2.Endoscopic management of sinonasal vascular tumors without embolization: Our experience with vessel ligation

Objective:
To present our surgical experience and technique in performing endoscopic sinus surgery for vascular sinonasal tumors without pre-operative embolization using intraoperative ligation of the external carotid artery or its distal branches.
Methods:
Design: Retrospective Series.
Setting: Tertiary Private Teaching Hospital.
Participants: Seven Patients.
Results:
Out of 7 patients (5 males, 2 females, aged 12 to 64 years old) with non-embolized vascular sinonasal tumors, 2 had juvenile angiofibroma, 3 had a benign vascular tumor (hemangiopericytoma, hemangioma and a vasoformative solitary fibrous tumor), and 2 had a malignancy (rhabdomyosarcoma, squamous cell carcinoma). Four (57.1%) had external carotid artery ligation, two (28.6%) had internal maxillary artery ligation and one (14.2%) had sphenopalatine artery ligation. The mean intraoperative blood loss was 2447.1 mL (range 900mL to 5,000mL) and average operation duration was 7.6 hours (range 2.9 hours to 14.5 hours). The average amount of transfused blood products was 1785.7mL (zero to 3,000mL). The average hospital stay was 7 days (range 2 to 13 days) with one post-operative complication (ICU admission for hypotension from intraoperative blood loss).
Conclusion
Intraoperative ligation of the ECA or its distal branches to disrupt the vascular supply of sinonasal tumors may provide a viable means of preventing excessive intraoperative blood loss in patients with non-embolized vascular sinonasal tumors.

Tumors of vascular origin are subdivided into two groups: those composed of pericytes (hemangiopericytoma and glomus tumor), and those composed of endothelial cells(hemangioendothelioma). 4.A Case of Hemangioendothelioma on the Nasal Septum.

Hemangioendothelioma(HE) is a rare intermediate-grade malignant vascular neoplasm first described in 1982 by Weiss and Enzinger as a specific entity. The HE is differentiated by both in its characteristic behavior and histologic appearance into benign (hemangioma) and malignant tumor (angiosarcoma). The tumor rarely occurs in the nose and paranasal cavity. At least four distinct entities are recognized within HE: epitheloid HE, spindle cell HE, kaposiform HE, malignant endovascular papillary angioendothelioma. However, it is difficult to classify a small remaining group of vascular neoplasms that show a considerable overlap in their histologic appearance. The composite and polymorphous HE are therefore classified separately. Surgical excision is the treatment of choice. Generally, the prognosis is good. The authors recently experienced a case of HE on the nasal septum which was treated by excision. We report this case with the review of literature.

Sclerosing angiomatoid nodular transformation (SANT) is a rare, benign vascular neoplasm. Most patients have no clinical symptoms, and the tumors are usually discovered incidentally on abdominal computed tomography or ultrasonography. Some studies have reported the clinical features and imaging findings of SANT, but the diagnosis is based on histopathologic examination of a tissue specimen obtained at splenectomy. We report herein an incidentally discovered case of SANT and review the related literature.

6.A Case of Spontaneous Hematomyelia.

This is a report of 23 year old soldier who developed sudden onset of flaccid paraplegia and loss of all sensory modalited below the level of T4. The clinical entities of hematomyelia were sudden onset of pain, repidly developing pyramidal signs and paraplegia. The mainly involving sites of this disease were cervical and thoracic regions. The causative factors were hemorrhagic diasthesis, vascular malformation and inflammatory process as well as trauma and neoplasms affecting the spinal cord. We experienced a case of hematomyelia which did not combine with vascular anomaly, inflammatory process, hemophilia or trauma history.

7.A Case of Angioma Serpiginosum Accompanied with Ipsilateral Nevus Flammeus on the Hand.

Angioma serpiginosum is an acquired benign vascular neoplasm characterized by minute purple-colored, grouped nonpalpable punta on the extremities. However, nevus flammeus is a congenital vascular malformation that shows unilateral red macules. Although the evidence for association between vascular tumor and malformation have been posed, angioma serpiginosum is known to be rarely associated with nevus flammeus. We herein report a rare case of angioma serpiginosum accompanied with nevus flammeus on the ipsilateral hand.

vascular tumor of the liver in infancy, can occur with acute postnatal liver and congestive heart failure. Nevertheless, its course is often benign, and many children can be diagnosed and treated without surgical intervention. The distinction from malignant diseases is not always easy and it not clear whether invasive procedures for diagnosis and therapy should be performed. Here we report our experiences in our Center for Pediatric Liver Disease and postulate that large studies are needed to avoid unnecessary invasive procedures for these patients in the future.]]>

9.Angiographically Occult Arteriovenous Malformation Mimicking Brain Tumor: Case Report.

The authors report a case of angiographycally occult intracranial vascular malformation. The patient had huge calcified mass at left frontoparietal area. The preoperative diagnosis was a slowly growing benign tumor. The mass was very hard and so vascular malformation was not suspected even during the operation. Pathologic report revealed arteriovenous malformation.

Hemangioendothelioma is a recently described uncommon vascular neoplasm, characterized by epithelioid tumor cells and borderline biologic behavior. Its histological features and clinical behavior are intermediate between those of hemangioma and angiosarcoma. Its four principal sites of occurrence are the soft tissue, liver, lung and bone.
