1.Affecting Factors of the Awareness of Biomedical Ethics in Nursing Students.

PURPOSE: This study was conducted to examine awareness of biomedical ethics, and to identify affecting factors of the awareness of biomedical ethics in nursing students. METHODS: The subjects consisted of 266 nursing students their third and fourth years of study. The data were collected from October to December, 2015 by self-report using questionnaires. Data analysis was performed using SPSS/WIN 18.0, descriptive statistics, t-test, ANOVA, Scheffé test, Pearson correlation coefficient, and multiple regression analysis. RESULTS: The mean score of the awareness of biomedical ethics was 2.81±0.22, perception of death was 3.15±0.36, and knowledge of brain death, organ donation, and organ transplant was 12.12±3.02. The prediction factors of awareness of biomedical ethics were gender (β=.29, p<.001), participation in religious activity (β=.23, p=.015), and perception of death (β=.20, p=.016). The explanation power was 17.1%. CONCLUSION: These results showed that education about biomedical ethics is necessary for nursing students, and the development of biomedical ethics educational programs should reflect affecting factors.

2.Development of Organ Transplantation Information System Associated with Hospital Order Communicating System (OCS) and Korean Networking for Organ Sharing (KONOS).

A well designed transplant software program with its database is a valuable tool in the management of transplant patients and transplant associated works. PURPOSE: The purpose of this study is to make the organ transplantation information system associated with Hospital Order Communicating System (OCS) and Korean Networking for Organ Sharing (KONOS) METHODS: We used the Hospital Order Communicating System in Asan Medical Center as a data capture source. Web EDI system was going to be used to connect the Korean Networking for Organ Sharing. RESULTS: Electronic link to Order Communicating System in our hospital can eliminate the necessity of double entry of data and eliminates the manual entry errors while increasing the efficacy of service. Multicentric accessibility to this new software system makes the data entry be easy. This program can capture the necessary data including text, numeric, scanned data at all stages of treatments, from the pretransplant to posttransplant follow-up using data transmission with Order Communicating System. The built-in tools for data analysis and other services can assist the coordinators in performing their daily tasks. Complying with timely reporting demanded by KONOS overburdened the transplant center staff and coordinators. We are already prepared to connect to KONOS reporting system using EDI system, and we are waiting for response of KONOS. CONCLUSION: Even though it was not simple to make the well designed transplant software because it involves several concerns, we were able to make the organ transplant information system linked to OCS successfully. We hope this system can be connected to KONOS in the near future.

3.A Study on the Experience of Patients with Chronic Renal Failure who have Received a Kidney Transplant.

