Double pylorus (DP) or duplication of the pylorus is an uncommon condition that is either congenital or acquired. Acquired double pylorus (DP) results from a peptic ulcer eroding through and creating a fistula between the duodenal bulb and the distal stomach. We report a case of an acquired double pylorus in an adult gentleman that resulted from the erosion of a duodenal and prepyloric ulcer.

Peptic ulcer is known to rank very high among the diseases of the gastrointestinal system which seem most prevalent in this country. Although it's incidence in this country is difficult to know with any degree of precision at present because few statistical reports are available, it is frequently encountered in our daily practice. Many studies on the pathogenesis of the disease from the clincial aspect as well as the experimental, have been undertaken by many investigators. No definite conclusions, however, have been arrived at, and thus more diverse and intensified research is urgently required. The following studies, carried out in an effort to further elucidate the pathogenic mechanism, are reported here.

PURPOSE: The aim of the present study was to assess the clinical, endoscopic and histologic findings of the Helicobacter pylori(Hp)-associated gastritis in children. METHODS: We have assessed 91 patient(age range 3-15 years) referred for upper intestinal endoscopy during 17 months. At least two antral biopsies were taken during endoscopy. The histological analysis and CLO test were done. The histologic changes were scored and semiquantitative assessment of the degree of Hp colonization was performed using Giemsa' stain. RESULTS: 1) Helicobacter pylori(Hp) was detected by CLO test and/or histologic analysis in 35 patients(38.5%). The CLO test was positive in 25 cases and H. pylori was colonized histologically in 10 patients. 2) The clinical symptoms were not significantly different between Hp positive and Hp negative patients(p>0.01). 3) Nodular antritis(37.1%) was a frequent endoscopic finding in Hp-infected patients, particularly in the subgroup with active chronic gastritis(93.8%). All patient with nodular antritis had Hp infection. 4) Helicobacter pylori has been associated both with active chronic gastritis(42.9%) and with nonactive chronic gastritis(40.0%). The common abnormal histologic finding was active chronic gastritis(42.9%) in Hp positive patients and nonactive chronic gastritis(66%) in HP negative patients. A significant correlation was found between Hp colonization score and the histologic categories(p<0.01). 5) The frequency of Hp infection(positiveness) as related to histologic categories was: active chronic gastritis 93.8 %; duodenal ulcer 66.7%; gastric ulcer 33.3 %; nonactive chronic gastritis 27.5%; normal 16.7%. 6) The histolgical colonization by Hp was assessed semiquantitatively, and a significantly greater Hp colonization score was observed in patients with signs of histological activity(p<0.01). CONCLUSIONS: This study suggests that Hp infection was significantly associated with endoscopic nodular antritis and the presence of gastroduodenal pathology.

PURPOSE: The advanced stage of chronic venous insufficiency (CVI) 4, 5, 6, which shown lipodermatosclerosis, venous ulcer might cause frustrating symptom compared with simple varicose vein. For successful treatment, precise anatomic, hemodynamic information of venous system is recommended. Although there is still controversy, duplex scan is the best way in evaluation of hemodynamic pathology. We studied key role of duplex scan in treatment of advanced CVI. METHOD: From November 1998, to February 2000, 125 patients were treated for CVI in our hospital. Of these 11 patients with class 4, 5, 6 were assessed by duplex scan, ascending venogram, hemodynamic functional studies. All venous segments were eagerly assessed for looking reflux. Except one patient who refuse surgery 10 patients received conventional surgical treatment and endoscopic perforator ligation. The one year follow up results were assessed. RESULT: 9 patients showed superficial venous reflux, 3 patients had deep venous reflux. In all 6 patients who had ulcer, incompetent perforator vein was identified. In 9 patients, stripping 8, high ligation 1, endoscopic perforator vein ligation 3 were done. During 1 year follow up clinical improvement was singificant in these 11 patients assessed with symptom scores. All ulcer were healed with average 25.3 days, and no recurrence. CONCLUSION: Although no single test was enough for perfect assessing CVI, scrupulous examination with duplex scan is best way for accomplishing successful outcome at presents.

The pathological changes of the gastric rumen produced by rocking stress in 38 young male albino rats fed on low protein diet for a short period and in 20 young male albino rats fed on low protein diet only as the control rats, were investigated macroscopically and histologically. The ulcerative lesions of the gastric rumen were observed in 17 out of 38 experimental rats accompanied with hyperplasia and cornification of the ruminal epithelial layer and more or less edematous changes of the tunica propria near the ulcer site. The authors, finding of the ruminal ulcer in albino rats produced by rocking stress associated with feeding on a low protein diet for a short period were compared with the results of others. The authors discussed other opinions concerning of the gastric ruminal ulcer of the albino rats produced in various ways.

A 32-year-old woman had a recurrent shallow ulcer on the flank. A biopsy specimen showed thromboses in the dermal vessels and she was found to have circulating antiphospholipid antibody with no associated systemic disease. A clean ulcer developed on the flank of a patient with primary antiphospholipid syndrome is considered to be a rarely encountered/unusual presentation of this syndrome.

OBJECTIVETo investigate the optimal operation method for the management of various chronic wounds in legs and feet.

METHODSFifty-one chronic wounds were evaluated according to infection, inflammatory response, and distribution in different areas of the leg and foot. Preoperative treatment was given accordingly, then transposition of skin flap, skin grafting, or amputation was performed. The healing rate after single session operation and average hospitalization were statistically analyzed.

RESULTSThe wound healing rate after single session operation was 86. 3% , the average hospital stay was (17. 8 +/- 2. 1) days, and the appearance and function of the leg and foot after operation was satisfactory.

CONCLUSIONThe appropriate preoperative treatment and operation method conforming to the wound location and evaluation are of vital importance in the management of chronic wounds in the leg and foot. Operation is one of the most effective ways to repair chronic wounds in the leg and foot, and it can shorten the wound healing process and restore the function.

Behcet disease is relatively rare in pediatric age group. And the bowel involvement is seen in only a small portion of Behcet disease. However, once the bowel is involved it is potentially life threatening event. We report a 15 year old boy with intestinal Behcet's disease who had a history of recurrent oral and genital ulcers for several years. He underwent right hemicolectomy under the impression of intestinal tumor. Pathologically the lesion was a large sharply delineated ulcer in the cecum. The ulcer was round and deep with elevating margin, and was associated with thickening of affected intestinal wall. Microscopically, the ulcer base consisted of granulation tissue with fissurings and underminings. Characteristic phlebitis and occlusive arterial lesion were seen in intestinal wall. The inflammatory lesion was most pronounced around the ulcer but could be recognizable throughout the resected specimen.
