundations leading to their future professions.
2.Treatment of postoperative gastrointestinal fistulas in patients with severe acute pancreatitis

Objective To explore the means of treating postoperative gastrointestinal fistulas in patients with severe acute pancreatitis(SAP). Methods A retrospective study was carried out on the clinical data of 28 cases of postoperative gastrointestinal fistulas(PGF) in patients with severe acute pancreatitis admitted in our department between 1996 and 1999.Results The amount of drainage decreased significantly by using somatostatin(SS) and growth hormone(GH). All the patients were recovered, the average disappearred time of the fistula was (52?16) days(24~227 days).85.7% of the fistula were cured by non operative treatment.Conclusions Non-operative treatment is suitable to most of SAP patients with PGF. SS and GH can accelerate the healing of gastrointestinal fistulas.
3.Design of an Incremental and Open Laboratory Automation System.

Recent years have witnessed great development of TLA (Total Laboratory Automation) technology, however, its application hit the bottleneck of high cost and openess to other parties' instruments. Specifically speaking, the initial purchase of the medical devices requires large sum of money and the new system can hardly be compatible with existing equipment. This thesis proposes a new thought for system implementation that through incremental upgrade, the initial capital investment can be reduced and through open architecture and interfaces, the seamless connection of different devices can be achieved. This thesis elaborates on the standards that open architecture design should follow in aspect of mechanics, electro-communication and information interaction and the key technology points in system implementation.

4.Senile Constipation Treated with Shenshen(Radix Ginseng and Fructus Mori) Bolus

400 cases of senile constipation were treated by Shenshen Bolus, a self-formulated remedy of Qi-benefitting, Yin-nourishing, blood-benefitting and blood-activating nature with a total effective rate of 98.0%. Experiments showed that as compared with Maren(Fructus Cannabis)Bolus, Shenshen Bolus not only possesses better cathartic action and lowers the tension of intestine in vitro. but also strengthens the function of the kidney. It reveals no side effects even in long-term administration.
5.Study on mother-to-child transmission route of transfusion transmitted virus infection

Objective To investigate the prevalence of TT virus infection in pregnant women in Shenzhen and the role of mother-to -child transmission route in the TTV infection. Methods TTV DNA were detected by nested PCR in sera, cord blood and breast milk samples from 400 mother-infant pairs. TTV DNA positive mothers were followed up for 6 months for monitoring TTV DNA. Thirty two TTV DNA PCR products amplified from blood and milk samples of 13 mother-infant fairs were cloned and sequenced. Results By means of PCR, TTV DNA were detected in 62 of mothers' serum samples, 4 of 400 cord blood samples, and 20 of 400 milk samples, respectively. Therefore, 6.5%(4/62) of infants are likely to be infected with TTV from their mothers during the fetal period. Direct sequencing of TTV DNA from 13 mother-child pairs showed identical isolates (nucleotide homology from 97.5% to 99%), Follow-up sera of 42children, whose mothers were TTV DNA positive, showed 16 cases turned out to be existence of TTV viremia at 6 months old. Conclusions Our data proved the mother-to-child transmission route of TTV infection.TTV can be transmitted transplacentally and postnatally by the way of breast feeding as well. The increase of TTV prevalence during the 6 months after birth, suggests the postnatal transmission route of TTV may be of more importance, and further study is needed to address whether preventive measure should be done.
6.Effect of endocrine hormone and cardiac function on patients with chronic heart failure by Huangqi Decoction

Objective To analyse change of neuroendocrine hormone and cardiac ultra ejection force in patients with heart failure before and after treatment with Huangqi Decoction.Methods 100 patients diagnosed with chronic heart failure were collected.According to the different treatment regimens divided into two groups ( control group: conventional western medicine; traditional Chinese medicine group: conventional western medicine treatment +Astragalus Decoction orally for 2 weeks) , before and after treatment,nerve endocrine hormone using echocardiography evaluation index and ejection force were detected in two groups, and compare the results of data.Results Compared with control group, Chinese medicine group with better anti heart failure treatment, performance: the plasma calcitonin gene related peptide (CGRP), atrial natriuretic peptide (ANP), C type natriuretic peptide ( CNP ) , neuropeptide Y ( NPY ) levels were significantly lower ( P<0.05 ) . Capacity index significantly improved cardiac function (P<0.05).Left atrial diameter, left atrial maximum, minimum volume decreased significantly (P<0.05).Ejection force of left atrium and left ventricular increased significantly ( P<0.05 ) .The results were statistically significant.Conclusion Huangqi decoction can inhibit the secretion of CGRP, ANP, CNP,NPY and other neuroendocrine hormones in patients with heart failure, reduce cardiac load, increase cardiac ejection force.
7.Pathogenicity of Treponema pallidum inoculated through different routes in rabbits:a preliminary study

