1.Evaluation for reliability of 13C-Urea breath test and other biopsy-based methods in H. pylori diagnosis.

Multiple tests are available to diagnose Helicobacter pylori. The aim of this study was to evaluate and compare the sensitivity, specificity and negative and positive predictive values of 13C-Urea breath test (13C-UBT) and other biopsy-based methods in H. pylori diagnosis. A total of 702 patients (mean age, 51.2+/-16.0 years; 327 males and 375 females) were tested for H. pylori infection using 13C-UBT, CLO tests (Delta West, Australia), histology (H&E and modified Giemsa stains), and culture. All patients were untreated for H. pylori infection. Infection status was established by Gold Standard (or culture positive of 2 out of 3 other tests positive). Results suggested that 13C-UBT was the most accurate method for H. pylori diagnosis, although CLO test and histology also produced excellent sensitivity and specificity. Culture method had the best specificity; however, it produced the lowest sensitivity. Combination of histology and CO test gave higher sensitivity and specificity than each method used individually. Conclusion: The noninvasive 13C-BUT is excellent accurate method for determining H. pylori infection when endoscopy is not required. Histology, CLO test and culture are also good methods for H. pylori diagnosis.

3.The prevalence of Helicobacter pylori infection in Vietnam and in Australia

Study on 732 Australia dyspeptic patients with infection of H. pylori, in which there are 96 Vietnamese immigrants. Results: Among dyspeptic patients, the prevalence of H. pylori infection in Vietnam (63.7%, 95%Cl: 62.7-64.7%) was statistically higher than that in Australia (46.4%, 95%Cl: 46.0-46.8%). The spectrum of GD diseases in Vietnamese dyspeptic patients is very different from that of Australian patients with higher DU and GU and lower GORD and Barrett's epithelium. The prevalence of H. pylori infection in Vietnamese patients in Hanoi is not different from that of Vietnamese immigrants in Australia. In contrast, GoRD is found about 7 times more frequent in Vietnamese immigrants compared with Vietnamese patients living in Hanoi.

215 patients with gastroduodenal ulcer were divided into 3 groups for treating with the precedures of OAM, OAC, and OMC. Results showed that the OAM gave lowest efficacy because of high resistance to metronidazole in community. OAC gave the most efficacy though it was affected evidently by clarithromycine resistance. OMC was affected lightly by metronidazole resistance. The results of treatment was satisfied relatively because of the combined effect of clarythromycine. In case of an allergy with penicillin, this is the procedure of choice

5.Esophagectomy technique with supported thoracoscopy

Background: Esophageal cancer is a common disease at Digestive Surgery Department of Binh Dan hospital. Surgical treatment is still a main procedure. Objectives: Study on a less invasive surgical technique in esophagectomy with supported thoracoscopy to provide a new esophageal cancer treatment. Subjects and method: Application on a new technique via thoracoscopy to esophagectomy totally in 6 patients (1 female, 5 males), aged 60 years on average, treated in Digestive Surgery Department of Binh Dan hospital from March 2006 to June 2006. Results: The average surgical time was 260 minutes. There was no event during operation. Postoperative complications included: 1 patient had to stitch abdominal wall after operative 7 days, 3 patients with pneumonia after successful operation, 2 patients with right pneumothorax, having to put siphonage. There was one case of death without related to operative technique. Conclusion: Esophagectomy is major operation that can be conducted via open surgery or laparoscopic surgery. Although small case studies, researchers found that laparoscopic surgery is a feasible technique, can be performed safely if the surgeons having experience in open surgery and good skills on laparoscopic surgery. However, thoracoscopic esophagectomy can only be considered as a surgical method, without an alternative method to traditional open surgery.

6.Phenolic Constituents from Balanophora laxiflora with their Anti-inflammatory and Cytotoxic Effects

Balanophora laxiflora Hemsl. (Balanophoraceae) is a traditional medicinal plant with a diverse array of biological activities. In our exploration of new bioactive constituents from B. laxiflora, we isolated five compounds, including a new lignan, balanophorone (5), and four known phenolic compounds (1–4). The chemical structures of these compounds were determined by extensive spectroscopic analyses, including 1D and 2D NMR, HR-ESI-MS, and CD. In addition, we evaluated the effects of each of the isolates (1–5) on the messenger RNA expression levels of tumor necrosis factor (TNF)-α and cyclooxygenase (COX)-2 in lipopolysaccharide (LPS)-stimulated RAW 264.7 macrophages and cytotoxicity against MCF-7 and MDA-MB-231 breast cancer cells. Compound 2 showed significant inhibition of LPS-induced COX-2 and TNF-α expression in RAW 264.7 macrophages, while compound 4 showed moderate cytotoxicity against MCF-7 and MDA-MB-231 breast cancer cells, with IC 50 values of 18.3 and 30.7 μM, respectively. No significant effects on the viability of normal mammary epithelial cells were observed.

All HCWs at a tertiary intensive care units were asked to undertake weekly rectal swabs. Among 40 participants, 65% (26/40) carried extended spectrum β-lactamases (ESBL)/AmpC β-lactamase-producing Escherichia coli. Two HCWs colonized with ESBL/AmpC β-lactamase-producing Klebsiella pneumoniae. One HCW colonized with Acinetobacter baumannii. No one carried Pseudomonas spp.. A quarter (10/40) of HCWs were identified as persistent and frequent carriers. There is an urgent need to screen antibiotic-resistant GNB among HCWs and improve HCWs' hand hygiene compliance to reduce the transmission of antibiotic-resistant GNB in the hospital.
