1.The Incidence of Venous Thromboembolism Following Shoulder Surgery: A Pilot Study.

BACKGROUND: To assess the incidence of venous thromboembolism (VTE) following shoulder surgery and to evaluate the role of postoperative duplex ultrasonography. METHODS: The study comprised a total of 224 patients who underwent shoulder surgery, including 180 shoulder arthroscopic surgeries, 28 shoulder arthroplasties, and 16 plate fixations for proximal humerus fracture between January 2014 and December 2014. The mean age of patients was 59.0 years, and there were 81 men and 143 women. Clinical data, including body mass index, blood tests, metabolic work-up for liver and renal function, previous, and present medical history, were evaluated. Duplex ultrasonography in the operative arm was performed on 2 to 4 days after surgery. RESULTS: The overall incidence of VTE following shoulder surgery was 0.45% (1/224). One patient with open reduction and plate fixation for proximal humerus fracture had asymptomatic deep vein thrombosis that showed complete remission after anticoagulant medication during a 2-month period. Four patients had asymptomatic superficial cephalic vein thrombosis and complete remission without any treatment. There was no case of pulmonary embolism. CONCLUSIONS: The incidence of VTE following shoulder surgery was extremely low in Asians. Duplex ultrasonography may be not considered a routine follow-up of shoulder surgery and can be selectively performed in high-risk or symptomatic patients for VTE.

2.Hydramnios related to fetal deformity in a Hanwoo cow: a case report

A Hanwoo cow with a delayed gestation and abdominal distension was delivered following PGF2α injection. There was excessive amniotic fluid, and a male calf was delivered but died immediately. The calf had no eyes and nose, and a cleft palate on the upper jaw. Gross appearance and computed tomography image showed that upper teeth were spread out on both sides due to cleft palate in the upper jaw, and lower jaw and teeth were positioned upward. There were no other brain parts except cerebellum. These findings show a rare case of hydramnios related to fetal congenital deformity in a Hanwoo cow.

The purpose of this study was to evaluate the effects of DFPR on differentiation of mouse calvarial cell in vitro, to examine the possibility for periodontal regeneration. 10microgram/ml of DFPR was used as experimental concentration. osteogenic medium only was assigned as control, Experimental 1 was supplemented with 10nM dexamethasone, Experimental 2 with 10microgram/ml DFPR and Experimental 3 with 10nM dexamethasone + 10microgram/ml DFPR. cellular activity was evaluated by MTT method at 8, 12, 16 days, expression of mRNA of ALP, osteopontin, osteocalcin, collagen type-1 was detected by RT-PCR method at 4, 8, 12, 16 days of culture . extent of mineralization was observed by Von Kossa staining at 16 day of culture. The results are as follows 1)Any acceleration of differentiation was not observed at expression of differentiation marker, 2) Decrease in expression of extracelluar matrix and in bone nodule formation was observed The results suggested that DFPR have negative effect on the rate of differentiation on rat calvarial cell, decrease extracelluar matrix formation ,decrease bone nodule formation. Ongoing studies are necessary in order to determine effect of DFPR on periodontal regeneration.

5.The impact of the duration of retained placenta on postpartum diseases and culling rates in dairy cows.

The objective of this study was to evaluate the duration of retained placenta (RP) on postpartum diseases and culling in dairy cows. Data were collected from 456 Holstein cows between 2008 and 2010. RP (> or = 24 h after parturition) and postpartum diseases were diagnosed by farm managers and a veterinarian according to standardized definitions. The overall incidence rate of RP was 14.0%, and the incidence rate of RP with fourth and higher parity was two times that of cows having offspring for the first time. The duration of RP was 2 to 15 days with a mean of 6.8 days, except for cases of fourth and higher parity which had a mean of 10.5 days. A total of 63% of cows with RP had postpartum diseases. Among the cows diagnosed with RP, 23.4% developed metritis, and of those, 35.3% developed endometritis or pyometra. A total of 25% (n = 16) cows with RP were culled within 60 days in milk (DIM) and of those culled, 75% (n = 12) had postpartum diseases. These results suggest that RP increases the risk of postpartum diseases such as metritis and mastitis and is a culling hazard up to 60 DIM.

6.Intraarticular Ganglion cyst under Patellar Tendon: Case Report.

Intraarticular ganglion of the knee joint is a cystic mass found around cruciate ligaments in infrapatellar fat pad or under patellar tendon. The first report on intraarticular ganglion of the knee joint along with some intermittent case reports or ganglion around the cruciate ligaments were in 1924 by Caan. Ganglion under the patellar tendon or in the patellar fat pad was first reported in 1972 by Muckle and Monahan. The two cases were treated with open arthotomy and mass excision. There was been no report on ganglion under patellar tendon since then. We managed two cases of ganglion under the patellar tendon with arthroscopic debridement. After 16 months of treatment, there has no recurrence of symptom. We report two cases of ganglion under patellar tendon with the review of the literature.