Objective To observe clinical manifestations and immune responses in rabbit models of syphilis established by inoculation with Treponema pallidum through different routes. Methods A total of 20 rabbits were randomly and equally divided into two groups: testis-inoculated group injected with Treponema pallidum (Tp) suspensions into the testes, back-inoculated group intracutaneously injected with Tp suspensions at six sites on the shaved back. All the rabbits received a single injection with the total injection amount of Tp (Nichols strains)being 2 × 107 in both groups. Clinical symptoms and immune responses were evaluated in both groups every other day. Statistical analysis was carried out by a two-sample t-test and rank sum test with the SPSS 20.0 software. Results On day 8 after the inoculation, the testes of rabbits in the testis-inoculated group started to swell with induration, and the swelling was most severe during days 13 - 16. Afterwards, the testes gradually diminished, and returned to normal in size without swelling or hardening on day 28. No skin lesions occurred in the other sites in the testis-inoculated group within 2 months after the inoculation. Erythematous swelling occurred at inoculation sites on day 7 after inoculation in the back-inoculated group, which was most obvious between days 15 and 45. Moreover, cartilage-like indurated ulcers were observed at 12 inoculation sites in 3 rabbits in the back-inoculated group, and dark-field examination of the ulcers showed the presence of Tp. There was a significant difference in the time required for the severity of lesions to peak between the testis-inoculated group and back-inoculated group (14.50 ± 1.08 days vs. 29.00 ± 10.30 days, t=5.02, P<0.05). Rank sum test showed significant differences in the distribution of highest RPR titers between the two groups(P<0.05), and RPR titers were higher in the testis-inoculated group than in the back-inoculated group. Conclusions The injection of Tp suspensions at different sites can cause different clinical symptoms and immune responses in rabbits. RPR turned positive earlier with a higher titer in the testis-inoculated group compared with the back-inoculated group. The clinical manifestations in the back-inoculated group were similar to those of chancre in primary syphilis in human.
8.Mechanism of emodin enhancing chemosensitivity of pancreatic cancer cells

Objective To investigate whether emodin enhances chemosensitivity of pancreatic cancer cells and the mechanism. Methods Normal human skin fibroblasts and pancreatic BXPC-3 cells were divided into control group, cisplatin (5 mg/L) treatment group, gemcitabine (0.5 μmol/L) treatment group, emodin (50 μmol/L)+ cisplatin (5 mg/L) co-treatment group and emodin (50 μmol/L) + gemcitabine (0.5 μmol/L) co-treatmentgroup. The cell viability was detected by MTT assay, the cell apoptosis rate by flow cytometry, the expression of the multidrug resistance-associated protein 1 ( MRP1 ), MRP2, breast cancer resistance protein (BCRP) and P-glycoprotein (P-gp) mRNA in different cell lines was determined by reverse transcription polymerase chain reaction.All data were analyzed using the t test. Results The cell viability and apoptosis rate of pancreatic BXPC-3 cells after treatment were 62.44% ± 3.42% and 27. 10% ± 4.24% in the cisplatin treatment group, and they were 30.53% ±0.05% and 66.33% ±9.37% in the emodin + cisplatin co-treatment group. There were significant differences in cell viability and apoptosis rate between the two groups (t = 13.20, 5. 35, P < 0.05 ). The cell viability and apoptosis rate of pancreatic BXPC-3 cells after treatment were 79.82% ±2.83% and 13.48% ± 1.65%in the gemcitabine treatment group, and they were 45.65% ± 2.46% and 62.74% ± 10. 18% in the emodin +gemcitabine co-treatment group. There were significant differences in cell viability and apoptosis rate between the two groups (t = 12.89, 8.28, P < 0. 05 ). There was no significant difference in the viability of normal human skin fibroblasts between the cisplatin treatment group and the emodin + cisplatin co-treatment group, and also between the gemcitabine treatment group and the emodin + gemcitabine co-treatment group (t = 2. 08, 0. 64, P >0.05 ). Expression of MRP1 mRNA was detected in pancreatic BXPC-3 cells, whereas the expression of MRP2,BCRP and P-gp mRNA was undetectable. The expression of MRP1 mRNA in pancreatic BXPC-3 cells was significantly down-regulated in the cisplatin treatment group, and cisplatin + emodin co-treatment had an additive effect on down-regulating the expression of MRP1 mRNA. Conclusion Emodin may enhance chemosensitivity of pancreatic cancer cells to cisplatin by down-regulating the expression of MRP1 mRNA.
9.The suppressive effects of IFN-α on human gastric carcinoma cell line BGC-823

