1.Changes of trace elements and nutritional proteins in children with acute leukemia at remission stage

Objective To investigate the changes of serum trace elements and nutritional proteins in children with acute lymphoblastic leukemia and acute myeloid leukemia at the stage of remission.Methods Erythrocyte count,hemoglobin,serum levels of total protein,albumin,iron,ferritin,transferrin,lactate dehydrogenase,ceruloplasmin,cuprum,zinc and their ratio were measured in 43 patients with acute lymphoblastic leukemia,19patients with acute myeloid leukemia at stages of remission(remission groups),and 30 healthy controls(control group)enrolled from Shanghai Children's Medical Center using atomic absorption spectrometry,nophelometry assay,dry chemical method,and chemiluminescence method.The differences of these indicators between remission groups and control group were analyzed.Results Serum levels of total protein(P=0.454),iron(P=0.769),transferrin(P=0.903),and zinc(P=0.343)were not significantly different between the remission groups and the control group.Serum levels of ferritin(P=0.000),lactate dehydrogenase(P=0.000),ceruloplasmin(P=0.000),cuprum(P=0.002),and Cu/Zn ratio(P=0.003)in the remission groups were significantly higher than those in control group.On the contrary,erythrocyte count(P=0.000),hemoglobin(P=0.000)and albumin(P=0.046)were significantly lowerin remission groups than those of control group.Serum levels of all detected indicators were not significantly different between the acute lymphoblastic leukemia remission group and acute myeloid leukemia remission group(P>0.05)except for lactate dehydrogenase(P=0.025).Conclusion At the remission stage of acute lymphoblastic leukemia and acute myeloid leukemia,serum levels of some trace elements and nutritional proteins gradually returned to normal,and the original balance is established again.

Objective To investigate the changes of serum trace elements and nutritional proteins in children with acute lymphoblastic leukemia and acute myeloid leukemia at the stage of onset. Methods Serum levels of cuprum, zinc, iron, magnesium, phosphorus, calcium, ceruloplasmin, ferritin, transferrin, lactate dehydrogenase, total protein, albumin, hemoglobin, and erythrocytes were detected in 73 patients with acute lymphoblastic leukemia, 26 patients with acute myeloid leukemia at stages of onset, and 30 healthy controls using methods including atomic absorption spectrometry, nophelometry assay, dry chemical method, and/or chemiluminescence method. The differences of these indicators among these three groups were analyzed by t test. Results Serum levels of all detected elements except for zinc and phosphorus were significantly different between the onset groups and the control group (P < 0.05 ). Serum levels of cuprum, magnesium, iron, ferritin, ceruloplasmin, and lactate dehydrogenase in the onset groups were significantly higher than those in control group ( all P < 0.05 ). On the contrary, calcium, transferrin, total protein, albumin, hemoglobin, and erythrocyte count were significantly lower in the onset groups than those in control group (P < 0.05). Serum iron, cuprum, zinc, and their metabolism were significantly different between acute lymphoblastic leukemia group and acute myeloid leukemia group ( P < 0.05 ).Conclusion Serum levels of some trace elements and nutritional proteins are disordered and out of balance in chil dren with acute lymphoblastic leukemia and acute myeloid leukemia at stages of onset.
3.Comparison of analgesic efficacy and postoperative recovery between multi-point and single-point fascia iliaca compartment block in patients with hip fracture: a randomized controlled trial

Objective:To compare the analgesic effect and postoperative recovery between multi-point and single-point ultrasound-guided fascia iliaca compartment block (FCIB) in elderly patients with hip fracture.Methods:From June 2019 to April 2020, 96 patients aged (71.1±5.4) years undergoing elective surgery for hip fracture in Renji Hospital were randomly assigned in multi-point block group or single-point block group, and the multi-point or single-point ultrasound-guided FCIB was performed for the two groups, respectively. The primary outcomes were the success rate for block of femoral nerve, lateral femoral cutaneous nerve and obturator nerve, and the 24 hours postoperative pain visual analog scale (VAS) score at rest and passive movement. The secondary outcomes were the onset time of sensory blocks of the above three nerves, complications of FICB, the satisfaction score of pain control during 48 hours after surgery and postoperative recovery of patients.Results:The success rate for block of lateral femoral cutaneous nerve in multi-point block group was 97.9% (47/48), which was significantly higher than that in single-point block group (83.3%(40/48)) ( P=0.031). The success rate for block of obturator nerve in multi-point block group was 95.8% (46/48), which was significantly higher than that in single-point block group which was 81.3% (39/48) ( P=0.025). The success rate of femoral nerve block in both groups was 97.9% (47/48) ( P=1.000). The 24 hours postoperative VAS scores at rest and passive movement in the multi-point block group were significantly lower than those in the single-point block group ( P<0.05). The onset time for block of lateral femoral cutaneous nerve and obturator nerve in multi-point block group was shorter than that in single-point block group ( P=0.025 and P<0.01). There was no significant difference in the onset time for block of femoral nerve between the two groups ( P=0.343). The satisfaction score of patients at 48 hours after surgery in multipoint block group was higher than that in single point block group ( P=0.024). The length of hospital stay in the multi-point block group was shorter than that in the single-point block group ( P=0.042), the proportion of intravenous analgesic drugs used after surgery was lower than that in the single-point block group ( P=0.041), and the complication rate within 30 days after surgery was also lower than that in the single point block group ( P=0.026). Conclusion:Compared with single-point block, ultrasound-guided multi-point block in iliac fascia space has advantages of increasing analgesic efficacy and promoting postoperative recovery in elderly patients with hip fracture.
4.A controlled study on identification for depression and effectiveness of its treatment in psychiatric clinics of general hospitals