The grafting of a kidney has been found to be the best medical treatment for patients who have renal insufficiency failure, but the patients still have experienced much trouble and apprehension. This study was done to further nursing theory developing for patients who have has a kidney graft from another person. The research method followed grounded theory methodology of Strauss and Corbin. The subjects were three female and four male patients. This study done between Oct. 1997 and Mar. 1998. All of the subjects were interviewed by the author. Interview were done by the long interview technique and observation. In the process of data analysis, 'heart-boiling' was found to be the core phenomenon. The results were composed to 101 concepts. These concepts were grouped into nineteen categories, and then to twelve categories. There were 12 super-class categories as follows ; 'pain', 'heart-boiling', 'experience of dialysis', 'term of admission to a hospital', 'support of other person', 'dependence on God', 'direction', 'negative reaction', 'positive reaction', 'comfortable', 'lacking', 'acceptance'. In this process, 14 hypotheses were derived from the categories as follows. (1) The more experience with dialysis that the patients have, the stronger the heart-boiling will tend to be. (2) The less experience with dialysis the patients have, the weaker the heart-boiling will tend to be. (3) The longer admission to hospital the patients have, the stronger the heart-boiling will be. (4) The shorter the admission to hospital the patients have, the weaker the heart -boiling will be. (5) The weaker the intense-grief is, the more positive the reaction to heart-boiling the patients wall have. (6) The stronger the intense-grief is, the more negative the reaction to heart-boiling the patients will have. (7) The stronger the support of other persons that the patients have, the more positive the reaction to heart-boiling the patients will have. (8) The weaker the support of other person that the patients have, the more negative the reaction to heart-boiling the patients will have. (9) The stronger the dependence on God that the patients have, the mare positive reaction to heart-boiling the patients will have. (10) The weaker the dependence on God that the patients have, the more negative reaction to heart-boiling the patients will have. (11) The more positive thoughts that the patients have, the more positive reaction to heart-boiling the patients will have. (12) The more negative thoughts that the patients have, the more negative reaction to heart-boiling the patients will have. (13) The more positive reaction the patients have, the more free from heart-boiling the patients tend to be. (14) The more negative reaction the patients have, the less free from heart-boiling the patients tend to be. From the analysis of observed data and comparing each class, 1 concluded that there are four formula relation types between reaction of patients and heart-boiling. (1) If patients have the experience of dialysis, have a long term admission to hospital, are strong in heart-boiling, depend on God, have positive thoughts and another's strong support, they experience release by positive reaction to the intense-grief. (2) If patients have the experience of dialysis, have a short term admission to hospital, are weak in heart-boiling, do not depend on God, have negative thoughts, and have few supports from others, they experience attachment to heart-boiling thought a negative reaction. (3) If patients have the experience of dialysis, have a long term admission to hospital, are strong in heart-boiling, do not depend on God, and have negative thoughts, they experience attachment to heart-boiling through negative reaction in spite of support from another. (4) If patients have the experience of dialysis, have a long term admission into hospital, are strong in heart -boiling and sat isfaction is low, but they have positive thoughts, then they experience acceptance and harmony through the positive reaction to heart-boiling. The results of this study are expected to help the way nurses care for patients who have had a kidney graft from another.

4.Comparison of Costochondral Graft and Customized Total Joint Reconstruction for Treatments of Temporomandibular Joint Replacement.

PURPOSE: We review published research on temporomandibular joint (TMJ) total replacement that compares costochondral graft and customized total joint reconstruction (especially TMJ concepts), focusing on effectiveness. METHODS: We searched PubMed databases, including prospective, retrospective, case-control or longitudinal studies and significant statistical analysis. In data analysis, we divided outcomes into 'Acceptable' or 'Non-acceptable'. RESULTS: There were seven articles found dealing with costochondral graft and 180 patients. The majority of patients had satisfactory treatment outcomes (n=109, 61%). There were six articles including 275 patients using the alloplastic material TMJ concepts. Almost all patients had satisfactory treatment outcomes (n=261, 95%). CONCLUSION: Comparing customized total joint reconstruction with costochondral graft, use of TMJ concepts resulted in increased quality of life and fewer complications. In conclusion, we judged that alloplastic material such as TMJ concepts is more effective device in total joint replacement than costochondral graft.

5.Kidney Transplantation in Patients with Focal Segmental Glomerulosclerosis.

Purpose: Kidney transplantation (KT) is one of the important treatment modality for the patients with focal segmental glomerulosclerosis (FSGS), but high recurrence rate and resulting graft failure is still a great obstacle. In order to compare the results of transplantation for recipient with FSGS, we reviewed our cases retrospectively. Methods: Thirty-six biopsy proven FSGS were reviewed and compared their clinical characteristics according to their recurrence status, retrospectively. Patient with significant proteinuria after KT was re-biopsied and light- and electron-microscopic study were done to confirm the recurrence of original disease. Results: Recurrence of FSGS was confirmed histologically in 13 (36%) recipients. Among 15 failed grafts, 9 grafts lost their function by recurrence of FSGS. Higher rate of acute rejection associated in recurred group (53% vs 39%). Five-year graft survival of recurred group was significantly lower than non-recurred group (65% vs 78%, P=0.0071). Cyclosporin group showed more frequent recurrence of FSGS after transplantation than tacrolimus group but no statistical significance (P>0.05). Plasmapheresis (PP) was done in 8 patients with early recurred FSGS and was effective in reducing the grade of proteinuria. Their long-term graft survival, however, was poor even though half of the recurred patients maintain their graft function until 5 years after PP. Conclusion: Our data showed that the recurred FSGS group showed higher rate of graft loss and poor graft survival. Since the FSGS recurrence is directly related with graft survival, large data analysis will be necessary to analyze the risk factor of recurrence and prevent adverse effect of recurrence of FSGS after KT.

