1.Clinical trial of oxantel pamoate(CP-14, 445) on Trichocephalus trichiurus infection.

The present clinical trial was conducted to determine the efficacy and tolerance of oxantel(CP-14, 445), one of pyrimidine derivative, in the treatment of Trichocephalus trichiurus infection. One hundred eighty two cases who harboured T. trichiurus were treated with oxantel in a single dose of 10 to 25 mg/kg of body weight. For the evaluation, the negative conversion rate and egg reduction rate were calculated before and 22 days after the treatment. The results obtained were as follows: The prevalence rate of T. trichiurus infection in present study was 64.2% by examining 1,536 fecal specimens collected. The overall negative conversion rate was 66.5%, while the highest rate(93.3%) was obtained in the group treated with 20 mg/kg of oxantel (Group III). The lowest rate(56.6%) was observed in Group I (10 mg/kg). The overall egg reduction rate on present study was 90.3%. The highest rate, 100% of egg reduction was also obtained in Group III (20 mg/kg). From the observation of 1,244 expelled worms, it was known that 45.0% of collected worms were expelled on the first of post-treatment day, and male and female worms were about the same in number(1.03 : 1). Average worm burden per capita was 11.1 in this observation. The medication was readily accepted and tolerated by all the treated cases. On the hemograms, urinalysis and liver function tests, no significant differences were noted before and after the drug administration. Side reactions were so mild and transient that none required specific treatment.

2.Incidence of intestinal parasites with fecal examination on the out patient of Pusan University Hospital.

Author studied on the incidence of intestinal parasites with fecal examination on the out patient Pusan University Hospital, between from January to December 1969. The method employed were Formalin-Ether and flotation method in 3,510 for the prevalence rate of intestinal helminthes(Ascaris lumbricoides, hookworm, Trichocephalus trichiurus, Trichostrongylus orientalis). The following results were obtained: The positive rate of intestinal helminthes of total was 65.4 percent among 3,510 persons. The infection rate of Ascaris lumbricoides, hookworm, Trichocephalus trichiurus, Trichostrongylus orientalis were 19.3 %, 6.5 %, 55.2 % and 46.1 %. The female (66.4%) had definitely higher infection rate than male (65.0 %). The age group of 51-60 year of the male showed highest infection rate 75.4 % and the age group 31-40 year of the female showed highest infection rate 72.9 %.

3.Effect of flubendazole against Ascaris lumbricoides, Trichocephalus trichiurus and Enterobius vermicularis in infected children.

Effectiveness of flubendazole, a newly introduced as an anthelmintic in Korea, was evaluated in 28 ascariasis, 28 whipworm infections and 17 pinworm infections in primary school children. All patients were orally given by flubendazole of 500 mg once. Stools for roundworm and whipworm and anal swabs for pinworm infections were examined both before and 25 days after treatment. After flubendazole treatment, all ascariasis and pinworm infected cases were cured. On the other hand, in whipworm infections, the cure rate was 82.1 percent (23 cured of 28 infected), and the egg reduction rate was 67.1 percent, respectively. Headache (one case), dizziness (2 cases) and abdominal pain (3 cases) were noticed as untoward effects, but they were mild and disappeared within 24 hours. These results demonstrate the excellent effectiveness of flubendazole in treating these helminth infections.

4.Measurement of blood loss using (51)Cr in heavy infection cases of Trichocephalus trichiurus.

This study was undertaken to find out the possibility of iron-deficiency anemia by the intestinal blood loss in massive infection cases of T. trichiurus. Among the children of a community in which the whipworm infections were highly prevalent, 7 cases were selected whose E.P.G. of whipworm ova were more than 1,000. The hematological findings and estimated blood loss using (51)Cr were undertaken. The results could be summarized as follows: The E.P.G. and E.P.D. of T. trichiurus ova in studied 7 cases were 10,500-37,200/gm of feces and 0.90-5.14 million/day respectively. Findings of hematological examination were in lower limit of normal in hemoglobin content and hematocrit. And erythrocytes were neither microcytic nor hypochromic. Differential count of leucocytes showed persistently low grade of eosinophilia. Serum iron content measured by modification of Barkan method showed subnormal value in 5 out of 7 cases. Erythrocyte radioactivity half disappearance time(T1/2) and intestinal blood loss were estimated using (51)Cr-tagging erythrocytes. And the results showed that T1/2 were shortened (13-19 days ) in all cases and the blood loss was 0.94-6.54 ml/day which were correlated with worm burden (E.P.D.) significantly. From the above results, it could be concluded that intestinal blood loss of T. trichiurus heavy infection were actually occurred but the amount of losses were easily compensated.

