The surgical treatment of urethral stricture diseases is continually evolving. Although various surgical techniques are available for the treatment of anterior urethral stricture, no one technique has been identified as the method of choice. This article provides a brief updated review of the surgical options for the management of different sites and different types of anterior urethral stricture. This review also covers present controversies in urethral reconstruction. Among the various procedures available for treating urethral stricture, one-stage buccal mucosal graft urethroplasty is currently widely used. The choice of technique for urethroplasty for an individual case largely depends on the expertise of the surgeon. Therefore, urologists working in this field should keep themselves updated on the numerous surgical techniques to deal with any condition of the urethra that might surface at the time of surgery.

4.Perineum-based Pediculated Scrotal Flap for Reconstructive Urethral Surgery.

Herein, we describe a perineum-based pediculated scrotal flap procedure for urethral reconstruction. A scrotal tubular flap was used as a substitute to correct a proximal penile urethral stricture in case 1. In case 2, a scrotal island patch was performed to treat an iatrogenic penile urethral injury. In both cases, the urethral catheter was removed on postoperative day 14 with simultaneous normal voiding cystourethrography. The excellent axial vascularization of this perineum-based pediculated scrotal flap procedure allows successful urethral reconstruction, regardless of extension, location, and etiology.

5.Internal Urethrotomy Using Optical Urethrotome for Urethral Stricture.

Urethral stricture has been most complicated problem in the field of urology. We have experienced 17 times of endoscopic internal urethrotomy in 13 cases with urethral stricture and reviewed results and complications with a brief literatures. Of 13 cases, 9 cases have been followed for 3 to 20 months. Satisfactory results were obtained in 7 cases(77.8%). The complication rate was 11.8%(2 cases). Average duration of retaining catheter was 12 days. In conclusion, endoscopic internal urethrotomy is thought to be safe and first choice of procedure for urethral stricture.

6.Treatment of urethral stricture by internal urethrectomy

We reviewed retrospectively the results in 39 patients who underwent endoscopic urethrotomy during two years 1/1998-12/1999. The results were good; most of patients could void easily. The time of operation was short and patients experienced few complications. Only 3 patients had hematuria, no need for transfusion. The time the catheter remainly depends on the character of the stricture

7.Primarily results of treatment of urethral stricture by intraurethral incision

A retrospective study on 28 patients with urethral stricture in Saint Paul during 1998-1999 was carried out of which 12 patients treated by the intraurethral incision. The results after 3 years of the operation have shown that good outcome (8), medium outcome (3) and poor outcome (1). The intraurethral incision can not substitute the reconstructive surgery in cases of complete and long urethral stricture. It only reduced the rate of reconstructive surgery

9.Clinical Assessment of Visual Internal Urethrotomy as Primary Treatment of Urethral Stricture.

PURPOSE: We describe clinical assessment of visual internal urethrotomy (VIU) as primary treatment modality in 69 men who presented with complete or partial obstructed urethral stricture. MATERIALS AND METHODS: The 63 patients with partial obstructed urethral stricture were treated by typical VIU. The 6 men with complete obstructed urethral stricture were treated by VIU using sound passed through previously placed suprapubic tract as guide. RESULTS: The procedure was completed successfully in 57 of the 63 men with partial obstructed (90.5%) and 3 of 6 patients with complete obstructed urethral stricture (50%). CONCLUSIONS: VIU is a safe and effective as primary treatment plan when urethral stricture is partial obstructed and the impassable urethral segment is shorter than 1.0cm.
