2.Interhemispheric Osteolipoma with Agenesis of the Corpus Callosum.

Osteolipoma is an ossified lipoma with distinct components of fat and bone. We present a case of interhemispheric osteolipoma associated with total agenesis of the corpus callosum. A 20-year-old man complained of severe headache, nausea and vomiting. Brain computed tomography showed a low-density mass in an interhemispheric fissure, with high T1 and T2 magnetic resonance signals compatible with fat. The mass measured 4.9 x 2.9 cm in size and showed peripheral calcifications. There was another small piece of same signal mass within the lateral ventricular choroid plexus. The interhemispheric lesion was removed by an interhemispheric approach. Osteolipoma is rare in interhemispheric region, however, it should be a differential diagnosis of lesions with fat intensity mass and calcifications.

3.An Experimental Study of Noncontact Transscleral Diode Laser Cyclophotocoagulation in Rabbits.

Acute and long-term effects of the noncontact transscleral diode laser cyclophotocoagulation were studied in 6 pigmented rabbits used laser power level was 0.15J, 0.25J, 0.5J and 0.75J. Each eyeballs was enuleated 1.7 and 21 days after cyclophotocoagulation for histopathologic examination. The findings with diode laser were coagulation necrosis of pigmented epitihelium, nonpigmented epitihelium and stroma, congestion of vessel, pigment dispersion and detachment of pigment epithelium. Twenty one days after laser treatment, the light microscopic examination revealed atrophy of nonpigmented epithelium and pigmented epithelium, fibrosls, pigment laden macrophages. These changes were obvious in rabbits treated with a power more than 0.5J. It is suggested that noncontact trasnscleral cyclophotocoagulation with diode laser is useful in lowering intraocular pressure.

4.Evaluation of an Automated Solid-phase Cell Adherence Assay in the Galileo System (Immucor) for Routine Pretransfusion Tests.

BACKGROUND: In spite of a trend of automation for conducting most clinical laboratory tests, many blood banks are still dependent on manual tests. The aim of this study was to evaluate a full automation system called the Galileo (Immucor, USA) for conducting pretransfusion tests. METHODS: From August to October in 2009, a total of 3,002 cases of ABO-RhD typing and 1698 cases of antibody screening were compared between using manual tests and the Galileo system at Seoul National University Hospital, Seoul, Korea. For the manual tests, we used the slide method for ABO-RhD typing and the anti-human immunoglobulin treated microplate method for antibody screening. The Galileo system used the microplate method for ABO-RhD typing and the solid-phase red cell adherence (SPRCA) method for antibody screening. We calculated the overall concordance rate and the false positive or negative rates regarding the manual method as a standard test and the Galileo system as a comparative test. RESULTS: When comparing 3,002 cases of ABO-RhD typing, 52 cases (1.7%) were retested. A discrepancy between the two tests remained in 17 cases (0.6%) after repetition, which led to a concordance rate of 99.4% (2,985/3,002). On the comparison of 1,698 cases of antibody screening, 54 cases (3.2%) were retested. A discrepancy between the two tests remained for 30 cases (1.8%) and the concordance rate was 98.2% (1,615/1,698). Among the 20 false negative cases (1.2%), nine were thought to be cold antibodies with no cases of confirmed warm antibody. CONCLUSION: The automated Galileo system and the preexisting manual tests showed very good concordance for ABO-RhD typing and antibody screening. The Galileo system can be used with confidence for routine pretransfusion tests.

5.Prognostic Significance of VEGF in Human Stomach Cancer.

PURPOSE: Angiogenesis plays an important role in the growth, progression and metastasis of solid tumors. Vascular endothelial growth factor (VEGF) was thought to be one such angiogenic factor and was also thought to be a selective mitogen for endothelial cells. The purpose of this retrospective study was to evaluate for prognostic significance of VEGF in stomach cancer. MATERIALS AND METHODS: The sections of formalin-fixed, paraffin embedded from 55 stomach cancer were stained immunohistochemically for VEGF. The rate of VEGF expression and correlation between expression of VEGF and other prognostic factor of stomach cancer were studied. RESULTS: There were 20 cases (36.4%) of VEGF-positive and 35 cases (63.6%) of VEGF- negative. There were no significant difference between VEGF expression and the histologic type, differentiation, depth of invasion of histologic stage, lymph node involvement. The frequency of hepatic recurrence was higher in patients with VEGF-positive tumor than that af patient with negative tumor (p=0.007). The prognosis of the patients with VEGF positive tumor was worse than that of patients with VEGF negative tumor (p=0.0214). CONCLUSION: There was a closely significant between positive expression of VEGF and a high incidence of hepatic metastasis, low survival rate. The expression of VEGF could be considered to be one of useful prognostic factor in human gastric carcinoma

PURPOSE: This study was aimed to determine the role of the VEGF and MVD expression in infiltrating ductal carcinoma of breast and to observe the correlation between the expression of these VEGF/MVD, and other prognostic factors. MATERIALS AND METHODS: Immunohistochemical staining of VEGF and MVD with monoclonal antibody in pathologic specimens of 35 patients of infiltrating ductal carcinoma of breast was carried out. Reiationship between the expression of the VEGF/MVD and prognostic factors were assessed. RESULTS: The VEGF/MVD expression was closely related to tumor size, lymph node metastasis, and clinical stage, but not related to histologic grade, nuclear grade, estrogen receptor, and progestrone receptor. And the VEGF expression was closely related to MVD. CONCLUSION: The VEGF expression and microvessel density in infiltrating ductal carcinoma of breast may play an important prognostic factors, closely related to the tumor size, lymph node metastasis, and stage.

