1.Title Delayed Central Conduction Time on Brainstem Auditory Evoked Potential Pathway in Diabetic Patients: Functional Origin? Or Structural Origin?.

BACKGROUND: It has been well known that absolute and interpeak latencies of brainstem auditory evoked potentials (BAEP) are usually prolonged in diabetics. However, Its etiology is still controversial. We tried to identify whether the cause is structural or metabolic in origin by performing BAEP and brain MRI in the diabetic patients. METHODS: BAEP were performed in both the diabetic patients (DM) group (16 males and 15 females) and the normal control group (25 males and 33 female). A brain MRI was performed in the DM group on those who showed abnormal BAEP and com-pared the results of BAEP of the DM group with those of the control group. RESULTS: 7 patients (22.6%) showed abnor-mal BAEP (male; 6, female; 1, unilateral; 4, bilateral; 3) when abnormal BAEP was defined as being larger than two and a half standard deviations of the control group BAEP results. Two males of the DM group who showed abnormal structural lesions of the pons in their brain MRI were not included in the statistical analysis. The remaining 14 diabetic male patients (mean age: 58.7 +/-9.1 years, mean disease duration: 6.1 +/-4.7 years, mean hemoglobin (Hb) A1C: 7.7 +/- 2.0%) and 15 diabetic female patients (mean age: 60.6 +/-10.8 years, mean disease duration: 5.4 +/-5.1 years, mean HgA1C: 7.8 +/-2.1%) were stastistically analyzed. Interpeak latencies of I-III, III-V, and I-V were found to be signifi-cantly prolonged in the DM group. The prolongation of interpeak latencies of I-III and I-V were found to be signifi-cantly correlated with the disease duration only in the diabetic female patients, but not with age and HbA1c. CONCLUSIONS: These findings suggest that both metabolic derangement and structural lesion contribute to prolonging the central conduction time on BAEP pathway in diabetics.

2.Studies on Fatty Acid Intake Patterns, Serum Lipids and Serum Fatty Acid Compositions of High School Students in Seoul.

The purpose of this study was to investigate the pattern of individual fatty acid intake and to compare serum lipid levels and total serum fatty acid composition of high school students in Seoul (total:234;male:91;female:143). In serum lipid levels, total cholesterol (Chol.), HDL -Chol. and LDL-Chol. levels of female students were significantly higher than those of male students and there was no significant difference between High Fish & Low Meat intake (HFLM) and Low Fish & High Meat intake (LFHM) groups. The average fat intake was 22 - 25 energy % of total subjects and especially, that of LFHM group was 29%, which were over the recommendation level. Although the average P/M/S ratio of dietary fat was 1.1/1.2/1.0, the average range of omega 6/omega 3 fatty acid ratio of dietary fat was found to be 17.9 - 20.7, which was far beyond the suggested range, 4 - 10. The average intake of cholesterol of total subjects was 360mg. LFHM group had more meats and beverages such as carbonated drinks and tended to have less beans, vegetables and mushrooms. In addition, LFHM group had more energy and fat intake than those of HFLM group, the P/S ratio of dietary fat (0.73) was lower than the recommended ratio. Serum C16:0 composition of LFHM group was significantly higher than that of HFLM group, and EPA and DHA composition of HFLM was significantly lower than that of LFHM. Therefore, in HFLM group, the P/S ratio of serum fatty acids was significantly higher and the omega 6/omega 3 ratio was lower. Dietary C18: 0 was negatively correlated with serum EPA and DHA composition. Individual PUFA intake was negatively correlated with serum C16:0 and sum of SFA, and positively correlated with serum C18:2omega 6 (LA), sum of omega 6 and sum of PUFA. Serum C18:1, C18:3omega 3 and C20:4omega 6 (AA) compositions were not correlated with dietary fatty acid. Only serum triglyceride (TG) levels were significantly correlated with serum fatty acid compositions. Sum of SFA, C14:0, C16:0, sum of MUFA and C18:1 compositions were positively correlated with serum TG levels, but LA, AA, sum of PUFA and P/S ratio were negatively correlated with it.

3.A Retrospective study on the survival rate of the sinus perforated implants.

Perforation of maxillary sinus is a common complication of implant placement in posterior maxilla. The purpose of this study was to evaluate the prognosis of sinus perforated implants placed in partially edentulous maxillae. Eighteen sinus perforated implants in 15 patients were examined for cumulative survival rate, radiographic preoperative bone level, and radiographic marginal bone level change. Twenty-two non-perforated implants in the same patients served as control. The results were as follows; 1. There was no statistically significant difference in cumulative survival rate between sinus perforated implants and non-perforated implants (P>0.05). 2. There was no statistically significant difference in the marginal bone level between sinus perforated implants and non-perforated implants (P>0.05). 3. There was no statistically significant difference in cumulative survival rate according to the preoperative bone level (P>0.05). These results suggests that perforation of maxillary sinus may not affect implant success in posterior maxillae.

