1.Active case finding to detect symptomatic and subclinical pulmonary tuberculosis disease: implementation of computer-aided detection for chest radiography in Viet Nam

Objective: In Viet Nam, tuberculosis (TB) prevalence surveys revealed that approximately 98% of individuals with pulmonary TB have TB-presumptive abnormalities on chest radiographs, while 32% have no TB symptoms. This prompted the adoption of the “Double X” strategy, which combines chest radiographs and computer-aided detection with GeneXpert testing to screen for and diagnose TB among vulnerable populations. The aim of this study was to describe demographic, clinical and radiographic characteristics of symptomatic and asymptomatic Double X participants and to assess multilabel radiographic abnormalities on chest radiographs, interpreted by computer-aided detection software, as a possible tool for detecting TB-presumptive abnormalities, particularly for subclinical TB.
Methods: Double X participants with TB-presumptive chest radiographs and/or TB symptoms and known risks were referred for confirmatory GeneXpert testing. The demographic and clinical characteristics of all Double X participants and the subset with confirmed TB were summarized. Univariate and multivariable logistic regression modelling was used to evaluate associations between participant characteristics and subclinical TB and between computer-aided detection multilabel radiographic abnormalities and TB.
Results: From 2020 to 2022, 96 631 participants received chest radiographs, with 67 881 (70.2%) reporting no TB symptoms. Among 1144 individuals with Xpert-confirmed TB, 51.0% were subclinical. Subclinical TB prevalence was higher in older age groups, non-smokers, those previously treated for TB and the northern region. Among 11 computer-aided detection multilabel radiographic abnormalities, fibrosis was associated with higher odds of subclinical TB.
Discussion: In Viet Nam, Double X community case finding detected pulmonary TB, including subclinical TB. Computer-aided detection software may have the potential to identify subclinical TB on chest radiographs by classifying multilabel radiographic abnormalities, but further research is needed.

Background:
Avian influenza (AI) is a contagious disease that causes illness and death in poultry and humans. High pathogenicity AI (HPAI) H5N6 outbreaks commonly occur in Quang Ninh province bordering China. In June 2021, the first HPAI H5N8 outbreak occurred at a Quang Ninh chicken farm.
Objectives:
This study examined the risk factors associated with HPAI H5N6 and H5N8 outbreaks in Quang Ninh.
Methods:
A retrospective case-control study was conducted in Quang Ninh from Nov 2021 to Jan 2022. The cases were households with susceptible poultry with two or more clinical signs and tested positive by real-time reverse transcription polymerase chain reaction. The controls were households in the same village as the cases but did not show clinical symptoms of the disease. Logistic regression models were constructed to assess the risk factors associated with HPAI outbreaks at the household level.
Results:
There were 38 cases with H5N6 clade 2.3.4.4h viruses (n = 35) and H5N8 clade 2.3.4.4b viruses (n = 3). Compared to the 112 controls, raising poultry in uncovered or partially covered ponds (odds ratio [OR], 7.52; 95% confidence interval [CI], 1.44–39.27), poultry traders visiting the farm (OR, 8.66; 95% CI, 2.7–27.69), farms with 50–2,000 birds (OR, 3.00; 95% CI, 1.06–8-51), and farms with ≥ 2,000 birds (OR, 11.35; 95% CI, 3.07–41.94) were significantly associated with HPAI outbreaks.
Conclusions
Combining biosecurity measures, such as restricting visitor entry and vaccination in farms with more than 50 birds, can enhance the control and prevention of HPAI in Quang Ninh and its spread across borders.
3.Identification of Phenolic Compounds from Vietnamese Artichoke (Cynara scolymus L.) Leaf and Their Antioxidant Activities

Artichoke (Cynara scolymus L.) is a perennial plant belonging to the Asteraceae family originating from the Mediterranean region. In Vietnam, there are some varieties of artichoke which are extensively cultivated and propagated in highland areas, however, there have been limited detailed scientific publications on the chemical composition and biological activity of artichoke grown in Vietnam. Therefore, this study provides a detailed description of the extraction, isolation, and structural determination of 20 natural secondary metabolites present in harvested artichoke. The antioxidant activity of the extract and the 9 isolated compounds are tested in 1,1-diphenyl-2-picrylhydrazyl radical scavenging and ex vivo malondialdehyde model. Among the selected compounds, 1-caffeoylquinic acid, 3-caffeoylquinic acid, chlorogenic acid, 4-caffeoylquinic acid, cynarin, 1,5-dicaffeoylquinic acid, 4,5-di-caffeoylquinic acid, cynaroside, and scolymoside exhibited strong radical scavenging activity with IC50 values ranging from 5.7 to 61.6 µM. In the malondialdehyde assay, 1,3-dicaffeoylquinic acid (or cynarin) showed the strongest activity with an IC50 value of 24.7 µM, followed by 1,5-di-caffeoylquinic acid (66.8 µM), and 4,5-di-caffeoylquinic acid (127.3 µM). This outcome contributes to establishing a database on the phytochemical and antioxidant activity of the Vietnamese artichoke.

