1.Association between high-risk human papillomavirus DNA load and cervical intraepithelial lesion.

OBJECTIVETo determine the association between viral load of high risk type human papillomavirus (HR-HPV) and stage of cervical intraepithelial neoplasia (CIN) lesion.
METHODSCervical exfoliated cells were collected from 1997 women aged 35-45 in a cross-sectional screening study. HPV DNA was detected by hybrid capture 2 (HC2) system, and viral load was measured by the ratios of relative light units compared to standard positive control (RLU/PC). Log10RLU/PC were categorized into four groups: negative (< 0), low viral load (0 - 1.12), medium viral load (1.13 - 2.23), and high viral load (2.24 - 3.37). Cervical lesions were diagnosed by biopsies as normal, CIN 1, CIN 2-3, and squamous cervical cancer (SCC). Association between HR-HPV and CINs were evaluated by unconditional multinomial logistic regression.
RESULTS100% (12/12) SCC, 97.3% (72/74) of CIN 2-3, 58.3% (74/127) of CIN 1, and 11.5% (205/1784) of normal women were positive for HPV DNA. The median log10RLUs for the positive women with SCC, CIN 2-3, CIN 1 and in normal women were 2.60, 2.32, 2.18 and 1.18 respectively. The odds ratio (OR) between low viral load of HPV DNA and CIN 1 was 3.8 (1.9 - 7.3) while between high viral load and CIN 2-3 was OR=865.9 (200.1 - 3738.0) which showed that higher viral load could increase the risk of cervical lesions (P <0.001).
CONCLUSIONBoth cervical cancer and CINs were highly influenced by HR-HPV viral load.

2.Association between human papillomavirus (HPV) type 16 infection and E6/E7 gene variant and the cervical lesions in Beijing.

OBJECTIVETo investigate the association between HPV16 infection, E6/E7 variations and the cervical lesions.
METHODSHPV subtypes were detected by using flow-through hybridization technique, E6/E7 gene was extracted from cervical lesions in 80 patients with HPV16 infection, PCR amplified, cloned into plasmid pMD18-T and sequenced.
RESULTSHPV 16 was the most common type which accounted for 33.3% (154/463), the HPV16 infection rates increased with the severity of cervical lesions (P < 0.05). Totally in 72 cases the complete E6 and E7 regions were successfully sequenced, the DNA mutation rate of E6/E7 was 88.9% (64/72). A mutation, E6-D32E (T96G) coincided with a specific type of E7 mutation, N29S (A86G). D32E/N29S mutation rate was 38.9% (28/72), the detection rate increased with the severity of cervical lesions (P < 0.05).
CONCLUSIONHPV 16 was the most common type in women with cervical lesions in Beijing, D32E/N29S variant associated with the cervical lesions.

3.Study on the genotyping of human papillomavirus using a new DNA liquid chip in women of high-risk group of Shandong province.

OBJECTIVETo evaluate the diagnostic applicability of human papillomavirus (HPV) liquid chip assay which is based on Luminex XMAP System, and perform a HPV epidemiologic study with the liquid chip in women of Shandong province.
METHODSTo detect HPV genotypes on a 96-well plate with the liquid chip which can simultaneously detect and identify 26 common HPV genotypes in a total of 2925 cervical scrapes obtained from gynecological outpatients as well as to analyze the relationship between HPV types and different cervical diseases by studying the distribution of HPV genotypes and pathologic diagnosis.
RESULTSAmong 639 cases who performed pathologic/cytological and histological diagnoses, 184 cases are in group of normal cytology, 266 cases in group of, 77 cases in group of cervical intra-epithelial neoplasia (CIN) I, 7 cases in group of CIN I - II, 46 cases in group of CIN I - II, 46 cases in group of CIN I - II and 13 cases in group of cervical cancer. The overall incidence of HPV in our samples is 36.0% (1054/2925) and 23 types of all 26 types on liquid chip are found. The most common genotypes found are HPV-16 (26.75%), HPV-52 (25.75%), HPV-58 (10.47%), HPV-18 (8.87%) and HPV-11 (6.94%). Among all the positive types, 87.32% are high-risk HPV and 13.68% are low-risk HPV genotypes. Both single and multiple types are easily identified, showing 66.22% ( n = 698) single type and 33.78% ( n = 356) multiple types. Of all the 1054 HPV-positive cases, 261 (24.8%) is occupied by women 21 to 25 years of age and progressively lower by older age groups, reaching 4.9% by women between 51 to 67 years old. The incidence of HPV in our samples is 23.37%, 33.08%, 54.54%, 57.14%, 82.61%, 91.30% and 100% for normal cytology, inflammation,CIN I ,CIN I - II, CIN II ,CIN III, and carcinomas specimens, respectively. Infections with more that one virus are common, accounted for 4.89%, 7.14%, 18.18%, 28.57%, 41.30%, 43.37% and 38.46% for normal cytology, inflammation, CIN I, CIN I - II, CIN II, CIN III, and carcinomas specimens, respectively. Based on the criteria of histology and pathology, the sensitivity, specificity, positive-predictive value and negative-predictive value of HPV liquid chip assay for detecting all cases of CIN II, III are 88.57%, 76.63%, 68.89% and 92.16% respectively. Conclusion The common types of HPV infection are 16, 52, 58, 18, 11, 6, 56 and 31. The HPV-positive rate increased along with the increase of grading on cervical lesions. There are more younger women among all the HPV-positive ones. Multiplex HPV genotyping by liquid chip appears to be highly suitable for diagnostic screening as well as the conduction of large-scale epidemiological studies.

