1.Energy expenditure during sport activities by means of a portable device for the measurement of oxygen.

Energy expenditure during sport activities has been determined traditionally by the Douglas Bag Method and the Motion Time Study. However, those two methods do not yield accurate values when used in long continuous and/or vigorous physical activities. This study, therefore, measured oxygen uptake by means of a portable device “Oxylog”, and determined the energy expenditure of many sport activities. The experiments were carried out with 13 untrained male subjects (UTS), and 30 trained male subjects (TS) . The 30 trained men consited of ten tennis players, ten badminton players and ten basketball players. UTS played one set of doubles (tennis), one set of singles (badminton), two sets of singles (table tennis), two games (bowling) and jogged 10 minutes, But TS played only their major sports : one set of doubles (tennis), one set of singles (badminton) and two 20-minute halves (basketball) . The results were 0.172±0.017kcal⋅kg-1⋅min-1 for jogging (UTS), 0.146±0.028kcal⋅kg-1⋅min-1 for badminton (TS), 0.133±0.021kcal⋅kg-1⋅min-1 for basketball (TS), 0.130±0.018kcal⋅kg-1⋅min-1 for badminton (UTS), 0.102±0.016kcal⋅kg-1⋅min-1 for tennis (TS), 0.096±0.014kcal⋅kg-1⋅min-1 for tennis (UTS), 0.089±0.019kcal⋅kg-1⋅min-1 for table tennis (UTS), and 0.055±0.009kcal⋅kg-1⋅min-1 for bowling (UTS) . In comparison with UTS, TS exhibited higher values in tennis and badminton. This is considered to result from TS's better training. This result indicates that energy expenditure in playing sports activities depends on the level of player's ability. The method employed in of this study is believed to be the best choice at present. Many other sport activities must be reexamined in detail using this method.
2.Introduction of collaborative drug therapy management (CDTM) into a community healthcare system in Japan

Introduction : As one of the strategies for the introduction of collaborative drug therapy management (CDTM) into a community healthcare system in Japan, we constructed a ‘positive list’ which was agreed by a family physician in advance, and subsequently validated the effects on healthcare insurance fees, the number of prescription question and the changes of utilization rate for generic drugs.
Methods : The items that replaced prescription questions with regard to ex post facto approval were defined as ‘Resolved Questions’ in our constructed positive list. By adopting the ‘Resolved Questions’, we then examined the effect on healthcare insurance fees, prescription questions and the changes of utilization rates for generic drugs.
Results : Among all prescription questions accepted from January through May 2012 inclusive, 178 (22.7%) were ‘Resolved Questions’, without there being change in the therapeutic strategies by the prescribing physicians. During this period, 17,455 healthcare insurance fees were reduced. In addition, the utilization rate for generic drugs were increased up to 46.6%.
Conclusion : The reduction of healthcare insurance fees, prescription questions and the increased utilization rates of generic drugs were facilitated by prudent pharmacist judgements based on ‘Resolved Questions’.
3.Medulloblastoma Manifesting as Sudden Sensorineural Hearing Loss.

We present a rare case of medulloblastoma which presented with unilateral sudden sensorineural hearing loss as an initial symptom. A 19-year-old man was admitted to our hospital with a chief complaint of dizziness and facial numbness on the right side. His illness had begun two years previously with sudden hearing loss on the right side, for which he had been treated as an idiopathic sudden hearing loss. Magnetic resonance imaging demonstrated abnormal signals located mainly in the right middle cerebellar peduncle. We performed partial resection of the tumor by suboccipital craniotomy. The histopathological diagnosis was medulloblastoma. Intrinsic brain tumor is an extremely rare cause of sudden sensorineural hearing loss and is therefore easily overlooked as was in the present case. The present case highlights not only the need to evaluate patients with sudden sensorineural hearing loss by magnetic resonance imaging but also the importance of paying attention to intrinsic lesions involving the brainstem. Although this condition like the presented case might be rare, intrinsic brain tumor should be considered as a potential cause of sudden sensorineural hearing loss, as it may be easily missed leading to a delay in appropriate treatment.
