3.Clinical Analysis of the Results of Reoperations for Recurrent Herniated lumbar Disc.

Thirty-one Patients who underwent reoperation from Jan. 1983 to Dec. 1988. were analysed retrospectively. The results of reoperation showed the excellent recovery in 13 cases(42%) and good recovery in 12 cases(39%) in terms of pain relief and working capability. But 6 cases(19%) couldn't be managed successfully. The most common intraoperative findings were true recurrences(14 cases: 45%), missed disc material(7 cases: 23%), and small recurrent fragment embedded in epidural fibrosis(7 cases: 23%). The clinical result is affected predominantly by the intraoperative pathology and the time interval between the first and second operation. The excellent or good results could be achieved among patients with true recurrences occurred after more than 6 months following initial surgery and among the patients who were operated again within one month because of missed disc material, hematoma, and inadequate laminectomy and discectomy. But the patients who underwent reoperation between 1 month and 6 months because of a small recurrence and fibrosis or epidural fibrosis showed the poor results. The complete removal of disc, adequate bleeding control, adequate laminectomy were considered to be helpful to reduce the chance of reoperation. Microsurgery is advisable in reoperations because it can facilitate the dissection between fibrous scar and nerve root, making the small recurrent disc material removed completely.

4.Renal Hemodynamic Changes in Acute Ureteral Obstruction.

Between August 1993 and Apri1 1994, intrarenal arterial color duplex Doppler sonography was performed on 55 patients(110 kidneys) with acute ureteral obstruction and 12 patients(24 kidneys) with painless ureteral obstruction by unilateral ureteral stones to assess renal hemodynamic changes and its clinical applications. The mean resistive index(RI) of the doppler waveforms obtained on the acutely obstructed 55 kidneys(0.663+/-0.055) was significantly higher than that of the contralateral normal kidneys (0.613+/-0.052) (P<0.05). We found no RI difference between the contralateral normal 55 kidneys(mean: 0.613+/-0.052) in patients with acute ureteral obstruction and the painless obstructed 12 kidneys(mean: 0.595+/-0.029). In the acutely obstructed 55 kidneys, the mean RI of the upper pole(0.674+/-0.054) was significantly higher than that of the mid portion(0.661+/-0.064) or the lower pole(0.654+/-0.065,r<0.05). No difference of the RI was found between the hydronephrotic 31 kidneys(mean: 0.671+/-0.063) and nonhydronephrotic 24 kidneys(mean: 0.65+/-0.041) in the acutely obstructed 55 kidneys(P>0.05). There was no correlation between the levels of ureteral obstruction and intrarenal RI values(P>0.05). The RIs of 3 significantly obstructed kidneys decreased after relief of obstruction. We conclude that the RI is easily obtained and measured, and it would provide important physiological informations on renal vascular impedance in acute ureteral obstruction. Renal doppler findings could be the one of the criteria for assessing the upper urinary tract obstruction.

5.A Study on Assessment of CAPs (Client Assessment Protocols) using MDS-HC 2.0 on City Elderly .

BACKGROUND: This study used MDS-HC 2.0 (Minimum Data Set-Home Care) to analyze the health and the state of function of the traveling health objects. This study was intended to make use of it with the basic materials for providing them with traveling health service suited for the requirement on the health of the traveling health objects. METHODS: The subjects of this study were 1160 people (over 65 years) living under management control of local Health Center from September 7th to October 3th, 2006. This study was analyzed with inter RAI program & SPSS/WIN 10.0, chi-square -test, t-test, and ANOVA. RESULTS: The result showed that 8.97 CAPs per an elderly person was identified and the subjects over 60% had an injury problem from a fall, health prevention service, vision, IADL, pain, and cognition. The number of CAPs in general was high in higher age and the less educated, and those without a job. But, in subjects that had a life partner and a spouse, the number of CAPs was low. In CAPs by the distinction of sex, CAPs which was much more in man than women in statistics were the improvement of health, the abuse of alcohol and drinking wine, bedsore, and the weak supply system. CAPs which women had much more were the function of the heart and the lungs, pain, the performance of the doctor's advice, health prevention service, and incontinence of urine and insertion of catheter. According to the results comparing CAPs by the level of the ADL, the number of CAPs was shown that the group of ADL 2 was higher than group ADL 1. The matter in which the traveling health service had to be applied in all both ADL1 and ADL2 was injury from a fall, health prevention service, and vision. Conculsion: MDS-HC is applicable to decide the care needs for health and social service supplies. The results can be further applicable for careplan, and referral criteria in continuum of care service over long-term care spectrums.

