1.Research Progress in Acupuncture in Treating Chemotherapy Induced Nausea and Vomiting

This article summarized clinical studies and mechanism research literature on acupuncture in treating chemotherapy induced nausea and vomiting in the latest 20 years. It found that the domestic and foreign researches tend to regard acupuncture as an effective method for the treatment of chemotherapy induced nausea and vomiting, which still requires further high-level clinical researches to verify. Acupuncture may prevent and treat chemotherapy induced nausea and vomiting through multiple mechanisms, and studies on these mechanisms show extensive research prospect.
2.The model of relationships between factors influencing physical activity under the theory of social ecological model in patients with type 2 diabetes

Objective To explore the mechanisms of individual, social and environmental factors influencing physical activity and the relationships among the factors in patients with type 2 diabetes. Methods A survey was conducted from January to July 2015 among 225 patients with type 2 diabetes using convenience sampling. The questionnaire included 9- item Patient Health Questionnaire, the International Physical Activity Questionnaire,Perceived Social Support Scale, Chinese Walkable Environment Scale, Bandura′s Exercise Self- efficacy Scale. Results The effective questionnaire recovery rate was 98.7%(222/225). Exercise self- efficacy was a mediator of factors influencing physical activity. Exercise self- efficacy and physical activity were positively correlated (β=0.24, P<0.05). Walkable environment only affected physical activity indirectly medicated by exercise self- efficacy (β=-0.26, P<0.05). Perceived social support directly associated with physical activity (β=0.14, P<0.05). Depression not only affected physical activity directly (β=-0.28, P<0.05), but also associated with physical activity indirectly medicated by exercise self-efficacy (β=-0.32, P<0.05). Conclusions Interventions should be taken to increase the perceived social support, improve depression and walkable environment, especially to enhance exercise self- efficacy, so as to improve the physical activity level of patients with type 2 diabetes.
3.CLINICAL COMPARISION OF THE TREATMENT OF CERVICAL PROLAPSE OF INTERVERTEBRAL DISC BY USING CAGE OR TITANIUM PLATE

Objective To compare the clinical effects of treating cervical prolapse of intervertebral disc by using Solis cage, titanium cage and autogenous iliac crest graft (AICG) combined with titanium plate. Methods 64 cases of cervical prolapse of intervertebral disc were analyzed retrospectively. All the patients were followed up for about one year. Group A is composed of 20 patients (30 intervertebral spaces) treated with microdiscectomy and Solis cage fusion;group B is composed of 21 patients(22 intervertebral spaces) treated with titanium cage and group C is involved of 23patients(28 intervertebral spaces) which were dealt with AICG combined with titanium plate. The differences in the to-tal X ray exposure time, time working on iliac bone, incidence of complications, fusion rate, incidence of JOA score recovery rate more than 50 % and rehabilitation time of the three groups are analyzed. Results All indexes from group A and B were more satisfactory than those from group C (P<0. 05). The statistic analysis results of the incidence of complications of cervical part, rehabilitation time in group A are 5.0± 1.8, 5.1 ± 1.2; and that is 14.3±2.6, 7.5 ± 1.6and 26.1±6.2, 8.6±2.3 in group B and group C respectively. There is significant difference between group A and group B on the incidence of complications of cervical part and rehabilitation time (P<0.05). Results of the incidence of JOA score recovery rate > 50 % in group A(95.8 ± 3.2) was more satisfactory than that in group B(93.6 ± 2.9),while there was no statistical difference between group A and group B (P>0.05). Conclusion The Solis cage is better in treating the cervical prolapse of intervertebral disc than other ways. Also, it makes the operation easier, rehabilitation time shorter, and it causes fewer complications.
4.Effects of different operations on respiratory function after colon replacement in esophageal cancer