4.Coaggregation between Porphyromonas gingivalis and Tannerella forsythia.

Dental plaque, a biofilm consisting of more than 500 different bacterial species, is an etiological agent of human periodontal disease. It is therefore important to characterize interactions among periodontopathic microorganisms in order to understand the microbial pathogenesis of periodontal disease. Previous data have suggested a synergistic effect of tow major periodontal pathogens Porphyromonas gingivalis and Tannerella forsythia in the periodontal lesion. In the present study, to better understand interaction between P. gingivalis and T. forsythia, the coaggregation activity between these bacteria was characterized. The coaggregation activity was observed by a direct visual assay by mixing equal amount (1 x 10(9)) of T. forsythia and P. gingivalis cells. It was found that the first aggregates began to appear after 5-10 min, and that the large aggregates completely settled within 1 h. Electron and epifluorescence microscopic studies confirmed cell-cell contact between two bacteria. The heat treatment of P. gingivalis completely blocked the activity, suggesting an involvement of a heat-labile component of P. gingivalis in the interaction. On the other hand, heat treatment of T. forsythia significantly increased the coaggregation activity; the aggregates began to appear immediately. The coaggregation activity was inhibited by addition of protease, however carbohydrates did not inhibit the activity, suggesting that coaggregation is a protein-protein interaction. The results of this study suggest that coaggregation between P. gingivalis and T. forsythia is a result of cell-cell physical contact, and that coaggregation is mediated by a heat-labile component of P. gingivalis and T. forsythia component that can be activated on heat treatment.

5.Influence of cavity size and restoration methods on the cusp deflection in composite restoration.

The aim of this study was to measure the cusp deflection during composite restoration for MOD cavity in premolar and to examine the influence of cavity dimension, C-factor and restoration method on the cusp deflection. Thirty extracted maxillary premolar were prepared to four different sizes of MOD cavity and divided into six groups. The width and depth of the cavity were as follows. Group 1; 1.5 x 1 mm, Group 2; 1.5 x 2 mm, Group 3; 3 x 1 mm, and Group 4-6; 3 x 2 mm respectively. Group 1-4 were restored using bulk filling method with Z-250 composite. However, Group 5 was restored incrementally, and Group 6 was restored with an indirect resin inlay. The cusp deflection was recorded at the buccal and lingual cusp tips using LVDT probe for 10,000 seconds. The measured cusp deflections were compared between groups, and the relationship between the cube of the length of cavity wall/the cube of the thickness of cavity wall (L3 / T3), C-factor and cusp deflection or %flexure (100 x cuspal deflection / cavity width) was analyzed. The cusp deflection of Group 1-4 were 12.1 microm, 17.2 microm, 16.2 microm and 26.4 microm respectively. The C-factor was related to the %flexure rather than the cusp deflection. There was a strong positive correlationship between the L3 / T3 and the cusp deflection. The cusp deflection of Group 5 and 6 were 17.4 microm and 17.9 microm respectively, which are much lower value than that of Group 4.

7.Esthetic evaluation of maxillary single-tooth implants in the esthetic zone.

PURPOSE: The aim of this study is to assess the influence exerted by the observer's dental specialization and compare patients' opinion with observers' opinion of the esthetics of maxillary single-tooth implants in the esthetic zone. METHODS: Forty-one adult patients, who were treated with a single implant in the esthetic zone, were enrolled in this study. Eight observers (2 periodontists, 2 prosthodontists, 2 orthodontists and 2 senior dental students) applied the pink esthetic score (PES)/white esthetic score (WES) to 41 implant-supported single restorations twice with an interval of 4 weeks. We used a visual analog scale (VAS) to assess the patient's satisfaction with the treatment outcome from an esthetic point of view. RESULTS: In the PES/WES, very good and moderate intraobserver agreements were noted between the first and second rating. The mean total PES/WES was 11.19 +/- 3.59. The mean PES was 5.17 +/- 2.29 and mean WES was 6.02 +/- 1.96. In the total PES/WES, the difference between the groups was not significant. However, in the WES, the difference between the groups was significant and prosthodontists were found to have assigned poorer ratings than the other groups. Periodontists gave higher ratings than prosthodontists and senior dental students. Orthodontists were clearly more critical than the other observers. The statistical analysis revealed statistically significant correlation between patients' esthetic perception and dentists' perception of the anterior tooth. However, the correlation between the total PES/WES and the VAS score for the first premolar was not statistically significant. CONCLUSIONS: The PES/WES is an objective tool in rating the esthetics of implant supported single crowns and adjacent soft tissues. Orthodontists were the most critical observers, while periodontists were more generous than other observers. The statistical analysis revealed a statistically significant correlation between patients' esthetic perception and dentists' perception of the anterior tooth.

