1.The Potential Utility of Iodinated Contrast Media (ICM) Skin Testing in Patients with ICM Hypersensitivity.

Both immediate and delayed hypersensitivity reactions to iodinated contrast media (ICM) are relatively common. However, there are few data to determine the clinical utility of immunologic evaluation of ICM. To evaluate the utility of ICM skin testing in patients with ICM hypersensitivity, 23 patients (17 immediate and 6 delayed reactions) were enrolled from 3 university hospitals in Korea. With 6 commonly used ICM including iopromide, iohexol, ioversol, iomeprol, iopamidol and iodixanol, skin prick (SPT), intradermal (IDT) and patch tests were performed. Of 10 patients with anaphylaxis, 3 (30.0%) and 6 (60.0%) were positive respectively on SPTs and IDTs with the culprit ICM. Three of 6 patients with urticaria showed positive IDTs. In total, 11 (64.7%) had positive on either SPT or IDT. Three of 6 patients with delayed rashes had positive response to patch test and/or delayed IDT. Among 5 patients (3 anaphylaxis, 1 urticaria and 1 delayed rash) taken subsequent radiological examinations, 3 patients administered safe alternatives according to the results of skin testing had no adverse reaction. However, anaphylaxis developed in the other 2 patients administered the culprit ICM again. With 64.7% (11/17) and 50% (3/6) of the sensitivities of corresponding allergic skin tests with culprit ICM for immediate and delayed hypersensitivity reactions, the present study suggests that skin tests is useful for the diagnosis of ICM hypersensitivity and for selecting safe ICM and preventing a recurrence of anaphylaxis caused by the same ICM.

Iodinated contrast media (CM) can cause immediate and late reactions. We treated a patient with a recurrent generalized maculopapular rash and a fever that occurred within two days of exposure to iodinated CM, iopromide (Ultravist(R)), for chest computed tomography. We performed skin testing including prick tests, intradermal tests, and patch tests. Our findings indicated a late skin reaction to Ultravist(R) in addition to cross-reactions to other iodinated CM such as ioversol (Optiray(R)), iohexol (Iobrix(R)), and iobitridol (Xenetix(R)). In this study, we report the case of a patient diagnosed with a late adverse reaction to Ultravist(R) in addition to cross-reactions to other iodinated CM.

BACKGROUND: This study was performed to evaluate the prevalence and risk factors on the adverse reactions caused by iodinated contrast media (CM) for computed tomography (CT) examination in a university hospital. METHODS: Clinical and demographic data among outpatients with CM use were collected at 0000 University Busanpaik Hospital in Busan, Korea between 2008 and 2010. Adverse reaction rate was calculated by the number of adverse reaction among total outpatients with CM use, which was stratified by seasons and sex. The association of risk factors on adverse reaction was investigated using logistic regression model. RESULTS: The total outpatients and events of administered CMs were 27,587 and 48,616, respectively. The administered CMs were iopromide, iohexol, iobitridol, and iodixanol. Adverse reactions occurred in 300 outpatients among the total outpatients (1.1 %). The number of outpatients administered CM more than twice were 8,348. Among them, outpatients who experienced adverse reaction(s) more than once and twice were 124 (1.5 %) and 26 (0.3 %), respectively. Adverse reaction rate was significantly different by sex(p=0.01). The other risk factors were cancer history (OR 2.57, 95 % CI 2.00-3.31) and previous CM administration (OR 1.89, 95 % CI 1.47-2.44). Urticaria was the most frequent symptoms. CONCLUSION: Total adverse reaction rate was 1.1 % with most common symptom in skin system. Related risk factors were female, cancer history, and previous CM administration. These results were similar to previous studies and will be contribute to clinical practice and future research especially in Koreans.

5.Study of the marker for a neuronavigation system.

UNLABELLEDWith different ratios, Omnipague mixed with cod liver oil and Meglumine Diatrizoate mixed with cod liver oil are compared with the stardard marker under the scanning of CT and MR in order to find the better ratio and materials for the marker. The experimental results show that the performances of both Omnipague mixed with cod liver oil and Meglumine Diatrizoate mixed with cod liver oil are better than the stardard marker.
CONCLUSIONOmnipague mixed with cod liver oil and Meglumine Diatrizoate mixed with cod liver oil can be used in making marker for neuronavigation system.

8.A Case Report of the Hypersensitivity Reaction with Urografin .

Since 1929, the iodine compounds were used intravenous pyelogram by the Swick, it was accepted for the use of intravenous pyelogram in the worldwide. Nevertheless, it has several minor and major side reactions, also it has been used to IVP or cholecystography at X-ray room without prepared for side reaction or resuscitation. We have experienced a case of sever hypersensitivity reaction with the urografin during IVP at X-ray room. Here, we review with the several articles including its etiology, symptoms and treatments. Especially, we classified the mild and severe reactionsby David and Frederick from Table 1 Table 6. Furthermore we considered that t is necessary to prepared the resuscitating instruments during IVP with urografin under local or general anesthesia.

PURPOSE: We compared the use of diluted gastrografin enema- and air enema CT for the evaluation of the diagnostic accuracy of preoperative lesion detection and the staging of colorectal carcinoma. MATERIALS AND METHODS: Forty-two patients (43 lesions) with colorectal carcinoma, diagnosed by barium enema and/or colonoscopy, underwent preoperative diluted (2.5%) gastrografin enema CT, and in another 26 patients (27 lesions) with colorectal carcinoma, air enema CT was performed. The presence or absence of lesion, its spread (T), and peritumoral lymph node (N) were analyzed. All patients underwent surgery and pathologic confirmation was obtained. RESULTS: Gastrografin enema CT and air enema CT demonstrated the primary tumor in 97.7% (42 of 43) and 96.3% of cases (26 of 27), respectively. A comparison of gastrografin enema CT and the pathologic results showed that the disease was correctly staged as T2 in five of six cases, as T3 in 28 of 31, and as T4 in two of five. Using air enema CT, the disease was correctly staged as T2 in three of three cases, as T3 in 17 of 21, and as T4 in one of two. Overall, carcinoma was correctly staged by gastrografin enema CT in 83% of cases (35 of 42) and by air enema CT in 81% (21 of 26). Nodal involvement was accurately detected in 64% of cases (27 of 42) using gastrografin enema CT and in 69% (18 of 26) using air enema CT. CONCLUSIONS: These findings suggest that the use of gastrografin enema or air enema CT does not significantly affect the diagnostic outcome.