Objective To explore the effects of different operations on respiratory function after colon replacement for esophageal cancer.Methods Fourty patients with esophageal cancer underwent esophagectomy from May 2004 to June 2008,were retrospectively analyzed.The site of esophageal anastomosis was at neck after sternum in 20 cages.at neck through esophageal bed in other 20 cages.Blood gas analysis and pulmonary fuction were monitored in these patients before operation and in 14 days after operation.Results VC% were (42.17±10.15)%、(49.52±9.56)%、(55.67±10.73)%、(60.27±10.52)%in patients after sternum and(37.65±9.52)%、(40.72±10.12)%、(47.02±10.65)%、(52.89±10.82)%in patients through esophageal bed in 5th、7th、10th、14th postoperative day with sigllificant statistical difference;FEV_1% were(60.55±16.71)%、(67.12±16.90)%in patients after sternum and(45.23±16.26)%、(50.52±16.72)%in patients through esophageal bed in 10th、14th postoperative day with significant statistical difference;PaO_2 were(17.56±7.32)mm Hg、(19.67±6.08)mm Hg、(17.17±4.85)mm Hg、(15.43±5.02)mm Hg、(11.32±3.79)mm Hg、(9.67±2.87)mm Hg、(6.98±3.26)mm Hg in patients after sternum and(20.17±7.04)mm Hg、(22.83±6.75)mm Hg、(20.67±4.31)mm Hg、(18.32±4.85)mm Hg、(16.02±3.71)mm Hg、(13.44±2.56)mm Hg、(9.01±3.17)mm Hg in patients through esophageal bed in 1st,2nd,3rd,5th,7th,10th,14th postoperative day with significant statistical difference. Conclusions After esophagectomy,esophageal anastomosis through esophageal bed has significant negative effect on respiratory function.
5.Efficacy of Acupuncture Point Injection for Fine Motor Functions of Children with Cerebral Palsy

ObjectiveTo investigate the efficacy of acupuncture point injection for fine motor functions in children with cerebral palsy.Methods57 children with cerebral palsy were treated by acupuncture point injection combined with occupational therapy. The efficacy was evaluated by the Peabody Developmental Motor Scale-Fine Motor (PDMS-FM).ResultsThe fine motor quotient (FMQ) of 34 children (59.7%) elevated. The raw scores of grasping subtest (Gr) elevated in 45 children (78.9%) and the raw scores of visual-motor integration (VI) elevated in 53 children (93.0%).ConclusionThe efficacy of acupuncture point injection combined with occupational therapy is significant in children with cerebral palsy.
6.Predictive value of MRI image-based scoring model for diagnosis and adverse clinical outcomes of invasive placenta accrete

Objective:To explore the predictive value of a scoring model based on MRI images for diagnosing invasive placenta accreta and associated adverse clinical outcomes.Methods:This retrospective cohort study involved 260 patients delivered at Peking University Third Hospital from January 2015 to December 2018, who were suspected to be placenta accreta with two or more ultrasound image findings and underwent MRI examination. Placenta accreta was finally diagnosed and classified based on the intraoperative clinical findings or pathological examination. Adverse clinical outcomes were defined as intraoperative bleeding ≥1 500 ml and/or having hysterectomy. Quantitative and qualitative interpretation of five MRI signs were performed, including intraplacental low-intensity band on T2 weighted imaging, abnormal intraplacental vascularization, vascularization of uterovesical interface, uterine bulging and cervical involvement. Chi-square and t test were used for univariate analysis of the five MRI signs and the receiver operating characteristics (ROC) curve of each MRI sign for predicting invasive placenta accreta and adverse clinical outcomes were drawn. The predictive value was assigned as 1 when ≥ the cutoffs that matched to the maximum Yoden index values, and was assigned as 0 when below the cutoffs. A scoring model based on the five MRI signs was established, ROC curves of the model for predicting invasive placenta accreta and adverse clinical outcomes were drawn and the area under the curve (AUC), sensitivity, specificity and Youden index were calculated. Results:(1) Univariate analysis showed that all five MRI signs were significantly associated with invasive placenta accreta and adverse clinical outcomes. Except for cervical involvement, the other four signs had an AUC value of greater than 0.5 in predicting invasive placenta accreta and adverse clinical outcomes. (2) The predictive cut-off values of abnormal intraplacental vascularization image and intraplacental dark band area on T2 weighted imaging were 2.0 cm 2 and 0.6 cm 2, respectively, and were all 1.0 for the other three signs. The AUC value of MRI signs-based scoring model for predicting invasive placenta accreta was 0.863. When the score was ≥ 2 points, the diagnostic sensitivity was 0.836 and the specificity was 0.726. The scoring model predicted adverse clinical outcomes with an AUC of 0.841. When the score was ≥3 points, the predictive sensitivity was 0.707 and the specificity was 0.818. Conclusions:The scoring model based on MRI signs is of good value for the diagnosis of invasive placenta accreta and the prediction of adverse clinical outcomes.
7.Clinical Characteristics of 100 Children with Cerebral Palsy