8.Effect of supportive periodontal therapy on the prevention of tooth loss in Korean adults.

PURPOSE: The purpose of this retrospective study was to evaluate the effect of patient compliance with supportive periodontal therapy (SPT) on tooth loss in Korean adults. METHODS: The periodontal records of 134 patients were reviewed for this study. They completed active periodontal treatment from 1999 to 2001 and were placed on a schedule of periodic follow-up visits for SPT. Patient compliance was classified into complete compliance (CC), erratic compliance (EC), and noncompliance (NC) groups. Re-examinations were carried out 11.0+/-0.8 years after the active periodontal treatment. The prognosis for each tooth was determined as good, questionable, or hopeless according to the bone loss observed in pretreatment radiographs. RESULTS: The rate of tooth loss of the CC group was significantly lower than that of the NC group. The tooth loss/patient and the tooth loss/patient/year were not significantly different between the three groups. The rates of tooth loss in the good, questionable, and hopeless prognosis groups were 6.7%, 9.5%, and 13.2%, respectively. For the teeth with a good prognosis, the rate of tooth loss of the CC group was significantly lower than that of the NC group (0.4% vs. 5.1%). For the teeth with a questionable prognosis, the CC group showed a significantly lower rate of tooth loss than did the EC group (4.1% vs. 30.7%) or the NC group (4.1% vs. 25.6%). For the teeth with a hopeless prognosis, the rates of tooth loss were not significantly different among the three groups. CONCLUSIONS: Within the limits of this study, the patients who showed a poor compliance with SPT were more likely to lose teeth than were the regularly compliant patients. However, the risk of tooth loss with a hopeless prognosis was high irrespective of the compliance.

9.Darkfield microscopic study of the bacterial morphotypes in the periodontal pockets of Korean adult periodontitis patients.

The purpose of this study was to evaluate the difference of subgingival bacterial compositions between periodontally healthy and diseased sites. Subgingival plaque samples were obtained from 100 sites in 20 untreated adult periodontitis patients(experimental group), and 100 sites in healthy individuals(control group). Before sampling, probing pocket depth(PPD) and clinical level of attachment(CAL), Plaque Index(PI), and Sulcus Bleeding Index(SBI) were recorded for each sampled sites. Microbial samples were collected from the bases of gingival sulci or periodontal pockets with sterile curettes. The samples were examined under darkfield microscope(x400). At least 150 bacteria were evaluated and categorized on the basis of bacterial morphology and motility, i.e. cocci, non-motile rods, motile rods, and spirochetes. In control group, subgingival microbial flora consisted of 73.7% of cocci, 20.0% of non-motile rods, 4.3% of motile rods, and 2.0% of spirochetes. The microbial samples from experimental group consisted of 51.5% of cocci, 19.4% of non-motile rods, 17.6% of motile rods, and 11.6% of spirochetes. The proportion of cocci was higher in control group than in experimental group. Proportions of motile rods and spirochetes were higher in experimental group than in control group. The proportion of nonmotile rods in experimental group and control group was not significantly different. Sulcus Bleeding Index and Plaque Index showed high correlation with the bacterial composition. These findings suggests that examination of subgingival bacterial proportion may serve as more sensitive mirror of the local periodontal status than clinical parameters.

10.Masticatory function following implants replacing a second molar.

PURPOSE: The aim of this study was to obtain objective and standardized information on masticatory function and patient satisfaction following second molar single implant therapy. METHODS: Twenty adult patients, who had restored second molar single implants more than 1 month before the study, were enrolled in this study. All patients received a chewing test using peanuts before and after insertion of the implant prosthesis, with a questionnaire and visual analogue scale (VAS) to evaluate the effect of second molar single implant therapy. RESULTS: This study obtained standardized information on the masticatory function objectively (e.g., P, R, X50) before (Pre-insertion) and after insertion (Post-insertion) of the implant prosthesis. Masticatory performance (P) after insertion of the implant prosthesis significantly increased from 67.8+/-9.9 to 84.3+/-8.5% (P<0.0001). With the implant prosthesis, the P value increased by 24%. The masticatory efficiency index (R) of Post-insertion is higher than that of Pre-insertion (P<0.0001). With the implant prosthesis, the R value increased by 29%. The median particle size (X50) of Post-insertion is lower than that of Pre-insertion (P<0.0001). More than 90% of the patients were satisfied with the second molar single implant therapy from a functional point of view. CONCLUSIONS: These findings indicate that a second molar single implant can increase masticatory function.