7.Preliminary Experience in Stereotactic Radiosurgery with the Linear Accelerator.

It was already reported in else where that a standard linear accelerator, with only slight modification can function in a manner similar to Leksell's Gamma unit device in the treatment of inoperable deep seated intracranial lesions. Using collimated narrow beams, a localization system and special computer programs for precise patient positioning and a high concentration of irradiation dose within the lesions, eleven patients with AVMs and brain tumor have now been treated since 1988. The 6 MeV linear accelerator and Hitchcock stereotactic frame was used of the all procedures. In 10 of the 11 patients, no side effects occurred. The first 7 patients, who could be observed 3 months or longer have been studied radiologically and clinically. In 6 patients marked decrease in contrast enhancement and sometimes dramatic clinical improvement was achieved. In one case a marked increase of the surrounding brain edema occurred 7days after radiosurgery, who was operated on. The radiantion dose was based on the volume, location and radiosensitivity of the lesion which ranged 15-40 Gy.

8.Correlation of VEGF with contrast enhancement on dual-phase dynamic helical CT in liver tumors: preliminary study.

The purpose of this preliminary study is to elucidate that vascular endothelial growth factor (VEGF) influences contrast enhancement of hepatic tumors on computed tomography (CT). Fourteen patients with hepatic tumors (11 hepatocellular carcinomas; 3 metastatic cancers) underwent a dual-phase dynamic helical CT or computed tomographic hepatic arteriography. The attenuation of each mass was determined as hyperattenuation, isoattenuation or hypoattenuation with respect to the adjacent nontumorous parenchyma. Gun-needle biopsy was done for each tumor, and paraffin sections were immunostained with anti- VEGF antibody by the avidin-biotin-peroxidase complex method. The pathologic grade was made by intensity (1 +, 2+, 3+) and area (+/-, 1 +, 2+). The tumor ranged 2.0-14.0 cm in size (mean, 5.8 cm). In arterial phase, the intensity was not correlated with the degree of enhancement (p=0.086). However, the correlation between the attenuation value of hepatic arterial phase and the area of positive tumor cells was statistically significant (p=0.002). VEGF may be the factor that enhances the hepatic mass with water-soluble iodinated contrast agent in CT.

9.Evaluation of Theophylline Dosing Equation in Korean Infants with Bronchial Asthma.

It has been reported that theophylline clearance is lower in Korean infants than in Americans, suggesting that dose requirements for theophylline in Korean infants will be 1ower than those in Americans. Therefore, we reviwed patients charts retrospectively to formulate the new theophylline dosing equation appropiate to Korean infants. We reviewed patient charts of 111 asthmatic infants who received intravenous aminophy1line and oral theophylline in Wa1lace Memorial Baptist Hospital. We set the therapeutic serum concentration at 5-15microgram/ml in Korean infants based on response rate to theophylline therapy, which is supported by National Institute of Health Expert Panel Recommendations. Doses required to achieve a therapeutic serum concentration increased with age. The mean dose among the infants less than 4 months of age was 578+/-154mg/kg/day and increased to 10.12+/-1.78mg/kg/day after 8 months (p<0.0001). The relationship between age and dosage requirements can be expressed by the least squares determination of the linear regression :Dose (mg/kg/day) = 557 + 0.14 X (age in weeks). The coefficient of correlation for this relationship is 0.67(p<0.0001). Half-lives of elimination among the infants studied correspondingly decreased with age. In conclusion, this study showed that the relationship between age and dosage requirements in Korean infants may be different from those in Americans. Therefore, we strongly suggest that it is required to formulate a new theophylline dosing equation appropriate for Korean infants through well-designed prospective studies.

10.The Prognostic Significance of Serum CA 125 Levels in Patients with Advanced Serous Epithelial Ovarian Cancer.

OBJECTIVE: In this retrospective study, we analyzed the clinicopathologic characteristics of advanced serous epithelial ovarian cancer and evaluated the prognostic factors which have an impact on survival of patients with epithelial ovarian cancer. We also compared several analyzing methods of CA 125 for predicting prognosis of advanced serous epithelial ovarian cancer. METHODS: A total of 57 patients with advanced serous epithelial ovarian cancer who were treated at Samsung Medical Center between Aug. 1995 and Jul. 2000 were included. Medical records including pathologic reports were reviewed to identify clinicopathologic characteristics and serum levels of CA 125. Five different methods of analyzing serum CA 125 were compared for predicting the prognosis of advanced serous epithelial ovarian cancer. Survival curves were analyzed by Kaplan-Meier method and Cox proportional hazards model. RESULTS: The median age of all patients was 54.2 years. In univariate analysis, age, FIGO stage, grade, residual tumor and ascites showed no significant correlation with prognosis. Among five different analyzing methods of serum CA 125 levels, normalization after 3rd chemotherapy was correlated with improved 5-year survival rate (p=0.026). CONCLUSION: This study showed that normalization of serum levels of CA 125 after 3rd chemotherapy is useful for predicting prognosis of advanced serous epithelial ovarian cancer patients.
