1.Relationship between the angiotensin Ⅱ type 1 receptor gene polymorphisms and cerebrovascular disease

Objective To explore the relationship between angiotensin Ⅱ type 1 receptor (AT1R) gene polymorphisms and cerebrovascular disease (CVD).Methods The 104 patients with CVD and 98 healthy individuals were detected the AT1R genotypes polymorphisms by restriction fragment length polymorphism in order to analysis.Results CC genotype was not found both in CVD and control group. In CVD group, genotypic frequency of AA was 40.4% and AC was 59.6%. The allele frequency of A was 70.1% and C was 29.9%. In control group, genotypic frequency of AA was 91.8% and AC was 8.1%. The allele frequency of A was 95.9% and C was 4.1%. 2.The study on clinical method of measurement in the marginal fitness of crown

Objective To study the clinical feasibility of micrometer method and thin carbon paper method in the marginal fitness of crown.Methods 10 ex vivo teeth were selected to be restored by Nickel-Chromium alloy PFM crown.Selected 6 measurement sites on shoulder in each ex vivo teeth.Micrometer method,thin carben paper method and SEM method were used to measure.and the data were analyzed by SPSS 13.0.Results 5 data in micrometer method and 6 data in thin carbon paper method were different to SEM method.The disparity was not statistically significant(t=0.392 and t=0.543).Conclusion Micrometer method and thin carbon paper method were two facility quantization measurements to the marginal fitness of crown.They had satisfied to effect in clinic.
3.Relationship between the ACE gene I/D and AT1R gene A1166C polymorphisms and cerebral infarction

Objective To explore the relationship between angiotensin I-converting enzyme(ACE)gene I/D and angiotensin Ⅱ type 1 receptor(AT1R)gene A1166C polymorphisms and cereral infarction(CI).Methods ACE and AT1R genotypes were investigated with the method of PCR-RLFP in 88 patients with CI and compared with 90 age-matched population controls.Results AC genotypic frequency(31.8%)and C allele frequency(15.9%)of AT1R gene in CI group were significently higher than those in control group(11.1%,5.6%)(all P0.05).Conclusions The polymorphism of AT1R A1166C is related to the incidence of CI.There are synergistic effects of ACE DD genotype and AT1R gene A1166C polymorphisms on the risk of CI.
4.In vitro effects of aprotinin on human blood platelets

The study was designed to evaluate the in vitro effects of aprotinin on platelets functions and arachidonic acid metabolism, Venous blood was drawn from healthy volunteers, and aprotinin was added to the blood at three different dose of aprotinin:50KIU/ml,150KIU/ml(low-dose)and 300KIU/ml (high-dose), respectively. Results demonstrated that aprotinin inhibited significantly the rates of platelet adhesion and aggregation induced by ADP or thrombin. Both the inhibitions were reversible and there was not significantly difference between high-dose and low-dose. The ADP-induced activities ofphospholipase A_2 (PLA_2) and cyclo-oxygenase(CLO)of platelet membrane were significantly suppressed by prophylactic aprotinin,but were not by therapeutic aprotinin. It is suggested that administration of low-dose aprotinin during CPB is reasonable,especially gaved just before the start of CPB.
5.Epidemic trend of viral hepatitis B in Shaoxing from 2008 to 2018

Objective:
To understand the epidemiological characteristics of viral hepatitis B in Shaoxing from 2008 to 2018,so as to provide basis for formulating the viral hepatitis B prevention and control strategy.
Methods:
The data of viral hepatitis B cases in Shaoxing from 2008 to 2018 were collected from infectious diseases reporting and management system. Descriptive epidemiological method was employed to analyze the time,place and population characteristics of viral hepatitis B cases in Shaoxing. Annual percentage change(APC)was calculated to analyze the development trend of viral hepatitis B.
Results :
From 2008 to 2018,a total of 22 764 cases of viral hepatitis B were reported in Shaoxing,with an average annual incidence rate of 42.24/100 000 and a standardized incidence rate of 39.19/100 000. The incidence rate of viral hepatitis B decreased from 2008 to 2018,among which the APC from 2012 to 2018 was -18.00% with a significantly downward trend(P<0.05). Six counties,cities or districts in Shaoxing reported cases of viral hepatitis B,of which Zhuji(49.40/100 000),Yuecheng(44.17/100 000)and Xinchang(43.28/100 000)ranked top three in average annual incidence rate. The incidence rate of viral hepatitis B in men was 57.07/100 000,which was higher than 27.33/100 000 in women(P<0.05).The age of onset was mainly 25-59 years old,with 16 941 cases(74.42%)reported. The top three occupations were farmers,workers and migrant workers,with 15 013(65.95%),2 730(11.99%)and 1 121(4.92%)cases reported,respectively.
Conclusions
The incidence of viral hepatitis B in Shaoxing from 2008 to 2018 showed a downward trend. Farmers,workers and migrant workers aged 25-59 years were at high risk of hepatitis B infection.
6.Bilateral big toe wrap-around flap for reconstruction of the defects of the finger

