In the field of surgery, there are a large number of studies in progress for more minimally invasive surgery and cosmetic improvements. Among these studies, NOTES (natural orifice transluminal endoscopic surgery) and single-port laparoscopic surgery are the 2 studies attracting the most attention. Single-port laparoscopic surgery is a procedure where laparoscopic surgery is performed through a single incision at the umbilicus or another region. Until now, many kinds of surgeries were performed by single-port laparoscopy. In this article, the authors attempt to explain the procedures of single-port laparoscopic appendectomy in detail based on our experiences.

3.Immediate Umbilical Reconstruction after Removal of Umbilicus due to Urachal Cancer: Defatting, Anchoring and Purse-String Suture.

Reconstruction of the umbilicus is not a frequent event. Umbilical restoring should always be aimed at creating an umbilicus of sufficient depth and good morphology(circular and cylinder shape) with less scarring. A wide range of techniques has been described to reconstruct neoumbilicus; nevertheless, a perfect result is difficult to obtain. I provide another alternative to reconstruct the umbilicus using defatting, anchoring and purse-string suture. The important steps are the following; the first step is defatting of 2.0cm umbilicus' circle. The second step is to umbilical fixation to the fascia wall using anchoring suture. The third step is purse-string suture to decrease new formative umbilicus and to make the circular and cylinder shape umbilicus. The author's method is a simple, easy and useful technique and renders better aesthetic result. So, I reviewed two cases and discussed the literatures.

Based on the theories of I-Ching and umbilicus-hologram, the navel acupuncture is considered as a new acupuncture therapy that only acupuncture at Shenque (CV 8). It has a good effect on the treatment of syndrome and provides a new treatment idea for syndrome. This article presents the definition, etiology and treatment of syndrome, and introduces the application of umbilical-holographic, the principle and method of positioning and needle-inserting, the adjustment of therapies and the analysis of cases, in order to introduce the idea of treating syndrome by I-Ching navel acupuncture.

Heterotopia refers to the finding of normal tissue in foreign sites, entirely separate from the main organ. Heterotopic gastric mucosa has been observed throughout the alimentary tract, everywhere from the oral cavity to the rectum. However, occurrences in the umbilicus are an extremely rare and peculiar phenomena. We report the case of heterotopic gastric mucosa in the umbilicus.

7.A Case of Omphalith.

Omphalith is a compaction of keratinous and sebaceous material in the deeply set umbilicus. It may be clinically mistaken for a malignant melanoma because of its pigmented surface and sudden appearance. However, careful probing and removal of this umbilical mass, followed by histopathological examination readily identifies its true nature. We describe a patient who presented with a firm, black umbilical mass diagnosed as an omphalith and reviewed the differential diagnosis of a firm, black umbilical mass.

9.Immediate Umbilical Reconstruction for Urachal Cancer.

The umbilicus is a central line of abdomen and can be an important aesthetic interest in the abdomen wall, so its loss can be a source of psychological distress. Many techniques for reconstruction of a lost umbilicus have been described, however none has achieved a perfect result. There are few reports that describe the procedure for immediate reconstruction of umbilicus after surgical ablation except Miller's Iris technique. We have performed umbilical reconstruction right after the umbilicus was resected during the surgery. The patient was a 34 year old man with urachal cancer, who wanted to have his lost umbilicus reconstructed immediately for personal, social, and aesthetic reasons. We tried immediate reconstruction with modified iris technique. Using three local flaps, we fixed the central portion of inferior flap at the area of 1 cm below umbilical defect on linea alba. The neoumbilicus retained in its depth and natural shape. This technique is considered to be simple and draw good results.

10.Reduction Umbilicoplasty for Bizarre Big Umbilicus after Abdominoplasty.

A cosmetically pleasing shape of the umbilicus is essential, especially in aesthetic operation. Umbilical restoring should always be aimed at creating an umbilicus of sufficient depth and good morphology with less scarring. But such a goal is often difficult to attain. The author's reduction umbilicoplasty is a useful technique in case of abnormal large umbilicus after abdominoplasty. The operation renders better aesthetic result. Technically, the procedure is easy to do and learn. The important steps are the following; the first step is reduction of original umbilicus' circle. The second step is umbilical fixation to the fascia wall using anchoring suture. The third step is cinching suture for decrease "the window" of abdominal flap and this step is more important than other steps. Finally, the suture performed between "the window" and reduced umbilicus. I operated the abdominal large umbilicus using above reduction umbilicoplasty. So, I reviewed that case and discussed the literatures.