5.An evaluation of cellophane thick smear technique for mass stool examination.

To evaluate the Kato's cellophane thick smear techcique in detection ability of various helminth ova, authors examined 1,843 nationwidely collected stool specimens by the two methods, formalin-ether technique and cellophane thick smear technique. And the results were analyzed. The positive rates of following helminth ova were superior in cellophane thick smear technique to those of formalin-ether technique: Ascaris lumbricoides(62.3% : 51.1%), Taenia species (1.5% : 0.9%). The difference in A. lumbricoides ova detection is highly significant one. The overall positive rate for helminths ova was also higher in Kato's method (92.3% : 90.0%). The positive fates of following helminth ova were infrior in cellophane thick smear technique to those of formalin-ether technique: Hookworm (5.8% : 14.6%), Trichostrongylus orientalis(3.2% : 17.8%), Clonorchis sinensis(8.3% : 11.0%), Metagonimus yokogawai (0.6% : 1.7%) and Hymenolepis nana(0.05% : 0.3%). The positive rate for the ova of Trichocephalus trichiurus by the two methods was incidentally same, 74.1%. The causes of inferior positive rates in some helminth ova by cellophane thick smear technique were shortly discussed and suggests some opinions in improvement of detection ability.

6.Study on the status of helminthic infections in Koreans.

This study was undertaken to evaluate the present status of intestinal helminthic infections in Koreans, from April 1967 through May 1969. The nationwidely collected stool specimens and scotch-tape anal swabs from primary schoolchildren, middle school students, draftees to army recruitment camp, soldiers and inhabitants of various parts of Korea were examined. The methods employed were Kato's cellophane thick smear technique and formalin-ether sedimentation technique in 40,581 for the prevalence rate of various intestinal helminths, scotch-tape anal swab technique in 8,585 for the prevalence rate of Enterobius vermicularis and by Stoll's dilution egg counting technique in 1,174 for the evaluation of the worm burden of some helminths. The results are as follows. Result of stool examination: The positive rate of intesinal helminthes of any kind was 90.5% among 40,581. In rural people, it was 92.3% in average in contrast to 85.0% in Seoul inhabitants. The infection rate of A. lumbricoides in 40,581 was 58.2%. It was higher in rural people 62.9% than in Seoul inhabitants and highest in 5-9 year-old group by age. The infection of Trichocephalus trichiurus showed the highest prevalence rate in Koreans and it was 74.5%. No remarkable difference was observed between Seoul inhabitants(72.2%) and rural people(75.2%). The tendency of higher prevalence rate with the older age group was observed. The infection rates of hookworm and Trichostrongylus orientalis in 40,581 examinations by cellophane thick smear were 5.1% and 5.8% respectively. But in 4,949 examinations for which refined cellophane thick smear technique and formalin-ether sedimentation technique were employed, the infection rates were turned out 17.6% in hookworm and 15.9% in T. orientalis. Although rural people showed higher infection rate of hookworm(18.8%) than Seoul inhabitants(13.8%), the reverse was true in T. orientalis infection rate(14.5% : 20.3%). Both kinds of infections were tend to be higher with the increase of age. The infection rates of Clonorchis sinensis and Metagonimus yokogawai were 4.7% and 0.4% respectively. In both kinds of infections male showed higher infection rates than female and the tendencies of higher infection rate in the older age group were observed. The provinces which showed the higher infection rate of C. sinensis were South and North Kyongsang Do and North Cholla Do, all of which are provinces of southern part of Korea. Some endemic foci of M. yokogawai were noticed in South Kyongsang Do and South Cholla Do. The infection rate of Taenia spp. by applying only the stool examination was 0.7% throughout the survey. No sex difference in infection rate of Taenia spp. was noticed, but in older age group it was increased. Cheju Do, the island of extreme southern part of Korea seemed the most endemic area and South and North Cholla Do were next in ranking of Taenia infection rate. The ova of Hymenolepis nana were observed in 0.2% and all of them were under the age of 20 except only one caes. The infection rate seemed higher in female (0.3%: 0.1%). No geographical difference was noticed except 1.1% of schoolchilderen and students of Cheju Do. Result of Scotch-tape anal swab technique for E. vermicularis infection. The infection rate of E. vermicularis in 8,585 urban and rural schoolchildren and rural inhabitants was 46.6%. Female (57.9%) had definitely higher infection rate than male (41.4%). The infection rate was significantly higher in rural people than Seoul schoolchildren. The exteme example was 79.7% in schoolchildren of South Cholla Do. Result of worm burden examination by Stoll's dilution egg-counting technique: Among 1,174 examinations, countable(E.P.G.> 100) cases of A. lumbricoides were 745(63.5%) and the mean E.P.G. was 9,723. The mean E.P.G. in Seoul inhabitants was 5,835 in contrast to 10,820 in rural people. The age group of 5-9 showed the heaviest mean E.P.G., 11,337. 70.6% of 745 cases were less than 10,000 in E.P.G. which means light infection. Countable cases by Stoll's technique for T. trichiurus ova were 63.5% and mean E.P.G. was 649. No difference between Seoul and rural inhabitants was observed. Grade I and II in which E.P.G. is less than 5,000 involve 99.3% of positive cases. 91 cases (7.9%) in 1,174 were positive for hookworm ova by Stoll's technique and mean E.P.G. was 501. All cases were in Grade I and II which were less than 5,000 in E.P.G. Trichostrongylus orientalis was positive only in 25 cases (2.1%) by Stoll's technique and mean E.P.G. was 340. No cases were found to have more than 5,000 in T. orientalis E.P.G. The mean E.P.G. of Clonorchis sinensis among the referred cases of clonorchiasis to Department of Parasitology, Seoul National University, was 9,416. On the other hand mean E.P.G. of C. sinensis was 21,376 among cases in the hyperendemic area of Kimhae, South Kyongsang Do, and 3,073 in cases from other various localities. Among referred clonorchiasis cases to our Department, 21.6% were turned out to have the E.P.G. more than 10,000. The cases who have the E.P.G. more than 10,000 in hyperendemic area, Kimhae were 40.7% and in other various localities were 9.1%