Objective To investigate the therapeutic effect of bilateral combined flap transfer in reconstruction and repairation of thumb and finger defects.Methods A partial fibular side wrap-around flap was harvested from both big toes.The flaps were tailored and designed based on measurements taken from the finger of the intact side.These two flaps were combined to reconstruct the defects of the finger.From March,2008 to December,2015,a total of 9 fingers (9 cases) which had defect degrees range from Ⅰ ° to Ⅲ° underwent reconstruction surgeries in this way.Appearance,sensation and function of the thumb and foot flap were followed-up.Simultaneously how to improve the outcoming and success rate were explored and investigated.Results All 9 thumbs and fingers in 9 cases with these procedures were survived.No vascular crisis happened.All patients were followed up from 3 to 24 months.The reconstructed thumbs and fingers were all with abundant blood supply,having similar shape to the normal thumbs and fingers,good pain and temperature sensation,with two-point discrimination of 4-8 mm.All patients were satisfied with the thumb and finger shape,regained life and work ability as before.The donor sites had no obvious discomfort,and walking and weight-bearing function remained normal.Judged by the tentative evalution standard of function of upper limb,which was made by Chinese Medical Association for Surgery of the Hand.The results were excellent in 6,good in 2,fair in 1.Conclusion There is less damage on foot and most hand function recovery using bilateral combined flap transplantation to repair multiple fingers defect.
7.Effects of early invasive mechanical ventilation on anoxia and prognosis of acute respiratory distress syndrome after chest injury

Objective To explore the clinical significance of early invasive mechanical ventilation for acute respiratory distress syndrome (ARDS) caused by thoracic trauma.Methods We retrospectively analyzed the clinical data of 93 patients of invasive mechanical ventilation in those with ARDS caused by thoracic trauma,and divided them into delayed invasive mechanical ventilation group (n =45) and timely invasive ventilation group (n =48) by the time of invasive mechanical ventilation.We compared the blood gas analysis,oxygen index,heart rate,lactic acid,muscle use score of the two groups before and after invasive mechanical ventilation and the differences of mechanical ventilation time,incidence of complications,mortality and length of hospital stay between the two groups.Results Each index of the delayed invasive mechanical ventilation group and the timely invasive ventilation group was calculated:PaO2 (63.2 ± 12.3,74.2 ± 12.1)mm Hg,PaCO2 (39.4 ±8.7,38.9 +7.6) mm Hg,PaO2/FiO2 (196.2 ± 17.8,231.4 ± 18.2),respiratory rate (27.4 ±3.7,21.1 ± 3.4) beats/min,heart rate (117.2 ± 26.8,96.0 ± 25.1) beats/min,accessory muscle use score (2.69 ± 0.15,1.67 ± 0.10),lactic acid (4.23 ± 1.70,2.12 ± 0.80) mmol/L Except for PaCO2,the differences of each index were statistically significant (t values were respectively 4.346,9.420,3.949,5.436,38.809,7.736; P < 0.05).The duration of mechanical ventilation treatment (10.21 ± 1.50,6.47 ± 1.20) d and the length of hospital stay (21.53 ± 1.90,16.32 ±2.10) d of the delayed group and the timely group were significantly different (t values were respectively 12.518,13.318; P < 0.01).The timely group had significantly lower mortality rate (26.7% (12/45),14.6% (7/48)),VAP (46.7% (21/45),22.9%(11/48)),acute gastric mucosal lesions (33.3% (15/45),12.5% (6/48)),MODS (40.0% (18/45),16.7% (8/48)) than the delayed group (x2 values were respectively 3.86,5.81,4.49,5.70; P < 0.05).There is no significant difference on the incidence of pneumothorax between the two groups (11.1% (5/45),8.3% (4/48)).Conclusion Invasive mechanical ventilation is an effective treatment of ARDS caused by chest trauma.Early invasive mechanical ventilation can reduce the mortality rate and shorten the duration of mechanical ventilation and hospital stay of the patients with ARDS after severe thoracic trauma.
8.Effects of miR-335-5p on the osteoblast function in high glucose condition

