1.The effect of Xuebijing Injection on inflammatory factor of large sized avulsion patients

Objective To investigate the infection of Xuebijing injection on inflammatory factor of large sized avulsion patients.Methods 70 patients being selected with large sized avulsion were randomly recruited into a treatment group(35 patients)and a control group(35 patients).The control group received traditional comprehensive treatment.On this basis,Xucbijing injection was injected to the treatment group on admission day,and Xuebijing injection 50 ml in 0.9%NaCl solution 100 ml was,intravenously infused for 60 min once,2 times daily,up to 7 d.TNF-α,IL-6,CRP,WBC and NEU%of the two groups were respectively detected before treatment and 7 days after the treatment.Exudation of wound surface was also observed.Results After 7 days of treatment,there was significant difierence in the treatment group compared with pretreatment(P<0.01).There was statistical difference between two groups after 7 days of treatment(P<0.01).Exudation of wound surface of the treatment group was less than the control group's.Conclusion Xuebijing injection has antagonistic effect on inflammatory factor of large sized avulsion patients and can lessen exudation of wound surface.
2.Distribution characteristics of astigmatism in preschool children from Tongzhou District of Beijing and its effect on visual acuity

AIM: To explore the characteristics of astigmatism distribution among preschool children from Tongzhou District, Beijing, discuss its categorizations, severity, and the effect on preschoolers' vision, and clarify the influence of cycloplegic refraction on the detection of astigmatism.METHODS:In this cross-sectional study conducted from December 2021 to January 2022, a cluster random sampling method was utilized to assess 1 498 preschool children(2 996 eyes)from Tongzhou District, Beijing. The sample comprised 791 males and 707 females, with 222 children aged 3 to <4 years, 521 children aged 4 to <5 years, 647 children aged 5 to <6 years, and 108 children aged 6 to <7 years. Evaluations included visual acuity, anterior segment, computerized optometry, and cycloplegic refraction.RESULTS:Prior to cycloplegic refraction, the prevalence of astigmatism was found to be 61.88%(927/1498). For post-cycloplegic refraction, this percentage slightly increased to 64.02%(959/1498, P=0.095). Following cycloplegic refraction, the distribution of astigmatism severity was as follows: 51.87%(777/1498)had mild astigmatism, 9.41%(141/1498)had moderate astigmatism, and 2.74%(41/1498)had severe astigmatism. Astigmatism was predominantly with-the-rule across all age groups, with compound hyperopic astigmatism being the most frequent type. In cases of subnormal vision caused by astigmatism: low degree accounted for 9.38%, moderate degree accounted for 25.4%,and high degree accounted for 52.6%.CONCLUSION:The findings reveal a high incidence of astigmatism in preschool children, predominantly in a mild nature. Cycloplegic refraction was observed to have a negligible effect on the rate of astigmatism detection. Moreover, its impact on vision becomes more significant as the degree of astigmatism increases.
3.Analysis of influencing factors of post-vaccination serologic testing in children exposed to hepatitis B virus in Tongzhou District, Beijing from 2020 to 2022

ObjectiveTo explore the importance of post-vaccination serological testing (PVST) for children exposed to hepatitis B virus (HBV), and analyze the factors affecting the progress of PVST. MethodsThe study focused on hepatitis B surface antigen (HBsAg)-positive pregnant women and their newborns residing in Tongzhou District, Beijing, who delivered at various obstetric institutions from January 1, 2020 to March 31, 2022. The obstetric institutions and community health service centers conducted follow-up visits 1 to 2 months after the children had received three doses of the hepatitis B vaccine (HepB). ResultsThe vaccination rate of hepatitis B immunoglobulin (HBIg) was 100.00% (800/800), with a successful PVST follow-up rate of 85.88% (687/800) in Tongzhou District. The initial non-response rate to immunization was 0.29% (2/687), but successful immunization was achieved after re-immunization. The mother-to-infant transmission rate of hepatitis B was 0. Children who did not undergo PVST accounted for 14.13% (113/800), with the main reasons being delays due to the COVID-19 pandemic, parents’ reluctance to allow venous blood collection due to the young age of the children, and loss to follow-up because children moved back to their parents’ place of origin. Logistic regression analysis showed that the proportion of PVST was higher among high-risk children (OR=30.009,P=0.001), children with family residing in Beijing (OR=2.218,P=0.002), and children whose mothers were <35 years old (OR=1.687,P=0.020). ConclusionPVST is necessary for assessing the status of HBV immune response in newborns after vaccination with HepB. The COVID-19 pandemic impacted the implementation of PVST for children exposed to HBV. Strengthening the management of non-high-risk children, those living outside Beijing, and children with mothers aged ≥ 35 years old may increase the rate of PVST in Tongzhou District.
4.Clinical observation on treatment of diabetic peripheral neuropathy with qi-supplementing and blood-activating therapy.

