1.Prevention of surgical complications of renal transplantation operation

Objective To investigate the potential complications of renal transplant patients, to develop appropriate treatment programs to reduce the incidence. Methods Clinical data of 42 patients with renal transplantation were retrospectively analyzed. Blood type of the donors and recipients were same in 32 cases; Blood type of the donors was 0 and recipients blood type was A,3 cases;Donors blood type was 0 and recipients blood type was B,2 cases; Donors blood type was O and recipients blood type was AB,4 cases; Donors blood type was B and recipients blood type was AB,lease. Results In alll cases, there were acute rejection in 2 cases,pulmonary infection in 2 cases, wound infection in 1 case,urinary leakage in 1 case,urinary tract infection in 2 cases.42 cases were all cured up. One patient lost kidney function and required dialysis therapy 4 years after kidney transplantation, the remaining 40 patients were good. Conclusion Early detection and timely management of complications of renal transplantation played a very important role in improving the survival rate.
2.Textual Research on the Editions' Origin and Circulation of Yizong Baojing (Master's Precious Mirror of Medicine)

[Objective] Make textual research on the version origin of Yizong Baojing. [Method] Take the Lingyunlou carving copy of Yizong Baojing in Jiaqing Wuwu year(1798) of Qing Dynasty as the master copy, and lithographic copy of Shanghai Feiying Publishing House in the end of Qing Dynasty as the main checked copy, in the same time, take lithographic copies of Shanghai Wenruilou and Shanghai Jinzhang Publishing House in Minguo Times as reference copies, make careful investigation to many wondering points on version origin of the book, point out the problems and make serious comparison and analysis.[Result]Yizong Baojing was firstly carved in Jiaqing Wuwu year 3(1798) of Qing Dynasty, and published by Lingyunlou, thus came the Lingyunlou carving copy in Jiaqing Wuwu year(1798) of Qing Dynasty; to the end of Qing Dynasty, it was issued lithographical y by Shanghai Feiying Publishing House referring to the primary carving copy, then published by Shanghai Wenruilou, i.e. the lithographic copy of Shanghai Feiying Publishing House. This book is combined by many predecessors'medical books, very possibly from booksel ers or merchant of literature.[Conclusion]The study on the version origin of Yizong Baojing offers basic foundation for offspring's col ecting and studying the book, helpful to ancient TCM books protection and u-tilization, as wel as the development of TCM undertaking.
3.Compare with the three methods of DNA extraction of sweat latent fingerprint on the adhesive side of tapes

Objective To comparatively analyze the DNA extracted from sweat latent fingerprints on the adhesive side of tapes by three kinds of methods. Methods DNA was extracted from sweat latent fingerprint on the adhesive side of tapes using silicon bead test,QIA micro Kit and combined silicon bead-QIA micro Kit. STR loci were detected by multiplex PCR procedures. The PCR product was electrophoresed on an ABI 3100 Genetic Analyzer. Results 36%of the sweat latent fingerprints on the adhesive side of tapes was successfully genotyped using combined silicon bead-QIA micro Kit, and 21% using QIA micro Kit. Conclusion DNA genotyping of the sweat latent fingerprints on the adhesive side of tapes reveals higher possibility using combined silicon bead-QIA micro Kit than using QIA micro Kit and is less time-consuming.
4.Combined chemotherapy with gemicitabine and cisplatin as preoperative induction treatment in patients of stage m a( N_2) NSCLC

Purpose: To study the clinical efficacy, toxicity of preoperative chemotherapy and survival time with Gemicitabine-Cisplatin combination in the treatment of stage Ma( N2) NSCLC. Methods: Thirty patients with stage IIIa( N2) NSCLC were included. Gemicitabine was administered on dl, 8 and 15 at a dose of 1000 mg/m" and Cisplatin at a dose of 100 mg/m on d2. The chemotherapy was repeated every 28days. Results: Thirty patients were evaluable for response. The overall response rate was 70%. Surgical'excision rate after preoperative chemotherapy was 93%. Total surgical excision rate was 70%. Median survival time was 15 months, one year survival rate was 67%. The main toxicity was hematological, thrombocytopenia of grade III-IV appeared in 46% course of treatment, but it did not lead to hemorrhage. Conclusions: Preoperative chemotherapy with Gemicitabine-Cisplatin combination is effective and well-tolerated in the treatment of stage IIIIa( N2) NSCLC. So it is worthy to be further studied and popularized.
5.Double-bundle versus single-bundle in individualized arthroscopic anatomical reconstruction of anterior cruciate ligament