ObjectiveTo analysis the clinical characteristics and the high risk factors of children with cerebral palsy.MethodsClinical data of 100 children with cerebral palsy were retrospectively analyzed.Results51% of children were born with asphyxia, 42% were born prematurely, 39% were low birth body weight. 85.7% of children involved had mental retardation, 12% had hearing loss and 7% had visual impairment. 88% of children involved had abnormal cranial CT results and 46.1% had abnormal EEG. 50.0% of the children had abnormal brain stem auditory evoked potentials.ConclusionAsphyxia, prematurely birth and low birth body weight are high risk factors of cerebral palsy. Most of the children with cerebral palsy in this group are mental retarded.
8.Evaluation of viability of myocardium by adenosine stress echocardiography combined with quantitative tissue velocity imaging and tissue tracking after acute myocardial infarction

Objective To seek a new method to identify viability of myocardium by adenosine stress echocardiography combined with quantitative tissue velocity imaging(QTVI)and tissue tracking(TT).Methods Fifteen healthy canines were selected to establish the models of acute myocardial infarction and reperfusion by ligating anterior descending branch of coronary artery for 90 minutes and then releasing the artery to get reperfusion.After reperfusion.peak velocity in systole(Vs),peak velocity in isovolumic contraction(VIVC)and the displacement in systole(Ds)were measured on anterior wall and anterior septum at baseline.The 2,3,5-triphenyl tetrazolium chloride(TTC)staining was set as a"gold standard"for defining the viable and non-viable groups.The sensitivity and specificity of assessing myocardial viability were determined with QTVI and TT.Comparison of variables between viable and non-viable group was made by using t test.One way analysis of variance and LSD-t test were used to estimate the significance of differences in different states.Results Compared with baseline,Vs,VIVC and Ds decreased significantly(P<0.01)after reperfusion in both viable and non-viable group.After administration of adenosine,Vs and Ds increased(P<0.05),but VIVC didn't change significantly compared with that before administration of adenosine in viable group(P>0.05).Variables in non-viable group didn't change significantly after administration of adenosine(P>0.05).By receiver operating characteristic(ROC)analysis for predicting myocardial viability,when a cutoff value of △Vs(%)rate was 17.9,the sensitivity and the specificity was 78.6%and 81.1%,respectively,and when the cutoff value of △Ds(%)rate was 18.4,the sensitivity and the specificity was 75.0%,83.6%,respectively.Combined △Vs(%)with △Ds(%),the sensitivity and specificity to prediction of myocardium viability could reach 94.6%and 68.0%,respectively.Conclusions When the viability of myocardium after myocardial infarction is assessed by using the method of adenosine stress echocardiography with QTVI and TT,the sensitivity and the specificity are greatly enhanced.

10.The outcome of monoclonal gammopathy of undetermined significance in elderly patients

Objective To explore the outcome of monoclonal gammopathy of undetermined significance (MGUS). Methods The data from 14 MGUS patients in our hospital including clinical features, outcome and change of M protein concentration were analyzed retrospectively. Results The MGUS didn't have the clinical manifestations of multiple myeloma (MM), the time of outcome from MGUS to MM was about 4-20 years (mean time, 10 years). The most types of MM were IgA and IgG, 6 cases were IgA type, 6 cases were IgG type and 2 cases were light chain type. The concentration of immune globulin in general showed an upward trend year by year. A few showed fold lines ascend. Conclusions The elevated monoclonal immunoglobulin may develop into MM after many years. We must follow up frequently to avoid error diagnosis and missed diagnosis.