Aims:
To isolate and characterize the lactic acid bacteria (LAB) strains from the “mam chua ca ro” (sour fermented fish) in the South of Vietnam and investigate their potential anti-bacterial properties.
Methodology and results:
Four LAB strains (MCR1, MCR2, MCR3 and MCR4) were isolated from the "mam chua ca ro" product and their anti-bacterial activity was determined using the spot assay and the paper disc diffusion method. The isolated LABs can inhibit Escherichia coli ATCC 25922, Staphyloccocus aureus ATCC 25923 and Vibrio parahaemolyticus BV016 and produce bacteriocin to control the growth of E. coli ATCC 25922 and S. aureus ATCC 25923, except V. parahaemolyticus. MCR2 was chosen to sequence 16S rRNA of Pediococcus acidilactic.
Conclusion, significance and impact of study
On the basis of their prominent anti-pathogenic bacteria activity, LAB strains isolated from Vietnamese sour-fermented fish products were verified as prospective probiotics.

5.Sexual awareness and cognitive social capital among high school students: a cross-sectional study in rural Vietnam

Objective: Since the late 1980s, Vietnam has seen numerous social changes, likely leading to changes in adolescent sexual awareness. Adolescents are currently exposed to a plethora of sexual information without adequate sex education and knowledge. Globally, researchers have identified sexual knowledge and self-esteem as determinants of adolescent sexual awareness and behavior, but little is known about the role of social capital, especially in rural areas. This study aimed to clarify the relationship between sexual awareness and cognitive social capital among high school students in rural Vietnam.Materials and Methods: We conducted a questionnaire-based survey assessing sexual knowledge, sexual awareness, perceptions of peers’ sexual experience, structural and cognitive social capital, and self-esteem (Rosenberg Self-Esteem Scale) in March 2017 among 1,583 11th graders at four public high schools in Hai Duong Province, Vietnam.Results: The final sample comprised 1,517 students (95.8% of total responses): 609 boys (40.1%) and 908 girls (59.9%). Multiple logistic regression analysis was performed with intolerance toward premarital sex as the dependent variable. In the first model, the independent variables were self-esteem, knowledge of proper timing for condom use, and perception that many peers are sexually experienced. The second model added an indicator of cognitive social capital. In the first model, self-esteem, knowledge of proper timing for condom use, and perception that many peers are sexually experienced were all significantly associated with intolerance toward premarital sex for both boys and girls. In the second model, for both boys and girls, only cognitive social capital was significantly associated with intolerance toward premarital sex.Conclusion: The results of this study regarding the effect of social capital in rural Vietnam are consistent with previous findings in urban areas. Social capital should be emphasized in efforts to improve sexual awareness and, in turn, sexual and reproductive health among adolescents in rural areas.
6.Trichoderma asperellum Chi42 Genes Encode Chitinase.

Four Trichoderma strains (CH2, SH16, PQ34, and TN42) were isolated from soil samples collected from Quang Tri and Thua Thien Hue provinces in Vietnam. The strains exhibited high chitinolytic secretion. Strain PQ34 formed the largest zone of chitinase-mediated clearance (> 4 cm in diameter) in agar containing 1% (w/v) colloidal chitin. Analysis of the internal transcribed spacer regions of these strains indicated that they were Trichoderma asperellum. The molecular weights of the chitinases were approximately 42 kDa. Chitinase genes (chi42) of T. asperellum strains TN42, CH2, SH16, and PQ34 were 98~99% homologous to the ech42 gene of T. harzianum CB-Pin-01 (accession No. DQ166036). The deduced amino acid sequences of both T. asperellum strains SH16 and TN42 shared 100% similarity.