Objective To compare clinical outcomes of double-bundle and single-bundle in individualized arthroscopic anatomical reconstruction of anterior cruciate ligament (ACL) . Methods The clinical data of 117 patients were reviewed who had received double-bundle or single-bundle arthroscopic ACL reconstruction from March 2007 through September 2009 in our hospital and had undergone complete follow-up. Of them, 35 cases had single-bundle ACL reconstruction and 82 double-bundle reconstruction. In the single-bundle group(group A), there were 31 men and 4 women, aged 28. 6 ±5. 1 years. In the double-bundle group(group B), there were 73 men and 9 women, aged 27. 6 ±5. 4 years. The 2 groups were comparable in the preoperative demographic data ( P > 0. 05). To evaluate the outcomes, Lachman and Pivot Shift exams , KT-2000, Lysholm and IKDC (International Knee Documentation Committee) scores, were adopted. Results The 117 patients received a mean follow-up of 15 months (from 11 to 25 months). The Lachman test showed 88. 6% (31/35) were normal in group A and 95. 1% (78/82) were normal in group B.The pivot-shift test showed 88. 6%(31/35) were normal in group A and 96. 3% (79/82) were normal in group B. Group A had a mean Lysholm score of 93. 4 ± 8. 2 and group B a mean Lysholm score of 93. 7 ±7. 0. There were no significant differences between the 2 groups in the above indexes ( P > 0. 05). By IKDC score, 71. 4% (25/135) were normal in group A and 93. 9% (77/82) were normal in group B. The KT-2000 test showed a mean of 1. 4 ± 0. 6 mm in group A and a mean of 1. 1 ± 0. 5 mm in group B. These 2 values were significantly different between the 2 groups ( P < 0. 05). Conclusions The individualized arthroscopic double-bundle anatomical reconstruction of ACL can maximally restore the anteroposterior and rotational stability. Arrangement of the ACL insertion site on the femoral and tibial side, three-portal technique and ruler application are keys for individualized anatomical double-bundle ACL reconstruction.
6.Clinical research in prevention and cure effect of anisodamine gel on phlebitis caused by intra-venous infusion of KLT

Objective To investigate the prevention and cure effect of anisodamine gel on phlebitis caused by intravenous infusion of KLT. Methods 96 patients with intravenous infusion of KLT were divided into the experimental group and the control group, 48 patients in each group, and the skin at the top of transfusion puncture points of the patients of the experimental group was evenly embrocated with 2% anisodamine gel along the vein before 3 to 5 minutes of intravenous infusion of KLT, while the control group was without any treatment. After observing the situation of venous injury, the skin at the top of transfusion puncture points of the positive patients would be embrocated with 2% anisodamine gel. The incidence, degree of phlebitis and treatment effect were compared between the two groups. Results The incidence rate of phlebitis of the control group was 68.7%, of which grade II, III occupied 60.4%, while the incidence rateof phlebitis of the experimental group was 10.4%, in which there was no occurrence of grade II, III phlebitis. All the venous injury symptoms of 33 patients of the control group and 5 patients of the experi -mental group disappeared after treatment, and after statistical processing, the incidence rate of phlebitis in the experimental group and the control group showed significant differences. Conclusions The embrocating of 2% anisodamine gel can effectively avoid the occurrence of the phlebitis caused by intravenous infusion of KLT.
7.Research in prevention and treatment of venous injury caused by intravenous infusion of Delisheng with anisodamine gel

Objective To investigate the effect of prevention and treatment of venous injury caused by intravenous infusion of Delisheng with anisodamine gel.Methods 100 patients with intravenous infusion of Delisheng were randomly divided into the experimental group and the control group,50 patients in each group,and the skin at the top of transfusion puncture points of the patients of the experimental group were evenly embrocated with 2% anisodamine gel along the vein before 3 to 5 minutes of intravenous infusion of Delisheng,while the control group received no treatment.After observing the situation of venous injury,the skin at the top of transfusion puncture points of the positive patients was embrocated with 2% anisodamine gel,and the effect on venous injury was observed.Results The incidence rate of intravenous pain of the experimental group was 4%,there wasn't any occurrence of poor intravenous infusion or swelling of the infusion position.The incidence rate of poor intravenous infusion of the control group was 34% and phlebitis incidence rate was 42%,and intravenous pain incidence rate was 68%.All the venous injury symptoms of 39 patients of the control group and 2 patients of the experimental group disappeared after treatment.Conclusions The embrocating of 2% anisodamine gel can effectively prevent and treat the venous injury caused by intravenous infusion of Delisheng.
8.Role of cAMP-PKA signal transduction pathway in lidocaine-induced up-regulation of expression of surfactant protein-A in rat alveolar epithelial type Ⅱ cells

