1.Correct measurement of the posterior wall size of adult male inguinal hernia used for herniorrhaphy

BACKGROUND:The mesh size used for herniorrhaphy mainly depends on clinicians' experience, which is less reliable. OBJECTIVE:To obtain the mesh size by measuring the posterior wall of inguinal hernia in Chinese adult men undergoing Rutkow surgery, and to assess the postoperative effect. METHODS:The posterior wall size of inguinal hernia in 180 Chinese adult male patients undergoing tension-free hernioplasty was measured, and then the optical mesh size was calculated. All patients were equivalently randomized into two groups, followed by subjected to Rutkow surgery according the calculated mesh size (experimental group) or subjective size (control group). Clinical indexes, the incidence of complications and recurrence rate were compared between groups. RESULTS AND CONCLUSION:The distance of pubic tubercle to internal oblique inguinal ligament starting point was (6.03±0.58) cm;the vertical distance of the highest point on the edge of the arcuate to inguinal ligament was (3.02±0.32) cm;therefore, the length of mesh should be larger than (13.04±0.74) cm, and width larger than (5.88±0.52) cm. The time of off-bed, dieting and hospitalization in the experimental group were significantly less than those in the control group (P<0.05);the time of operation and recovery of normal activities showed no significant difference between groups (P>0.05). The rates of postoperative complication and recurrence in the experimental group were significantly lower than those in the control group (P<0.01). These results suggest that after measuring the posterior wall size of inguinal hernia, the Rutkow used for inguinal hernia repair, obtains desired treatment outcomes with low incidence of complications and recurrence.
2.Efficacy of Prolene 3D patching in tension-free hernioplasty for femoral hernia via femoral approach

A total of 114 cases of femoral hernia undergoing 3D Prolene patching via femoral approach were examined for duration of surgery , length of stay , recurrence , postoperative pain and complications.Among them, 53 cases received both pre-and post-operative Doppler ultrasonic examinations.The vein thrombus ,diameter and flow rate of femoral vein were observed to compare with the pre-operative data to learn whether it had difference or not.The operative duration was 13.3 minutes.There was no event of recurrence ,compression,stricture or thrombosis of femoral vein.
3.Polymorphisms of the thymidylate synthase gene promoter in primary colon cancer and its relationship with clinical pathological parameters

Objective To investigate the relationship between polymorphisms in the thymidylate synthase promoter enhancer region(TSER) and clinical pathological characteristics of primary colon cancer.Methods TSER polymorphisms and thymidylate synthase(TS) protein were detected by PCR and immunohistochemical staining respectively in primary foci.Results There were three genotypes of TSER including homozygous with double tandem repeats(2R/2R),homozygous with triple tandem(repeats)(3R/3R) and heterozygous with both alleles(2R/3R)in 68 cases of colon cancer(6,40,22 cases respectively).The ratio of 3R/3R was significantly higher in stage of Dukes C than stages in Dukes B and Dukes A.(Dukes A16%;Dukes B30%;Dukes C 93%,?~2=28.56,P
5.Determination of Lead in Cosmetics by Potential Stripping Voltammetry

Objective To establish a method for determination of lead in cosmetics. Methods Samples were digested. The lead content was detected by isochronous mercury-plating method with the condition of lead cation being enriched for 40 s at -1.20V(vs. SCE). Results The stripping potential peak of lead appeared at - 0. 20 V( vs. SCE) . The detection limit was 0.03 jug Pb. Six parallel determinations of a sample were done with the mean of 3.574mg/ kg and the relative standard deviation of 4. 1% . Recovery rates obtained by standard addition test ranged from 90% -100.5% . Conclusion The method is suitable for detecting lead in cosmetics.
6.Application of Grafting Vessels for Coronary Artery Bypass Grafting

Objective To summarize the experience of obtaining and maintaining grafting vessels correctly in order to prevent grafting vessels from occlusion and assure the success for myocardial revascularization.Methods The grafting vessels was obtained by the method of "no touch", active anticoagulant therapy and maintaining normal level of blood pressure and blood glucose after operation.Results Stable postoperative hemodynamic parameters and myocardial functional improvement without perioperative myocardial infarction and pectoris angina occurred in 15 patients with coronary heart disease.Conclusions Manipulating carefully and gently by the method of "no touch"and active anticoagulant therapy play an important role in the prevention of grafting vessels occlusion and the success of coronary artery bypass grafting.