4.Identification and genotyping of oncogenic type of human papillomavirus in paraffin-embedded cervical cancer samples in Guangzhou.

OBJECTIVETo investigate human papillomavirus (HPV) infection and genotyping in patients with cervical cancer in Guangzhou in the last 3 decades.
METHODSHPV L1 gene fragment in paraffin-embedded cervical cancer samples was amplified by HPV-specific PCR with consensus primers, and typing of HPV strains was performed on the basis of sequence analysis of the PCR products.
RESULTSThe positivity rates of HPV DNA was 26.2% in the 99 cases of cervical cancer. Five HPV genotypes were identified including HPV16, 18, 33, 52 and 58.
CONCLUSIONHPV16, 58 and 33 are the most common genotypes of HPV, accounting for over 88.4% in the total infected cases, suggesting that the HPV genotypes closely related to cervical cancer is more centralized in Guangzhou.

7.Analysis of human papillomavirus infection and typing in Shanxi province.

OBJECTIVETo investigate human papilloma virus (HPV) infection and typing in 7 640 cases of women in Shanxi province in order to provide theoretical basis for the prevention and treatment of the cervical cancer.
METHODSTotally, 7 640 cases of cervical cell specimens in Shanxi provincial tumor hospital, screening and physical examination from 2012 January to 2013 May and 23 HPV genotypes were analyzed by PCR and reverse dot blot gene chip technology.
RESULTSA total of 1 441 cases of patients with HPV infection were to be found in 7 640 cases of women with an average age of (42.26 ± 19.15)years old. The total infection positive rate, infection rate of high-risk HPV, infection rate of low-risk HPV and infection rate of mixed high and low risk were 18.86% (1 441/7 640), 16.03% (1 225/7 640, including multiple high-risk of HPV infection), 4.88% (373/7 640, including multiple low-risk of HPV infection) and 2.05% (157/7 640) respectively. The rate of high-risk HPV infection was 85.01% (1 225/1 441) in total infection positive women. The most common subtype was HPV16 (34.70%, 523/1 507) and followed by HPV58 (11.48%, 173/1 507), HPV18 (7.43%, 112/1 507), HPV33 (7.10%, 107/1 507), HPV56 (6.04%, 91/1 507) and HPV52 (5.51%, 83/1 507) respectively in tested 18 high-risk HPV subtypes, and there was no HPV82. The most common subtype was HPV43 (38.13%, 151/396) and followed by HPV42 (22.22%, 88/396), HPV81 (20.45%, 81/396), HPV6 (11.87%, 47/396) and HPV11 (7.32%, 29/396) respectively in tested 5 low-risk HPV subtypes. The HPV infection positive rates were significantly different in different age groups and HPV total infection rate, high-risk infection rate in 41-50 year-old age group was the highest, 23.23%. The infection rates of single subtype of HPV and single high-risk subtype of HPV were 75.71% (1 091/1 441) and 61.35% (884/1 441) respectively in all cases of infection women and single high-risk subtype infection rate was 81.03% (884/1 091) in all cases of single subtype infection women. The infection rate of multiple subtype of HPV was 24.29% (350/1 441) in all cases with HPV infection. The double infections was most common (18.18%, 262/1 441) in which the women of double high-risk infections of HPV were 151 cases (10.48%, 151/1 441). The rates of double infection, triple infection and quadruple or more infection of HPV were 74.86% (262/350), 20.29% (71/350) and 4.89% (17/350) respectively in all cases with HPV multiple infection. The positive rates of HPV infection in different age groups were obvious difference. The positive rate of single infection of HPV was 17.80% (574/3 224) in cases of 41-50 years old group which higher than that in other groups(χ² = 20.18, P < 0.05).
CONCLUSIONThe more common high-risk HPV subtype is HPV16, HPV58, HPV18, HPV33, HPV56 and HPV52 and low-risk HPV subtype is HPV43, HPV42 in Shanxi province. HPV infection is most common in the age group of 41- 50 years old female.

