2.Study on hepatic vein and retrohepatic venous injuries in 4 years (2000-2003) in Viet Duc hospital

The study carried out on 58 patients (45 men and 13 women) from 4 to 53 years old with hepatic venous injuries who were operated in Viet Duc Hospital. The result showed that there were 4 common kinds of hepatic venous injuries. Among them, retrohepatic venous injuries commonly causes death due to acute and uncontrol blood loss. The methods to treat hepatic venous injuries tend to don’t use shunt because of disadvantages. The methods include early surgery, attach liver to the diaphragm to stop the bleeding, blood transfusion with high amount and rapid speed to raise blood pressure, liver resection according to injury, pining liver stalk, chocking antlers, in case of need pining inferior vena cava, deal with vein injury was the most common and effective method to reduce mortality rate of patients with hepatic venous injuries

A reconstruction following a resection of malignant oral cavity tumors is one of the most difficult problems in recent oral oncology. For a better understanding of oral and maxillofacial reconstructive procedures, basic and advanced microvascular anastomosis techniques must be learned and memorized. The aim of this article was to clarify and define the microvascular anastomosis methods, such as primary closure after an arteriotomy, end to side anastomosis, end to end anastomosis, and side to side anastomosis with an artery and vein. This review article discusses the basic skills regarding microvascular anastomoses with brief schematic diagrams in the Korean language. This article is expected to be helpful, particularly to young doctors in the course of the Korean national board curriculum periods for oral and maxillofacial surgery.

4.Anatomical research of hepatic venous system that coming in to the inferior aortic venous and its application for hepatectomy; plastic reconstruction of hepatic veins and liver transplantation

168 healthy livers from lead patients were studied. The results have shown that 4 basic anatomical forms of the right liver and 6 basic anatomical forms of left liver have been found. 36.9% of cases can be dissected the left hepatic vein. This finding can be applied for the liver transplantation and heapatectomy and plastic reconstruction of hepatic veins.

5.Perinatal transmission of hepatitis B virus and vaccination in high-risk neonates

To study on 65 HBsAg positive mothers and 61 their newborns received differents dosages and types of hepatitis B vaccine at 0, 1, 2 months. HBeAg was detected in 25 (38.5%) mothers and all of them were HBV DNA positive. HBsAg and HBV DNA was detected in 19 (76%) and 23 (92%) respectively of the 25 cord blood from HBeAg positive mothers, while detected in 16 (40%) and 12 (30%) respectively of the 40 cord blood from HBeAg negative mothers. Testing at 6 months after completion of the vaccineation schedule, the failure risk (HBsAg positive) of vaccines for dosage of 2.5 g/ml is higher than others (5 g/ml and 10 g/ml). Hepatitis B vaccine with hepatitis B immune globulin or vaccine dosages of 5 and 10g alone may be recommended for high-risk neonates.

9.Value and safety of the surgery with vascular resection and reconstruction for pancreatic cancer.

OBJECTIVETo investigate the value and safety of the surgery with vascular resection and reconstruction during pancreatectomy for pancreatic cancer.
METHODSThe clinical data of 206 patients with pancreatic cancer who underwent radical resection were retrospectively analyzed from January 2009 to March 2014 in Lihuili Hospital, Medical center of Ningbo.All cases were divided into non-vascular resection group(132 cases), the combined vein resection group(66 cases) and the combined arterial resection group(8 cases). The peri-operation data, the incidence of postoperative complications and the survival were compared in pairs among three groups.All patients were followed up till September 2014.
RESULTSThere were no statistical differences for the preoperative data among three groups.The operation time and the blood loss (M(QR)) were (347±96)minutes and (500(400)) ml in non-vascular resection group, (425±91)minutes and (800(500))ml in combined vein resection group, (508±120)minutes and (1 750(2 075))ml in combined arterial resection group, with significant differences among three groups(all P<0.01). The incidence of postoperative complication was 16.7%(22/132) in non-vascular resection group, 28.8%(19/66) in combined vein resection group, and 6 cases in combined arterial resection group, respectively.There were significant differences between non-vascular resection group and combined vein resection group(P<0.05), non-vascular resection group and combined arterial resection group(P<0.05), as well as between combined vein resection group and combined arterial resection group(P<0.05). The median survival time was 15 months for non-vascular resection group, 15 months for combined vein resection group, and 12 months for combined arterial resection group.No significant difference was found among three groups(all P>0.05). The postoperative mortality was nil for all of groups.
CONCLUSIONSCompared with non-vascular resection, combined vein resection can be performed safely with a similar prognosis. The surgery of combined arterial resection could only be justified when R0 resection for pancreatic cancer could be achieved for highly selected patients.

10.Treatment of erectile dysfunction by embedding deep dorsal vein of penis for 5 cases.

OBJECTIVESTo evaluate the curative effect of embedding deep dorsal vein of penis on erectile dysfunction (ED).
METHODSThree patients with venous ED and 2 patients with mixed factors of artery and vein were treated by the embedding deep dorsal vein of penis. For evaluating curative effect the 5 patients were visited after operation.
RESULTSTwo months after surgery, the 3 patients of venous ED achieved satisfactory intercourse and the 2 patients with mixed factors of artery and vein had sufficient erection for intercourse after taking 50 mg Sildenafil. The 5 cases kept the above-mentioned curative effect during the follow up period between 3 to 12 months (7 months on avenage).
CONCLUSIONSThe embedding deep dorsal vein surgery, having small surgical wounds and few complications, is an effective treatment for venous ED.