7.Prevalence of intestinal parasites in ROKA solidiers.

A survey of intestinal parasites was performed during the period from April 1970 to December 1971 in ROKA (Republic of Korea Army) soldiers. Samples were collected from 591 Army troops(enlisted men), 1,755 recruits at completion of 6 weeks basic training, 304 recruits during basic training and 245 stool specimens of students of ROKA Nursing School at Taegu area. Each of the specimens was examined one time or twice under the microscope, one for protozoan cysts stained with iodine solution and the other for helminth eggs. Findings were summerized as follows: A positive rate of intestinal parasites was 88.0 percent by Formalin-Ether concentration method and 89.2 percent by Kato's method. Of the 541 soldiers examined by Formalin-Ether concentration method, the infection rate of A. lumbricoides was 28.1 percent, T. trichiurus 79.3 percent, hookworm 14.2 percent, T. orientalis 30.7 percent, C. sinensis 7.8 percent and M. yokogawai 0.6 percent. Of the 1,532 soldiers examined by Kato's method A. lumbricoides showed positive rate of 45.8 percent, T. trichiurus 78.3 percent, T. orientalis 1 4.5 percent, hookworm 7.5 percent, C. sinensis 6.6 percent and Taenia sp. 0.6 percent. The incidence of E. vermicularis out of 822 samples was 19.8 percent from the anal swabs. E. histolytica was recovered in 4.2 percent among 541 samples, E. coli 6.1 percent and also infected G. lamblia, E. nana, I. butschlii and Chilomastix mesnili. Most of the infected cases revealed single or double infections.

8.Parasitologial studies of Korean Forces in South Vietnam II. A comparative study on the incidences of intestinal parasites.

