1.Immunomodulatory effects of cryolytic lung cancer cells on bone marrow-derived dendritic cells

BACKGROUND: Cryoimmunology has been paid more attention in recent years, but the effect of cryoablation on cellular function has not been clearly understood, especially for the cellular apoptosis and dendritic cells.OBJECTIVE: To evaluate whether NCI-H446 lung cancer cells could activate bone marrow-derived dendritic cells (DCs) which stimulate lymphocytes to be cytotoxic lymphocytes (CTLs) to kill cancer cells?DESIGN, TIME AND SETTING: Cellular immunology experiment was performed at the Hematological Laboratory, Beijing Naval General Hospital, and Genetics Center of Military Medical Science Academy of the PLA between August 2002 and April 2003.MATERIALS: NCI-H446 cells were small lung cancer cells purchased from Shanghai Institute of Sciences.METHODS: Cultured lung cancer cells were cryo-treated and added to cultured DCs in the presence of naive lung cancer cells. Morphological changes of cancer cells were observed; mixed lymphocyte reaction (MLR) and antitumor activity of cytotoxic T lymphocyte (CTLs) were also evaluated.MAIN OUTCOME MEASURES: Morphology of DCs and cell surface immunophenotype changes; MLR of cultured cells and cell apoptosis.RESULTS: ① of 51 patients,18 were followed-up for 6-12 months,2 for 13-18 months,and 8 for 19-24 months.② A total of 49 (96.1%) out of 51 patients preserved the eyeballs and the visual acuity improved to different degrees in 48 (94.1%) patients.③ After surgery,fungal infection recurred in 6 eyes (11.6%),4 of which were controlled by antifungal medication and 2 was enucleated because of uncontrolled endophthalmitis.Graft rejection was found in 18 (35.3%) eyes, 13 of which recovered transparent by medication and 5 received secondary PKP.Graft ulceration was present in 4 (7.84%) eyes, 3 of which were cured and the remaining one was re-grafted because of severe endothelial cell loss. Secondary glaucoma appeared in 7(13.7%) eyes,and the intraocular pressure was controlled medically and surgically.Complicated cataract occurred in 6 (11.8%) eyes,3 of which underwent cataract extraction. Most complications were successfully controlled.In the final follow-up period,45 (88.2%) grafts were transparent.CONCLUSION: Cryo-treated lung cancer cells exhibited cell lysis and membrane collapse, which activate bone marrow-derived DCs to kill cancer cells specifically.
2.Clinical efficacy of laparoscopic and open method in the treatment of gastric cancer radical surgery

Objective To compare the clinical efficacy of laparoscopic and open method in the treatment of gastric cancer radical surgery.Methods 1 1 0 patients who underwent radical surgery for gastric cancer were selected. According to its different operation method,they were divided into observation group and control group,55 cases in each group.The observation group received laparoscopic surgery,and the control group received open operation treat-ment.The general surgery situation,the C -reactive protein (CRP)levels at preoperation and postoperative 1 ,7,1 0d were compared in the two groups.Results The surgical incision length,intraoperative blood loss,postoperative bowel movements and postoperative hospital stay in the observation group were (24.6 ±3.7)cm,(99.9 ±31 .8)mL,(5.8 ± 2.7)d,(1 2.8 ±7.7)d respectively,which were significantly lower than (5.6 ±1 .4)cm,(61 .3 ±1 3.6)mL,(4.5 ± 0.7)d,(9.2 ±3.9)d in the control group(t =31 .472,5.289,3.1 94,2.398,all P <0.05),the number of the lymph node in the two groups had no significant difference(P >0.05).CRP level at 1 d preoperation,1 d postoperation,7d postoperation,1 0d postoperation of the control group were (1 8.21 ±3.67)mg/L,(42.96 ±7.28)mg/L,(22.88 ± 4.35)mg/L,(15.89 ±4.34)mg/L respectively,which of the observation group were (17.82 ±3.57)mg/L,(29.97 ± 4.58)mg/L,(1 6.01 ±3.1 6)mg/L,(8.65 ±1 .77)mg/L respectively,it had no significant difference between the two groups at preoperative 1 d (P >0.05 ),postoperative 1 d both elevated obviously,postoperative 7 d,1 0 d both significantly dropped than postoperative 1 d,and the differences between the two groups were significant (t =7.497, 6.521 ,8.339,all P <0.05).The incidence rate of complications was 5.45%(3 /55)in the observation group,which of the control group was 1 8.1 8%(1 0 /55),the difference between the two groups was statistically significant(χ2 =6.371 ,P <0.05).Conclusion Compared with the traditional laparotomy,laparoscopic gastric cancer radical has mild trauma,rapid recovery,the effect is good,quite cure degree,which is worthy of clinical popularization and application.

