OBJECTIVETo improve the recognization and treatment for oral venous lakes (OVL).

METHODSClinicopathologically analysis of 20 cases of oral venous lakes was performed. All the samples were removed by surgically.

RESULTSAll the lesions occurred in senile persons and located under the mucosa. Among the 20 cases, 11 were male patients and 9 were female; 18 of 20 occurred in the lower lip, another two in the upper lip and buccal mucosa. The lesions were 0.3 approximately 1.2 cm in size. Pathologically, the oral venous lake was composed of single or a few large, dilated and irregular veins. Thrombosis and organization were found within the lesions sometimes.

CONCLUSIONSOral venous lakes are a kind of focal anomalies of venous structure. All the 20 cases (with a follow-up time from 8 months to 4 years) has no recurrence after surgical treatment.

We reviewed the data of 38 neonates who died of respiratory failure. Paraffin sections of the autopsy lung samples were examined with HE staining or immunolabeling for CD34, CD68 and CK to observe the development of the pulmonary vessels and detect potential pulmonary vascular diseases (PVDs). Five cases were identified to have PVDs, including pulmonary hypertensive vascular remodeling in 3 cases and alveolar capillary dysplasia in 2 cases. The result indicated that PVD was one of the important reasons for respiratory failure in these neonates.

Rectus sheath hematoma is an uncommon entity of the anterior abdominal wall and an unusual cause of painful abdominal mass. Rectus sheath hematoma have been in association with trauma, infections, debilitating disease, collagen vascular disease, pregnancy. and the puerperium. Sudden disruption of a deep epigastric vessel may result in an abdominal wall hematoma, which depending upon its location and size, can produce symptoms and clinical findings compatible with a variety of acute intra-abdominal conditions. Such hematoma are infrequently encountered and early accurate diagnosis could prevent surgical intervention. Unfortunately, the clinical manifestations of rectus muscle hematoma are sometimes so dramatic that laparotomy is performed under the belief that intra-abdominal pathology is present. The diagnosis was confirmed at surgery in both patient. We present two cases of rectus sheath hematoma which were diagnosed by clinical & ultrasound examination preoperatively.

Spontaneous hematomyelia (intramedullary spinal hematoma) is an uncommon event of an unknown cause. A 35-year-old man experienced sudden paresthesia over the chest, radiating pain, and motor weakness followed by complete paraplegia appeared after 1 hour. The preoperative diagnosis was made by magnetic resonance imaging which revealed hemorrhages from T4 to T9 cord segments. The selective spinal angiography, CSF study, blood laboratory, and pathology revealed no apparent cause for the hemorrhages.

Epithelioid hemangioendothelioma (EHE) is a relatively rare and moderately aggressive type of vascular tumor. We describe a case of EHE that presented in a 24-year-old woman as a palpable nodule in the left cervical area. Radiological examinations and fine-needle aspiration cytology led to a preliminary diagnosis of metastatic lymphadenopathy with an unknown primary site. However, during surgery, we determined that the nodule was an intra-vascular tumor arising from the left internal jugular vein. The histopathological examination revealed cords of epithelioid endothelial cells distributed in a hyaline stroma, compatible with a diagnosis of EHE.

Angioma serpiginosum is an uncommon, acquired vascular nevoid disorder with capillary dilation and proliferation in the papillary dermis. The eruptions are asymptomatic and characterized by grouped, erythematous to violaceous, serpiginous and punctate macules. The condition usually appears in females during adolescence on unilateral lower extremities and the buttocks. We report a rare case with a late onset and atypical distribution of lesions in a 48-year-old female patient who had groups of punctate lesions on her left foot for four to five years. Histopathological examination showed hyperkeratosis and multiple dilated and proliferated capillaries in the papillary dermis. Inflammation and extravasation of red blood cells were not found. According to the clinical and pathological findings, we established a diagnosis of angioma serpiginosum. She was treated with a pulsed dye laser, and the angiomatous lesions subsequently improved.

BACKGROUNDThe ankle brachial index (ABI) is a simple, inexpensive, noninvasive tool that correlates well with angiographic disease severity and functional symptoms. The aim of this study was to identify the manifestation of lower extremity atherosclerotic lesions in patients with high ABI by retrospective clinical study.

METHODSA cohort of 184 diabetic patients, (63 +/- 14) years old, 144 males, who underwent simultaneously ABI testing and low extremity arterial duplex ultrasound within one week, were enrolled randomly into this study. According to the ABI value, they were divided into three groups: the high, normal and low ABI groups. The severity and location of atherosclerotic lesions in the lower extremity were determined based on the results of low extremity artery duplex ultrasound. The chi-square test was used to compare the atherosclerosis severity grade and lesion location across the three groups.

RESULTSThe prevalence of low extremity artery occlusion was significantly lower in the high ABI group than in the low ABI group (3.3% vs. 63.5%, P < 0.01), and the main atherosclerotic lesions were diffuse dot-like hyperechogenicity spots or small plaques (86.7%). In addition, the atherosclerotic lesions were mostly found in the distal segment of the lower extremity in patients with high ABI (46.3%).

CONCLUSIONA high ABI may be an integrative marker for intimal and medial calcification, which has a high positive predictive value for artery calcification.
