Study on 229 newborn babies transferred from lower-level hospital into Neonatology Department of Tu Du Hospital. The results showed that: The main causes were respiratory distress, neonatal infection and asphyxia in premature infants, asphyxia and respiratory distress in the term infants. The more premature the newborn infants were, the earlier they were hospitalized. 157/229 infants were transferred into the hospital by rudimentary vehicles, the others were transferred by ambulance in which only 9.6% had oxygen supply. 163/229 patients (71.18%) were admitted in very serious status that required active intensive care and had high mortality rate

The food intoxication is considered only when there were acute or subacute clinical symptoms. Those were a food borne diseases. They comprised diseases with or without the symptoms such as diarrhea, vomiting, fever, and appeared in 50% of the food borne diseases. 70% of the cases of diarrhea caused by infected food or water. There were 2 kinds of the food intoxication: due to the endotoxics of staphylococci with very short time of incubation and the toxics generated during growth of microbial in the infected food must be a certain number called level for disease. It should assure the measures of antibiotics or containment of microbial under level for disease.

The food intoxication has impacted badly on the human health and economy. Every year, the rate of the food intoxication was very high such as in America (175/1000 rehabitant), England and New Zealand (190/1000), Australia (220/1000). In Vietnam, there are about 7,000 cases suffered the food intoxication with 46 deaths in 60/61 provinces in 1997. This paper introduced the concerns and solution for management of the food intoxication.

4.The first evaluation of clinical efficacy of high-frequency oscillatory ventilation in the treatment of seriously respiratory distress syndrome in preterm babies at Tu Du hospital.

Background:Pulmonary disease is still the main reason cause diseases and mortality for preterm babies. Objectives:This study aims to evaluate of the clinical efficacy of high-frequency oscillatory ventilation in the treatment of preterm infants who have seriously respiratory distress syndrome required mechanical ventilation. Subjects and method:A prospective clinical trail was conducted at Neonatal Unit of Tu Du Hospital from 1st December 2005 to 30th November 2006. Intervention: Patients were randomized to high-frequency oscillatory ventilation (20 patients) or conventional mechanical ventilation-CMV (59 patients). High-frequency oscillatory ventilation(HFOV) was used in high lung volume strategy. Results:The two groups of patients were similar in demographic distribution of birth weight, gestational age, gender, surfactant after birth. Patients on HFOV were ventilator \ufffd?dependent (2.14 \xb1 1.76 vs 3.48 \xb1 2.70 day. p=0.004) and respiratory pressure support (4.01 \xb1 2.23 vs 6.55 \xb1 day. p =0.02) for a shorter time than patients on CMV. Conclusion: First intention HFOV with high volume strategy shortened the time of ventilation and pressure support in preterm infants with seriously respiratory distress syndrome. In the future, it will be necessary to have the tremendous research for the efficacy of HFOV in order to decrease the ratio of bronchopulmonary dysplasia of infants who have seriously respiratory distress syndrome required mechanical ventilation.

5.Results of neglected clubfoot treatment in older children and adults by gradual correction using the ilizarov method combined with minimal surgery

Background: Neglected clubfoot is a common congenital abnormality of uncertain aetiology. It presents with abnormal foot, adductus, varus and equinus of the foot. In Vietnam, the number of people with clubfoot rather high.\r\n', u'Objectives: To evaluate the results of using compression distraction counter direction threaded rod (CDCTR) and complications in clubfoot orthopedic surgery for older children and adults.\r\n', u'Subjects and methods: We studied and treated 118 severe clubfeet in older children and adults by compression distraction counter direction threaded rod (CDCTR) combined with minimal operative intervention.\r\n', u'The mean age of patients at the time of the surgery was 28.5 years (with the range of 3.5-54 years) and the mean follow-up time was 7.2 years (6 months-14 years). \r\n', u'Results: All cases achieved a plantigrade foot with better walking ability and great satisfaction with the results. There were no complications of note. The gradual correction by CDCTR combined with minimal operative intervention to treatment neglected clubfoot in the older children and adults achieved satisfactory results for functional and aesthetic aspects of the foot. \r\n', u'Conclusion: The role of physical therapy after clubfeet orthopedic surgery is very important in recovering the shape and function of the foot.\r\n', u'

6.Ankle arthroscopy in Viet Duc Hospital: 10 cases report

Background: Ankle arthroscopy has been used in Viet Duc Hospital for 4 years and now has become a routine surgery with about 200 patients, who has had ankle arthroscopy to reconstruct the anterior cruciate ligament or operate ragged meniscectomy. Objectives: Evaluate the result of ankle arthroscopy in Viet Duc Hospital and review the indication and surgical technique of ankle arthroscopy in the literature. Subject and methods: Retrospective study on 10 patients with ankle arthroscopy in Viet Duc Hospital from May 2006 to May 2007. Results: 5 patients with impingement, 4 patients with OCD lesion and 1 case of an old displaced fracture of lateral malleolar. Postoperative evaluation with Olerud Molander, ankle function improves at least 10 points and all the patients are satisfied with the results. Conclusions: Ankle arthroscopy gives good result and can be recommended for other cases.