Objective To evaluate the role of cyclic adenosine monophosphate-protein kinase A (cAMP-PKA) signal transduction pathway in lidocaine-induced up-regulation of the expression of surfactant protein-A (SP-A) in rat alveolar epithelial type Ⅱ cells (AEC Ⅱ).Methods Healthy male Sprague-Dawley rats were sacrificed and AEC Ⅱ were isolated,purified and incubated in 24-well culture plates (100μd/hole) with density of 1 × 106/ml.After being incubated for 2 h,the culture medium was replaced with serum-free medium DMEM.The cells were randomly divided into 4 groups (n =48 each):control group (group C),forskolin (adenylate cyclase agonist) group (group F),lidocaine 200 μg/ml group (group L),and PKA inhibitor H89 + L group (group P+ L).Forskolin 10 μmol/ml was added to DMEM in group F.Lidocaine 200 μg/ml was added to DMEM in group L.H89 10μnol/ ml was added to DMEM and AEC Ⅱ were incubated for 10 min,and then lidocaine 200 μg/ml was added in group P + L.At 6,12 and 24 h of incubation (T1-3),cAMP content and PKA activity (using ELISA),and expression of SP-A mRNA (by real-time fluorescent quantitative PCR) and SP-A (by Western blot) were measured.Results Compared with group C,the expression of SP-A mRNA and SP-A was significantly upregulated,and cAMP level and PKA activity were increased at T1-3 in group F and at T2,3 in group L.Compared with group L,the expression of SP-A and SP-A mRNA was down-regulated,PKA activity was decreased,and no significant change was found in cAMP level at T1-3 in group P + L.Conclusion Lidocaine can up-regulate the expression of SP-A in AEC Ⅱ of rats through activating cAMP-PKA signal transduction pathway.
9.The effect of ethanol embolization for vascular malformations on cardiopulmonary function:recent progress in research

The treatment of vascular malformations has been a difficult clinical subject. At present, the main therapeutic methods include embolization/sclerotherapy, surgical excision, laser treatment, etc. However, it is often difficult to obtain a satisfactory clinical effect. As it can induce the vascular endothelial denudation resulting in protein degeneration, ethanol embolization can obtain the effect of complete obliteration of the diseased vascular lumen. Although ethanol embolization of vascular malformations has already achieved satisfactory clinical effect, the fear of cardiac and pulmonary accidents has limited the application of this technique in clinical practice. This paper aims to make a comprehensive review concerning the effect of ethanol embolization for vascular malformations on the cardiopulmonary functions.
10.Clinical value of DR and CT in the secondary pulmonary tuberculosis

Objective To discuss different performance of DR and CT examination on secondary pulmonary tuberculosis,to improve the clinical guidance value of CT in the diagnosis of secondary pulmonary tuberculosis. Methods The image data of 52 cases with secondary pulmonary tuberculosis confirmed by pathology were collected, including chest DR radiography and CT scans.The different image characteristics of two kinds of imaging examination were analyzed and compared.Results In 52 cases of secondary pulmonary tuberculosis,chest X -ray plain film found lesions calcification in 3 cases,2 cases of bronchiectasis,pleural thickening and adhesion in 12 cases,pleural effusion in 1 case,empty cavity in 4 cases,emphysema in 1 case,the lung nodules in 5 cases.Chest CT revealed lesions calcification in 9 cases,16 cases of bronchiectasis,pleural thickening and adhesion in 40 cases,pleural effu-sion in 4 cases,empty cavity in 10 cases,emphysema in 8 cases,the lung nodules in 10 cases,6 cases of pulmonary bullae,pulmonary interstitial change:4 cases of lung reticulocyte shadows,interlobular septal thickening in 4 cases, ground glass sample density in 4 cases.Conclusion DR and CT can be used for inspection of secondary pulmonary tuberculosis,but CT is a better tool,it has important guiding role for clinical diagnosis and treatment.