4.Hematologic manifestations of immunodeficiency disease

6.Clinical observation about 18 cases of polymyalgia rheumatica by using Biqi capsule

Objective:To compare the effect of curing polymyalgia rheumatica either by using both Biqi capsule and prednisone or by using prednisone alone. Methods: Thirty-nine patients of polymyalgia rheumatica were randomly divided into two groups, 18 patients in the treatment group treated with Biqi capsule and small dosage of prednisone; 21 patients in the control group treated with small dosage of prednisone. The therapeutic course was 12 weeks. The patients' symptoms of muscle ache, inflexibility in the morning and T-cell subsets determination (CD4, CD8)were observed and scores of clinical practice was made. Results: The respective total effective rate was 94.44% for the treatment group and 66.67% for the control group. There was distinctive difference in the scores for the treatment group and the control group in relieving the patients of the symptoms of polymyalgia rheumatica. The treatment group would help the patients of polymyalgia rheumatica to improve their immunity ability. Conclusion: The combination of Biqi capsule and prednisone can help to improve the effect of curing polymyalgia rheumatica.
7.Clinical analysis of mirror therapy combined with rehabilitation training on stroke patients upper limb motor function

Objective:To evaluate the curative effect of mirror therapy combined with rehabilitation training in the treatment of stroke hemiplegic upper limb motor dysfunction. Methods:To choose 48 patients, were randomly divided into control group, 24 cases in each group to observe, are basic rehabilitation training, observation group joint mirror therapy, using FMA, ARAT, BMI, respectively, evaluation of patients with general motor function, upper limb motor function, daily life ability, numerical measurement of radial carpal extensor emg. Results:After the treatment, the control group, the observation group the FMA level, ARAT level, MBI level, EMG level was higher than that before treatment, the observation group than that in the control group, the observation group FMA, ARAT, MBI, the difference of EMG signals is higher than that of control group, with significant difference(t=14.14,t=4.55,t=5.84, t=9.14;P<0.05).Conclusion: mirror therapy combined with rehabilitation training is really helpful to upper extremity motor functional recovery of stroke patients with hemiplegia, is of positive significance to improve the living ability of the patients.
8.The myocardium protection of the joint application of adenosine and cold-blood cardioplegia containing potassium in coronary artery bypass grafting