AIM: To observe the inhibitory effect of interferon-α ( IFN-α) on the growth invasiveness and metastasis of human gastric carcinoma cell line BGC-823, and mechanism of its action. METHODS: We detected the influence of IFN-α on the proliferative ability of BGC-823 in cell culture system, the cell vitality with the MTT colorimetric assay, and the cell cycle with flow cytometer (FCM). The regulatory functions of IFN-α to the expression of E-cadherin and matrix metalloproteinase-2 ( MMP-2) in tumor cells were estimated by immunohistochemical analysis ( S-P). The ultrastructural changes of the junction among the tumor cells were observed under electron microscope. RESULTS : IFN-α can significantly inhibit the growth of human gastric carcinoma cell line BGC-823 in a dose-dependent manner. When the concentration of IFN-α was ≥106 U/L, the cell proliferation can be effectively suppressed,the suppression rate was ≥ 12. 2%, and the blockage appeared at the phase of G1-S of the cell cycle. Under the induction of IFN-α, the expression level of the cell E-cadherin increased while the MMP-2 decreased. The changes on ultrastructure of the cells showed the increased adhesive junctions and the relative compact structure. CONCLUSION: IFN-α can suppress the growth of human gastric carcinoma cell line BGC-823 through its influence on cell cycle. IFN-α can regulate the expression of E-cadherin and MMP-2, make the cell junction closely, so that it has the potential on restricting the invasion and metastasis of gastric carcinoma cells.
10.Clinicopathological features and risk factors of lymph node metastasis of gastrointestinal neuroendocrine neoplasms in 467 patients

Objective To investigate the clinicopathological features and risk factors of lymph node metastasis of gastrointestinal neuroendocrine neoplasms (GI-NENs).Methods The retrospective case-control study was conducted.The clinicopathological data of 467 patients with GI-NENs who were admitted to the Fourth Hospital of Hebei Medical University from January 2006 to December 2015 were collected.Observation indicators:(1) occurrence sites and pathological classification of GI-NENs;(2) pathological characteristics of surgical specimens of GI-NENs;(3) univariate analysis and multivariate analysis affecting lymph node metastasis of GI-NENs:sex,age,tumor location,tumor diameter,pathological classification,pathological stage and tumor invasive depth.The univariate analysis and multivariate analysis were respectively done using the chi-square test and Logistic regression model.Results (1) Occurrence sites and pathological classification of GI-NENs:of 467 patients with GI-NENs,tumors of 304,15,7,14 and 127 patients were located at stomach,duodenum,small intestine,colon and rectum,respectively.Tumor diameter was 0.3-12.0 cm,with an average diameter of 2.2 cm.Of 467 patients with GI-NENs,G1 and G2 of neuroendocrine tumors (NETs),G3 of neumendocfine carcinomas (NECs) and mixed adenoneuroendocfine carcinomas (MANECs) were respectively detected in 209,64,146 and 48 patients.Lymph node metastasis rate of GI-NENs was 31.48% (147/467).(2) Pathological characteristics of surgical specimens of GI-NENs:NETs were high-differentiated NENs.Ceils of NETs were solid and nest-,trabeculum-and tubular-shaped,and consisted of small or medium cells,with moderate amount or massive cytoplasms,round or oval nucleus,particle-shaped chromatin,unobvious nucleolus and positive endocrine markers.There were abundant of small blood vessels and surrounding fibrous stroma in peripheral tumor cell nests.NECs were low-differentiated NENs and included small cell carcinoma and large cell NEC.Cells of small cell carcinoma were small round or oval and looked similar to lymphocytes,with few amount cytoplasms,fine granularshaped or hyperchromatic nucleus and common mitosis figures.Cells of large cell NEC were large and greater than 3 lymphocytes,arrayed in organoid-or chrysanthemum-shape,with massive cytoplasms,coarse particle-shaped chromatin,obvious nucleus,clear mitosis figures and large laminar-shaped necrosis.There were different positive expressions of endocrine markers between small cell carcinoma and large cell NEC.MANECs had the characteristics of glandular cavity formation of traditional adenocarcinoma and NENs.Results of immunohistochemical staining in 467 patients showed that Ki-67 of 467 patients was positive;CD56 in 379 of 428 with CD56 test was positive;synaptophysin (Syn) in 416 of 422 with Syn test was positive;cytokeratin (CK) in 354 of 396 with CK test was positive;chromogranin (CgA)in 264 of 388 with CgA test was positive;neuron specific enolase (NSE) in 287 of 346 with NSE test was positive.(3) Univariate analysis and multivariate analysis affecting lymph node metastasis of GI-NENs:results of univariate analysis showed that sex,tumor location,tumor diameter,pathological classification,pathological satge and tumor invasive depth were related factors affecting lymph node metastasis of patients with GI-NENs (X2 =20.654,18.182,26.788,184.709,163.738,195.391,P< 0.05).Results of multivariate analysis showed that pathological classification and pathological stage were independent influenced factors affecting lymph node metastasis of patients with GI-NENs (HR =2.129,7.171,95% confidence interval:1.273-3.561,-2.327-22.098,P<0.05).Conclusions GI-NENs are mostly located on the stomach and rectum.Results of immunohistochenical staining could help diagnosis of GI-NENs.Pathological classification and pathological stage are independent influenced factors affecting lymph node metastasis of patients with GI-NENs.
