1.Clinical Observation on Chronic Prostatitis.

A clinical observation was performed on 154 cases of chronic prostatitis in the Department of Urology, National Seoul Hospital from January 1981 to March 1982. The results are as follows: 1. 121 cases or 78.4 percent of the cases were observed among the ages between 20 to 40 years are more than one half of these cases were seen between the ages of 26 to 35 years. 2. Previous diseases were Non-gonococcal urethritis on 82 cases (53.2%), Gonococcal urethritis on 45 cases(29.2%) and previous prostatitis on 15 cases (9.7%). 12 cases (7.8%) were free of previous diseases. 3. Most frequently observed symptoms consisted of those of mild chronic urethritis such as morning drop, urethral discomfort, perineal discomfort and frequency. 4. In microscopic findings of wet smear of the prostatic secretion, 125 cases (81.2%) showed W.B.C. more than10/H.P,F. 29 cases (18.8%) were within normal limit. 5. In 125 cases in which prostatic gram stain was done, microorganisms were gram (+) cocci on 26 cases (20.8%),gram (-) bacilli on 12 cases (9.6%) and gram (+) bacilli on 10 cases (8.0%). No microorganism were on 77 cases (61.6%).

5.Clinical Observation of Urine Shred.

There have been very rare studies or articles about the shreds of urine. The author had observed microscopically the shreds of urine which were taken from the 624 healthy adult Korean males and obtained the following interesting results: 1) The shreds of urine, in the first urine taken early in the morning, were found in 195 cases (31.3%) among the 624 symptomless adult males. 2) In the above 195 cases, the shreds of urine were found in 45 cases (68.2%) among the 66 cases with a history of previous urethritis. And the shreds of urine were found in 150 cases (26.9%) in 558 cases without a history of previous urethritis. 3) As the result of microscopic examination about the shreds of urine in 624 cases, we found out 57 cases (9.1%) with 5~10 leukocytes/H.P.F. These 57 cases consist of 24 cases (36.4%) in 66 cases with a history of previous urethritis and 33 cases (5.9%) in 558 cases without a history of urethritis.

6.Treatment of Uncomplicated Male Gonococcal Urethritis with Rosoxacin.

Between September to October 1985, 61 male patients with uncomplicated genital gonorrhea, including 33 PPNG urethritis, were treated with single oral dose of rosoxaci, a new oral quinolone derivative, 300 mg Fifty-three patients returned for a test of cure 3 5 days after the treatment. When a patient was found to be not cured, a second course of treatment was given and followed as above. The overall failure rate of initial treatment was 11.3%(6/53). It was 15. 2%(5/33) for PPNG urethritis and 5%(1/20) for non-PPNG urethritis. During the course of this trial the rate af PPNG among pretreatment isolates was exceptionally high(62.2g) compared to 277. during the period of 198l~4. If the PPNG prevalance rate had been a little lower, for instance 30g the overall failure rate would have been much lower(4.9%). When the second treatment was given to the failure cases 4-6 days after the first treatment. The overall failure rate dropped to 3.8%, 3.1% for PPNG urethritis and 5.0% for non-PPNG urethritis. Ten patients (2l, 3p,) developed postgoriococcal urethritie. Susceptibility test, with disks containing rosoxacin 5mcg, showed the inhibition zone to be greater than 34mm. Eight patients developed minor side effects: dizziness or gastritis. The rosoxacin, oral dose of 300 mg, regimen gave the best result yet by oral treatment for gonorrheae in recent years in Korea.

7.Doxytycline in Treatment of Acute Gonococcal Uriethritis.

This series represents 128 cases of acute gonococcal urethritis treated with doxycycline at the Seoul Red Cross Hospital from the 1st. May to.5th, Oct., 1969. Results of this treatment were divided in three groups: Group I ; Single dose of doxycycline, 300 mg. was administered for this group. 36 cases fall into this category. Cure rate for this group was 36%. Group II; Doxycycline, 200mg. was given on the 1st treatment day and daily dose of 100 mg. for 4 ensuing days. 42 cases were included to this group. Cure rate was 72%. Group III Daily dose of 200 mg. was administered for the remainders, 50 cases for 5 consecutive days. Cure rate was 91%. It was concluded that 200 mg. of doxycycline daily for 5 days dose regime can be recommended for the treatment of acute gonococcal urethritis, especially in cases allergic to penicillin or in which penicillin treatment fails. No anaphylactic shock or some other undesirable side reaction in administering this drug was experienced.

8.Surveillance for Spectinomycin Resistant Neisseria Gonorrhoeae.

In 1982, we reported a case of recalcitrant urethritis caused by a strain of spectinomycin resistant PPNG. It caused a great fear that it might be the signal of an end of spectinomycin era, The disk-prediffusion method was used on all isolates from the Choong-Ku VD Clinic between August 1984 and December 1985 to screen out strains of N. gonorrhoeize resistant to spectinomycin, with disks containing 100pg of spectinomycin, Strains with inhibition zone greater tha.n 25 mm were considered susceptible, We could not disclose a single strain which wa.s resistant to spectinornycin, The reason why we do not see any more of spectinomycin. N. gonorrhoeae requires some speculation, It is possible that the mutant which had resistant to spectinornycin might not have teen quite stable and, after series of passage through human hosts, mutated back to original spectinornycin sensitive states. It is suggested that continual surveillance for spectinomycin resistant N. gonorrhoeae should be conducted.

9.The Use of Eradacil(Rosoxacin) in Acute Gonococcal Urethritis.

From July 1982,30 male outpatients with acute gonococcal urethritis, bacteriologically confirmed to be positive for gonococci, were entered in this study. New bactericidal agent Eradacil was administered orally in a signal dose of 300mg in 30 case of acute gonococcal urethritis and following results were obtained. 1. All cases of acute gonococcal urethritis were cured. 2. In 4 cases, there was recurrence of urethral signs and symptoms 3 to 10 days after initial clearing following Eradacil therapy. These 4 case were retreated successfully with Doxycycline. 3. No serious side effects were observed except slight dizziness in one case.
