1.Perspectives on molecular diagnosis and targeted therapy for complex vascular malformations in pediatrics.

Vascular malformations are due to abnormal development of blood and/or lymphatic vessels during embryonic life without endothelial cell proliferation. Most of the previous treatments were symptomatic methods as surgery and sclerotherapy because the pathogenic mechanism was not clearly understood. With advances in molecular biology, the pathogenesis of vascular malformations is thought to be related to inherited and/or somatic mutations that eventually activate the PI3K/ATK/mTOR, Ras/Raf/MEK/ERK pathways. Also, related studies have promoted the use of targeted inhibitors. This article provides a review of current causative genes and targeted drugs for pediatric vascular malformations, aiming to provide a basis for promoting accurate molecular diagnosis and precision targeted therapy for these diseases.

4.Multiple Pyogenic Granulomas in Port-wine Stain after Laser Therapy.

Pyogenic granuloma (PG) and port-wine stain (PWS) are vascular lesions that rarely occur in association. While PWSs are congenital capillary vascular malformations, PGs are acquired benign vascular proliferations. Although the pathogenesis of PG is unknown, many authors feel that they represent reactive hyperplasias, especially as they are often seen in response to trauma. We report the multiple pyogenic granulomas developed in a port-wine stain following Nd-YAG laser therapy.

5.Argon Laser Therapy of Nevus Flammeus.

Nevus flammeus is benign but cosmetically devastating congenital vascular malformation whose past treatments have been marred by unacceptable complication or by unacceptable ineffectiveness. The argon laser is a therapeutic device which has been newly applied to this condition. Blue-green argon light is absorbed by the intraluminal red hemoglobin molecule, leading to the destruction of blood vessels. This study was undertaken to evaluate the therapeutic effect, to look for complications caused by the argon laser on nevus flammeus and to investigate the prognostic values of the color, the degree of blanching and the histological pattern of lesion after laser treatment. All patients were treated with Coherent Radiation Model No. 1,000 Laser with a spot size of 1mm diameter. The power range was 1.8W (irradiance of 229W/cm2), the pulse duration was 0.4 second and the repetition rate was 120 impulses per minute. The result was evaluated according to the criteria of Gilchrest(Table 1). Among the 109 patients who have been treated between November, 1981 and July, 1982 at the Department of Dermatology, Seoul National University Hospital, data from the 49 patients who could be followed up was evaluated and analyzed. 6.Comparative Study between Clinical Morphology and Histopathology in Nevus Flammeus.

Nevus flammeus is benign but cosmetically devastating congenital vascular malformation whose past treaments have been marred by unacceptable complication or by unacceptable ineffectiveness. Argon laser is a therapeutic device which has been newly applied to this condition with fairly good result. However, all lesions do not show good response and it depends on the clinical and histopathological types. Therefore, it is very important to select patients who will show good response to argon laser therapy. Histological pattern was indicated as the most important prognostic factor. However, it is not always possible to get the histological data. This study was undertaken to evaluate whether it is possible to anticipate histologic patterns of nevus flammeus from color and clinical morphology of the lesions when biopsy is difficult to be taken. Color and clinical morphology of the lesions were recorded, and 4-mm punch biopsies were performed in 74 patients who visited the Laser Clinic in our dermatologic department during past 2 years, The study results were as follows. Eleven pink cases were all patch form morphologically but 8 cases(72, 7%) were constricted type and 8 cases(27 Q%) were intermediate type histopathologically. In 28 red cases, 27 cases(96 4gp) were patch form and only 1 case was smooth elevated form morphologically. And 2p cases(74, l%) were dilated or intermediate type and 7 cases(25 9%) were constricted or deep located type histologically.

7.Utility of sodium tetradecyl sulfate sclerotherapy from benign oral vascular lesion.

BACKGROUND: Hemangioma and vascular malformation are benign vascular lesions that often occur in cephalic and cervical region. Currently, surgical resection, laser therapy, angiographic embolization, use of steroids, and sclerotherapy are used as treatments. CASE PRESENTATION: This study reports three cases of benign vascular lesions that are remarkably treated by sodium tetradecyl sulfate (STS) injection, of which occurred in oral cavity and around the mouth. Three percent of STS was diluted with 0.9 % of normal saline, and it was injected to the lesion site at least once. The result of treatment was evaluated based on clinical findings. CONCLUSION: Surgical treatment of hemangioma and vascular malformation occurred in oral cavity is not normally used because of esthetic issues and potential hemorrhage. On the other hand, sclerotherapy using STS is an effective therapy compare to surgical treatment. Despite the number of STS injection was different for each patient, all three patients had reached satisfactory level through the treatment with gradual diminution of lesions.

8.Acute paraplegia following embolization of spinal dural arteriovenous fistula.

Embolization therapy has been used as the initial treatment for spinal dural arteriovenous fistula (SDAVF) only for certain patients or in certain medical institutions due to its minimal invasiveness, but the recurrence of embolization remains a clinical challenge. The recurrent patient usually exhibits a gradual onset of symptoms and progressive deterioration of neurological function. Developing paraplegia several hours after embolization is commonly seen in patients with venous thrombosis-related complications, for which anticoagulation therapy is often administered. This article reports on a SDAVF patient who had weakness of both lower extremities before embolization and developed complete paraplegia several hours after embolization therapy, later confirmed by angiography as fistula recurrence. The symptoms were relieved gradually after second embolization. The pathophysiology of this patient is also discussed.

vascular malformation are the most common benign tumors that are caused by congenitally or traumatic events. Theses tumors represent approximately 1/3~1/4 of all hemangiomas and vascular malformations in the head and neck. There are many forms of treatment for hemangioma and vascular malformation including closed observation, surgery, radiotherapy, laser therapy, steroid therapy, compression, embolization, and sclerotherapy. Ethanolamine oleate is an unsaturated fatty acid salt that has been used as a sclerosing agent because of its excellent thrombosing properties. This paper presents 1 case of intraoral multiple venous malformations treatment with 1.25% ethanolamine oleate (3.6~9.6 mg dose) intralesionally injected for 6 to 14 weeks over 2 week intervals. After the sclerotherapy, lesions almost completely disappeared without side effects. In conclusion, sclerotherapy using ethanolamine oleate is very effective against venous malformations, and sufficiently provides alternative support for surgical and other methods.]]>

10.Treatment of the Portwine Stain with Dermabrasion and Dermal Overgraft: A Clinical Experience.

Portwine stains(PWSs) are congenital vascular malformations, commonly found in the face, caused by progressively ectactic postcapillary venules of the superficial vascular plexus. They are reported to occur in 0.3% of newborn, and depending on the location and size, they may lead to psychosocial complications. Recently, with the advent of the lasers, ruby, dye, argon and CO2 lasers have been used for the treatment of PWSs. However, the results are variable and unpredictable. We present a case of congenital hemifacial PWS treating with dermabrasion and overgraft with very thin split-thickness skin rather than laser therapy. We achieved good result by using this method.