6.Psychosocial Adjustment of the Organ Transplantation Recipients in Korea.

PURPOSE: This study was aimed to understand the experience and adjustment process of organ transplantation recipients in Korea, using grounded theory method. METHODS: Eight patients after liver transplantation and two patients after heart transplantation were selected for this study, for whom minimum follow-up duration was at least 6 months after transplantation. Data was collected from July to November 2005 through in-depth individual interviews either at home or hospital. The data were analyzed using a constant comparative method in which new data was continuously coded into categories and properties. RESULTS: Data analysis revealed that "safekeeping the graft" was the core category in experience of organ transplantation recipients. In order to safekeep transplanted organ, recipients used seven different strategies to deal with various problems and conflicts which occurred during the recovery process: 1) altering the role function of family, 2) complying the treatment regimen, 3) developing the know-how for self-care, 4) returning to the normal life, 5) keeping the positive thinking, 6) concerning for the donor, and 7) improving the self-fulfillment. CONCLUSION: The results of this study provided deep understanding on the recovery process after organ transplantation and these would help establishing more appropriate intervention to improve quality of life for organ transplantation recipients.

7.Effect of Stem Cell Therapy on Induced Diabetic Keratopathy in Albino Rat.

BACKGROUND AND OBJECTIVES: Type 2 diabetes mellitus (DM) is a prevalent disorder. Diabetic keratopathy is a well-known ocular complication secondary to type 2 DM. Topical insulin application did not affect apoptosis and necrosis levels in corneal epithelium. Autologous cell transplantation is not a viable option for diabetic patients with bilateral limbal stem cell deficiency. The present study aimed at assessing the possible effect of hemopoeitic stem cell (HSC) therapy on induced diabetic keratopathy in albino rat. METHODS AND RESULTS: Fifteen male albino rats were divided into control group of 2 rats, diabetic group of 8 rats receiving single intraperitoneal (IP) injection of 50 mg/kg streptozotocin (STZ). 3 animals were sacrificed 6 weeks following confirmation of diabetes to confirm keratopathy and 5 rats were sacrificed 4 weeks following confirmation of keratopathy. SC therapy group included 5 rats injected with HSCs 6 weeks following confirmation of diabetes and sacrificed 4 weeks following SC therapy. Cord blood collection, stem cells isolation and labeling were performed. Eye specimens were subjected to histological, histochemical, immunohistochemical, morphometric and statistical studies. In diabetic group, the central cornea showed multiple cells with vacuolated cytoplasm and dark nuclei, focal epithelial discontinuity, reduced corneal thickness and less number of layers of corneal and conjunctival epithelia. In stem cell therapy group, few cells with vacuolated cytoplasm and dark nuclei were found in the corneal and conjunctival epithelia with more number of epithelial layers. CONCLUSIONS: A definite ameliorating effect of HSC therapy was detected on diabetic keratopathy. The therapeutic cells were effective in limiting corneal epithelial changes.

8.Consciousness of Biomedical Ethics in Nursing Students and Non-Nursing Students.

