8.A Case of Acute Aggravation of Fungal Maxillary Sinusitis after Zygomaplasty.

In patients with sinusitis, nasal obstruction and purulent discharge from the maxillary sinus are frequently observed. Mostly, it responds to antibiotic treatment. But in case of not responding, although fungal infection is a rare entity, we have to consider fungal sinusitis. We recently experienced a case of acute aggravation of fungal maxillary sinusitis after zygomaplasty. Endoscopic sinus surgery was performed and foreign materials were observed inside the right maxillary sinus. They were gauzes, which had been used during previous surgery. Intraoperative examination demonstrated a dark brown, cheese-like material in the sinus. After surgery, the patient's symptoms disappeared gradually. The culture examination identified the surgical sample to be Trichosporon asahii. So we reported with a review of literature.

9.A Case of Toenail Onychomycosis Caused by Trichosporon asahii.

We report the first case of toenail onychomycosis caused by Trichosporon asahii in a 60-year old Korean man. The patient presented with a typical distal subungual onychomycosis. Direct microscopic examination of scrapings on the potassium hydroxide(KOH) preparation revealed fungal elements. On modified dermatophyte agar, the colony was white color and wrinkled, folded with furrows and convolutions and microscopic morphology showed long rectangular arthroconidia and lateral blastoconidia of trichosporon species. The fungus was identified as T. asahii using API 20C AUX (bioMerieux S.A., l'Etolile, France) kit.

10.A Case of Trichosporon beigelii Esophagitis Presenting Hematemesis with a Large Amorphous Material.

We experienced a case of Trichosporon beigelii esophagitis in a 16-month-old boy who was presented with hematemesis with a large amorphous material. A spit-out material was silky, 10 x 1.2 cm in size and like a part of hollow viscus organ. Emergent gastrofiberscopy revealed that this silky material was teared up from upper and to lower esophagus and was threw with hematemesis. It was suggested that pseudomembrane on esophagus was peeled off followed by mucosal bleeding. Pathologic study revealed this material was pseudomembrane with esophageal mucosa of T. beigelii esophagitis and was teared at lamina propria level from submucosa.
