1.Evaluating the acceptance of the community on field training of medical students

Background: Training community-oriented general practitioners with the aims to provide the medical knowledge and skills of working in the community for medical students is appropriate with the socioeconomic context in Vietnam. Objectives: To evaluate the acceptance of the community in two districts Kim Bang (Ha Nam province) and Dong Hy (Thai Nguyen province) on field training for medical students. Subjects and method: The cross - sectional study was carried out on 300 households that were chosen randomly from 2 rural districts on April 2006. The heads of households were interviewed by open - ended questionnaires. Another 120 key informants from these two districts were invited to participate in a focus group discussion. Results: 81% of households voluntarily participated in the organization of field training for the medical students. More than 97% households paid attention to the medical students\u2019 information about health care and environmental hygiene. With medical community-based education, the people from communities receives\u2019 improvement in their knowledge of child care and personal hygiene in order to prevent diseases. Conclusion: Being exposed to health problems in communities, medical students can enhance their working and communication skills. People in the communities had a chance to improve their knowledge of disease preventions.

Background: \r\n', u'In the antituberculosis chemotherapy, shortening treatment course, and applying new high effective methods are top prioritized. Of which,the immunotherapy with M vaccea an environmental saprophyte, combined with the antituberculosis medicines all over the world has been applied since 1985, aimed to enhance the host immune responses.\r\n', u'Objectives:\r\n', u'To evaluate the results after 6 months of shortened course anittuberculosis chemotherapy by combining the immunotherapy withM vaccae.\r\n', u'Subjects and method: \r\n', u'233 newly diagnosed pulmonary tuberculosis patients with AFB smear (+) selected from Centers for Tuberculosis Control in some districts of Ha Tay and Ha Noi. They are 15 above, and not the pregnant, breastfeeding, diabetes, HIV infections, and hepatitis B (Unnecessary) \r\n', u'Dividing randomly into 2 groups, group A was in treatment of the standard 8 month chemotherapy regimen, and group B was treated in a shortned 6 month chemotherapy regimen in combination with immunotherapy by using M. vaccae vaccine. \r\n', u'Results:\r\n', u'The rates of AFB (-) sputum of group B were 98,2% and 100% respectively, the rates of the group A were 97,4% and 99% respectively after 2 and 6 months of treatment. There are 88.5% and 100% for group B, 85.6% and 98.8% for group A at the same time to achieve a negative sputum culture for M. tuberculosis. IgM responses against M. tuberculosis sonicated antigen before and after the follow-up time in Group A are not changed, whereas this level of the immunotherapy group had reduced significantly at the 6th moth (P<0.05). There is no responses in IgC to M. tuberculosis of both groups during the same follow-up period. The mean weight of patients in group B was higher than that of group A after 2 and 6 months of treatment.\r\n', u'Conclusion:\r\n', u'M. vaccae vaccine brings good benefits when be combined with the chemo-immunotherapy for newly diagnosed pulmonary tuberculosis patients with AFB smear (+). The negative sputum indicators and clinic improvements in the chemo- immunotherapy group of treatment are better than that of chemotherapy only. 3.Surveillance for Vibrio cholerae 01 and vibriophages in surface water - the factors for prediction of cholera outbreaks

Background: In countries that cholera outbreaks commonly occurs, surveillance of surface water is important to control the epidemics. Kappa phage (K139) of Vibrio cholerae is related to epidemic El Tor and O139 strains in various countries. Objectives: To study the relationships between V. cholerae 01 and vibriophages in surface water in Thai Binh and Hai Phong city and to determine the predictors of cholera outbreaks. Subjects and method: Total of 82 water samples were collected by shrimp swab method in Hai Phong and Thai Binh in June and September 2007, where cholera outbreaks occurred. The enriched culture method and direct peR were applied to define the relationships between vibriophages and V. cholerae 01 in surface water. Results: The rates of detecting kappa junction with chromosomes of V. cholerae 01 in Hai Phong and Thai Binh were 10% and 15% in June 2007, 35% and 41% in September 2007. The rates of detecting vibriophages in Hai Phong and Thai Binh were 15% and 15% in June 2007, 25% and 36% in September 2007. The rates of Fs1 and Fs2 detected by PCR method were 70% and 100% in Hai Phong at June and September 2007, and were 57% and 100% in Thai Binh at June and September 2007, respectively. V. cholerae 01, O139 weren\u2019t found in water samples. Conclusion: The presences of vibriophages (kappa phage and fs1 fs2) in surface water contaminated with V. cholerae 01 were dependent on the ability of the vibriophages to infect and lyse these bacteria. Vibriophages played a role in the transfer of cholera toxin and pathogenic factors. Surveillance by detecting V. cholerae 01 and vibriophages in surface water may be a feasible means of predicting outbreaks of cholera before a significant number of cases occur.