6.Antihypertensive Effect of Low-Dose Clonidine Monotherapy.

The antihypertensive effect of clonidine was evaluated in 129 patients with essential hypertension of mild to moderate severity with a dosage of one tablet(0.075mg) a day for 71 patients of Group I and two tablets a day for 58 patients of Group II. The results were as follows : 1) Of 71 patients of Group I, 34 patients(47.9%)showed good antihypertensive effect(greater than 15 mmHg drop in mean diastolic pressure) and 11 patients(15.5%) showed fair effect(10-14 mmHg drop in mean diastolic pressure). 2) Of 58 patients of Group II, 39 patients(67.2%) showed good effect and 7 patients(12.1%) showed fair effect. 3) The side effect observed was transient and tolerable dry mouth, which developed in 5.6% of Group I patients and 13.6% of Group II patients.

8.High-Dose intravenous immune globulin therapy for hyperbilirubinemia caused by ABO incompatibility.

Four newborn infants with hyperbilirubinemia, caused by ABO blood group incompatibility, were treated with high-dose intravenous immune globulin(IVIG). As soon as the diagnosis was clinically suspected, these infants received conventional treatment including phototherapy and were monitored closely for bilirubin levels. When bilirubin concentrations reached the risk point in spite of phototherapy, IVIG was given at a dose of 1g/kg for 6 hours. In all cases, bilirubin levels declined within 12 hours after LVIG therapy, and to rebound effect was seen, No side effects of IVIG treatment were observed. We suggest that high-dose IVIG therapy may be useful in the treatment of hyperbilirubinemia due to ABO incompatibility, and reduce the need for exchange transfusion.

9.MR Imaging of the Dural Ligaments' Cadaveric and Clinical Study.

OBJECTIVE: The purpose of this study was to describe the MR imaging appearance of the dural ligaments which is connected between the anterior dura mater and the posterior longitudinal ligament. MATERIALS AND METHODS: In 10 cadavers with formalin fixation, the dural ligaments were evaluated and photographed after removal of the posterior element of the spinal canal. MR studies of 12 patients who had distinct dural ligaments were evaluated. Spin-echo MR images were obtained on a 1. 5T unit. RESULTS: In seven of the ten cadavers, the midline dural ligament was observed; the lateral dural ligament was observed in 3 cadavers. On MR, the dural ligaments were imaged as thin lines of low signal intensity between the anterior dural margin and the posterior longitudinal ligament. Dural ligaments were well-visualized at the level of the lower lumbar spine which had a more prominent anterior epidural space than that of the upper level of the spine. Eight of 12 cases had well developed midline dural ligaments. Two cases showed distinct lateral dural ligaments and the remaining 2 cases had distinct midline and lateral dural ligaments. CONCLUSION: MR images of the dural ligament demonstrate a thin low signal line at the anterior epidural space.

10.A Case of Bilateral Adrenal Masses due to Tuberculosis.

Bilateral adrenal tuberculosis is a rare disease and often occurs bilaterally. We report a case of bilateral adrenal masses due to tuberculosis with adrenal insufficiency. The patient was a 39- year-old man who had complained of intermittent pain of right upper quadrant and general weakness. The plasma levels of cortisol and catecholamine were normal. The levels of 24-hour urinary catecholamine and VMA were also normal. But the levels of 24-hour urinary 17- hydroxycorticosteroids and 17-ketosteroids were reduced. Abdominal CT showed about 6.5 x4.8 x 5.4cm sized left adrenal mass and 4.0 x 2.8 x 3.6cm sized right adrenal mass with calcification. The result of sono-guided percutaneous needle biopsy was adrenal tuberculosis. The patient was treated with antituberculous chemotherapy and hormonal replacement. But the masses are unchanged during 5-months follow-up.
