2.Urethral reconstruction in transvaginal eversion of urinary bladder caused by hydrochloride application.

A case of massive transvaginal eversion of urinary bladder through a large vesicovaginal fistula is reported. It has developed by spillage of hyprochloride on prolapse uteri and accompanied with rectovaginal fistula. Satisfactory surgical repair of the fistula was obtained by reduction of bladder and urethral reconstruction using anterior bladder wall.

3.Clinical Observation on Urinary Fistula.

Twenty-three cases of urinary fistulas which were admitted to the Department of Urology, Chosun University Hospital during the period from Jan. 1973 to Dec. 1979 were analysed clinically. The following results were obtained. 1. Eight cases of all fistulas were in men and 15 in women. 2. The most prevalent age group was 51 to 60 years. 3. The most commonly involved site was urethra (37.5%) and the most common fistula type wag vesicovaginal fistula (30.4%). 4. The most common etiology of vesicovaginal fistula was obstetric cause. 5. Seventeen of 23 urinary fistulas were operated upon and all but 3 were repaired successfully on the first surgical attempt.

Isolated congenital urethrocutaneous fistula of the anterior urethra is an extremely rare anomaly in which, along with a normal urethra and meatus, a fistula is present. These cases usually present in the pediatric age group. To date, only one such case has been described in the adult population. We describe one such case in an adult and discuss the management of this uncommon anomaly.

5.Clinical Observation on Urinary Tract Fistula.

24 cases of urinary fistulae which were admitted to The Department of Urology, Chonnam University Hospital, during the period from Sept. 1968 to Aug. 1971 were analyzed clinically. 1. 15 cases of all fistulae were in women and 9 in men. 2. The most common age was from 30~35 years with 50%. 3. The most commonly involved site was bladder with 66% and the most common fistula type was vesicovaginal fistula (40%). 4. The most common etiology of vesicovaginal fistula was obstetrical cause 5. 11 cases (44%) of all fistulas occurred after the operation.

6.Clinical Observation on Vesicovaginal Fistula.

A clinical observation was made on 20 cases of vesicovaginal fistula admitted to the Department of Urology, presbyterian Medical Center during the period from Aug., 1970 through July. 1977 and the following results were obtained. 1. Among 36 cases of urinary tract fistula, the most commonly involved site was bladder(22cases) and the most common fistula type was vesicovaginal fistula (20cases). 2. Among 20 cases of vesicovaginal fistula, the most common age was between 30 and 50 years. 3. The most common etiology of vesicovaginal fistula was gynecological cause(9 cases). 4. The most common location of vesicovaginal fistula was trigone(13 cases) mainly. 5. On I.V.P. finding of vesicovaginal fistula, 11 cases were normal and 8 cases abnormal. 6. On cystoscopic findings of vesicovaginal fistula, 7 cases were normal, 11 cases abnormal, 3 cases combined with bladder stone and 1 case with vaginal stone. 7. On treatment of vesicovaginal fistula, suprapubic transvesical approach was performed in 9 cases, transvesical transperitoneal approach 4 cases, Foley catheter drainage 3 cases and ileal conduit 1 case. 8. Success rate of repair was 76.9%, suprapubic transvesical 66.7 % and transperitoneal transvesical 100%.

7.Very Late Onset of Radiation-induced Complicated Vesicocutaneous Fistula.

Vesicocutaneous fistula (VCF) secondary to radiation therapy is a rare event. There are difficulties in establishing the early diagnosis and choosing the proper management option. We present a very unusual case of postradiotherapy vesicocutaneous fistula which developed more than 30 years later. Temporary urinary diversion was performed because of poor performance status and anatomical condition. However, it failed to achieve spontaneous closure of VCF. It is important to recognize that late onset of radiation induced VCF could develop even after a substantial period of time has lapsed. In addition, conservative treatment appears to be unsuccessful in patient with complicated VCF. Therefore, it must be counselled carefully after a making synthetic judgment based on different individual situation.

8.Clinical Survey on Urologic Complications Developed after Treatments of Obstetric & Gynecologic Diseases (1972 - 1984).

We reviewed 28 patients admitted due to urologic complications after obstetric & gynecologic operation and management since May 1972. The age distribution, developed urologic complications and it`s treatment are discussed. The prevalent age groups of urologic complications were 30`s and 40`s. The most common urologic complication was urinary fistula : 8 ureterovaginal, 8 vesicovaginal and 4 other fistulas. The results after 1st operation of urinary fistula showed an over-all success rate of 72.6%.

9.Clinical Observation on 52 Cases with Failed Hypospadias Repair.

A clinical observation, giving the first consideration to the complications after repair, was made on 52 hypospadias of the in-patients in the Department of Urology, Yonsei University Hospital during the period Jan. 1972 through Dec. 1981 and the following results were obtained. 1. Of 52 straightenings, 40 cases (91%) were succeed by Duplay's method and 8 cases (100%) were succeed by Nesbit's method. 2. Success rate of 52 urethroplasties was 53.8%. Among them 4 cases by Thiersch-Duplay, 22 cases by Ikoma, 2 cases by Cecil and Denis-Brown's method respectively. 3. The penile type showed best success rate of 66.6%. 4. Urinary fistula complicated the urethroplasty in 22 cases (42.3%). 5. Most of the fistulae developed in original meatus (81.8%). 6. Urethral stricture complicated the urethroplasty in 5 cases (9.6%), 3 of them combined with fistula.

10.Repair of Anorectal Malformation.

Major advances in the management of anorectal malformations have been achieved during the last 20 years. Alberto Pena introduced the posterior sagittal anorectoplasty (PSARP) in 1982. He divided all the sphincteric muscles at the exact posterior midline and fully exposed the crucial anatomy. He was able to manage the associated fistula under direct vision with minimal urinary tract injury. The rectum could be placed in the middle of the sphincteric muscle complex. Even with PSARP by Alberto Pena himself, only 37.5% of all cases were considered totally continent. Anorectal malformation is still acontinuing challenge for the pediatric surgeons.