8.Determination of Chloride in Drinking Water by Chloride Ion-Selective Electrode

Objective To establish a simple,rapid,accurate and practical method for determination of chloride in drinking water by chloride ion-selective electrode.Methods The chlorine ion-selective electrode was used as indicator electrode and type217saturated calomel electrode of the double salt bridge(the lower salt bridge was filled with a solution of0.1mol/L potassium nitrate)was used as reference electrode.The contents of chloride in water were calculated using the calibration curve method in the electrolyte containing0.1mol/L potassium nitrate.Results The linear range of the method was10 -5 -10 0 mol/L and the best lin ear range was10 -4 -10 -1 mol/L with r=0.9999and the slope=53.5?0.667.The detection limit,recovery rate and coefficient of variation(CV)were0.3545mg /L,94.2%-102.2%(mean98.1%)and2.8%respectively.The suitable pH for the determination of the method was2-10.In the solution containing3.545mg/L chloride,50times more BrO -3 ,NO -2 ,CH 3 COO - ,SO 42- ,F - ,CrO 42- and10times more CO 32- could not interfere with the detection,but Br - and I - were interfering factors.The re-sults obtained by the chlorine ion-selective electrode method and the national standard determination method showed no statistic differences.Conclusion The method was easy to operate,rapid,accurate and suitable for determination of chloride in water.
9.Critical ethical issues for initiation of clinical trials of xenotransplatation

There are some widely discussed issues on xenotransplatation, such as animals'moral rights, allocation of medical resources and endangering public health,etc. The article raises four critical ethical issues, e.g. harm-benefit thresholds, informed consent from research patients, informed consent from third-party groups and oversight and approval of clinical trials as the prerequisites to initiation of clinical trials of xenotransplatation.
10.Adenovirus vector-mediated RNA interference inhibits expression of vascular endothelial growth factor receptor in colon carcinoma cells and tumor growth

Objective:To study the inhibitory effects of an adenovirus(Ad)-based short hairpin RNA(shRNA) expression system on expression of vascular endothelial growth factor receptor(VEGFR) and on growth of colon carcinoma cells in vitro and in vivo.Methods: RNA interference pAd-Easy/VEGFR vector was used to transfect 293 cells via Lipofectamine 2000.The adenoviral vector carrying VEGFR was used to transfect CW2 cells and the transfection efficiency was determined by fluorescent microscope and flow cytometry.The expression of VEGFR was examined by RT-PCR and Western blotting.The cell growth was observed with MTT method and the growth curve was plotted.Nude mouse was transplanted with CW2 cells to establish colon cancer tumor model and the growth of tumor was observed daily.Micro-vessel density(MVD) was detected by immunohistochemistry.Results: The recombinant pAd-Easy carrying shRNA against VEGFR was verified by sequencing.Flow cytometry showed that the transfection efficiency of CW2 cells was 99.7%.RT-PCR and Western blotting showed that the expression of VEGFR in pAd-Easy/VEGFR group was obviously decreased.The growth curve showed that the cell growth in the pAd-Easy/VEGFR group was obviously slowed down.We also found that the tumor growth in the nude mouse model was obviously inhibited and the MVD was also decreased.Conclusion: pAd-Easy/VEGFR-mediated VEGFR shRNA can effectively inhibit the expression of VEGFR in CW2 cells and suppress tumor growth in vivo.