OBJECTIVETo observe the clinical efficacy of Qi-supplementing and blood-activating (QSBA) in treating diabetic peripheral nephropathy (DPN).
METHODSSixty-eight type 2 diabetes mellitus patients with Qi deficiency-blood stasis Syndrome were randomly divided into two groups, the neurotrophic agents were used in both groups, while QSBA herbs were used in the treated group additionally. The treatment course was 2 months. Blood glucose (BG), triglyceride (TG), total cholesterol (TC) and nerve conduction velocity (NCV) were detected before and after treatment.
RESULTSAfter treatment, the BG, blood lipid, NCV were improved significantly (P < 0.05) in both groups, but the improvement was more significant in the QSBA treated group than in the control group (P < 0.05).
CONCLUSIONQSBA, in treating DPN, can not only improve its symptoms, but also ameliorate the NCV.

5.Consumption of nutritional supplements among the residents aged eighteen years and below in Tongzhou District

Objective:
To understand the consumption of nutritional supplements among the residents aged eighteen years and below in Tongzhou District of Beijing,so as to provide reference for supplements exposure assessment and health education.
Methods:
A hundred people aged 18 years and below were selected from each of the eight neighborhood committees of Tongzhou Town,Tongzhou District,Beijing. The questionnaire for the Nutritional Supplements Guidelines for Chinese Residents Study was used to collect the rate of taking nutritional supplements,influencing factors,types and frequency.
Results:
The rate of taking nutritional supplements was 44.16%. The rates of taking nutritional supplements in people aged 0-5 years,6-12 years and 13-18 years were 45.56%,42.86% and 41.00%,respectively,without statistically significant difference among them (P>0.05). The rates of taking nutritional supplements in males and females were 44.82% and 43.49%,without statistically significant difference between them (P>0.05). The Results of multivariate logistic regression analysis showed that annual per capita household income of more than 50 000 yuan (OR=1.688,95%CI: 1.171-2.435) and parents taking nutritional supplements (OR=4.104,95%CI: 3.023-5.573) were the promoting factors for the intake of nutritional supplements in people aged 18 years and below. The rates of taking calcium,vitamin D and multivitamin were 26.10%,21.56% and 8.31%,respectively. There were 272 (80.00%) people taking 1-2 kinds of supplements,and 200 (58.82%) people taking them daily.
Conclusions
In Tongzhou District,44.16% of the population aged 18 years and below takes nutritional supplements,mainly calcium and vitamin D. Family income and whether their parents taking or not are determinants.
6.Improved anatomical locking plate internal fixation through tarsal sinus incision in treating Sanders III-IV calcaneal fractures.

OBJECTIVETo investigate clinical effects of improved anatomical locking plate internal fixation through tarsal sinus incision in treating Sanders III and IV calcaneal fractures.
METHODSFrom February 2015 to October 2016, 35 patients with Sanders III and IV calcaneal fractures treated by improved anatomical locking plate internal fixation through tarsal sinus incision were collected, including 22 males and 13 females aged from 22 to 68 years old with an average of (42.3±12.7) years old. According to Sanders classification, 23 patients were type III and 12 patients were type IV. Postoperative complications were observed, Bö hler angle and Gissane angle before and after operation were compared, and Maryland foot function scoring standard was evaluated at 12 months after operation.
RESULTSAll patients were followed up from 12 to 20 months with an average of (14.5±2.0) months, the length of incision ranged from 4.0 to 5.5 cm with an average of (4.7±0.4) cm. Superficial infection occurred in 1 patient, delayed union in 2 patients, gastrocnemius nerve injury in 1 patient. No complications such as steel plate exposure and fracture reduction loss occurred. Wound healing time ranged from 14 to 28 days with an average of(15.4±4.7) days, the fracture healing time ranged from 8 to 14 weeks with an average of (9.8±1.9) weeks. Bö hler angle increased from preoperative (9.81±14.28)° to 3 days after operation (26.35±11.04)°, and (25.96±10.79)° at 12 months after operation(<0.05). Gissane angle ranged from preoperative (122.54±16.79)° to 3 days after operation (120.85±11.88)°, and (120.62±11.44)° at 12 months after operation and had statistical meaning. Maryland score increased from 12.66±4.10 before operation to 92.20 ±7.82 at 12 months after operation, and 32 patients got excellent results, 2 good and 1 moderate.
CONCLUSIONSImproved anatomical locking plate internal fixation through tarsal sinus incision in treating Sanders III and IV calcaneal fractures, which has advantages of less incision, less soft tissue injury, better fracture reduction and fixation, could receive good reduction and fixation. It is an effective method for Sanders III and IV fracture of calcaneus fracture.
7.Analysis of serum uric acid levels and associated factors among primary and middle school students in Tongzhou District, Beijing