7.The preliminary report of occult HBV infection in Vietnamese

Background: Vietnam is located in the endemic region of hepatitis B virus (HBV) infection, but no data of occult HBV infection was reported at present. Objectives: To investigate the prevalence of occult HBV infection in different ethnics of people and generations. Subjects and method: 80 voluteers with HbsAg negative from five different ethnics: Kinh, Tay, Mong, Giay and Dao in a Chino \ufffd?Vietnamses border province (Lao Cai) were enrolled in the study. After HBV-DNA was extracted, nested PCR of S gene and of Core-promoter/Pre-core region were used to detect HBV-DNA. Specifying nucleotide sequence was confirmed by direct sequencing. Results:The prevalence of occult HBV infection in population study was very high 73/80 (91,3%) by nested PCR of Core-promoter/Pre-core, significantly more sensitive than nested PCR of S gene (26,3%) (p<0,0001). The prevalence of occult HBV infection was notdifferent between ethnics of people or between children, adults. Conclusion: Occult HBV infection in Vietnamese is very common; however, nationwide further studies should be carried out to confirm this preliminary results and evaluate the impact of occult HBV infection in Vietnam.

Background: Hepatitis B is an infectious illness caused by hepatitis B virus (HBV) which infects the liver of hominoidea, including humans, and causes an inflammation called hepatitis. Objectives: The aim of study is to clarify clinical features and molecular characteristics of HBV in chronic HBV-infected patients with A 1899 mutation. Subjects and method: HBV genotype, HBV-ONA level, HBeAg and anti-HBe in 29 chronic HBV-infected patients were determined by PCR-RFLP, Real-time PCR and ELISA, respectively. Mutations were analyzed by direct sequencing. Results: Mutations in core-promoter/precore regions of HBV genome can suppress HBeAg secretion and stimulate HBV-ONA replication. The prevalence of hepatocel- lular carcinoma (HCc): 10/29, liver cirrhosis (LC) : 15/29 are significantly higher than that in chronic hepatitis (CH) : 4/29 (P < 0.001). HbeAg seroconversion rate in CH (75%) is higher than that in HCC \r\n', u'(40%) and in LC (53.3%), but not significant (P > 0.05). ALT level is the highest in CH and the lowest in HCC \r\n', u'(P = 0.02), 8/10 (80%) HCC patients have normal range of ALT. HBV-ONA level in HCC and in LC is significantly higher than that in CH (P = 0.024). The emerging of A 1899 is often accompanied by C/G1753 mutation (37.9%) and dual core-promoter mutation T1762A1764 (79.3%). Background: Hepatocellular carcinoma (HCC) is the most common primary hepatic tumor and one of the most common cancers worldwide. HCC is a primary malignancy of hepatocellular origin. Objectives:The aim of study is to combinate therapy of transarterial chemoembolization and percutaneousethanol injection afterward emerging metatasis caused by fine needle aspiration cytology. Subjects and method: A 50 years old male patient with hepatocellular carcinoma having a diameter of tumor more than 5 cm was treated by combination of transarterial chemoembolization and percutaneous ethanol injection from December 2000. Results & Conclusion: Results of study showed that: Transarterial chemoembolization and percutaneous ethanol injection are the two of non-surgical methods for treatment of hepatocellular carcinoma which are most commonly available in applied clinical activities at present. Up to now, the patient's life expectancy after therapy is more than 6 years that means the result of treatment is very good. However, the emerging metatasis into the anterior-right-Iower chest wall that was caused by fine needle aspiration cytology should be reviewed for further evaluating clinical experience, especially in cases with quite clear imaging features of untrasonography and significantly elevated AFP level higher than 200 ng/rnl.

10.Study on the thionylation to synthesize thiosalicylanilide derrivatives with antifungal and antibacterial activities

Condensation of salicylic acid with anilines made salicylanilid derivatives. From salicylanilids, 3-phenyl-2H-1, 3-benzoxazin-2, 4(3H)-dion derivatives were created by same circle reaction. Thionylation reaction 3-phenyl-2H-1, 3-benzoxazin-2,4(3H)-dion in dioxan solvent had good result. Products of thionylation created thiosalicylanilides after circle opened hydrolysis. Clearness of thiosalicylanilides was determined by thin layer chromatography and their structure was determined by UV, IR.H1 MNR. Antibacterial effect of synthetic products, especially Streptococcus feacalis E.coli was higher than control substance as salicylanilid. Antifungal effect of Bromo and clorothiosalicylanilide derivatives was stronger than other derivatives