4.Antidiabetic and antioxidant activities of red seaweed Laurencia dendroidea

Objective: To investigate antidiabetic and antioxidant activities of the extract and fractions from Vietnamese red seaweed Laurencia dendroidea. Methods: The seaweed Laurencia dendroidea was extracted by using microwave-assisted extraction method in 80% methanol. The seaweed extract was then fractionated using different solvents (n-hexane, chloroform, ethyl acetate, butanol and water). These obtained fractions were evaluated for α-glucosidase inhibitory and antioxidant activities. Antioxidant activities were tested using DPPH, nitric oxide radical scavenging and metal chelating assays. The enzyme inhibition mode was determined using Lineweaver-Burk plot. For acidic and thermal stabilities, the ethyl acetate fraction was treated at pH 2.0 and 100 ℃, respectively. The residual inhibitory activity of the fraction was calculated based on the initial inhibitory activity. For in vivo antidiabetic activity, mice were divided into four groups, including normal control, diabetic control, diabetic mice treated with ethyl acetate fraction and diabetic mice treated with gliclazide. Blood glucose level of treated mice during acute and prolonged treatments was measured. To evaluate the toxicity of the ethyl acetate fraction, the body weight changes and activities of liver function enzymes (aspartate transaminase, alanine transaminase and gammaglutamyl transferase) were carried out. Results: The extract of Laurencia dendroidea showed strong α-glucosidase inhibitory and DPPH radical scavenging activities. Methanolic concentrations affected both α-glucosidase inhibitory and antioxidant activities. A 80% aqueous methanol was the suitable solvent for extraction of enzyme inhibitors and antioxidants. Among solvent fractions, ethyl acetate fraction had the highest inhibitory activities against α-glucosidase with a mixed type of inhibition and the strongest antioxidant activities, and was stable under acidic and thermal conditions. The ethyl acetate fraction treated diabetic mice significantly reduced blood glucose level compared with the diabetic control group (13.16 mmol/L vs. 22.75 mmol/L after 3 hours of treatment). Oral administration of ethyl acetate fraction did not exhibit toxicity at a dose of 100 mg/kg body weight as determined by body weight changes and liver biochemical parameters. Conclusions: Laurencia dendroidea could be a potential source for production of antidiabetic and antioxidative agents.
5.Identification of Trombiculid Chigger Mites Collected on Rodents from Southern Vietnam and Molecular Detection of Rickettsiaceae Pathogen

Trombiculid “chigger” mites (Acari) are ectoparasites that feed blood on rodents and another animals. A crosssectional survey was conducted in 7 ecosystems of southern Vietnam from 2015 to 2016. Chigger mites were identified with morphological characteristics and assayed by polymerase chain reaction for detection of rickettsiaceae. Overall chigger infestation among rodents was 23.38%. The chigger index among infested rodents was 19.37 and a mean abundance of 4.61. A total of 2,770 chigger mites were identified belonging to 6 species, 3 genera, and 1 family, and pooled into 141 pools (10-20 chiggers per pool). Two pools (1.4%) of the chiggers were positive for Orientia tsutsugamushi. Ricketsia spp. was not detected in any pools of chiggers. Further studies are needed including a larger number and diverse hosts, and environmental factors to assess scrub typhus.

METHODS: A total of 120 participants with newly diagnosed T2D and 120 adults without T2D were recruited from a large hospital in Hanoi, the capital city of Vietnam. All participants wore an ActiGraph GT3X accelerometer for at least 5 days, including 1 weekend day. Freedson cut-off points were used to estimate different intensities of PA. In addition, comparisons between groups were made with respect to achieving the World Health Organization (WHO) and International Diabetes Federation (IDF) recommended PA guidelines. RESULTS: Men with T2D had significantly lower levels of PA than men without T2D. The respective multivariable-adjusted mean values of daily step count, daily light-intensity, moderate-intensity, and moderate-to-vigorous-intensity PA were approximately 14%, 19%, and 22% lower in the men with T2D than in their non-T2D counterparts. However, women with T2D accumulated a greater number of steps per day than women without T2D. Only 59.2% of the adults with T2D met the minimum recommended level of PA (WHO and IDF), compared to 74.2% of adults without T2D (p<0.05). After adjusting for potential confounders, participants with T2D experienced 50.0% significantly lower odds of achieving PA recommendations. CONCLUSIONS: Vietnamese men with T2D were less physically active than those without T2D, and adults with T2D were less likely to meet PA guidelines. The results suggest a need for integrating PA into the self-management of this chronic condition.

s/Aims: Parenchymal-sparing anatomical hepatectomy (Ps–AH) based on portal ramification of the right anterior section (RAS) is a new technique to avoid unnecessarily transecting too much liver parenchyma, especially in cases of major anatomical hepatectomy.
Methods:
We prospectively assessed 26 patients with primary hepatic malignancies having undergone major Ps–AH based on portal ramification of the RAS from August 2018 to August 2022 (48 months). The perioperative indications, clinical data, intra-operative index, pathological postoperative specimens, postoperative complications, and follow-up results were retrospectively evaluated.
Results:
Among the 26 patients analyzed, there was just one case that had intrahepatic cholangiocarcinoma The preoperative level of α– Fetoprotein was 25.2 ng/mL. All cases (100%) had Child–Pugh A liver function preoperatively. The ventral/dorsal RAS was preserved in 19 and 7 patients, respectively. The mean surgical margin was 6.2 mm. The mean surgical time was 228.5 minutes, while the mean blood loss was 255 mL. In pathology, 5 cases (19.2%) had microvascular invasion, and in the group of HCC patients, 92% of all cases had moderate or poor tumor differentiation. Six cases (23.1%) of postoperative complications were graded over III according to the Clavien–Dindo system, including in three patients resistant ascites or intra-abdominal abscess that required intervention.
Conclusions
Parenchymal-sparing anatomical hepatectomy based on portal ramification of the RAS to achieve R0-resection was safe and effective, with favorable short-term outcomes. This technique can be used widely in clinical practice.