Objective:
To examine uric acid (UA) levels and their influencing factors among primary and middle school students in the Tongzhou District of Beijing, and to provide referential guidance for the prevention and control of hyperuricemia in teenagers.
Methods:
A total of 1 483 students from four middle schools and four primary schools in three cities and three towns in Tongzhou District were selected for a questionnaire survey, physical examination, and blood examination using a multi-stage stratified cluster sampling method. The results of the serum uric acid (SUA) examinations were statistically analyzed and the influencing factors were evaluated.
Results:
The UA level of the students was skewed, with a median value of 296.0 mol/L and an interquartile distance of 96.0 mol/L. The overall detection rate of hyperuricemia was 14.83% (220/1 483). Middle school students (342 μmol/L), students from villages and towns (307 μmol/L), males (309 μmol/L), family dining students (298 μmol/L), students with high levels of triglyceride (TG) (318 μmol/L), students with low levels of high-density lipoprotein cholesterol (HDL-C) (329 μmol/L), and obese students (303 μmol/L) all had statistically higher SUA levels(H/U value was 119 938, 229 525, 212 043, 156 347, 42 052, 63 375, 33.49, P<0.05). The univariate analysis showed that the HUA detection rate was higher in older age groups, middle school students, township students, overweight or obese students, and low HDL-C students(χ 2=614.55, 73.20, 6.16, 30.56, 14.50, P<0.05). Multivariate Logistic regression showed that older age, being male, low HDL-C, high LDL-C, overweight, and obesity were risk factors for HUA(P<0.05).
Conclusion
The SUA level and the detection rate of hyperuricemia among primary and middle school students in Tongzhou District were high, and the risk of disease and influencing factors had significant characteristics. Therefore, targeted prevention and control measures should be strengthened.
8.An analysis of the abnormal results of individual dose monitoring for radiation workers in Tongzhou District, Beijing, China, 2015-2019

Objective To analyze the abnormal results of individual dose monitoring for radiation workers in Tongzhou District, Beijing, China, and to propose corresponding improvement measures. Methods Questionnaires were used to investigate the radiation workers who had abnormal dose monitoring results. Results In 2015—2019, among the 12 595 individual dose monitoring results for radiation workers in Tongzhou District, 23 were abnormal. The main type of work with abnormal monitoring results was medical diagnostic radiographer (69.60%). A total of 17 (73.90%) radiation workers had an exposure dose ranging from 1.25 mSv to 5 mSv. Conclusion The abnormal dose results were all from non-occupational exposure, mainly due to the dosimeter left in the workplace. The key to solving the problem is to further strengthen the education and training for radiation workers, to improve the institutional radiation protection management, and to supervise and inspect the relevant work strictly.
9.Nested case-control study on the risk factors and interaction analysis of positive screening for infant motor development

【Objective】 To identify the influencing factors of infant motor development, and to explore the potential interactive factors, so as to provide scientific basis for early prediction and targeted prevention of infant motor developmental delay. 【Methods】 Data of infants receiving "0~1 motor development screening" at 21 community health service centers in Tongzhou District, from January 1st, 2020 to August 31st, 2021 were collected through the Beijing Maternal and Child Information System.Using the method of nested case-control study, 416 infants with positive screening results were selected as case group, 416 infants with negative screening result were selected into the control group by 1∶1 case-control matching of screening age.Chi-square test, Logistic regression analysis, and Exhaustive CHAID decision tree model were used to explore the influencing factors of infant motor development, the interaction effect was investigated by using the additive and multiplicative interaction models. 【Results】 Multivariate Logistic analysis showed that birth weight<2 500g (OR=3.28), cesarean section (OR=1.63), outdoor activity≤ 1h/d (OR=3.07), and no supplementation of vitamin D (OR=2.68) were risk factors for positive screening results of infant motor development (P<0.01).The Exhaustive CHAID decision tree showed that low birth weight was the primary risk factor for infant with positive screening results, followed by outdoor activity, vitamin D supplementation and delivery.The interaction analysis showed that there was a multiplicative interaction (OR=13.76, 95% CI:1.20 - 158.08) and an additive interaction (RERI=10.22, 95%CI:5.05 - 15.38) between non supplementation of vitamin D and low birth weight on infant motor development with positive screening results. 【Conclusions】 Attention should be paid to the early motor development of infants with low birth weight and those born by cesarean section, with emphasis on the dosage of vitamin D supplementation and outdoor activity duration.Moreover, the early screening and hierarchical management of infant motor development should also be strengthened.
10.Exploration on Surrogate Outcome Measures in Clinical Trials of Traditional Chinese Medicine for Diabetic Kidney Disease

Traditional Chinese medicine (TCM) has unique advantages in alleviating the symptoms of diabetic kidney disease (DKD) and slowing its progression. However, traditional clinical trials often use the occurrence of end-stage renal disease as the end point, requiring long-term follow-up, which increases trial complexity and costs, thereby limiting the feasibility of TCM clinical studies. This paper suggested that in clinical trials of TCM for DKD, both the estimated glomerular filtration rate (eGFR) change rate (≥30%) and eGFR slope can serve as potential surrogate outcome measures. If the intervention course is short (<1 year), the eGFR change rate (≥30%) is recommended as a surrogate outcome measure, whereas in long-term interventional studies (≥1 year), the eGFR slope may be more appropriate. Furthermore, based on biochemical indicators such as eGFR slope and urinary albumin-to-creatinine ratio (UACR) change rate, integrating TCM symptom evaluation, TCM syndrome evaluation, and quality of life scales can help develop internationally recognized patient-reported outcome measures (PROMs) for TCM clinical trials, which will be a key step in enhancing the evaluation system for the effectiveness of TCM in treating DKD.