Objective To investigate the effects of miR-335-5p on the proliferation and apoptosis of osteoblasts which were exposed to high glucose condition, and explore its possible molecular mechanisms. Methods MC3T3-E1 osteoblasts were divided into four groups:control group(5. 5 mmol/L glucose), high glucose group(HG group, 22. 0 mmol/L glucose), agomir-335-5p group(transfected with agomir-335-5p and exposed to 22. 0 mmol/L glucose) , and agomir negative control group( agomir NC group, transfected with agomir negative control and exposed to 22. 0 mmol/L glucose), cultured for 7 days. Cell proliferaton, cell apoptosis, expressions of miR-335-5p and dickkopfhomolog1(DKK1)mRNA,proteinlevelsofDKK1andcysteinylaspartate-specificproteinase-3(caspase-3) were detected using MTT, flow cytometry, quantitative realtime PCR and western blot, respectively. Results Compared with control group, the expression of miR-335-5p mRNA and cell proliferation in HG group were significantly decreased(P<0. 05), while cell apoptosis and the protein levels of DKK1 and caspase-3 were increased significantly(P<0. 05), the expression of DKK1 mRNA did not change (P>0. 05). The miR-335-5p mRNA expression and cell proliferation in agomir-335-5p group were higher than those in HG group and agomir NC group(P<0. 05). However, Cell apoptosis and the protein levels of DKK1 and caspase-3 in agomir-335-5p group were lower than those in HG group(P<0. 05). Conclusion High glucose inhibits the proliferation and induce the apoptosis of MC3T3-E1 osteoblast through decreasing the expression of miR-335-5p and subsequently increasing the DKK1 expression.
9.Influence of bone quality on initial stability of implantable distraction A three-dimensional finite element analysis

BACKGROUND: The initial stability of implantable distractor depends on the stress distribution of bone-distractor interface. The understanding of the biomechanical change in initial stage can improve the clinical success ratio of implantable distractor used in alveolar crest.OBJECTIVE: To evaluate the influence of bone quality on stress distribution and deformation in initial distraction stage.METHODS: Four three-dimensional models with 10 079-11 456 cells and 17 299-20 101 nodes were prepared by finite element methods (11 mm in length and 3.7-4.1 mm diameter). Implantable distractor was embedded in a segment of mandible. The elastic modulus of cancellous bone and the thickness of cortical bone, stress and deformation of bones and distractor were calculated.RESULTS AND CONCLUSION: The highest stress in the bone was concentrated in transportable section and the maximum deformation of transportable section was observed at the edge of the cortical bone, both of which were increased with bone quality decreased. The subsidence of distractor was observed with bone quality decreased. Bone quality influenced the initial stability and the result of the implantal distraction. The decrease of bone elastic modulus would increase the failure risk of distraction osteogenisis.
10.Programming of alveoli-implant finite elemental modeling software

BACKGROUND: The finite element method has been wildly used in stomatology,but,the modeling methods and models were different and lack of standard.There is not biomechanical analysis software which specialized indental implantology.OBJECTIVE: To compile software that can generate alveoli-implant complex finite element models.METHODS: Based on PC,CAD,finite element analysis,database and electronic table software,software that includes the functions of modeling,database and assembles was programmed using Visual Basic for Application programming language.RESULTS AND CONCLUSION: Various kinds of alveoli-implant models could obtain though the software.A software package that can generate alveoli-implant finite element models rapidly and automatically could be compiled with the complex of AutoDesk Mechanical Desktop 2009DX,ANSYS Workbench 10.1 and Visual Basic for Application.