PURPOSE: The purpose of this study was to provide basic data necessary for educating nursing students by establishing a desirable sense of ethics values. METHOD: In this descriptive research, 101 nursing students and 191 non-nursing students (in other health related fields) from Daejeon City were selected. Tools used for data analysis were descriptive statistics, t-test, one-way ANOVA, Scheffe test and step wise multiple regression with SPSS/WIN 12.0 version. RESULTS: Average scores for consciousness of biomedical ethics in nursing students was 2.94, and in non-nursing students, 2.77. When differences in the consciousness of biomedical ethics of nursing and non-nursing students were compared, nursing students had significantly higher scores for artificial abortion, artificial insemination, prenatal diagnosis of fetus, right to life of newborn, euthanasia, organ transplantation and human biotechnology, but significantly lower for brain death. Variables which influenced consciousness of biomedical ethics were religion and economic status in the nursing students and intent to attend a class in biomedical ethics, quantity and quality of biomedical ethics in the current curriculum and religion in the non-nursing students. CONCLUSION: Continuing educational programs need to be considered and attention given to the significant variables that can promote consciousness of biomedical ethics in nursing students.

9.Analysis of Nursing Researches about Organ Donation and Transplantation in Korea.

PURPOSE: This study analyzes the trends of nursing researches about organ donation and transplantation, and recommends the direction of future nursing studies in Korea. METHODS: Ninety-nine researches based upon organ donation and transplantation in Korea were analyzed by descriptive statistics. RESULTS: Among them 58 papers were master's theses and 9 were doctoral dissertations. Articles about organ beneficiaries were seventy. The 47 articles among them were for kidney transplantation. By the types of research design, there were 73 quantitative studies, 19 qualitative studies, and 9 methodological studies. In correlation studies, the quality of life of subjects were evaluated the association with stress, social support, self-efficacy, and compliance. In experimental studies, the independent variables were self efficacy promotion exercise, steroid medication, educational programs regarding an organ transplantation and a brain death, Danjeon breathing exercise, and telephone counseling. The methods of qualitative studies were based on the grounded theory, phenomenology, interpretive phenomenology, and ethnography. The dominant concepts of qualitative researches were experiences of a decision-making of donors and of recipients for organ transplantation. CONCLUSION: Descriptive surveys or correlation studies were predominant on the nursing research about organ transplantation. Qualitative studies were conducted to some extent. It is recommended to conduct clinically applicable interventional researches with the experimental design.

10.MR Imaging of Stable Posterior Cruciate Ligament Grafts in 21 Arthroscopically Proven Cases.

OBJECTIVE: To describe the magnetic resonance (MR) appearance of intact posterior cruciate ligament (PCL) grafts. MATERIALS AND METHODS: Thirty-one postoperative MR examinations were performed in 21 grafts of 20 patients after PCL reconstruction. All 21 grafts were proven to be intact on second-look arthroscopic examination. Two musculoskeletal radiologists retrospectively analyzed the MR findings and reached decisions by consensus. The signal intensity (SI) of the graft on proton density-weighted and T2-weighted images, as well as the shapes, locations, and segments of increased SI were recorded. The graft thickness was also recorded and correlated to elapsed time since reconstructive surgery. RESULTS: The SI of the graft was high (15/31, 48%), intermediate (10/31, 32%), or low (6/31, 19%) on proton density-weighted images, and high (9/31, 29%), intermediate (6/31, 19%), or low (16/31, 52%) on T2-weighted images. The graft SI decreased significantly as postoperative time elapsed. The shape of the increased SI within the grafts was band-like (14/25, 56%) or focal (11/25, 44%). The increased SI was located in the proximal (18/25, 72%), middle (21/25, 82%), and distal (12/25, 48%) segments. In the axial plane, the location of increased SI was intrasubstance (19/25, 76%) or peripheral (10/25, 40%). A 'focal' shape of increased SI was found significantly more in Achilles tendon allografts, while a band-like shape was more frequent in autogenous double-loop hamstring tendon grafts. Graft thickness ranged from 5-15 mm. The difference in graft thickness relative to postoperative time was not statistically significant (p = 0.79). CONCLUSION: Stable PCL grafts commonly showed an increased SI at any segment or location, even though they were stable. The shape of increased SI differed according to allograft donor sites. However, SI tended to decrease as time elapsed.