7.The efficacy of sodium bicarbonate for treatment of gout

Background: Sodium bicarbonate is highly alkaline and may be helpful in relieving symptoms of gout. Sodium bicarbonate helps to adjust your body's pH level, slightly creating an alkaline-forming state that lessens the likelihood of gout attacks. We can prepare sodium bicarbonate in several ways to treat gout symptoms. Objective: To examine the efficacy of sodium bicarbonate for treatment of gout. Subjects and method: A study was conducted in 54 patients with gout at Department of Rheumatology of Bach Mai hospital, from September 2006 to March 2007. Patients were divided into two groups, group 1 included 29 patients treated with intravenous sodium bicarbonate and group 2 included 25 patients treated with oral sodium bicarbonate. This was a descriptive, cross-sectional study. Results: No significant changes showed between serum uric acid level, 24-hour urine uric acid level at before and after treatment in 2 groups. Gout patients with renal failure had increased level of 24-hour urine uric acid after treatment (p < 0.05). There were significantly increased level of urine volume, urine pH and decreased level of urine specific gravity after treatment. The effect of sodium bicarbonate was not different between intravenous usage and oral usage. Conclusion: Sodium bicarbonate could change urine environment, limit crystallization of urine uric acid of gout patients.

8.Effectiveness and safety of electroacupuncture analgesia in controlling intraoperative pain and hemodynamics during total thyroidectomy: A randomized controlled trial

Objective:
To evaluate the effectiveness and safety of electroacupuncture in conjunction with additional medications in providing analgesia and stabilizing hemodynamic parameters during total thyroidectomy.
Methods:
This randomized controlled trial included 100 patients who underwent a total thyroidectomy between October 2022 and October 2023 at the Vietnam National Hospital of Acupuncture. The patients were randomized into two groups. The electroacupuncture analgesia (EA) group received EA stimulation at five acupuncture points: Hegu (LI 4), Neiguan (PC 6), Shuitu (ST 10), Quepen (ST 12), and Yifeng (SJ 17), while the control group received a bilateral superficial cervical plexus block. Primary outcomes included the level of analgesia and perioperative vital signs in both groups. Additionally, pain thresholds and serum β-endorphin levels were measured before and after electroacupuncture in the EA group.
Results:
Complete analgesia (Level A) was attained in 86% and 76% of the patients in the EA and control groups, respectively, with no significant difference between the two groups (P = 1.00). In the EA group, the mean pain threshold after receiving EA doubled (648.7 (77.4) g/s vs. 305.3 (45.3) g/s, P < .001), and the mean serum β-endorphin level increased by approximately 13.5 pg/mL (P < .001). All patients remained hemodynamically stable throughout the surgery.
Conclusion
EA, in conjunction with additional medications that stimulate five acupuncture points, LI 4, PC 6, ST 10, ST 12, and SJ 17, was well tolerated and effectively maintained a suitable level of analgesia and hemodynamic stability during total thyroidectomy.
9.Situation of extremity injury caused by traffic accident in Viet Duc hospital from 2000 to 2004

Background: Situation of extremity injury caused by traffic accidents and occupational accidents are increasing in Vietnam. Therefore, the prevention and fight against these accidents are one of the key objectives to reduce the burden on the families of victims as well as for society. Objectives: To outline situation of extremity injury patients caused by traffic accident, treated in Viet Duc hospital from 2000-2004. Subjects and method: A retrospective study was conducted on 19.845 patients (14.562 males, 5.283 females, aged from 1-97 years old). The subjects were divided into 3 main groups: alone upper extremity injury, alone lower extremity injury and multiple fractures. Results: Extremity injury was common seen in people of working age (78.1%), men more than women (2.76/1 ratio), left foot more than right foot and it was common been in multiple fracture situation due to complex mechanisms of injury (50.3%), alone extremity injury (49.7%). The lower extremity injury was more common seen than upper extremity injury. Open fracture injury (27.3%), in which open fractures in lower extremity were more common seen. Open fractures in both 2 leg bones was the most common seen, accounted for 57.4% among total of open fractures. Amputation accounted for 8.3% among all case of open fractures and 2.3% among all case of extremity injuries. Conclusion: Extremity injury (lower extremity and upper extremity) caused by traffic accidents is common seen. The average, there are 3 emergency surgeries due to open fracture injury were performed daily in Viet Duc hospital per a total of about 11 traumatic emergency surgeries.

10.Performance and kap of army medical officers at the border area

Background: The combination medical between army and pepople in protection, healthcare for people and army in the whole country was a great policy of The Party and State. Objectives: Evaluate result of combination between armed forces in epidemic control and preventive medicine programs at mountainous and coastal areas of Thua Thien Hue in 2006. Studying knowledge, attitude, practice of army medical officers and men canvassed masses about epidemic control and preventive medicine programs. Subjects and method: Officers, men, army medical officers in 41 communes (28 coastal communes and 13 mountainous communes) of 5 district have combination medical between army and pepople. Using epidemiology method to description, analysis, cross-sectional study. Results: In 2006, home-flies spraying was carried out in 18 out of 20 markets; 1,030 all size restaurants and 18 residential areas of 5 districts. The number of chemically treated latrines in high epidemic areas was accounted fof 15,1%. A population-based latrine construction project was initiated to build 838 new latrines, contributing to minimize the invironmental pollution. 9,17% of waterwells and water tanks were hygienically treated; 11,434 people used clean water source; and 46,436 people washed their hands with soap. Studying knowledge, attitude, practice on preventive medicine programs of army medical officers showed very good results. Conclusion: The combination medical between army and pepople in preventive medicine, epidemic control gave wide efficiency for mountainous and coastal communes of Thua Thien Hue. It actively supported for Health station of 28 coastal communes and 13 mountainous communes in preventive medicine work.