7.Comparison of blood parameters according to fecal detection of Mycobacterium avium subspecies paratuberculosis in subclinically infected Holstein cattle

Background:
Mycobacterium avium subspecies paratuberculosis (MAP) causes a chronic and progressive granulomatous enteritis and economic losses in dairy cattle in subclinical stages.Subclinical infection in cattle can be detected using serum MAP antibody enzyme-linked immunosorbent assay (ELISA) and fecal polymerase chain reaction (PCR) tests.
Objectives:
To investigate the differences in blood parameters, according to the detection of MAP using serum antibody ELISA and fecal PCR tests.
Methods:
We divided 33 subclinically infected adult cattle into three groups: seronegative and fecal-positive (SNFP, n = 5), seropositive and fecal-negative (SPFN, n = 10), and seropositive and fecal-positive (SPFP, n = 18). Hematological and serum biochemical analyses were performed.
Results:
Although the cows were clinically healthy without any manifestations, the SNFP and SPFP groups had higher platelet counts, mean platelet volumes, plateletcrit, lactate dehydrogenase levels, lactate levels, and calcium levels but lower mean corpuscular volume concentration than the SPFN group (p < 0.017). The red blood cell count, hematocrit, monocyte count, glucose level, and calprotectin level were different according to the detection method (p < 0.05). The SNFP and SPFP groups had higher red blood cell counts, hematocrit and calprotectin levels, but lower monocyte counts and glucose levels than the SPFN group, although there were no significant differences (p > 0.017).
Conclusions
The cows with fecal-positive MAP status had different blood parameters from those with fecal-negative MAP status, although they were subclinically infected. These findings provide new insights into understanding the mechanism of MAP infection in subclinically infected cattle.
8.Korean native calf mortality: the causes of calf death in a large breeding farm over a 10-year period.

Calf losses have an economic impact on larger Korean native cattle (KNC) breeding farms due to replacement, productivity, and marketing. However, little research on KNC calf mortality or causes of calf death on large-scale breeding farms has been conducted. Based on medical records and autopsy findings from the Hanwoo experimental station of the National Institute of Animal Science, calf death records from 2002 to 2011 were used to identify the causes of mortality. Mortality rate of KNC calves was 5.7%. Large differences (1.8~12.6%) in year-specific mortalities were observed. Calf deaths were due to digestive diseases (68.7%), respiratory diseases (20.9%), accidents (6.0%), and other known diseases (2.2%). The main cause of calf death was enteritis followed by pneumonia, rumen indigestion, and intestinal obstruction. The greatest number of calf deaths occurred during the fall followed by summer. These results indicated that enteritis and pneumonia were the main reasons for calf death. However, autopsy findings demonstrated that other factors also caused calf death. This study suggested that seasonal breeding and routine vaccinations are the most important factors for preventing calf death, and improving calf health in high land areas with low temperature.

9.The clinical study of oral care for early radiation therapy in the head and neck cancer patients.

INTRODUCTION: Tooth requiring extraction before radiotherapy in head and neck cancer patients should be performed as long as possible before the initiation of radiation therapy. Conventionally, a minimum 2-week waiting primary healing period is recommended. Although the above 2-week period is ideal, it is not uncommon for the radiotherapist and cancer patient to feel an urgent need to proceed with radiotherapy despite the need for dental care. Therefore, alternative approaches for early radiotherapy, including conservative endodontic treatment and a 1-week waiting primary healing period after dental extraction at the time of radiotherapy were considered and applied based on a literature review MATERIALS AND METHODS: The clinical study involved 120 head and neck cancer patients who were treated at Wonju Christian Hospital, Wonju College of Medicine, Yonsei University, from January 1995 to December 2004. RESULTS: In the clinical study, there were no specific complications, such as, post-extraction wound infections, radiation osteitis and osteoradionecrosis over the recent 10 years despite the early radiotherapy. CONCLUSION: Based on the clinical study, a minimum 1-week waiting primary healing period for oral care before radiotherapy is suitable for early radiotherapy in head and neck cancer patients.

10.The Clinical Study of Fetal Death in Utero.

OBJECTIVE: To investigate the clinical characteristics and etiologic factors of pregnancies with fetal death in utero (FDIU). METHODS: Retrospective review of medical records of 184 pregnancies with FDIU between March 1996 and March 2006 was conducted and descriptive analysis was done. Medical records were unavailable in 14 cases which were excluded in the analysis of etiology and diagnostic evaluation. RESULTS: The overall incidence was 1.31%. There was no significant difference in the yearly incidence during the study period. Age distribution of FDIU was between 18 and 44 and the incidence was highest in 25~29 year-old age group. The risk analysis showed statistically significant risk in the age group under 25 (OR, 2.455) and 25~29 (OR, 1.590) compared to 30~34 year-old age group. The risk of age group beyond 35 has a tendency to increase but was not statistically significant. FDIU was the most prevalent (38.58%) among pregnancies less than 29 weeks of gestation. Most of cases were delivered vaginally (86.5%). Etiologic factors included unexplained causes (37.1%), fetal factors (29.4%), placental and cord factors (18.2%) and maternal factors (15.3%). Autopsy was done in 128 cases (75.3%) and placental pathology was examined in 148 cases (87.1%). Among the workups done, autopsy and placental pathology were the most informative. CONCLUSION: Despite the advance of prenatal care, the incidence of FDIU was steady throughout the study period. The etiology of the largest proportion was unexplained. Once FDIU is diagnosed, prompt delivery should be done and appropriate diagnostic tests should be offered to aid in next pregnancy.
