2.The anal verge: localization with multi-slice spiral CT

Objective To determine and evaluate the method of localization of anal verge by mutislice spiral CT. To provide an imaging reference for operative guidance of low-rectal cancer. Methods Forty eight consecutive adult patients suspected of abnormalities other than rectal disease were evaluated with abdominal and pelvic CT scans since August, 2009. They were divided into two groups based on sex and age. There were 23 men and 25 women. The ages of young group were 28 to 50 years and the average age was 41 years. The ages of elderly group were 52 to 81 years and the average age was 64 years. A small cotton ball dipped with contrast media was put at the anal verge as a marker and CT scans were performed with 64-slice spiral CT scanner. The distances between the cotton balls and the lower margin of the pubis combination (La), the lower margin of the 5th sacral vertebra (Lb), the inferior aperture of minor pelvis(Lc) and the lower margin of the basement of external anal sphincter (Ld) were measured on the midsagittal images obtained by MPR. The averages, the standard deviations(s), the 95% and 80% confidence intervals of La, Lb, Lc and Ld were calculated. We took the intervals of ± 1.96 s or ± 1.28 s < 5 mm as the standard of meeting the clinical requirement. The distances meeting the clinical requirement between different sex groups or age groups were compared with t test. Results The Ld was (9.8 ± 1.2) mm, ofwhich intervals of ± 1.96 s and ± 1.28 s were 4. 8 mm and 3.0 mm respectively, meeting the clinical need.This distance was also measured conveniently. There were no significant differences ( P > 0.05 ) between two different sex groups [ male group, ( 10.0 ± 1.2) mm], female group, (9.6 ± 1.2) mm and between two age groups [ young group, (9.6 ± 1.2) mm, elderly group, ( 9.9 ± 1.3 ) mm ]. Conclusions The lower margin of the basement of external anal sphincter was a useful anatomic lanmark for localizing the anal verge,and could be definitely identified on the middle sagittal pelvic CT image. The distance between the structure and anal verge is constant enough and can be used in measuring distance from low rectal lesion to the anal verge.
3.Effect of community nurse training in peripherally inserted central catheter on nursing quality of tumour patients with indwelled catheter during intermission of chemotherapy

Objective To evaluate the effect of community nurse training in peripherally inserted central catheter (PICC) on community nursing quality of tumour patients with indwelled PICC during the intermission of chemotherapy.Methods A prospective study was performed with a total of 220 patients with esophageal or lung cancer enrolled from September 2011 to March 2013 in the Department of Cardiothoracic Surgery,the First Affiliated Hospital of Chongqing Medical University.The 220 patients included 174 males (79.1%) and 46 females (20.9%),at the age of (61.1 ±9.0) years.With the start of community nurse training as the cut-off line,the patients were divided into pre-training group (n =119) and post-training group (n =101).PICC-related complications and patient satisfaction were compared between the 2 groups.Results The overall PICC-related complication rate was 20.9% (46/220) during community nursing,including,in the order of declining incidence,infection (7.2%,16/220),and venous thrombosis (4.5%,10/220),catheter occlusion (3.6%,8/220),catheter dropping (2.7%,6/220),and leaking/damage (2.7%,6/220).The total complication rate and incidence of infection in the post-training group were both significantly lower than those in the pre-training group (14.9% vs.26.1%,P =0.047 ; 3.0% vs.10.9%,P =0.035),and patient satisfaction (including satisfaction for answers to questions about PICC,feelings about PICC indwelling and PICC position,and overall satisfaction) was significantly higher than in the pre-training group (P =0.003,P =0.011,P =0.001).Conclusion Community nurse training programmes have positive effects on improving PICC-related nursing and patient satisfaction in cancer patients with PICC indwelling during intermissions of chemotherapy.
4.Metabolic disturbance and cardiovascular events in primary aldosteronism:recent advance

Metabolic disturbances such as hypertension and insulin resistence,as well as cardiovascular events are more frequent in primary aldosteronism patients than in essential hypertension patients.When the diagnosis of primary aldosteronism is made,more attention should be paid to the associated metabolic disturbances and cardiovascular events,which may benefit the prognosis of patients.
5.FURTHER OBSERVATIONS ON THE DEVELOPMENT AND CONNECTIONS OF THE MESENCEPHALIC NUCLEUS OF THE TRIGEMINAL NERVE IN HUMAN BRAIN

Serial sections of human embryos and the brain stem together with cerebellum offetuses, young and adults were studied by Cajal I and Nissl methods for demonstratingthe development and connections of mesencephalic nucleus of the trigeminal nerve. It was found that at C.R. 12.4 mm the mesencephalic root with some scattered cellgroups made its appearance along the sulcus limitus from the entrance of the trigeminalnerve up to the isthmus region. At C.R. 56mm it extended into the midbrain regionand the base of the cerebellum. Later on, the root cells differentiated in a caudo-rostraldirection. They were therefore somewhat larger in its caudal part than those in the ros-tral. With the increase of age the morphology of the mesencephalic root cells alsochanged from the fetal multipolar type to the adult unipolar. The present study confirm the previous observations that the mesencephalic root hadconnections with some cranial nerves and the cerebellum. And in addition, some scat-tered mesencephalic root cells were found in the posterior commissure, in its nucleus andin the interstitial nucleus. Discussion on their functional significance were made.
6.A SOMATOTOPICAL LOCALIZATION WITHIN THE RED NUCLEUS WITH RESPECT TO THE RUBROSPINAL TRACT IN THE RABBIT