One hundred patients underwent coronary artery bypass grafting and thirty patients underwent replacement of mitral valve were included in this study, and patients were randomized to two groups. Adenosine group (group I):6 mg adenosine was diluted with physiological saline and injected from the root of the ascending aorta after blocking it. The aorta was then perfused with high-potassium cold-blood cardioplegia. Control group (group II):the aorta was just perfused with high-potassium cold-blood cardioplegia. Adenosine was only added in first perfusion in adenosine group. Both groups were reperfused half-amount of cold-blood cardioplegia containing potassium every 30 minutes. Relevant clinical indexes and myocardial enzymological determination were compared between two groups. Results For patients who underwent CABG, the cardiac arrest induced time,ICU dwell and assisted ventilation time after surgery and dopamine usage were all less in adenosine group than those of control group (P < 0.05). There were no significant differences in the total number of cardioplegia of perfusion fluid, the total amount of perfusion, clamping aorta and assisting circulation times and automatic rebeating between two groups (P>0.05). For patients who underwent MVR, the cardiac arrest induced time and dopamine usage were all less in adenosine group than those of control group (P<0.05). There were no significant differences in times and total number of cardioplegia of perfusion fluid, clamping aorta and assisting circulation times, ICU dwell and assisted ventilation time after surgery automatic rebeating between two groups (P>0.05). For both operations, 4 hours after clamping aorta, creatine kinase isoenzyme (CK-MB) was less in adenosine group than that of control group (P<0.01 or P<0.05), and 4 hours and 24 hours after bypass, cardiac troponin I (cTnI) was significantly higher in control group than that of adenosine group (P < 0.05). Conclusion After aorta is clamped, immediate injection of adenosine diluent and cold-blood cardioplegia containing potassium from the root of the ascending aorta can lead to quick heart arrest, reduce the release of myocardial enzymes and dosage of vasoactive agents, and shorten ICU dwell and assisted ventilation time.
9.Microsurgical resection of large acoustic neurinoma through suboccipital retrosigmoid approach

Objective To evaluate the safety and efficacy of microsurgical resection of large acoustic neurinoma via the suboccipital retrosigmoid approach.Methods Forty-nine patients with large acoustic neurinoma(≥4 cm) underwent microsurgical resection through suboccipital retrosigmoid approach.The craniotomy was performed by way of an unilateral S-shaped suboccipital incision.With microsurgical techniques the outmost layer of the arachnoid membrane was preserved in order to avoid damaging to the surrounding vital structures.The tumor was resected intracapsularly from the superior pole and the internal auditory meatus was finally opened.The last pieces of tumor were removed by sharp dissection from the facial nerve bidirectionally,and were resected cautiously in a piecemeal fashion.Results Of the 49 patients,45 patients(92%) received a total resection of the tumors,and 4 patients(8%) subtotal resection.No patients died.The facial nerve was preserved anatomically in 42 patients(86%) and functionally in 36 patients(73%).The acoustic nerve was preserved anatomically in 7 patients(14%) and functionally in 3 patients(6%).One patient experienced a postoperative haematoma,and a re-operation was required.No recurrence was seen in 37 patients during a follow-up for 6 months ~ 5 years(mean,2.8 years).These patients had recovered for normal work and daily activities.Conclusions Microsurgical operation through suboccipital retrosigmoid approach is a favorable treatment for large acoustic neurinomas.The procedure can improve the rate of total resection,decrease the morbidity and mortality,and effectively protect the function of the acoustic and facial nerves.
10.Pterional approach microsurgical removal of giant tuberculum sellae meningiomas

Objective To summarize the experience of pterional craniotomy microsurgical resection of giant tuberculum sellae meningiomas. Methods A total of 18 patients with giant tuberculum sellae meningiomas were treated microsurgically in this hospital from February 2000 to November 2004. The patients were operated on by use of pterional craniotomy on the side of worse vision. The basal part of the tumor was firstly dissected to control the blood supply of the lesion. Through the cerebral cisterns at the sellar region and the interfaces between the tumor and the adjacent structures, the tumor was removed to the greatest possible extent with minimal invasion to the neighbouring structures. Results[WTBZ] A total resection was conducted in 16 patients (88.9%), and a subtotal resection, in 2 patients (11.1%). No surgery related death was observed. Follow-up examinations in 14 patients for 3 months ~ 4 years (mean, 2.5 years) found no recurrence of meningiomas. Conclusions[WTBZ] The pterional approach provides excellent exposure of the middle fossa and the parasellar area. Pterional approach microsurgical technique can improve the rate of total resection of giant tuberculum sellae meningiomas.