8.Human Papillomavirus Prevalence and Genotype Distribution among HIV-Infected Women in Korea.

The epidemiology on human papillomavirus (HPV) among human immunodeficiency virus (HIV)-infected women in Korea is not well established. A retrospective study was conducted to determine the prevalence and genotype distribution of HPV infection among HIV-infected women in Korea. HPV DNA genotype and cervical cytology were examined in 60 HIV-positive women and 1,938 HIV-negative women. HPV genotypes were analyzed by using a HPV DNA chip. HIV-infected women had higher prevalence of high-risk HPV (hr-HPV) infection (30% vs 4.9%, adjusted odds ratio [AOR], 6.96; 95% confidence interval [CI], 3.63-13.34, P<0.001) and abnormal cervical cytology (18.3% vs 1.8%, AOR, 10.94; 95% CI, 5.18-23.1, P<0.001) compared with controls. The most common hr-HPV genotype detected in HIV-infected women was HPV 16 (10%), followed by 18 (6.7%) and 52 (5%). Prevalence of quadrivalent vaccine-preventable types (HPV 6, 11, 16, and 18) was 21.7% and 2.3% in HIV-positive women and HIV-negative women, respectively. Age was a significant risk factor for hr-HPV infection in HIV-infected women (P=0.039). The presence of hr-HPV was significantly associated with abnormal cervical cytology (P<0.001). These findings suggest that HPV testing for cervical cancer screening in HIV-infected women would be necessary, particularly among young age group.

9.Clinical Efficacy of HPV DNA Chip Test in the Era of HPV Vaccination: 1,211 Cases, A Single Institution Study.

BACKGROUND: Human papillomavirus (HPV) prophylactic vaccines, bivalent types for HPV-16/18 with 70% prophylactic expectation, have been developed based on the genotypes found prevalent in the western countries, but little is known for those in Korea. Using a DNA chip test, we evaluated the clinical efficacy of HPV genotype based on cervical abnormalities. METHODS: As the initial diagnostic tests, HPV DNA chip tests and Papanicolaou smear (PAP) were used for 1,211 subjects. Cervical colposcopy directed biopsies were performed for 626 among the 1,211 subjects within one month. RESULTS: The most frequently found genotypes in all HPV-positive specimens (n=445) were HPV-16 (22.0%), 58 (13.9%), 52 (11.0%), 51 (9.0%), 56 (8.5%), and 18 (7.2%). HPV prevalence was significantly higher in specimens where PAP and biopsy results were closer to malignancy. The HPV genotype distribution of the histologically confirmed cervical high-grade squamous intraepithelial lesions (HSIL) or carcinoma cases showed HPV-16, 58, 52, 18, and 33, in descending order. The HPV DNA chip sensitivity, specificity, positive predictive value (PPV), and negative predictive value (NPV) for the detection of cervical HSIL or carcinoma were 76.9%, 70.1%, 72.1%, and 75.8%, respectively, Of these, the sensitivity and NPV were higher than those of PAP. PPV and NPV of HPV-16 were 90.5% and 60.7%, respectively, being the highest among the genotypes. CONCLUSIONS: We confirmed that HPV-16 genotype was also very important for the diagnosis of HSIL and cervical carcinoma in Korea. However, contrary to the findings in the western countries, the prevalence of HPV-58 was higher than that of HPV-18. Moreover, as the other HPV genotype reports were rare in Korea, further studies are required with the HPV DNA chip test before the nationwide adoption of the vaccines.