17 young healthy rabbits weighing from 520 to 720 gins,ageing 38~48 postnatal days,were used for this study.Seven to fourteen days following spinal lesions at diffe- rent levels the animals were killed and serial transverse sections of thc brain stem were stained with thionin,the retrograde cell degenerations within the red nucleus were studied. An analysis of the chromatolysis indicates that the rubrospinal fibers are largely crossed and a few fibers to the cervical cord are uncrossed.Furthermore,a somatotopical localization exists within the contralateral red nucleus with respect to the rubrospinal tract,i.e.the cervical projections of the fibers system take origin from the dorsal part of the red nucleus almost throughout its whole extent,the thoracic projections from the ventral part of the caudal five-sixths of the red nucleus,and the lumbar projections from the centro-ventral part of the caudal third of the nucleus. It also indicates that all types of cells in the red nucleus,not only the large ones, give rise to the rubrospinal tract.The spinal porjections of this fiber tract may descend as far as the lumbar cord.

Objective To explore the curative effect of dissection steel plate treatment in humerus near end bone fracture curative effect.Methods From June 2000 to March 2006,dissection steel plate treatment was used in 31 cases of humerus near end bone fracture and Constant-Murley scores was used to evaluate the curative effect.Results All patients were made a follow-up for 10 to 24 months,the average 16.5 months.31 examples surgeries patient only superior 16 examples,good 10 examples,may 2 examples,difficult 3 examples.Conclusion Center 3 and 4 humeruses near ends bone fracture uses the dissection steel plate treatment to Neer dividing will be possible to obtain the good clinical effect.
8.An Improved Experimental Myocardial Ischemia Reperfusion in Rat Model

[Objective] To improve the way of establishing myocardial ischemia reperfusion rat model.[Methods]30 SD rats,quality(220?20)g,male,randomly split into normal model group and improved model group,improved established model ligation with a 0.6mm diameter line,and drawn after 30min.Compare such a method with normal model.[Results] In the context of keeping experimental stability,the reduced heart rate,wake time and survival rate of the way of improved established model respectively are(50?20)/min,(15?5)min,86.67%,but it is just(120?20)/min,(80?20)min,66.67% of the normal model group.[Conclusion]The improved myocardial ischemia reperfusion model offers significant superiority to the former one.
9.The value of contrast-enhanced ultrasound guiding normal saline injection in the detection of residual common bile duct stones

Objective To investigate the diagnostic value of contrast-enhanced ultrasound (CEUS) guiding normal saline (NS) injection through endoscopic nasobiliary drainage duct(ENBD) on evaluation for residual stones in common bile duct.Methods Fifty-five patients with bile duct stones were treated by endoscopic retrograde cholangio-pancreatography (ERCP) and duodenoscopic sphincterotomy incision surgery (EST) and ENBD.All patients received normal ultrasonography and CEUS guiding NS injection ultrasonography after EST.The length and width of common bile duct and the detection rate of residual stones before and after NS injection were compared.Results In the 55 patients,1 patient failed in injection of contrast agent into the ENBD.In the other 54 patients,the difference of the length and width of common blie duct before and after NS injection were statistically significant [(2.94±1.76)cm vs (6.09±1.46)cm,(0.58±0.30)cm vs (1.11±0.98)cm](all P<0.001).The full display rate of the common bile duct before and after NS injection were 13.0%(7/54) and 90.7%(49/54),respectively.Before injection,none of common bile duct stones was suspected.After injection,5 cases of common bile duct stones were suspected.Three cases were confirmed by ERCP,1 case was confirmed by operation and 1 case was false positive.Conclusions CEUS of the common bile duct through ENBD performs its patency and course.On this basis the injection of NS increases the display rates of common bile duct,thus improving the detection rate of residual stones.
10.Physiological changes and implications during the fetal-neonatal transition

During the fetal-neonatal transition, the body must undergo many important physiological changes to adapt the extrauterine environment. After birth, the blood and energy supply through placenta is stopped with clamping of the umbilical cord and, meanwhile, the pulmonary ventilation function is established when exposure to the air, which results in a series of changes in the respiratory, circulatory and endocrine systems and energy metabolisms, etc. The physiological transition can be relfected in heart rate, blood pressure, oxygen saturation, temperature, and other physiological indicators. The changes of these indicators can be used as references for prevention, diagnosis and treatment of neonatal diseases. This review provides an overview of physiological changes and implications in the lung function, circulatory and endocrine systems, and energy metabolism during the transition at birth as well as intervention measures for abnormal fetal-neonatal transition.