Objective The study is designed to evaluate the difference in identification for depression and clinical effectiveness of its treatment between psychiatrists and non-psychiatric Dhysieians at out-patient departments of general hospitals and to analyze its related factors.Methods Totally,680 patients who visited psychiatric clinics in nine general hospitals of Shanghai at first time were screened by psychiatrists using Composite International Diagnostic Interview(CIDI)for depression and the screening resuhs were compared to the diagnosis made by non-psychiatric physicians as goal-keepers there.Altogether 297 patients with depression were recruited and assessed using Hamilton Depression Rating Scale(HAMD)and Hamilton Anxiety Rating Scale(HAMA).A self-made questionnaire for basic information was used to assess the ability for identification of depression in non-psychiatric physicians of general hospitals.Results Psychiatrists identified depression correctly in 337 of 680 patients who visited psychiatric clinics in general hospitals at their first visit,but non-psychiatric physicians only identified 216 patients,with statistically significant difference(χ2=30.73,P=0.000).A consistent agreement on depression diagnosis between the findings by psychiatrists and by CIDI was reached with Kappa of 0.774,but Kappa for that between the findings by non-psychiatric physicians and CIDI was 0.439.Effectiveness of treatment for depression by psychiatrists was better than that by non-psychiatric physicians,with higher total score and scores for each item of HAMD four,eight and 12 weeks after treatment,respectively(P<0.05 or P<0.01).Length of professional work of non-psychiatric physicians and annual time for professional training in management of mental disorders associated with their ability of identification for depression(P<0.05).Conclusion Ability of non-psychiatric physicians to identify alQd cope with depression in the psychiatric department of general hospitals was insufficient,suggesting that more importance should be attached to the management of service quality at psychiatric department of general hospitals.
5.Research progress on helper T cell-17 and interleukin-17 in oral lichen planus

Oral lichen planus (OLP) is a chronic inflammatory disease occurring in the oral mucosa. Clinically, OLP presents with various lesion morphologies, attributed to differences in host immune responses. T-helper 17 cells (Th17) are a crucial component of the cellular immune response, primarily functioning through the secretion of interleukin 17 (IL-17). IL-17 plays a dual role in the oral mucosa: on one hand, it exerts a protective effect by promoting the recruitment of neutrophils driven by chemokines, enhancing the secretion of antimicrobial peptides, and strengthening the mucosal barrier; on the other hand, it binds to target cells in the mucosal tissue, activating downstream inflammatory signaling pathways such as nuclear factor kappa-B(NF-κB) and mitogen-activated protein kinase(MAPK), thereby initiating a pro-inflammatory cascade. This process increases the secretion of pro-inflammatory factors and promotes the recruitment and activation of immune cells, exacerbating inflammation. Current research extensively explores the correlation between the Th17/IL-17 axis and the pathogenesis and progression of OLP. This paper aims to review these developments to provide a research foundation for further elucidating the immunological mechanisms of OLP. Literature review results indicate that upregulation of Th17 and IL-17 in local lesion tissues and peripheral blood of OLP patients may be a key molecular event in the development of OLP. Compared to non-erosive OLP, higher expression levels of Th17 and IL-17 in the tissues and blood of patients with erosive OLP suggest a positive correlation between Th17/IL-17 and disease severity. Clinical studies demonstrate that targeted drugs against the Th17/IL-17 axis, by directly blocking IL-17 or inhibiting the production of Th17 cells, can effectively improve mucosal damage in OLP patients, showcasing potential as a new target for immune therapy. However, whether Th17 and IL-17 influence the pathogenesis of OLP by regulating the oral microbiome remains unclear. In summary, the Th17/IL-17 axis holds potential value as a new target for the immune therapy of OLP, warranting further in-depth research into its biological functions and signaling mechanisms within the inflammatory process of OLP.

Objective By summarizing the literature on the treatment of bronchial asthma with"Tonifying the spleen to help the lung"as the guiding ideology and"tonifying spleen drugs"published in recent 25 years,analyzing the domestic research situation,to understanding the research status of the mechanism of"Tonifying the spleen to help the lung"in the treatment of asthma,so as to provide ideas for the research of traditional Chinese medicine in the treatment of asthma.Methods Taking"Tonifying the spleen to help the lung","tonifying the spleen","strengthening the spleen","transporting the spleen","benefiting the spleen","Shenling Baizhu Powder","Yigong Powder","Ginseng Schisandra Decoction","Maimendong Decoction","Liujunzi Decoction"and"asthma"as the keywords,searching the literature in the Chinese Journal Full-text Database(CNKI),extracting the data recorded in the literature,including prescriptions,drug composition,test indexes and experimental results,and summarize and analyze the data.Results A total of 54 literatures were included in this literature study.The prescriptions of"Tonifying the spleen to help the lung"method in the treatment of asthma are complex.The classic prescriptions often involved include Liujunzi Decoction,Yupingfeng Powder,Yigong Powder,Buzhong yiqi decoction,Shenling baizhu powder,Maimendong decoction,Ginseng Schisandra decoction and Erchen decoction.The commonly used drugs include Atractylodes macrocephala,Astragalus membranaceus,Poria cocos,licorice,tangerine peel,etc.Experimental research mainly involves inflammatory cells,immunoglobulins,cytokines,signaling pathway,T cell subsets,cell receptors,intestinal flora and other aspects,and the specific detection indicators are complex and diverse.Conclusion The method of"Tonifying the spleen to help the lung"is effective in the treatment of bronchial asthma.Many studies show that its effect has a clear scientific basis,but its mechanism is complex and diverse and is gradually becoming clear.