A survey of intestinal parasites was performed in South Vietnam. Samples were collected from 717 Vietnameses, 1,933 U.S. Forces, 433 stool specimens of Korean Troops in South Vietnam. And 114 of Korean Army patients in home land were also examined as a control. Findings were discussed with others and summarized as follows: A parasitic rate, incidence in general, was 44.8 %. Koreans in South Vietnam showed 82.4 % of infection rate, Vietnamese 64.6% and U.S. Forces 26.1 %. Korean home patients revealed 93.9 %. Following 10 species of helminths ova and 5 kinds of protozoa were found: Ascaris lumbricoides, Hookworm, Trichocephalus trichiurus, Strongyloides stercoralis, Trichostrongylus orientalis, Enterobius vermicularis, Metagonimus yokogawai, Clonorchis sinensis, Hymenolepis nana, Taenia sp., Entamoeba histolytica, Entamoeba coli, Endolimax nana, Giardia lamblia and Trichomonas hominis. The incidence of Ascaris lumbricoides among Koreans in South Vietnam was 15.9 %, much less than Korean home patients. The infection rate of A. lumbricoides in Saigon residents was 47.2% but in Chulai 36.8% and in Quinhon 35.8 %. Trichocephalus trichiurus was most frequently found in Koreans. Quinhon residents showed 78.7 % positive in hookworn infection and Chulai 24.7 %. In Saigon, on the contrary, infection rate was only 3.1 %. No Strongyloides stercoralis, but two cases of Trichomonas hominis were found among Koreans in South Vietnam. U.S. Forces showed higher incidences than Koreans and Vietnameses in the incidences of Entamoeba histolytica and Endolimax nana. Most of the U.S. Forces revealed single infection. Korean home patients showed the highest per centage of polyparasitism. Non-autochthonous infections in Korea could not be found among Koreans in this survey.

9.Anthelminthic effectiveness of 2,3,5,6,-tetrahydro 6-phenyl-imidazole (2,1-b) thiazole hydrochloride (=tetramisole) upon intestinal parasites.

A single dose of Tetramisole, 2.5 mg/kg body weight, was given to the infected cases of intestinal parasites. The number of cases were: Ascaris lumbriocides 96, hookworm 16, Trichostrongylus orientalis 10, Trichocephalus trichiurus 114 and Clonorchis sinensis 19. No dietary restriction before and after the administration of Tetramisole was required. In Ascaris infection the egg negative conversion rate and the egg reduction rate were 92.7 per cent and 99.5 per cent, respectively. In Trichostrongylus orientalis infections, 9 out of 10 cases were resulted egg negative after the single dose of Tetramisole, and hookworm, 12 out of 16 administered showed egg negative. However, there were no appreciable effectiveness to the cases of Trichocephalus trichiurus and Clonorchis sinensis. Mild and transient side effects were noted in 75 cases (54.5%) out of 140 cases. The main symptoms were dizziness (25.5%), anorexia (25.5%), abdominal pain (18.6%), diarrhea (16.6%), headache (15.2%), nausea (14.4%) and fever (11.0%). From the above results, it is anticipated that Tetramisole is an effective anthelminthic for elimination of Ascaris, Trichostrongylus and hookworm.

10.Prevalence of intestinal parasites in Korea.

A survey of intestinal parasites infection among Korean people has been carried out during July 1969 to December 1970. A total of 2,250 stool specimens (male 1,101, female 1,146) was collected from all the provinces and Seoul city in Korea. The specimens were examined routinely by direct fecal smear, zinc sulfate flotation and formalin-ether sedimentation techniques. The results are summarized as follows: Of 2,250 specimens examined, l,803(80.l per cent) were positive for intestinal parasites. The positive rates of intestinal helminths were 1,644(73.1 per cent) among 2,250; Ascaris lumbricoides 46.0 per cent, Trichocephalus trichiurus 46.8 percent, hookworm 6.8 per cent, Trichostrongylus orientalis 7.0 percent, Clonorchis sinensis 12.1 percent, Enterobius vermicularis 1.6 per cent, Hymenolepis nana 0.7 percent, Taenia species 0.3 per cent, Metagonimus yokogawai 0.04 percent, Fasciolidae 0.04 per cent and one case of lung fluke Paragonimu westermani. The positive rstes of intestinal protozoa were 786(34.9 per cent); Entamoeba histolytica 6.4 per cent, Entamoeba coli 20.5 percent, Endolimax nana 10.0 per cent, Giardia lamblia 5.1 per cent, Trichomonas hominis 1.1 percent, Chilomastix mesnili 0.5 percent, Iodamoeba butschlii 0.6 percent, Enteromonas hominis 0.7 percent, Dientamoeba fragilis 0.1 per cent and one case of Isospora hominis. Sexual distribution of helminths and protozoan infections showed higher rate in female than that of male, except C. sinensis, H. nana, Taenia species or G. lamblia Infections of T. trichiurus, hookworm, T. orientalis, C. sinensis, Taenia species, E. histolytica, E. coli and E. nana increased with age. Conversely, H. nana and G. lamblia infections were more predominent in younger ages.
